Aplicació de la microdià lisi cerebral a l'estudi de la resposta metabòlica i inflamatòria en el traumatisme cranioencefà lic moderat i greu
Consultable des del TDXTÃtol obtingut de la portada digitalitzadaEls traumatismes cranioencefà lics (TCE) són la primera causa de mort i discapacitat en la població menor de 40 anys de qualsevol païs industrialitzat. Les xifres de mortalitat són encara elevades, oscil·lant, en la majoria de centres, entre el 30-40%. Els mals resultats neurològics supera en molts casos el 55%. Aquests resultats comporten importants repercusions sanità ries, econòmiques i socials, i expliquen el progressiu interés en la investigació traslacional de les lesions neurotraumà tiques. Malgrat el millor coneixement de la fisiopatologia del TCE i de l'avenç en les tècniques de neuromonitoratge i en el tractament, el pronòstic d'aquests pacients ha assolit una meseta. És conegut que tot i que una part considerable de les lesions es produeixen de forma immediata a l'impacte (lesions primà ries), moltes d'elles apareixen en un periode variable de temps després del traumatisme (lesions secondà ries). La hipòxia tissular d'origen isquèmic o no isquèmic constitueix la lesió secondà ria de major prevalència als TCE. A més, genera o potencia l'aparició de processos metabòlics anòmals i pot induïr una resposta inflamatòria a nivell tissular. Tanmateix, les dades obtingudes d'estudis en models experimentals i clÃnics han demostrat de forma inequÃvoca que la resposta inflamatòria cerebral juga un paper predominant en la lesió secondà ria postraumà tica, facilitant l'alteració de la permeabilitat de la barrera hematoencefà lica (BHE), l'edema cerebral i secondà riament la tumefacció cerebral i l'augment de la pressió intracranial. En els darrers anys, la introducció de noves tecnologies en l'à mbit clÃnic ha permès utilitzar tècniques revolucionà ries de neuromonitoratge a la capçalera del pacient. Una d'elles ha estat la microdià lisi cerebral, que permet el monitoratge neuroquÃmic quasi on-line dels nivells extracel·lulars de certs metabolits. El principi bà sic d'aquesta tècnica és la introducció al parènquima cerebral d'un catèter que conté una membrana semipermeable al seu extrem distal. La membrana de dià lisi permet la difusió lliure d'aigua i soluts entre l'espai extracel·lular cerebral i una solució salina perfosa mitjançant una microbomba. La possibilitat d'obtenir informació del metabolisme cerebral a la capçalera del pacient ens permet estudiar la resposta metabòlica a determinats procediments terapèutics, aixà com preveure l'establiment del dany tissular degut a la hipòxia tissular cerebral. A més, la recent aparició al mercat de noves membranes de microdià lisi, amb porus de filtratge majors, ha obert la possibilitat d'estudiar proteïnes de major tamany, implicades en els processos neuroinflamatoris desencadenats després d'un TCE, com l'estudi del paper de les metal·loproteïnases de matriu (MMPs) a la lesió cerebral traumà tica. La present tesi pretèn aportar noves aplicacions de la microdià lisi cerebral en l'estudi del metabolisme i de la neuroinflamació del pacient neurotraumà tic. En primer lloc l'aplicació de la microdià lisi com a eina diagnòstica en la detecció de la hipòxia cerebral isquèmica, ens permet validar per una banda la superioritat d'aquesta tècnica respecte una altra de més convencional, com és el cas del cà lcul de les diferencies arterio-jugulars de lactat i l'Ãndex lactat-oxigen. A més, la utilització combinada de la microdià lisi amb la pressió tisular d'oxigen ens permet determinar l'efecte de la hiperòxia normobà rica en el metabolisme cerebral d'aquests pacients. En segon lloc, la tesi aprofundeix en l'estudi d'unes proteases que es sobreexpressen com a conseqüència dels processos inflamatoris desencadenats a la fase aguda del TCE, les MMPs, que es relacionen amb l'alteració de la BHE. La possibilitat de poder recuperar proteïnes amb un pes mol·lecular superior al dels catèters de primera generació, obre pas a múltiples opcions terapèutiques. Una d'elles seria dissenyar estratègies farmacològiques per bloquejar o modular la sobreexpressió d'aquestes proteases a la fase aguda del TCE, amb la finalitat de limitar l'alteració de la BHE i el dany tissular postraumà tic cerebral.Traumatic brain injury (TBI) is the leading cause of death and disability worldwide among the population under the age of 40 years. Mortality is still high, and it fluctuates between 30% and 40% in the majority of centers. The poor neurologic outcome exceeds in many cases the 55%. These results lead to important sanitary, economic and social consequences, and explain the progressive interest in the translational research of brain traumatic lesions. In spite of the better understanding of TBI pathophysiology and the progress achieved in neuromonitoring techniques and treatment, the prognosis of these patients has reached a plateau. It is well known that even though a considerable part of injury is produced immediately after impact (primary brain injury), a great part appear in a variable period of time after traumatism (secondary brain injury). Tissular hypoxia, with ischemic or not ischemic origin, is the most prevalent secondary brain injury in TBI patients. Moreover, it generates or boost metabolic alterated processes and can induce an inflammatory tissular response. Data obtained from experimental and clinical models have firmly shown that inflammatory response has a crucial role in the posttraumatic secondary injury, promoting blood-brain barrier (BBB) increased permeability, brain edema and secondly brain tumefaction and increases in intracranial pressure. New clinical technologies have recently permitted to use revolutionary neuromonitoring techniques at the patient bedside. One of them is brain microdialysis, that allows brain neurochemistry monitoring of several metabolites in the extracellular space. The basic principle about this technique is the implantation of one catheter in the brain parenchyma, that contains a semipermeable membrane in the distal part. The dialysis membrane allows free difusion of water and molecules between the extracellular space and a saline solution, which is perfused by using a micropump. Due to the fact that we are able to obtain brain metabolic information at the patient bedside, we can study the metabolic response to some therapeutic procedures, as well as prevent tissular injury due to brain hypoxia. Moreover, the new generation membranes, with larger filtration membrane pores, has opened the possibility to study proteins with higher molecular weight implicated in the neuroinflammatory processes triggered after a TBI, such as the matrix metaloproteinases (MMPs). The present thesis aims to provide new applications of brain microdialysis in the metabolic and neuroinflammation field from the brain trauma patient. First, the microdialysis application as a diagnosis tool to detect brain ischemic hypoxia enables to validate the superiority of this technique in front of another one more conventional, like the arteriojugular venous differences of lactate and lactate-oxygen index. Moreover, the combinated use of brain tissue oxygen pressure and brain microdialysis allows us to analyse the normobaric hyperoxia effects on the brain metabolism of these patients. Second, the thesis explores the role of certain proteases that are overexpressed after a TBI, named MMPs, which are related to BBB disruption. The possibility to recover proteins with higher molecular weight opens multiple therapeutic options. One of them would be to design a pharmacological blockade or modulation of these proteases in the acute phase from TBI, with the goal to reduce BBB permeability and brain tissular damage
Per què rebroten les plantes?
La capacitat de les plantes d'assimilar diòxid de carboni i els seus requeriments d'aquest element no són constants en el temps, i els pics en la demanda poden correspondre amb condicions poc adequades per fer molta fotosÃntesi. Per coordinar aquests desajustos, les plantes tenen reserves de carbohidrats. Investigadors del CREAF i de la UAB han analitzat el paper de les reserves que les plantes empren per coordinar aquests desajustos, una recerca que pot fer reconsiderar la manera com entenem el balanç de carboni.La capacidad de las plantas de asimilar dióxido de carbono y sus requerimientos de este elemento no son constantes en el tiempo, y los picos en la demanda pueden corresponder con condiciones poco adecuadas para llevar a cabo mucha fotosÃntesis. Para coordinar estos desajustes, las plantas tienen reservas de carbohidratos. Investigadores del CREAF y de la UAB han analizado el papel de las reservas que las plantas emplean para coordinar estos desajustes, una investigación que puede hacer reconsiderar la forma en que entendemos el balance de carbono
Redisseny de la falca hospitalà ria
En aquest projecte es dissenya una falca hospitalà ria. L'objectiu és fer un producte més còmode i higiènic que el que s'utilitza actualment als hospitals i a altres facilitats. Es fa un estudi previ on l'usuari és el centre. Es realitza una enquesta i una entrevista i s'agafen resultats d'estudis previs per entendre la problemà tica d'aquest producte. A partir d'aquÃ, mitjançant diverses metodologies de disseny es comença una fase de generació d'idees tenint en compte tots els conceptes apresos a la fase d'investigació. Amb tots els croquis plantejats a la fase anterior, es crea un producte definitiu el qual es prototipa amb el programa Solidworks. El que fa destacar aquest projecte és que es fa servir la silicona mèdica com a material de la falca hospitalà ria. Els principals beneficis de la silicona són que és un material que s'emmotlla al cos i té un procés de desinfecció efectiva. Per evitar un dels principals problemes estudiats de la falca, el vessament, s'ha afegit una sortida per poder connectar el mesurador de diüresi. Un cop fet el disseny s'estudia el mètode de fabricació pel producte i es fa un estudi d'esforços. Per validar el producte es fa un prototip amb impressió 3D. Aquest prototip no s'ha pogut fabricar amb silicona, però es pot comprovar la capacitat i les mesures de la falca. Finalment, es pot concloure que amb el disseny de la falca s'han solucionat tres dels principals problemes de la falca amb el nou disseny innovador.En este proyecto se diseña una cuña hospitalaria. El objetivo es hacer un producto más cómodo e higiénico que el que se utiliza actualmente en los hospitales y otras facilidades. Se realiza un estudio previo donde el usuario es el centro. Se realiza una encuesta y una entrevista y se toman resultados de estudios previos para entender la problemática de este producto. A partir de ahÃ, mediante diversas metodologÃas de diseño se comienza una fase de generación de ideas teniendo en cuenta todos los conceptos aprendidos en la fase de investigación. Con todos los croquis planteados en la fase anterior, se crea un producto definitivo que se prototipa con el programa Solidworks. Lo que hace destacar este proyecto es que se utiliza la silicona médica como material de la cuña hospitalaria. Los principales beneficios de la silicona són que es un material que se amolda al cuerpo y tiene un proceso de desinfección efectiva. Para evitar uno de los principales problemas estudiados de la cuña, el vertido, se ha añadido una salida para poder conectar el medidor de diuresis. Una vez realizado el diseño se estudia el método de fabricación por el producto y se realiza un estudio de esfuerzos. Para validar el producto se realiza un prototipo con impresión 3D. Este prototipo no se ha podido fabricar con silicona, pero puede comprobarse la capacidad y las medidas de la cuña. Por último, se puede concluir que con el diseño de la cuña se han solucionado tres de los principales problemas de la cuña con el nuevo diseño innovador.In this project a hospital bedpan is designed. The objective is to make a more comfortable and hygienic product than the one currently used in hospitals and other facilities. A previous study is carried out where the user is the center. A survey and an interview are conducted and results of previous studies are taken to understand the problems of this product. From there, through various design methodologies, a phase of idea generation begins, taking into account all the concepts learned in the research phase. With all the sketches created in the previous phase, a definitive product is created and prototyped with the Solidworks program. What makes this project stand out is that medical silicone is used as the material for the hospital bedpan. The main benefits of silicone are that it is a material that molds to the body and has an effective disinfection process. To avoid one of the main problems of bedpans, the spillage, an outlet has been added to connect the diuresis meter. Once the design is completed, the manufacturing method for the product is studied and a stress study is carried out. To validate the product, a prototype is made with 3D printing. This prototype could not be manufactured with silicone, but the capacity and measurements of the bedpan can be checked. Finally, it can be concluded that with the wedge design, three of the main problems of the wedge have been solved with the new innovative design
Association of depression phenotypes and antidepressant treatment with mortality due to cancer and other causes: a community-based cohort study
Fà rmac antidepressiu; Cà ncer; SÃndrome depressiuFármaco antidepresivo; Cáncer; SÃndrome depresivoAntidepressant drug; Cancer; Depressive syndromeObjective: This study aimed to assess the association of somatic depressive symptoms (SDS), cognitive/emotional depressive symptoms (C-EDS), and antidepressant treatment on mortality due to cancer and other causes in a community cohort.Methods: A community-based sample recruited in 1995, 2000, and 2005 aged between 35 and 75 years was examined in two waves and followed for a median of 6.7 years. SDS and C-EDS phenotypes were assessed using the Patient Health Questionnaire-9. Medication used by participants was collected. Deaths and their causes were registered during follow-up. Cox proportional hazard models stratified by sex were performed to determine the association between depressive phenotypes and mortality.Results: The cohort consisted of 5,646 individuals (53.9% women) with a mean age of 64 years (SD = 11.89). During the follow-up, 392 deaths were recorded, of which 27.8% were due to cancer. C-EDS phenotype was associated with an increased risk of cancer mortality in both men (HR = 2.23; 95% CI = 1.11–4.44) and women (HR = 3.69; 95% CI = 1.69–8.09), and SDS was significantly associated with non-cancer mortality in men (HR = 2.16; 95 CI % = 1.46–3.18). Selective serotonin reuptake inhibitors (SSRIs) were significantly associated with both cancer (HR = 2.78; 95% CI = 1.10–6.98) and non-cancer mortality (HR = 2.94; 95% CI = 1.76–4.90) only in the male population.Conclusion: C-EDS phenotype was related to an increased risk of cancer mortality at 6 years. In addition, the use of SSRIs in the male population was associated with cancer and all-cause mortality.Objectiu: aquest estudi pretenia avaluar l'associació de sÃmptomes depressius somà tics (SDS), sÃmptomes depressius cognitius/emocionals (C-EDS) i tractament antidepressiu sobre la mortalitat per cà ncer i altres causes en una cohort comunità ria.
Mètodes: una mostra basada en la comunitat reclutada el 1995, 2000 i 2005 d'entre 35 i 75 anys es va examinar en dues onades i es va seguir durant una mitjana de 6,7 anys. Els fenotips SDS i C-EDS es van avaluar mitjançant el Patient Health Questionnaire-9. Es van recollir els medicaments utilitzats pels participants. Durant el seguiment es van registrar les morts i les seves causes. Es van realitzar models de risc proporcional de Cox estratificats per sexe per determinar l'associació entre fenotips depressius i mortalitat.
Resultats: La cohort estava formada per 5.646 individus (53,9% dones) amb una edat mitjana de 64 anys (DE = 11,89). Durant el seguiment es van registrar 392 defuncions, de les quals el 27,8% van ser per cà ncer. El fenotip C-EDS es va associar amb un augment del risc de mortalitat per cà ncer tant en homes (HR = 2,23; IC 95% = 1,11-4,44) com en dones (HR = 3,69; IC 95% = 1,69-8,09), i el SDS es va associar significativament. amb mortalitat no per cà ncer en homes (HR = 2,16; IC 95 % = 1,46–3,18). Els inhibidors selectius de la recaptació de serotonina (ISRS) es van associar significativament tant amb el cà ncer (HR = 2,78; IC 95% = 1,10-6,98) com amb la mortalitat no per cà ncer (HR = 2,94; IC 95% = 1,76-4,90) només a la població masculina.
Conclusió: el fenotip C-EDS es va relacionar amb un augment del risc de mortalitat per cà ncer als 6 anys. Antimicrobial susceptibility pattern of porcine respiratory bacteria in Spain
The monitoring of antimicrobial susceptibility of pig pathogens is critical to optimize antimicrobial treatments and prevent development of resistance with a one-health approach. The aim of this study was to investigate the antimicrobial susceptibility patterns of swine respiratory pathogens in Spain from 2017 to 2019. Bacterial isolation and identification were carried out following standardized methods from samples coming from sacrificed or recently deceased pigs with acute clinical signs compatible with respiratory tract infections. Minimum inhibitory concentration (MIC) values were determined using the broth microdilution method containing a total of 10 and 7-8 antimicrobials/concentrations respectively, in accordance with the recommendations presented by the Clinical and Laboratory Standards Institute (CLSI). The obtained antimicrobial susceptibility varies between pig respiratory pathogens. Actinobacillus pleuropneumoniae (APP) and Pasteurella multocida (PM) were highly susceptible (≥90%) to ceftiofur, florfenicol and macrolides (tilmicosin, tildipirosin and tulathromycin). However, the antimicrobial susceptibility was intermediate (>60% but <90%) for amoxicillin and enrofloxacin in the case of APP and sulfamethoxazole/trimethropim and tiamulin in the case of PM. Both bacteria showed low (<60%) antimicrobial susceptibility to doxycycline. Finally, Bordetella bronchiseptica was highly susceptible only to tildipirosin and tulathromycin (100%) and its susceptibility for florfenicol was close to 50% and <30% for the rest of the antimicrobial families tested. These results emphasize the need of determining antimicrobial susceptibility in pig respiratory cases in order to optimize the antimicrobial treatment in a case-by-case scenario
Brain Perihematoma Genomic Profile Following Spontaneous Human Intracerebral Hemorrhage
BACKGROUND: Spontaneous intracerebral hemorrhage (ICH) represents about 15% of all strokes and is associated with high mortality rates. Our aim was to identify the gene expression changes and biological pathways altered in the brain following ICH. METHODOLOGY/PRINCIPAL FINDINGS: Twelve brain samples were obtained from four deceased patients who suffered an ICH including perihematomal tissue (PH) and the corresponding contralateral white (CW) and grey (CG) matter. Affymetrix GeneChip platform for analysis of over 47,000 transcripts was conducted. Microarray Analysis Suite 5.0 was used to process array images and the Ingenuity Pathway Analysis System was used to analyze biological mechanisms and functions of the genes. We identified 468 genes in the PH areas displaying a different expression pattern with a fold change between -3.74 and +5.16 when compared to the contralateral areas (291 overexpressed and 177 underexpressed). The top genes which appeared most significantly overexpressed in the PH areas codify for cytokines, chemokines, coagulation factors, cell growth and proliferation factors while the underexpressed codify for proteins involved in cell cycle or neurotrophins. Validation and replication studies at gene and protein level in brain samples confirmed microarray results. CONCLUSIONS: The genomic responses identified in this study provide valuable information about potential biomarkers and target molecules altered in the perihematomal regions
Antimicrobial resistance and virulence characterisation among Escherichia coli clinical isolates causing severe obstetric infections in pregnant women
The virulence markers and the antimicrobial resistance profile of 78 Escherichia coli isolates causing obstetric infections accompanied or not by sepsis were studied. Adhesion-related virulence factors were the most prevalent. Low rates of resistance to the antimicrobial agents used as first line therapy suggest their correct implementation in stewardship guidelines
