    Financial strain, parental smoking, and the great recession: an analysis of the UK Millennium Cohort Study

    Introduction: During the recent “Great Recession,” many families in the United Kingdom experienced increased financial strain (FS). The aim of this study was to determine if increases in FS, occurring over the period of the “Great Recession,” were associated with increased risks of persistent and relapsed tobacco use among parents. Methods: We analyzed the Millennium Cohort Study, a longitudinal study of 18819 children born in the United Kingdom between 2000 and 2002. Surveys at 7 (T1, 2008) and 11 years (T2, 2012) spanned the “Great Recession.” Three measures of increased FS were defined; “became income poor” (self-reported household income dropped below the “poverty line” between T1 and T2); “developed difficulty managing” (parental report of being “financially comfortable” at T1 and finding it “difficult to manage” at T2); “felt worse off” (parental report of feeling financially “worse off” at T2, compared to T1). Poisson regression was used to estimate risk ratios (RR), adjusted RRs (aRR), and 95% confidence intervals for three outcomes: “persistent tobacco use,” “new reported tobacco use,” and “relapsed tobacco use.” Results: Parents in households which “became income poor” over the period of the “Great Recession” were significantly more likely to report “persistent tobacco use” (aRR = 2.17 [1.83–2.57]) or “new reported tobacco use” (aRR = 1.72 [1.04–2.83]). Ninety-five percent of “new reported tobacco users” had evidence of prior tobacco use suggesting the majority were “relapsed tobacco users.” Similar patterns were seen for those who “developed difficulty managing” and “felt worse off.” Conclusions: Increased tobacco use among financially strained families has the potential to widen inequalities and undermine the public health policies that have had positive impacts on tobacco consumption in the United Kingdom

    Looking back at the stare-in-the-crowd effect: Staring eyes do not capture attention in visual search

    The stare-in-the crowd effect refers to the finding that a visual search for a target of staring eyes among averted- eyesdistractersismoreefficientthanthesearchforan averted-eyes target among staring distracters. This finding could indicate that staring eyes are prioritized in the processing of the search array so that attention is more likely to be directed to their location than to any other. However, visual search is a complex process, which not only depends upon the properties of the target, but also the similarity between the target of the search and the distractor items and between the distractor items themselves. Across five experiments, we show that the search asymmetry diagnostic of the stare- in-the-crowd effect is more likely to be the result of a failure to control for the similarity among distracting items between the two critical search conditions rather than any special attention-grabbing property of staring gazes. Our results suggest that, contrary to results reported in the literature, staring gazes are not prioritized by attention in visual search

    Assessing the impact of verbal and visuospatial working memory load on eye-gaze cueing

    Observers tend to respond more quickly to peripheral stimuli that are being gazed at by a centrally presented face, than to stimuli that are not being gazed at. While this gaze-cueing effect was initially seen as reflexive, there have also been some indications that top-down control processes may be involved. Therefore, the present investigation employed a dual-task paradigm to attempt to disrupt the putative control processes involved in gaze cueing. Two experiments examined the impact of working memory load on gaze cueing. In Experiment 1, participants were required to hold a set of digits in working memory during each gaze trial. In Experiment 2, the gaze task was combined with an auditory task that required the manipulation and maintenance of visuo-spatial information. Gaze cueing effects were observed, but they were not modulated by dual-task load in either experiment. These results are consistent with traditional accounts of gaze cueing as a highly reflexive process

    Learning approach and its relationship to type of media use and frequency of media multitasking

    Research has demonstrated that learning is impaired if students multitask with media while encountering new information. However, some have gone further, and suggested that media-multitasking (as a general activity) may have a negative impact on cognitive control processes. If this were the case, students who are heavy media-multitaskers generally would have difficulties with goal-directed behaviour, and organising their time effectively to meet their learning goals. The study described here explored links between total levels of self-reported media-multitasking, academic achievement and approaches to learning. Well-established measures of media-multitasking and learning approach were given to 307 students. Total levels of media-multitasking did not relate to either learning approach or academic achievement. However, surface learning approach related negatively to academic achievement, and time spent engaging with printed media. Deep learning approach was positively related to time spent using printed media, email and other computer applications. These findings suggest that patterns of media use differ according to current learning approach, and that these patterns may be more relevant for learning than overall tendency to media-multitask

    Scattering variability detected from the circumsource medium of FRB 20190520B

    Fast radio bursts (FRBs) are millisecond-timescale radio transients, the origins of which are predominantly extragalactic and likely involve highly magnetized compact objects. FRBs undergo multipath propagation, or scattering, from electron density fluctuations on sub-parsec scales in ionized gas along the line-of-sight. Scattering observations have located plasma structures within FRB host galaxies, probed Galactic and extragalactic turbulence, and constrained FRB redshifts. Scattering also inhibits FRB detection and biases the observed FRB population. We report the detection of scattering times from the repeating FRB 20190520B that vary by up to a factor of two or more on minutes to days-long timescales. In one notable case, the scattering time varied from 7.9±0.47.9\pm0.4 ms to less than 3.1 ms (95%95\% confidence) over 2.9 minutes at 1.45 GHz. The scattering times appear to be uncorrelated between bursts or with dispersion and rotation measure variations. Scattering variations are attributable to dynamic, inhomogeneous plasma in the circumsource medium, and analogous variations have been observed from the Crab pulsar. Under such circumstances, the frequency dependence of scattering can deviate from the typical power-law used to measure scattering. Similar variations may therefore be detectable from other FRBs, even those with inconspicuous scattering, providing a unique probe of small-scale processes within FRB environments.Comment: 10 pages, 9 figures, accepted to MNRA

    Subclinical Tuberculosis Disease-A Review and Analysis of Prevalence Surveys to Inform Definitions, Burden, Associations, and Screening Methodology.

    While it is known that a substantial proportion of individuals with tuberculosis disease (TB) present subclinically, usually defined as bacteriologically-confirmed but negative on symptom screening, considerable knowledge gaps remain. Our aim was to review data from TB prevalence population surveys and generate a consistent definition and framework for subclinical TB, enabling us to estimate the proportion of TB that is subclinical, explore associations with overall burden and program indicators, and evaluate the performance of screening strategies. We extracted data from all publicly available prevalence surveys conducted since 1990. Between 36.1% and 79.7% (median, 50.4%) of prevalent bacteriologically confirmed TB was subclinical. No association was found between prevalence of subclinical and all bacteriologically confirmed TB, patient diagnostic rate, or country-level HIV prevalence (P values, .32, .4, and .34, respectively). Chest Xray detected 89% (range, 73%-98%) of bacteriologically confirmed TB, highlighting the potential of optimizing current TB case-finding policies

    A double-blinded randomised dietary supplement crossover trial design to investigate the short-term influence of medium chain fatty acid (MCT) supplement on canine idiopathic epilepsy : study protocol

    Abstract Background Epilepsy is the most common brain disease in dogs. Recently, diets have been reported to have a positive impact on seizure activity and behaviour in various species including dogs with idiopathic epilepsy (IE). Historically, classic high fat ketogenic diets (KD) and medium chain triglycerides (MCT) KD have been successfully used to manage drug-resistant epilepsy. Similarly, an MCT enriched diet has been shown to improve seizure control and behavioural comorbidities in some dogs with IE. However, it is unknown whether an MCT dietary supplement (DS) may provide similar positive effects. Methods A 6-month prospective, randomised, double-blinded, placebo-controlled, crossover, multicentre dietary trial is designed comparing a 9% metabolic energy based calculated medium-chain triglyceride (MCT) oil supplement to a conventional ‘control’ DS. Only dogs which will have an International Veterinary Epilepsy Task Force Tier II level like diagnosis of IE which satisfied the following inclusion criteria are included: age between 6 months and ≤ 12 years; weighing between 4 and ≤ 65 kg; unremarkable interictal neurological examinations; no clinically significant findings on routine laboratory diagnostics; unremarkable brain MRI scan; have had at least 3 seizures in the previous 3 months prior to enrolment; treated with at least one ASD and being classified as resistant. All dogs are fed initially for 90 ± 2 days with either the control oil or the MCT oil alongside their normal diet, followed by 97 ± 2 days with the other supplement including a 7-day washout period. Overall, the aim is to recruit thirty-six patients at five different centres and to investigate the effect of MCTs as DS on seizure activity, tolerability, behavioural comorbidities and quality of life (QoL). Discussion Dietary interventions are rarely studied in a standardised form in veterinary medicine. The background diet, the cohort of animals and ASD received is standardised in this prospective diet trial to ensure representative data about the potential effect of MCT DS. If the study data confirms former findings, this would provide further evidence for the efficacy of MCTs as a management option for canine epilepsy. This publication should offer a repository of trial conditions and variable description with forecasted statistical analysis

    Regional and racial variations in the utilization of endoscopic retrograde cholangiopancreatography among pancreatic cancer patients in the United States

    BackgroundPancreatic cancer is projected to become the second leading cause of cancerâ related deaths by 2030. Endoscopic retrograde cholangiopancreatography (ERCP) is recommended as firstâ line therapy for biliary decompression in pancreatic cancer. The aim of our study was to characterize geographic and racial/ethnic disparities in ERCP utilization among patients with pancreatic cancer.MethodsRetrospective cohort study using the US Surveillance, Epidemiology, and End Results (SEER)â Medicare database to identify patients diagnosed with pancreatic cancer from 2003â 2013. The primary outcome was receipt of ERCP, with or without stent placement, vs any nonâ ERCP biliary intervention.ResultsOf the 36 619 patients with pancreatic cancer, 37.5% (n = 13 719) underwent an ERCP, percutaneous drainage, or surgical biliary bypass. The most common biliary intervention (82.6%) was ERCP. After adjusting for tumor location and stage, Blacks were significantly less likely to receive ERCP than Whites (aOR 0.84, 95% CI 0.72, 0.97) and more likely to receive percutaneous transhepatic biliary drainage (PTBD) (aOR 1.38, 95% CI 1.14, 1.66). Patients in the Southeast and the West were more likely to receive ERCP than those in the Northeast (Southeast aOR 1.21, 95% CI 1.04, 1.40; West aOR 1.16, 95% CI 1.01, 1.32).ConclusionRacial/ethnic and geographic disparities in access to biliary interventions including ERCP exist for patients with pancreatic cancer in the United States. Our results highlight the need for further research and policies to improve access to appropriate biliary intervention for all patients.To date, disparities in the receipt of endoscopic therapies among patients with pancreatic cancer have not been reported.     Resistance to the impact of interruptions during multitasking by healthy adults and dysexecutive patients

    Two experiments (one with healthy adult volunteers and the other with controls and dysexecutive patients) assessed the impact of interruptions on a novel test of multitasking. The test involved switching repeatedly between four tasks (block construction, bead threading, paper folding, alphabetical searching) over a 10 minute period. In Experiment 1, there were 4 groups of 20 healthy participants. One group attempted multitasking with no interruption, a second group was interrupted early in the test, a third group late in the test and a fourth group was interrupted both early and late. Interruption involved carrying out a fifth, unexpected task for a period of one minute before returning to the four main tasks. There was no difference in multitasking performance between the groups. In Experiment 2 the participants were seven dysexecutive patients and 14 age-matched controls. A repeated measures approach was employed to assess the impact of two interruptions (early and late) for both groups. Contrary to predictions, the patients as well as controls were resistant to the effects of interruptions, despite their clearly impaired multitasking performance. These results suggest that the ability to deal with interruptions may be separable from the ability to organise and execute multiple tasks within a limited time frame
