    Estudo fitoquímico e propriedades de Andira paniculata Benth (Fabaceae) na inibição de linhagens tumorais de leucemia, próstata e carcinoma de cólon

    The Brazilian Cerrado is home to numerous plant species, including the genus Andira, or angelins, known mainly for its anthelmintic and insecticidal properties. This work aimed to evaluate the toxic, antibacterial and cytotoxic activity of the leaf fractions from Andira paniculata Benth and investigate the chemical constituents present in this material, whose phytochemical profile is unknown in the literature. In toxicity tests against Artemia salina, it was observed that the fractions in methanol, ethyl acetate and dichloromethane are non-toxic, LC50 above 1000 ”g.mL-1. For the antibacterial assay, the fractions showed no inhibitory or bactericidal effect against any of the bacteria tested: Staphylococcus epidermidis, Staphylococcus aureus, Escherichia coli and Pseudomonas aeruginosa, with values of minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) greater than 1000 ”g.mL-1. In the cytotoxicity test, dichloromethane fraction showed growth inhibition against two tumor cells: HL60 (leukemia, IC50 33.06 ”g. mL-1), HCT-116 (colon carcinoma, IC50 70.07 ”g. mL-1) and PC3 (prostate, IC50 80,04 ”g.mL-1), not inhibiting the growth of non-tumor cells (L929, mouse fibroblast, IC50 >100 ”g.mL-1). The phytochemical study revealed the presence of three flavonoids in the ethyl acetate fraction: (-)-epicatechin, engeletin and astilbin, whose structures were confirmed by 1D and 2D NMR and mass spectrometry. This is the first report of the occurrence of these substances in A. paniculata.Introdução: o Cerrado brasileiro abriga inĂșmeras espĂ©cies de plantas, incluindo as do gĂȘnero Andira, ou angelins, conhecidas por suas propriedades anti-helmĂ­nticas e inseticidas. Objetivo: avaliar a atividade tĂłxica, antibacteriana e citotĂłxica das fraçÔes das folhas de Andira paniculata Benth e investigar os constituintes quĂ­micos presentes neste material. Metodologia: a toxicidade das fraçÔes foi avaliada frente ao microcrustĂĄceo Artemia salina. Os ensaios antimicrobiano e citotĂłxico foram realizados in vitro atravĂ©s dos testes de microdiluição em caldo e MTT [brometo de 3-(4,5-dimetil-2-tiazolil)-2,5-difenil-2H-tetrazĂłlio], respectivamente. O isolamento das substĂąncias foi realizado por meio de cromatografia em coluna e as estruturas foram identificadas por RMN (RessonĂąncia MagnĂ©tica Nuclear) 1D e 2D e espectrometria de massas Resultados: as fraçÔes metanĂłlica, acetato de etila e diclorometĂąnica foram atĂłxicasfrente Ă  A. salina. NĂŁo foi observado efeito inibitĂłrio ou bactericida das fraçÔes frente Ă  nenhuma das bactĂ©rias testadas: Staphylococcus epidermidis, Staphylococcus aureus, Escherichia coli e Pseudomonas aeruginosa.     Rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART): Study protocol for a randomized controlled trial

    Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients.     Ticks are arthropods widely distributed in tropical and subtropical regions, which can transmit infectious agents also responsible for zoonoses. Excessive use of conventional acaricides has resulted in the onset of drug resistance by these parasites, thus the need to use alternative methods for their control. This study evaluated the acaricidal activities of Chrysopogon zizanioides (vetiver) essential oils containing different zizanoic and khuzimol (high and low acidity) acid concentrations on Amblyomma cajennense and Rhipicephalus microplus (Acari: Ixodidae). To this aims, toxicity tests of different concentrations of examined essential oils were conducted on adult females and larval stages. Results showed that the essential oils of C. zizanioides with high and low acidity reduced oviposition of females, eggs hatch and larval survival, being more effective than some commercial products widely used to control these ectoparasites. These results indicate that the C. zizanoides essential oils are promising candidates as acaricidal agents and represent also an add value to vetiver oil with high acidity, which is commercially undervalued in the cosmetic industry

    Evaluation of a quality improvement intervention to reduce anastomotic leak following right colectomy (EAGLE): pragmatic, batched stepped-wedge, cluster-randomized trial in 64 countries

    Background Anastomotic leak affects 8 per cent of patients after right colectomy with a 10-fold increased risk of postoperative death. The EAGLE study aimed to develop and test whether an international, standardized quality improvement intervention could reduce anastomotic leaks. Methods The internationally intended protocol, iteratively co-developed by a multistage Delphi process, comprised an online educational module introducing risk stratification, an intraoperative checklist, and harmonized surgical techniques. Clusters (hospital teams) were randomized to one of three arms with varied sequences of intervention/data collection by a derived stepped-wedge batch design (at least 18 hospital teams per batch). Patients were blinded to the study allocation. Low- and middle-income country enrolment was encouraged. The primary outcome (assessed by intention to treat) was anastomotic leak rate, and subgroup analyses by module completion (at least 80 per cent of surgeons, high engagement; less than 50 per cent, low engagement) were preplanned. Results A total 355 hospital teams registered, with 332 from 64 countries (39.2 per cent low and middle income) included in the final analysis. The online modules were completed by half of the surgeons (2143 of 4411). The primary analysis included 3039 of the 3268 patients recruited (206 patients had no anastomosis and 23 were lost to follow-up), with anastomotic leaks arising before and after the intervention in 10.1 and 9.6 per cent respectively (adjusted OR 0.87, 95 per cent c.i. 0.59 to 1.30; P = 0.498). The proportion of surgeons completing the educational modules was an influence: the leak rate decreased from 12.2 per cent (61 of 500) before intervention to 5.1 per cent (24 of 473) after intervention in high-engagement centres (adjusted OR 0.36, 0.20 to 0.64; P &lt; 0.001), but this was not observed in low-engagement hospitals (8.3 per cent (59 of 714) and 13.8 per cent (61 of 443) respectively; adjusted OR 2.09, 1.31 to 3.31). Conclusion Completion of globally available digital training by engaged teams can alter anastomotic leak rates. Registration number: NCT04270721 (http://www.clinicaltrials.gov)
