    The Communal I : Exclusion and Belonging in American Autobiography

    The Communal “I” in American autobiography emerges as an aesthetic response to the pressure of using “the master’s tools” to write from a community on the margins to disclose identity in the conflicts of exclusion and belonging. In this case “the master’s tools” to refer to several distinct elements the communal “I” is tasked with navigating: the use of what we have come to identify as standard English, the form and function of European autobiography as a celebration of individual exceptionalism, and the contradictory pressures on these autobiographies to both elevate and protect the communities in question from further marginalization. In addition, the work examines how three writers from these communities (Frederick Douglass, Maxine Hong Kingston and Gloria Anzaldúa) have been absorbed and framed within the emerging minority/diversity canon in American universities, and how one (Jesús Colón) has been conspicuously left out. The intention of effecting social change complicates and shapes the autobiographical in many ways.     Intracoronary brachytherapy for in-stent restenosis of drug-eluting stents

    AbstractPurposeGiven the limited salvage options for in-stent restenosis (ISR) of drug-eluting stents (DES), our high-volume cardiac catheterization laboratory has been performing intracoronary brachytherapy (ICBT) in patients with recurrent ISR of DES. This study analyzes their baseline characteristics and assesses the safety/toxicity of ICBT in this high-risk population.Methods and materialsA retrospective analysis of patients treated with ICBT between September 2012 and December 2014 was performed. Patients with ISR twice in a single location were eligible. Procedural complications included vessel dissection, perforation, tamponade, slow/absent blood flow, and vessel closure. Postprocedural events included myocardial infarction, coronary artery bypass graft, congestive heart failure, stroke, bleeding, thrombosis, embolism, dissection, dialysis, or death occurring within 72 hours. A control group of patients with 2 episodes of ISR at 1 location who underwent percutaneous coronary intervention without ICBT was identified. Unpaired t tests and χ2 tests were used to compare the groups.ResultsThere were 134 (78%) patients in the ICBT group with 141 treated lesions and 37 (22%) patients in the control group. There was a high prevalence of hyperlipidemia (>95%), hypertension (>95%), and diabetes (>50%) in both groups. The groups were well-balanced with respect to age, sex, and pre-existing medical conditions, with the exception of previous coronary artery bypass graft being more common the ICBT group. Procedural complication rates were low in the control and ICBT groups (0% vs 4.5%, P = .190). Postprocedural event rates were low (<5%) in both groups. Readmission rate at 30 days was 3.7% in the ICBT group and 5.4% in the control group (P = .649).ConclusionsThis is the largest recent known series looking at ICBT for recurrent ISR of DES. ICBT is a safe treatment option with similarly low rates (<5%) of procedural and postprocedural complications compared with percutaneous coronary intervention alone. This study establishes the safety of ICBT in a high-risk patient cohort

    Gestión sanitaria y su impacto en la adherencia terapéutica en pacientes hipertensos de un Centro de Salud- Santa Elena 2022

    The present study was developed with the objective of determining the impact of health management on therapeutic adherence in hypertensive patients in a health center - Santa Elena 2022. The methodology was with a quantitative approach, basic type, with a non-experimental, descriptive design. cross-sectional correlational. The sample consisted of 61 health professionals: doctors and nurses, to whom two questionnaires were applied as an instrument to collect information on both variables: health management and therapeutic adherence. The results were obtained through inferential statistical analysis, using Spearman's non-parametric Rho test, where it was demonstrated that there is a significant correlation between both variables (Rho=0.927; p&lt;0.000) and between the dimensions of participation (Rho=0.732; p&lt;0.000) and control (Rho=0.475; p&lt;0.000) with therapeutic adherence. It was concluded that health management significantly impacts the therapeutic adherence of patients with hypertension.El presente estudio se desarrolló con el objetivo de determinar el impacto de la gestión sanitaria en la adherencia terapéutica en pacientes hipertensos de un centro de salud - Santa Elena 2022. La metodología fue con enfoque cuantitativo, tipo básica, con diseño no experimental, descriptivo, correlacional de corte transversal. La muestra consistió en 61 profesionales de salud: médicos y enfermeros/as, a quienes se les aplicó como instrumento, dos cuestionarios para recoger información de ambas variables: gestión sanitaria y adherencia terapéutica. Los resultados se obtuvieron a través del análisis estadístico inferencial, mediante la prueba no paramétrica Rho de Spearman, donde se demostró que existe correlación significativa entre ambas variables (Rho=0.927; p&lt;0,000) y entre las dimensiones de participación (Rho=0.732; p&lt;0,000) y control (Rho=0.475; p&lt;0,000) con la adherencia terapéutica.     Erros de medicação em Unidade de Terapia Intensiva adulto: revisão integrativa: Medication errors in an adult Intensive Care Unit: an integrative review

    Objetivo: identificar estudos sobre erros de medicação intravenosa cometidos pela equipe de enfermagem em Unidade de Terapia Intensiva (UTI) adulto. Métodos: revisão integrativa de estudos publicados entre 2015 a 2021 que respondiam à pergunta de pesquisa: “quais erros ocorrem no processo da medicação intravenosa realizado pela equipe de enfermagem nos pacientes adultos internados em UTI?” em 4 bases de dados: Medical Literature Analysisand Retrieval System Online (MEDLINE/PubMed), Biblioteca Virtual em Saúde (BVS), Scopus e Isi Web of Science. Resultados: 39 publicações selecionadas abordam três temas: erros de medicação; incompatibilidade; e intervenções para a redução dos erros de medicação. Identificou-se prevalência de erros de medicação variada entre 1,93% a 64,38%. As incompatibilidades envolvem infusões de midazolam, insulina, norepinefrina, antibióticos e inibidores da bomba de próton. As intervenções aplicadas foram heterogêneas. Conclusão: Por conta de vários fatores, erros de medicação são frequentes na UTI no momento da administração e prescrição

    Administration of BPX-501 Cells Following Αβ T and B-Cell-Depleted HLA Haploidentical HSCT (haplo-HSCT) in Children with Acute Leukemias (AL)

    Background Allogeneic HSCT is a well-established treatment for children with AL. For pts lacking a compatible matched related or unrelated donor, HLA-haplo-HSCT represents an alternative. Promising results were reported with selective depletion of αβ T and B cells (Locatelli, Blood 2017). PX-501 is an allogeneic product consisting of T cells modified to express the inducible caspase-9 (iC9) safety switch and truncated CD19 to allow monitoring and expansion of BPX-501 following transplant. BPX-501 provides broad virus and tumor-specific immunity; the safety switch provides the unique ability to promptly and durably resolve graft-versus-host disease (GvHD) symptoms following the administration of rimiducid. Aims Evaluate the safety and efficacy of BPX-501 in pediatric pts with AL by determining whether BPX-501 infusion can increase efficacy outcomes through an enhanced graft-versus-leukemic (GvL) effect, while maintaining a low risk of GvHD. Methods A subset of pts had high-risk ALs. BPX-501 was planned to be infused on day14±4 after the allograft with no post-transplant GvHD prophylaxis allowed. Pts who developed steroid-resistant GvHD could receive ≥1 dose of rimiducid. Results As of June 30, 2018, 100 pts with AL (described in Table 1) were efficacy evaluable. Median time for neutrophil and platelet engraftment was 16 and 12 days, respectively. Four pts (4.1%) experienced primary graft failure. Of 96 evaluable pts, 5 (3.1%) developed Grade III-IV aGvHD. Of 82 evaluable pts, 12 developed cGvHD (18.1%), with 3 moderate-severe. Rimiducid was administered to 10 pts. Best overall clinical response (CR/PR) post-rimiducid was 80% (8 pts). Among responding patients, 7 (87.5%) had a CR. Six (6.6%) pts died after transplantation. Efficacy outcomes in AL subsets are in Table 2. CD3+ and CD3+CD4+ T cells above 500 cells/ml were achieved by 180 and 270 days, respectively. IgA and IgM levels achieved normal values by 180 days. Conclusion BPX-501 following αβ-T and B-cell depleted haplo-HSCT represents a highly effective transplantation strategy for pediatric pts with AL. Rimiducid was an effective treatment for pts with steroid-resistant GvHD

    Valor compartido en las empresas de generación de energía eléctrica en el Perú

    La presente investigación tuvo por objetivo describir el nivel de aplicación del valor compartido (VC) en las empresas de generación de energía eléctrica en el Perú en el año 2016. Para lograr este objetivo, se realizó un estudio cuantitativo utilizando una entrevista semiestructurada que incluyó cinco partes. Los datos recolectados provinieron de fuentes primarias y secundarias relacionados con las siete empresas más importantes de generación de energía eléctrica en el Perú, las cuales fueron: Edegel SAA, Engie Perú S.A., Kallpa Generación S.A., Fenix Power Perú S.A., Statkraft Perú S.A., Duke Energy Egenor S. en C. por A. y Electricidad del Perú – Electroperú S.A. Los resultados de la investigación indicaron que solo una de las empresas conoce el concepto de VC. Asimismo, ninguna de las siete empresas investigadas se encontró preparada para la aplicación de VC. De otro lado, solo una empresa aplicó el VC según las tres formas propuestas por Porter y Kramer (2011). Finalmente, cinco empresas de la muestra se encontraban en el Nivel 2 con respecto al VC con enfoque de stakeholders. To achieve this objective, a quantitative study was conducted using a semi-structured interview that include five parts. The data collected came from primary and secondary sources related to the top seven Peruvian electric power generation companies: Edegel SAA, Engie Perú SA, Kallpa Generación SA, Fenix Power Perú SA, Statkraft Perú SA, Duke Energy Egenor S. en C. por A. y Electricidad del Perú – Electroperú SA. The results of this research showed that one Company only knows the shared value concept. Also, none of the seven companies under this study was prepared for applying shared value. On the other hand, one Company only applied shared value using the three ways proposed by Porter and Kramer (2011). Finally, five companies from the sample were in Level 2 with respect to shared value associated to stakeholders.     Procrastination and Its Relationship to the Academic Burnout of First-Year College Students in a State University

    The abrupt shift in learning mode demands students to adjust from the comfort of their homes, as well as the challenges of face-to-face learning. As a result, as the pandemic fades, institutions in the Philippines have begun to reopen their doors to students. Hence, this study employed a correlational design to investigate the relationship between procrastination and academic burnout among 150 first-year college students in a state university. Based on the statistical analysis, the r coefficient of 0.67 indicates a moderate positive correlation between the variables. The p-value of 0.00, which is less than 0.05, leads to the decision to reject the null hypothesis. Hence, a significant relationship exists between procrastination and academic burnout among first-year college students. Implications and recommendations were discussed in the study
