Factors Associated with SARS-CoV-2 Infection among Oral Health Team Professionals
Objective:
To analyze the factors associated with the SARS-CoV-2 infection among oral health professionals.
Material and Methods:
This was a cross-sectional study in the city of São Paulo, São Paulo State, Brazil. Professionals from three different categories were included: dental surgeons (DS), dental assistants (DA), and dental hygienists (DH). A questionnaire was created on a digital platform and sent via institutional email to each subject. The questionnaire contained 32 questions about sociodemographic, work, and behavior factors. The data about SARS-CoV-2 infection was confirmed through RT-PCR exams. Descriptive (absolute and relative frequencies) and inferential analyses (chi-squared or Fisher’s exact test) (p<0.05) were performed.
Results:
There was a SARS-CoV-2 infection prevalence of 3.8% for DS, 30.0% for DH, and 33.3% for DA. SARS-CoV-2 was associated with a lower income (p=0.027), a lower education level (p=0.011), the category of technical professionals (DA and DH) (p=0.025), and using public transportation to commute to work (p=0.009).
Conclusion:
Sociodemographic factors like lower income and education levels and work factors like job category and public transportation were associated with COVID-19 among professionals on the oral health teams

Product Development in Digitalization of Home Products
Under de senaste åren har digitaliseringen av fysiska produkter ökat, och allt fler företag har därmed börjat implementera digitala komponenter i sina produkter. Att implementera mjukvara i en analog produkt innebär nya utmaningar för produktutvecklingsteam som tidigare arbetat med att ta fram analoga produkter. Många företag har i och med digitaliseringen valt att anpassa sina produktutvecklingsmetoder med målet att integrera de digitala och analoga produktutvecklingsprocesserna med varandra. Syftet med studien är att undersöka hur produktutvecklingsprocesserna ser ut idag på produktutvecklande företag som har genomgått en digitalisering. De aspekter som har tagits extra hänsyn till är projekttid, produktutvecklingsmetod, företagets organisering och struktur samt insamling och implementering av användardata i produktutvecklingsprocessen. Företag som utvecklar uppkopplade produkter för hemmet är exempel på företag som just nu genomgår en digitalisering av tidigare analoga produkter, därför har företag med detta spår valts som inriktning vid denna studie. Studien har utförts genom en inledande litteraturstudie följt av kvalitativa intervjuer med fyra responderande företag, som samtliga utvecklar uppkopplade hemprodukter vilka innehåller IoT-teknologi. Studien visar att företagen strävar mot ett agilt arbetssätt, men att det finns svårigheter med att integrera hårdvaru- och mjukvaruutveckling i produktutvecklingsprocesserna. Trots detta upplevs utvecklingstiden i projekt som oförändrad jämfört med innan digitaliseringen. Det framkommer även att tvärfunktionalitet hos utvecklingsteamen är en fördel i samspelet mellan de digitala och analoga delarna av produktutvecklingen. Studien visade slutligen att kunddata som samlas in via digitaliserade produkter används av företag som ett verktyg för att effektivisera produktutvecklingen. In recent years digitalization of physical products has increased, and many companies has therefore started to implement digital components in their products. To add software to an analog product creates new challenges for the product development teams, which up until then mainly have been developing analog products. Many companies have, as a result of the digitalization, chosen to adapt their product development methods to manage the integration between digital and analog development processes. The purpose of this study is to investigate what the product developing process looks like today in companies that have digitalized their products. The aspects that are specifically considered are; project duration, product development method, organizational structure of the company, and implementation of big data in the product development process. Companies that develop products for home use is one example of companies that are going through a digitalization process of their previously analog products, which is why this branch of companies is targeted in this study. The study was conducted through an initial literature study, followed by interviews with four responding companies, who all develop connected home products containing IoT-technology. The study shows that the companies aim for a more agile work procedure, but that there are problems with integrating hardware and software development in product development processes. Nonetheless, the time duration of the projects does not appear to have changed significantly in comparison to pre-digitalization. It is also revealed that cross-functional teams are an advantage within the collaboration between the digital and analog parts in the development process. The study finally shows that big data, collected through digitalized products, is used by the companies as a tool for increasing the effectiveness of the product development
The variation of the burden of hypertension and diabetes in two large districts of the city of São Paulo, Brazil, based on primary health care routinely-collected data.
BackgroundNoncommunicable diseases (NCDs) were responsible for 72.3% of global deaths in 2016, with cardiovascular diseases accounting for almost half of those deaths and low- and middle-income countries carrying the biggest burden. As a result, the prevention and control of NCDs is recognized as urgent, while better surveillance at the country level could result in more effective policies. Hence, the objective of this study was to obtain more detailed information on the distribution of the prevalence of hypertension and diabetes among the population of two large districts of the city of São Paulo in Brazil, and to compare these findings to the results of a citywide health survey.Methods and findingsThis cross-sectional study used primary health care (PHC) routinely-collected data. The study population included 187,110 individuals 20 years of age or older registered in 13 public PHC facilities at two districts of the city of São Paulo in 2015. Data extracted from SIAB, a primary care database, was used to calculate age and sex directly standardized prevalences for diabetes and hypertension for each PHC facility. The prevalence of hypertension among women was significantly higher than the prevalence among men in the entire study population, and in every PHC facility. There was great variation among PHC facilities that was more pronounced among women. The prevalence of diabetes among women was significantly higher than the prevalence among men in the entire study population, and in every PHC facility, but there was little variation among PHC facilities.ConclusionsThis study provided information that could help with policy planning and allocation of resources, and demonstrated the use of PHC routinely-collected data to generate important insights that if replicated could have a substantial impact given the broad coverage of the national public PHC program in Brazil
Efeito de um programa educativo no conhecimento e na qualidade dos registros da pressão arterial
Objective
to evaluate the effect of an educational program on blood pressure recording for nursing professionals in relation to theoretical knowledge and the quality of these records.
Method
quasi-experimental study conducted in a hospital service located in the city of São Paulo. The theoretical knowledge of 101 professionals was measured using a validated questionnaire before and after the educational intervention; the quality of blood pressure records was evaluated using a validated form which was applied to 354 records in the pre-intervention period and 288 in the post-intervention period. The educational program was based on active teaching-learning methodologies and consisted of two strategies: expository/dialogue class and a board game. The Wilcoxon, Mann-Whitney, Fisher and Chi-Square tests were used for comparisons, adopting a level of significance of α=0.05.
Results
the median of the professionals’ scores increased from 19 to 22 points in the post-intervention period (p<0.001). There was an improvement in the quality of the blood pressure recordings regarding the variables: cuff size (p<0.001), arm used in the procedure (p<0.001) and patient position (p<0.001).
Conclusion
the educational program showed positive results in the promotion of knowledge among nursing professionals and in the improvement of the quality of blood pressure recording.Objetivo

estudio cuasi-experimental conducido en un servicio hospitalario ubicado en la ciudad de São Paulo. El conocimiento teórico de 101 profesionales fue medido a través de un cuestionario validado antes y después de la intervención educativa; la calidad de los registros de la presión arterial fue evaluada utilizando formulario validado y aplicado a 354 prontuarios en el período pre-intervención y 288 en el período post-intervención. El programa educativo fue fundamentado en metodologías activas de enseñanza-aprendizaje y constituido por dos estrategias: clase expositiva-dialogada y un juego de tablero. Las pruebas de Wilcoxon, Mann-Whitney, Fisher y Qui-cuadrado se utilizaron para comparaciones, adoptando un nivel de significancia de α=0,05.
Resultados
la mediana de aciertos de los profesionales pasó de 19 a 22 puntos en el período post-intervención (p<0,001). Se observó una mejoría en la calidad de los registros de presión arterial en cuanto a las variables: tamaño del manguito (p<0,001), miembro utilizado en el procedimiento (p<0,001) y posición del paciente (p<0,001).
Conclusión
el programa educativo mostró resultados positivos en la promoción del conocimiento de los profesionales de enfermería y en la mejora de la calidad de los registros de la presión arterial.Objetivo

Resultados

Conclusão

Detecting latent safety threats in an interprofessional training that combines in situ simulation with task training in an emergency department
Abstract Background During in situ simulation, interprofessional care teams practice in an area where clinical care occurs. This study aimed to detect latent safety threats (LST) in a training program, which combined in situ simulation scenarios with just-in-time and just-in-place self-directed task training in an emergency department. We hypothesized this simulation-based training in actual care areas allows the detection of at least one LST per simulation scenario. Methods This prospective observational study (April 2015–March 2016) involved 135 physicians, nurses, and nurse technicians. Training themes selected were arrhythmia, respiratory insufficiency, shock, and cardiopulmonary resuscitation. Simulation weeks occurred every 3 months, with three 10-min scheduled in situ simulation scenarios alternating for each theme daily. The scenarios were followed by co-debriefing by two facilitators (a physician and a nurse). LST were identified by facilitators using a debriefing checklist. Additionally, a room was set up with task-trainers related to each theme. Results The number participants in scenarios was 114 (84% of the population) and in task-training, 101. The number of scenario cancelations was nine, making the final total number to 49 of 58 proposed. Fifty-six LST were observed, with an average of 1.1 per scenario. LST were divided into four categories: equipment (n = 23, 41.1%), teamwork (n = 12, 21.4%), medication (n = 11, 19.6%), and others (n = 10, 17.9%). There was a higher proportion in equipment-related LST (p < 0.01). Conclusions The training allowed a high rate of detecting LST regardless of theme. Equipment-related LST were more frequently found
Consensus for vaginal stenosis prevention in patients submitted to pelvic radiotherapy.
ObjectiveTo propose a consensus for prevention of vaginal stenosis in patients submitted to pelvic radiotherapy.MethodIn this methodological study, Delphi technique was applied for content validation on vaginal stenosis prevention. Data regarding content validation were collected from 32 specialists practicing in the oncology profession. The content validity index of items in the consensus was calculated based on the evaluations by the specialists.ResultsIn the first round, of the 38 items evaluated, 29 items reached a Content Validity Index (CVI-I) greater than 80%, and 9 items had a CVI lower than 80%. Of the items that did not obtain agreement, 2 items were excluded, and 7 were reformulated and included in the second round. In the second round, all 7 items obtained a CVI-I greater than 80%. The final instrument consisted of 29 items validated in the first round, plus 7 items reformulated and consolidated in the second round. The judges agreed that it is the responsibility of the health professionals to consult the patients undergoing radiation therapy in the area of sexuality to patients. The radiation oncologist should be the first professional to address this issue and the nurse oncologist in the follow-up consultation should pass the guidelines to the patients as comprehensively as possible. Patients should be informed about vaginal dilation, regardless of whether they are sexually active or have a partner. They should also be informed of when they can resume sexual activity. The procedure of vaginal dilation should be individualized. The prescribed vaginal dilators should be used with a lubricant for a duration of at least 5-10 minutes, 2-3 times a week, as per the need of each patient (sexual activity and/or clinical follow-up) for an indefinite time. Patients should seek medical help in case they experience pain, discomfort, or bleeding during dilation.ConclusionThe Brazilian version of the consensus for vaginal stenosis prevention in patients submitted to pelvic radiotherapy was validated with 36 items in 7 categories related to Responsibility; Target population; Rationale; Vaginal dilator; Content instructions; Information provision; and Patient support. In Brazil, the educational practices on vaginal dilation for patients submitted to radiotherapy partly revealed similar difficulties as identified in other studies as well as countries with reference to specific guidelines for the start and duration of vaginal dilation. The final consensus developed in this study could strengthen the guidelines for education of patients in Brazil and provide a future scope to establish a single and safe guideline
