IDENTIFICATION OF SAFETY ALERT BY MONITORING ANALYTICAL PARAMETERS AND HIGH-RISK DRUGS
Early detection of adverse drug reactions (ADR) increases patient safety. Our objective was to identify ADR by monitoring laboratory parameters and high-risk drugs. We carried out a two-month prospective observational study in a Internal Medicine Department, with daily recording of drugs prescribed and the following parameters: Na, K, Ca, serum creatinine, glomerular filtration rate (GFR), INR, glucose, haemoglobin, platelets, ALT, AST, bilirubin, GGT, alkaline phosphatase, TSH, T4, and blood digoxin. High-risk drugs were closely monitored. 52 patients included, of whom 46.2% experienced an ADR. We observed an association with drugs in 25.5%, as follows: reduction in GFR, 26.9% (associated with loop diuretics [41.7%], angiotensin-converting enzyme [ACE] inhibitors [33.3%], angiotensin II receptor blockers [ARB] [16.6%], andanti-diabetic drugs [8.3%]); hypokalemia, 22.3% (associated with loop diuretics [50.0%], potassium-free fluid [37.5%], and salbutamol [12.5%]); hyperkalemia, 14.4% (associated with ACE inhibitors [60.0%] and ARB [40.0%]); INR out of range, 10.8% (associated with drug interactions [66.7%]); hyperglycemia, 8.1% (associated with corticosteroids [66.7%] and anti-diabetic drugs [33.3%]); and other conditions, 18.8%. We conclued that patient safety could be improved by implementing warnings in electronic prescriptions in cases of a decrease in GFR or modification of potassium levels in patients who are prescribed loop diuretics, ACE inhibitors, or ARBs.Key words: Adverse drug reaction, clinical decision support, high-risk drug, safety
Acciones que elevan la calidad de vida en el adulto mayor. Jiguaní
The aging or normal process of changes related to the passage of time, begins with the birth and continues throughout life; elderly is the final phase of life. It was performed a descriptive study in the Elderly House Jose Reyes Arencibia located in the north popular council of Jiguani municipality in Granma Province. There were taken 20 elders from a universe of 25 with the objective to describe the actions that increase the quality of life of our elderly population through actions developed all over the country and particularly in our health system. There were valued some variables like: age, sex, family description, needs for help, necessities and actions. For the development of this work it was applied a survey taking 20 elders at random from a universe of 25 what represented the 80%. The main results were the following: 20 elders (100%) referred that the actions that have high quality of life are the elderly houses where a strict nursery follow up is performed to them, for example: the checking of vital signs, coordination with INDER to carry out strict control of compliance with diet and physical exercises, coordination of geriatric and nutrition consultations. Other actions which have been taken at the national level by the government and which were also considered by elders were the currency pensions, 18 to 90%. They are needed indeed to face and live life, 14 to 70% are also necessary to educate children and do the errands, 12 elders for 60%.El envejecimiento o proceso normal de cambios relacionados con el paso del tiempo, se inicia con el nacimiento y continúa a lo largo de la vida, la ancianidad es la fase final de la vida. Se realizó un estudio descriptivo en la casa de abuelos ¨José Reyes Arencibia¨ ubicada en el consejo popular norte del municipio Jiguaní provincia Granma en el periodo comprendido a 20 adultos mayores de un universo de 25 con el objetivo de describir las acciones que eleven la calidad de vida de nuestra población adulta mayor a través de acciones llevadas a cabo por el país y en particular nuestro sistema de salud. Se valoraron variables como: edad, sexo, descripción de la familia, relaciones familiares, necesidad de ayuda, necesidades y acciones. Para la realización de este trabajo se aplicó una encuesta al azar a 20 adultos mayores, de un universo de 25 lo que representa un 80 %. Los principales resultados fueron que 20 adultos mayores(100%) refirieron que las acciones que más han elevado su calidad de vida son las casas de abuelos donde se les realiza por parte de enfermería un estricto seguimiento en cuanto a salud se refiere por ejemplo: chequeo de signos vitales, coordinación con el INDER para la realización de ejercicios físicos y control estricto del cumplimiento de la dieta, se les coordinan consultas geriátricas y nutrición ;otras de las acciones que se han tomado a nivel nacional por parte del gobierno y que también fueron consideradas por los adultos mayores fueron las pensiones monetarias, 18 para un 90 %. Se necesitan mucho para enfrentar y vivir la vida, 14 para un 70 %, son necesarios también para educar a los hijos y hacer mandados, 12 adultos mayores para un 60 %

Cardiovascular diseases constitute one of the leading causes of death in developed countries. Among them emergencies are considered one of the leading causes of death. The statistics recorded a rising mortality between the years 2008-2009 ranged from a rate of 135 to 186 for every 100 000 inhabitants. A retrospective descriptive study was performed in the polyclinic "Kings Edor Martínez Arias" of Jiguaní during the period of the year 2009. The studied universe was made by 221 patients, coinciding with the sample and the measure was performed through data collection templates, in order to evaluate the impact of trombolisis rooms in cardiovascular emergencies. There will be considered all of the patients assisted in cardiovascular emergencies, evaluating variables like: age, sex, race, origin, if they were recovered, remitted, or dead, as well as the time that cardiovascular emergencies presented. Statistical method of percentage was used and the results were evidenced in tables, depending on the type of variable. To obtain the main results there were attended 221 patients. The 85% of them were over 46 years old. The mixed race and rural area had the greatest percentage( 65%). Within the pathologies the hypertensive urgencies had the major incidence with 43.5%. From the assisted patients 60 % was recovered in the institution and only 33% was remitted while only 12% of them passed away.Las enfermedades cardiovasculares constituyen en los países desarrollados una de las primeras causas de muerte, dentro de ellas las urgencias constituyen hoy una de las primeras causas de muerte. Las estadísticas registran una mortalidad ascendente entre los años 2008-2009 que osciló desde una tasa de 135 hasta 186 por cada 100 000 habitantes. Se realizó un estudio descriptivo retrospectivo en el Policlínico “Edor de los Reyes Martínez Arias “de Jiguaní en el período comprendido del año 2009. El universo estudiado está comprendido por 221 pacientes coincidiendo con la muestra y la medición la hicimos a través de plantillas de recolección de datos, con el objetivo de evaluar el impacto de la sala de trombolisis en las urgencias cardiovasculares. Se tendrán en cuenta todos los pacientes atendidos como una urgencia cardiovascular, valorando variables como la edad, sexo, raza, procedencia, si fueron recuperados, remitidos o fallecidos, así como el tiempo de urgencia cardiovascular que presentaron. Se utilizó el método estadístico porcentual y los resultados se vertieron en tablas según su tipo de variable. Para lograr los principales resultados fueron atendidos 221 pacientes, de ellos 85% tenían más de 46 años, siendo la raza mestiza y el área rural la de mayor por ciento con un 65% así como dentro de las patologías las urgencias hipertensivas la de mayor incidencia con un 43,5%. De los pacientes atendidos se recuperaron en la institución el 60% y solo el 33% fue remitido mientras que solo falleció el 12%
Antenatal Fear of Childbirth as a Risk Factor for a Bad Childbirth Experience
Giving birth is one of the most impressive experiences in life. However, many pregnant women suffer from fear of childbirth (FOC) and experience labour in very different ways, depending on their personality, previous life experiences, pregnancy, and birth circumstances. The aim of this study was to analyse how fear of childbirth affects the childbirth experience and to assess the related consequences. For this, a descriptive cross-sectional study was carried out in a sample of 414 women between 1 July 2021 and 30 June 2022. The Birth Anticipation Scale (BAS) was used to measure fear of childbirth and the Childbirth Experience Questionnaire (CEQ-E) was applied to measure satisfaction with the childbirth experience. Fear of childbirth negatively and significantly predicted the childbirth experience. In addition, women who were more fearful of childbirth were found to have worse obstetric outcomes and a higher likelihood of having a caesarean delivery (p = 0.008 C. I 95%). Fear behaved as a risk factor for the birth experience, so the greater the fear, the higher the risk of having a worse birth experience (OR 1.1). Encouraging active listening and support strategies may increase pregnant women's confidence, thus decreasing their fear of the process and improving their childbirth experience
Linfangioma gigante abdominal como causa de dolor abdominal inespecífico: presentación de un caso clínico
El linfangioma es una lesión benigna y poco frecuente que resulta de la malformación congénita del sistema linfático. Suele localizarse con mayor frecuencia en cabeza, cuello, axilas e ingles, aunque puede afectar cualquier localización corporal. El diagnóstico supone un reto para el pediatra, ya que, en la mayoría de las ocasiones, cursa de forma asintomática o con clínica inespecífica. Raramente cursa con síntomas derivados de alguna de sus complicaciones (rotura, hemorragia, infección). El tratamiento es quirúrgico en la mayoría de los casos. Presentamos el caso de un paciente de 9 años que acude al Servicio de Urgencias por dolor abdominal y estreñimiento de corta evolución, objetivándose en ecografía abdominal una gran masa quística cuya biopsia y PAAF revela el diagnóstico de linfangioma gigante. La resección parcial del tumor mejora el cuadro, permaneciendo a partir de entonces el paciente asintomático. Como conclusión, aunque el dolor abdominal y el estreñimiento son síntomas de etiología habitualmente banal, es importante plantearnos dentro del diagnóstico diferencial la posibilidad de masa abdominal.Lymphangioma is a benign and rare injury resulting from the congenital malformation of the lymphatic system. It is usually located in head, neck, armpits and groins, although it can affect any body location. The diagnosis is a challenge for the pediatrician, because, in most cases, is asymptomatic or nonspecific clinical. Rarely presents with symptoms resulting from some of its complications (rupture, hemorrhage, infection). The treatment is surgical in most cases. We report the case of a 9 years old boy who comes to the emergency department for abdominal pain and short evolution constipation. A large abdominal cystic mass was found with ultrasound, which biopsy revealed the diagnosis of giant lymphangioma. Partial resection of the tumor improves the symptoms, remaining thereafter asymptomatic. Although abdominal pain and constipation are usually banal symptoms, we must consider in the differential diagnosis the possibility of abdominal mass

Clinical Characteristics and Prognosis of COPD Patients Hospitalized with SARS-CoV-2
Objective
To describe the characteristics and prognosis of patients with COPD admitted to the hospital due to SARS-CoV-2 infection.
Methods
The SEMI-COVID registry is an ongoing retrospective cohort comprising consecutive COVID-19 patients hospitalized in Spain since the beginning of the pandemic in March 2020. Data on demographics, clinical characteristics, comorbidities, laboratory tests, radiology, treatment, and progress are collected. Patients with COPD were selected and compared to patients without COPD. Factors associated with a poor prognosis were analyzed.
Results
Of the 10,420 patients included in the SEMI-COVID registry as of May 21, 2020, 746 (7.16%) had a diagnosis of COPD. Patients with COPD are older than those without COPD (77 years vs 68 years) and more frequently male. They have more comorbidities (hypertension, hyperlipidemia, diabetes mellitus, atrial fibrillation, heart failure, ischemic heart disease, peripheral vascular disease, kidney failure) and a higher Charlson Comorbidity Index (2 vs 1, p<0.001). The mortality rate in COPD patients was 38.3% compared to 19.2% in patients without COPD (p<0.001). Male sex, a history of hypertension, heart failure, moderate–severe chronic kidney disease, presence of cerebrovascular disease with sequelae, degenerative neurological disease, dementia, functional dependence, and a higher Charlson Comorbidity Index have been associated with increased mortality due to COVID-19 in COPD patients. Survival was higher among patients with COPD who were treated with hydroxychloroquine (87.1% vs 74.9%, p<0.001) and with macrolides (57.9% vs 50%, p<0.037). Neither prone positioning nor non-invasive mechanical ventilation, high-flow nasal cannula, or invasive mechanical ventilation were associated with a better prognosis.
Conclusion
COPD patients admitted to the hospital with SARS-CoV-2 infection have more severe disease and a worse prognosis than non-COPD patients
Aplicación de potasio en variedades de caña de azúcar: efectos en la productividad y en el ataque del taladrador de la caña
La caña de azúcar posee múltiples usos, tanto en la alimentación como en la producción de energía. Para la obtención de altas productividades se debe aplicar tecnologías y técnicas que mejoren la calidad del producto en armonía con el ambiente. El objetivo de esta investigación fue evaluar el efecto de la fertilización potásica sobre la producción de dos cultivares de caña de azúcar y el ataque del taladrador (Diatraea saccharalis), en condiciones de campo. Se condujo un experimento en el área experimental del Departamento de Suelos de la Universidad Federal de Santa María, RS, Brasil. El arreglo experimental fue bifactorial, siendo el primer factor dos cultivares (RB-956911 y SP81-3250), el segundo factor la aplicación de cuatro dosis de potasio (0, 40, 80 y 120 kg ha-1 de K2O). Se evaluaron: a) la longitud de la parte industrial (LPI), b) diámetro del tallo, c) número de entrenudos, d) número de tallos, e) rendimiento, f) °Brix, g) Intensidad de Infestación (II) y h) el Índice de Intensidad de Infestación (III). El rendimiento del cultivar RB-956911 y SP81-3250 fue de 102,3 y 121,8 Mg ha-1, respectivamente, el oBrix de 18,61 y 18,03, respectivamente. La II y el III no fueron afectados por el cultivar, presentando valores de II de 3,71 y 4,08 y de III de 4,27 y 4,82, respectivamente para el cultivar RB-956911 y SP81-3250. La fertilización potásica no afecta la producción de caña de azúcar, ni el oBrix, sin embargo, tiende a disminuir el ataque del taladrador de la caña de azúcar
