    Longitudinal Aging Study in India: Vision, Design, Implementation, and Some Early Results

    India is poised to experience a dramatic rise in its aging population in coming decades, yet comprehensive research and effective policy to confront this transition are lacking. According to projections constructed by the United Nations Population Division, the share of Indians aged 60 and over will increase from 8% today to 19% by 2050 (representing 323 million people, more than the entire US population in 2011). This demographic shift will pose significant challenges. India’s traditional reliance on private family networks to provide older people with care, companionship, and financial support will be stressed not only by the increasing number of aging Indians who rely on it, but also by changing household dynamics and patterns of spatial mobility among younger family members. The Longitudinal Aging Study in India (LASI) is intended to inform the design and expansion of a new generation of institutions – public and private – for the care and support of India’s population of older people by providing comprehensive data to the scientific and policy community. LASI is an evidence base for analyzing the (1) health, (2) economic and financial resources, and (3) living arrangements and social connections of older Indians. It enhances opportunities for cross-national analysis by adding India to the growing number of countries with harmonized data on their older populations. LASI surveys will be carried out every two years, providing longitudinal data to support research and policy development. This paper provides an overview of the conception and content of the 2010 LASI pilot survey that was conducted in four states: Punjab, Rajasthan, Karnataka, and Kerala. We highlight key aspects of the field work, such as response rates and interview duration, and discuss the breadth and quality of the economic, health, and social data collected. We pay close attention to the cultural and geographic diversity LASI is able to capture, and bring to light interesting patterns in, and relationships among, measures of health, social connectedness, labor force participation, and hardship among the elderly.aging, longitudinal, India

    Prevalence, circumstances and consequences of non-fatal road traffi c injuries and other bodily injuries among older people in China, Ghana, India, Mexico, Russia and South Africa

    Unintentional injuries are one of the main contributors to mortality and disability in elderly populations in low- and middle-income countries. The aim of this study was to examine the annual road traffic and other bodily (not including falls) injury prevalence and associated risk factors among older adults across six lower and upper middle-income countries. A cross- sectional survey involving face-to-face household interviews were conducted in China (n=13,177), Ghana (n=4305), India (n=6560), Mexico (n=2318), the Russian Federation (n=3938) and South Africa (n=3840), resulting in population-based cohorts of persons aged 50+ years.  Measures included questions on injury, self-rated visual difficulties, alcohol use, depression treatment, sleeping problems, self-reported health status, and vision assessment using LogMAR (logarithm of Minimum Angle of Resolution) eye charts. It comprises rows of letters and is used to measure visual acuity. Results indicate that the overall annual non-fatal road traffic injury prevalence was 2.0% and for other bodily injury 2.1% (not including falls) across the six countries. The multivariate logistic regression analysis found that residing in a rural area, taking medications or other treatment for depression in the past 12 months and having a sleeping problem were associated with road traffic injury, while younger age, residing in a rural area, hazardous or harmful alcohol use and having a sleeping problem were associated with other bodily injury. Visual impairment was not associated with prevalence of road traffic injuries. This study provides the burden of non-fatal road traffic injury and other bodily injury and their associated risk factors across the six countries’ studies. The findings of this study improves the understanding of non-fatal road traffic injury and other bodily injury upon which policy makers, programme developers and researchers in public health can design strategic interventions to reduce these preventable injuries as well as improve safety associated with unintentional injuries.Keywords: injury, traffic, ageing, China, Ghana, India, Mexico, Russian Federation, South Africa

    Bullying, Eating Disorders and Coping Strategies among Adolescents in Malaysia

    Understanding how bullying leads to eating disorders can help develop effective intervention strategies. This phenomenological study examines the relationship between bullying and coping strategies Malaysian adolescents diagnosed with eating disorders use. Using the snowball sampling technique, 12 Kuala Lumpur participants, six females and six males aged 10 to 19, responded to interview questions based on the literature reviews, theories, and a published questionnaire. By using thematic analysis, the researcher found out that all 12 participants with EDs were bullied about their weight or size. Most of the bullying and teasing were verbal or via online platforms. The findings suggest that bullying can be a significant cause of the development of eating disorders among adolescents in Malaysia. Proper coping strategies can be essential in overcoming the trauma of being bullied and treating eating disorders

    Diverse material based geopolymer towards heavy metals removal: A review

    Metakaolin is a commonly used aluminosilicate material for the synthesis of geopolymer based adsorbent. However, it presents characteristics that restrict its uses such as weak rheological properties brought on by the plate-like structure, processing challenges, high water demand and quick hydration reaction. Industrial waste, on the other hand, contains a variety of components and is a potential source of aluminosilicate material. Geopolymer adsorbent synthesized by utilizing industrial waste contains a wide range of elements that offer better ion-exchangeability and increase active sites on the surface of geopolymer. However, limited studies focused on the synthesized of geopolymer based adsorbent by utilizing industrial waste for heavy metal adsorption in wastewater treatment. Therefore, this paper reviews on the raw materials used in the synthesis of geopolymer for wastewater treatment. This would help in the development of low cost geopolymer based adsorbent that has a great potential for heavy metal adsorption, which could deliver double benefit in both waste management and wastewater treatment

    Predictors of unintended pregnancy in Kersa, Eastern Ethiopia, 2010

    <p>Abstract</p> <p>Background</p> <p>In Ethiopia, little is known about pregnancy among rural women. Proper maternal health care depends on clear understanding of the reproductive health situation. The objective of this study was to identify predictors of unintended pregnancy in rural eastern Ethiopia.</p> <p>Methodology</p> <p>This study was part of pregnancy surveillance at Kersa Demographic Surveillance and Health Research Center, East Ethiopia. Pregnant women were assessed whether their current pregnancy was intended or not. Data were collected by lay interviewers using uniform questionnaire. Odds Ratio, with 95% confidence interval using multiple and multinomial logistic regression were calculated to detect level of significance.</p> <p>Results</p> <p>Unintended pregnancy was reported by 27.9% (578/2072) of the study subjects. Out of which, 440 were mistimed and 138 were not wanted. Unintended pregnancy was associated with family wealth status (OR 1.47; 95% CI 1.14, 1.90), high parity (7 +) (OR 5.18; 95% CI 3.31, 8.12), and a longer estimated time to walk to the nearest health care facility (OR 2.24; 95% CI: 1.49, 3.39).</p> <p>In the multinomial regression, women from poor family reported that their pregnancy was mistimed (OR 1.69; 95% CI 1.27, 2.25). The longer estimated time (80 + minutes) to walk to the nearest health care facility influenced the occurrence of mistimed pregnancy (OR 2.58; 95% CI: 1.65, 4.02). High parity (7+) showed a strong association to mistimed and unwanted pregnancies (OR 3.11; 95% CI 1.87, 5.12) and (OR 14.34; 95% CI 5.72, 35.98), respectively.</p> <p>Conclusions</p> <p>The economy of the family, parity, and walking distance to the nearest health care institution are strong predictors of unintended pregnancy. In order to reduce the high rate of unintended pregnancy Efforts to reach rural women with family planning services should be strengthened.</p

    Prevalence, distribution and correlates of tobacco smoking and chewing in Nepal: a secondary data analysis of Nepal Demographic and Health Survey-2006

    <p>Abstract</p> <p>Background</p> <p>Nearly four-fifths of estimated 1.1 million smokers live in low or middle-income countries. We aimed to provide national estimates for Nepal on tobacco use prevalence, its distribution across demographic, socio-economic and spatial variables and correlates of tobacco use.</p> <p>Methods</p> <p>A secondary data analysis of 2006 Nepal Demographic and Health Survey (DHS) was done. A representative sample of 9,036 households was selected by two-stage stratified, probability proportional to size (PPS) technique. We constructed three outcome variables 'tobacco smoke', 'tobacco chewer' and 'any tobacco use' based on four questions about tobacco use that were asked in DHS questionnaires. Socio-economic, demographic and spatial predictor variables were used. We computed overall prevalence for 'tobacco smoking', 'tobacco chewing' and 'any tobacco use' i.e. point estimates of prevalence rates, 95% confidence intervals (CIs) after adjustment for strata and clustering at primary sampling unit (PSU) level. For correlates of tobacco use, we used multivariate analysis to calculate adjusted odds ratios (AORs) and their 95% CIs. A p-value < 0.05 was considered as significant.</p> <p>Results</p> <p>Total number of households, eligible women and men interviewed was 8707, 10793 and 4397 respectively. The overall prevalence for 'any tobacco use', 'tobacco smoking' and 'tobacco chewing' were 30.3% (95% CI 28.9, 31.7), 20.7% (95% CI 19.5, 22.0) and 14.6% (95% CI 13.5, 15.7) respectively. Prevalence among men was significantly higher than women for 'any tobacco use' (56.5% versus 19.6%), 'tobacco smoking' (32.8% versus 15.8%) and 'tobacco chewing' (38.0% versus 5.0%). By multivariate analysis, older adults, men, lesser educated and those with lower wealth quintiles were more likely to be using all forms of tobacco. Divorced, separated, and widowed were more likely to smoke (OR 1.49, 95% CI 1.14, 1.94) and chew tobacco (OR 1.36, 95% CI 0.97, 1.93) as compared to those who were currently married. Prevalence of 'tobacco chewing' was higher in eastern region (19.7%) and terai/plains (16.2%). 'Tobacco smoking' and 'any tobacco use' were higher in rural areas, mid-western and far western and mountainous areas.</p> <p>Conclusions</p> <p>Prevalence of tobacco use is considerably high among Nepalese people. Demographic and socioeconomic determinants and spatial distribution should be considered while planning tobacco control interventions.</p

    Data Resource Profile: The World Health Organization Study on global AGEing and adult health (SAGE)

    Population ageing is rapidly becoming a global issue and will have a major impact on health policies and programmes. The World Health Organization's Study on global AGEing and adult health (SAGE) aims to address the gap in reliable data and scientific knowledge on ageing and health in low- and middle-income countries. SAGE is a longitudinal study with nationally representative samples of persons aged 50+ years in China, Ghana, India, Mexico, Russia and South Africa, with a smaller sample of adults aged 18-49 years in each country for comparisons. Instruments are compatible with other large high-income country longitudinal ageing studies. Wave 1 was conducted during 2007-2010 and included a total of 34 124 respondents aged 50+ and 8340 aged 18-49. In four countries, a subsample consisting of 8160 respondents participated in Wave 1 and the 2002/04 World Health Survey (referred to as SAGE Wave 0). Wave 2 data collection will start in 2012/13, following up all Wave 1 respondents. Wave 3 is planned for 2014/15.     Determinants of linear growth from infancy to school-aged years: a population-based follow-up study in urban Amazonian children

    Background: Although linear growth during childhood may be affected by early-life exposures, few studies have examined whether the effects of these exposures linger on during school age, particularly in low-and middle-income countries. Methods: We conducted a population-based longitudinal study of 256 children living in the Brazilian Amazon, aged 0.1 y to 5.5 y in 2003. Data regarding socioeconomic and maternal characteristics, infant feeding practices, morbidities, and birth weight and length were collected at baseline of the study (2003). Child body length/height was measured at baseline and at follow-up visits (in 2007 and 2009). Restricted cubic splines were used to construct average height-for-age Z score (HAZ) growth curves, yielding estimated HAZ differences among exposure categories at ages 0.5 y, 1 y, 2 y, 5 y, 7 y, and 10 y. Results: At baseline, median age was 2.6 y (interquartile range, 1.4 y-3.8 y), and mean HAZ was -0.53 (standard deviation, 1.15); 10.2% of children were stunted. In multivariable analysis, children in households above the household wealth index median were 0.30 Z taller at age 5 y (P = 0.017), and children whose families owned land were 0.34 Z taller by age 10 y (P = 0.023), when compared with poorer children. Mothers in the highest tertile for height had children whose HAZ were significantly higher compared with those of children from mothers in the lowest height tertile at all ages. Birth weight and length were positively related to linear growth throughout childhood; by age 10 y, children weighing &gt;3500 g at birth were 0.31 Z taller than those weighing 2501 g to 3500 g (P = 0.022) at birth, and children measuring &gt;= 51 cm at birth were 0.51 Z taller than those measuring &lt;= 48 cm (P = 0.005). Conclusions: Results suggest socioeconomic background is a potentially modifiable predictor of linear growth during the school-aged years.     Under-Five Mortality in High Focus States in India: A District Level Geospatial Analysis

    <div><h3>Background</h3><p>This paper examines if, when controlling for biophysical and geographical variables (including rainfall, productivity of agricultural lands, topography/temperature, and market access through road networks), socioeconomic and health care indicators help to explain variations in the under-five mortality rate across districts from nine high focus states in India. The literature on this subject is inconclusive because the survey data, upon which most studies of child mortality rely, rarely include variables that measure these factors. This paper introduces these variables into an analysis of 284 districts from nine high focus states in India.</p> <h3>Methodology/Principal Findings</h3><p>Information on the mortality indicator was accessed from the recently conducted Annual Health Survey of 2011 and other socioeconomic and geographic variables from Census 2011, District Level Household and Facility Survey (2007–08), Department of Economics and Statistics Divisions of the concerned states. Displaying high spatial dependence (spatial autocorrelation) in the mortality indicator (outcome variable) and its possible predictors used in the analysis, the paper uses the Spatial-Error Model in an effort to negate or reduce the spatial dependence in model parameters. The results evince that the coverage gap index (a mixed indicator of district wise coverage of reproductive and child health services), female literacy, urbanization, economic status, the number of newborn care provided in Primary Health Centers in the district transpired as significant correlates of under-five mortality in the nine high focus states in India. The study identifies three clusters with high under-five mortality rate including 30 districts, and advocates urgent attention.</p> <h3>Conclusion</h3><p>Even after controlling the possible biophysical and geographical variables, the study reveals that the health program initiatives have a major role to play in reducing under-five mortality rate in the high focus states in India.</p> </div
