    ECG study in practical labs for biomedical engineering training

    Non-invasive biomedical measurements are one of the most important technological contributions whitin the biomedical engineering field. On this paper, a dual laboratory session student oriented is designed to simulate and implement a cardial signal monitor. During the first session, ORCAD PSpice software is used to simulate the whole process. The students can acquire knowledge on the process by configuring and running both the instrumentation amplifier and a passive filter to improve the signal quality. The second session requires from the student basic laboratory skills to use a specific printed circuit board (PCB) to measure its very own cardiac potential. As a result, from this session, the student can visualize the ECG signal acquired directly on the laboratory oscilloscope

    The Use of Corticosteroids or Tocilizumab in COVID-19 Based on Inflammatory Markers

    SEMI-COVID-19 Network.[Background] The inflammatory cascade is the main cause of death in COVID-19 patients. Corticosteroids (CS) and tocilizumab (TCZ) are available to treat this escalation but which patients to administer it remains undefined.[Objective] We aimed to evaluate the efficacy of immunosuppressive/anti-inflammatory therapy in COVID-19, based on the degree of inflammation. .[Design] A retrospective cohort study with data on patients collected and followed up from March 1st, 2020, to May 1st, 2021, from the nationwide Spanish SEMI-COVID-19 Registry. Patients under treatment with CS vs. those under CS plus TCZ were compared. Effectiveness was explored in 3 risk categories (low, intermediate, high) based on lymphocyte count, C-reactive protein (CRP), lactate dehydrogenase (LDH), ferritin, and d-dimer values.[Patients] A total of 21,962 patients were included in the Registry by May 2021. Of these, 5940 met the inclusion criteria for the present study (5332 were treated with CS and 608 with CS plus TCZ).[Main Measures] The primary outcome of the study was in-hospital mortality. Secondary outcomes were the composite variable of in-hospital mortality, requirement for high-flow nasal cannula (HFNC), non-invasive mechanical ventilation (NIMV), invasive mechanical ventilation (IMV), or intensive care unit (ICU) admission.[Key Results] A total of 5940 met the inclusion criteria for the present study (5332 were treated with CS and 608 with CS plus TCZ). No significant differences were observed in either the low/intermediate-risk category (1.5% vs. 7.4%, p=0.175) or the high-risk category (23.1% vs. 20%, p=0.223) after propensity score matching. A statistically significant lower mortality was observed in the very high–risk category (31.9% vs. 23.9%, p=0.049).[Conclusions] The prescription of CS alone or in combination with TCZ should be based on the degrees of inflammation and reserve the CS plus TCZ combination for patients at high and especially very high risk.Peer reviewe

    Influence of obstructive sleep apnea on systemic inflammation in pregnancy

    Background: Obstructive sleep apnea (OSA) is prevalent in pregnancy and it is associated with adverse pregnancy-related outcomes such as gestational diabetes, pre-eclampsia, and low birth weight. Maternal systemic inflammation is proposed to be one of the main intermediate mechanisms. However, the effects of OSA on systemic inflammation are unknown in normal pregnancy. Methods: Women in the 3rd trimester underwent hospital polysomnography to evaluate whether OSA increases systemic inflammation in normal pregnancy and its potential association with adverse fetal outcomes. OSA was defined as an apnea–hypopnea index (AHI) of = 5 h-1. Plasma cytokines levels (TNF-a, IL-1ß, IL-6, IL-8, and IL-10) were determined by multiple immunoassays. Results: We included 11 patients with OSA and 22 women with AHI < 5 h-1, who were homogeneous in age, and body mass index (BMI). Women with OSA had significant higher levels of TNF-a, IL-1ß, IL-8, and IL-10. We found significant correlations between AHI during REM and TNF-a (r = 0.40), IL-1ß (r = 0.36), IL-6 (r = 0.52), IL-8 (r = 0.43), between obstructive apnea index and TNF-a (r = 0.46) and between AHI and IL-1ß (r = 0.43). We also found that CT90% was related to IL-8 (r = 0.37). There were no significant differences in neonatal characteristics; however, we found inverse correlations between TNF-a and IL-8 with birth weight (both r = -0.48), while IL-8 showed a significant inverse relationship with neonatal gestational age (r = -0.48). Conclusions: OSA in our normal pregnancy population was associated with higher systemic inflammation, which was related to obstructive events, especially during REM sleep.     A chromosome-level genome assembly enables the identification of the follicule stimulating hormone receptor as the master sex-determining gene in the flatfish Solea senegalensis

    Sex determination (SD) shows huge variation among fish and a high evolutionary rate, as illustrated by the Pleuronectiformes (flatfishes). This order is characterized by its adaptation to demersal life, compact genomes and diversity of SD mechanisms. Here, we assembled the Solea senegalensis genome, a flatfish of great commercial value, into 82 contigs (614 Mb) combining long- and short-read sequencing, which were next scaffolded using a highly dense genetic map (28,838 markers, 21 linkage groups), representing 98.9% of the assembly. Further, we established the correspondence between the assembly and the 21 chromosomes by using BAC-FISH. Whole genome resequencing of six males and six females enabled the identification of 41 single nucleotide polymorphism variants in the follicle stimulating hormone receptor (fshr) consistent with an XX/XY SD system. The observed sex association was validated in a broader independent sample, providing a novel molecular sexing tool. The fshr gene displayed differential expression between male and female gonads from 86 days post-fertilization, when the gonad is still an undifferentiated primordium, concomitant with the activation of amh and cyp19a1a, testis and ovary marker genes, respectively, in males and females. The Y-linked fshr allele, which included 24 nonsynonymous variants and showed a highly divergent 3D protein structure, was overexpressed in males compared to the X-linked allele at all stages of gonadal differentiation.     Virtualizing university teaching through Open Educational Resources by means of ArcGIS Online (REARGOL)

    La pandemia provocada por el virus SARS-CoV-2 (COVID19) ha demostrado la necesidad de acelerar la digitalización de la docencia universitaria. Las herramientas digitales para la transferencia ciencia-educación, que ya eran esenciales para asegurar la calidad de la docencia presencial, se han transformado en imprescindibles cuando las circunstancias han impuesto la docencia virtual. El proyecto REARGOL ha desarrollado y ensayado en ArcGIS online instrumentos para la virtualización de contenidos en asignaturas de grado y máster, sobre geomorfología, gestión de desastres, patrimonio natural y patrimonio cultural. Ha sido un ensayo piloto, replicable en todas las titulaciones y temáticas susceptibles de generar información geoespacial (prácticamente todos los títulos y áreas de conocimiento). El único límite es la imaginación. El proyecto ha priorizado la participación de estudiantes de grado, máster y doctorado, que han desarrollado 4 tipos de aplicaciones: Mapas Web y Web AppBuilder (visores cartográficos interactivos), encuestas Survey 123 (formularios recogida de datos), Cuadros de Mandos (plataformas online que permiten combinar varias aplicaciones) y Story Maps (presentaciones para mostrar conjuntamente información y aplicaciones ArcGIS online). Las aplicaciones que se ensayaron con éxito durante el curso 2020-2021, en asignaturas de grado y máster, así como en TFMs y TFGs, continúan utilizándose en el curso 2021-2022.The SARS-CoV-2 (COVID19) pandemic has shown the urgent need to improve university teaching. Digital resources for Science-Education transfer, which already were crucial for ensuring the quality of face-to-face teaching, turned to be indispensable when the health crisis forced virtual teaching. The REARGOL project has developed and tested ArcGIS Online tools for the virtualization of Bachelor’s and Master’s courses focused on geomorphology, natural disaster management, and natural and cultural heritage. This has been a preliminary test that could be applied to all undergraduate and postgraduate degrees, that can produce geospatial information in all knowledge areas. Imagination is the only constraint. The project has prioritized the participation of undergraduate and postgraduate students (Master and PhD). They have developed four types of applications: Web Maps and Web AppBuilder (interactive cartographical viewers), Survey 123 (data collection forms), Dashboards (online platforms allowing to combine several applications) and Story Maps (presentations for displaying information and ArcGIS online applications).     Current clinical spectrum of common variable immunodeficiency in Spain: The multicentric nationwide GTEM-SEMI-CVID registry

    Common variable immunodeficiency (CVID) constitutes a heterogenic group of primary immunodeficiency disorders with a wide-ranging clinical spectrum. CVID-associated non-infectious morbidity constitutes a major challenge requiring a full understanding of its pathophysiology and its clinical importance and global variability, especially considering the broad clinical, genetic, and regional heterogeneity of CVID disorders. This work aimed to develop a nationwide, multicenter, retrospective study over a 3-year period describing epidemiological, clinical, laboratory, therapeutic, and prognostic features of 250 CVID patients in Spain. The mean diagnostic delay was around 10 years and most patients initially presented with infectious complications followed by non-infectious immune disorders. However, infectious diseases were not the main cause of morbimortality. Non-infectious lung disease was extraordinarily frequent in our registry affecting approximately 60% of the patients. More than one-third of the patients in our cohort showed lymphadenopathies and splenomegaly in their follow-up, and more than 33% presented immune cytopenias, especially Evans' syndrome. Gastrointestinal disease was observed in more than 40% of the patients. Among biopsied organs in our cohort, benign lymphoproliferation was the principal histopathological alteration. Reaching 15.26%, the global prevalence of cancer in our registry was one of the highest reported to date, with non-Hodgkin B lymphoma being the most frequent. These data emphasize the importance of basic and translational research delving into the pathophysiological pathways involved in immune dysregulation and diffuse lymphocytic infiltration. This would reveal new tailored strategies to reduce immune complications, and the associated healthcare burden, and ensure a better quality of life for CVID patients
