Use of a Crop Simulation Model to Provide Long-Term Data for Economic Analysis: the Case of Early Maturing Soybeans
A target MOTAD model is used to investigate incorporation of early maturing soybeans by a crop farm in southeastern Kansas. Weather (WGEN) and crop simulation (SOYGRO) models are used to generate a long-term series of soybean yields. Extrapyramidal signs in normal pressure hydrocephalus: an objective assessment
<p>Abstract</p> <p>Background</p> <p>Beyond the classic Normal Pressure Hydrocephalus (NPH) triad of gait disturbance, incontinence, and dementia are characteristic signs of motor dysfunction in NPH patients. We used highly sensitive and objective methods to characterize upper limb extrapyramidal signs in a series of NPH subjects compared with controls. Concentrated evaluation of these profound, yet underappreciated movement disorders of NPH before and after techniques of therapeutic intervention may lead to improved diagnosis, insight into pathophysiology, and targeted treatment.</p> <p>Methods</p> <p>Twenty-two (22) consecutive NPH patients and 17 controls performed an upper limb motor task battery where highly sensitive and objective measures of akinesia/bradykinesia, tone, and tremor were conducted. NPH subjects performed this test battery before and more than 36 h after continuous CSF drainage via a spinal catheter over 72 h and, in those subjects undergoing permanent ventriculo-peritoneal shunt placement, at least 12 weeks later. Control subjects performed the task battery at the same dates as the NPH subjects. Statistical analyses were applied to group populations of NPH and control subjects and repeated measures for within subject performance.</p> <p>Results</p> <p>Twenty (20) NPH subjects remained in the study following CSF drainage as did 14 controls. NPH subjects demonstrated akinesia/bradykinesia (prolonged reaction and movement times) and increased resting tone compared with controls. Furthermore, the NPH group demonstrated increased difficulty with self-initiated tasks compared with stimulus-initiated tasks. Following CSF drainage, some NPH subjects demonstrated reduced movement times with greater improvement in self- versus stimulus-initiated tasks. Group reaction time was unchanged. Resting tremor present in one NPH subject resolved following shunt placement. Tone measures were consistent for all subjects throughout the study.</p> <p>Conclusion</p> <p>Clinical motor signs of NPH subjects extend beyond gait deficits and include extrapyramidal manifestations of bradykinesia, akinesia, rigidity, and propensity to perform more poorly when external cues to move are absent. Objective improvement of some but not all of these features was seen following temporary or permanent CSF diversion.</p
Candidate genes that may be responsible for the unusual resistances exhibited by Bacillus pumilus SAFR-032 spores
The spores of several Bacillus species, including Bacillus pumilus SAFR-032 and B. safensis FO-36b, which were isolated from the spacecraft assembly facility at NASA's Jet Propulsion Laboratory, are unusually resistant to UV radiation and hydrogen peroxide. In order to identify candidate genes that might be associated with these resistances, the whole genome of B. pumilus SAFR-032, and the draft genome of B. safensis FO-36b were compared in detail with the very closely related type strain B. pumilus ATCC7061(T). 170 genes are considered characteristic of SAFR-032, because they are absent from both FO-36b and ATCC7061(T). Forty of these SAFR-032 characteristic genes are entirely unique open reading frames. In addition, four genes are unique to the genomes of the resistant SAFR-032 and FO-36b. Fifty three genes involved in spore coat formation, regulation and germination, DNA repair, and peroxide resistance, are missing from all three genomes. The vast majority of these are cleanly deleted from their usual genomic context without any obvious replacement. Several DNA repair and peroxide resistance genes earlier reported to be unique to SAFR-032 are in fact shared with ATCC7061(T) and no longer considered to be promising candidates for association with the elevated resistances. Instead, several SAFR-032 characteristic genes were identified, which along with one or more of the unique SAFR-032 genes may be responsible for the elevated resistances. These new candidates include five genes associated with DNA repair, namely, BPUM_0608 a helicase, BPUM_0652 an ATP binding protein, BPUM_0653 an endonuclease, BPUM_0656 a DNA cytosine-5- methyltransferase, and BPUM_3674 a DNA helicase. Three of these candidate genes are in immediate proximity of two conserved hypothetical proteins, BPUM_0654 and BPUM_0655 that are also absent from both FO-36b and ATCC7061(T). This cluster of five genes is considered to be an especially promising target for future experimental work
The state of the art in clinical knowledge management: An inventory of tools and techniques
Purpose
To explore the need for, and use of, high-quality, collaborative, clinical knowledge management (CKM) tools and techniques to manage clinical decision support (CDS) content.
Methods
In order to better understand the current state of the art in CKM, we developed a survey of potential CKM tools and techniques. We conducted an exploratory study by querying a convenience sample of respondents about their use of specific practices in CKM.
Results
The following tools and techniques should be priorities in organizations interested in developing successful computer-based provider order entry (CPOE) and CDS implementations: (1) a multidisciplinary team responsible for creating and maintaining the clinical content; (2) an external organizational repository of clinical content with web-based viewer that allows anyone in the organization to review it; (3) an online, collaborative, interactive, Internet-based tool to facilitate content development; (4) an enterprise-wide tool to maintain the controlled clinical terminology concepts. Even organizations that have been successfully using computer-based provider order entry with advanced clinical decision support features for well over 15 years are not using all of the CKM tools or practices that we identified.
Conclusions
If we are to further stimulate progress in the area of clinical decision support, we must continue to develop and refine our understanding and use of advanced CKM capabilities

Provider diversity in the English NHS: a study of recent developments in four local health economies
Objectives: The overall objective of the research was to assess the impact of provider diversity on quality
and innovation in the English NHS. The aims were to map the extent of diverse provider activity, identify
the differences in performance between Third Sector Organisations (TSOs), for-profit private enterprises,
and incumbent organisations within the NHS, and the factors that affect the entry and growth of new
private and TSOs.
Methods: Case studies of four Local Health Economies (LHEs). Data included: semi-structured
interviews with 48 managerial and clinical staff from NHS organizations and providers from the private
and Third Sector; some documentary evidence; a focus group with service users; and routine data from
the Care Quality Commission and Companies House. Data collection was mainly between November
2008 and November 2009.
Results: Involvement of diverse providers in the NHS is limited. Commissioners’ local strategies
influence degrees of diversity. Barriers to the entry for TSOs include lack of economies of scale in the
bidding process. Private providers have greater concern to improve patient pathways and patient
experience, whereas TSOs deliver quality improvements by using a more holistic approach and a greater
degree of community involvement. Entry of new providers drives NHS Trusts to respond by making
improvements. Information sharing diminishes as competition intensifies.
Conclusions: There is scope to increase the participation of diverse providers in the NHS, but care must
be taken not to damage public accountability, overall productivity, equity and NHS providers (especially
acute hospitals, which are likely to remain in the NHS) in the process
X-ray Spectroscopy of the Virgo Cluster out to the Virial Radius
We present results from the analysis of a mosaic of thirteen XMM-Newton
pointings covering the Virgo Cluster from its center northwards out to a radius
r~1.2 Mpc (~4.5 degrees), reaching the virial radius and beyond. This is the
first time that the properties of a modestly sized (M_vir~1.4e14 M_sun, kT~2.3
keV), dynamically young cluster have been studied out to the virial radius. The
density profile of the cluster can be described by a surprisingly shallow
power-law with index 1.21+/-0.12. In the radial range of 0.3r_vir<r<r_vir, the
best fit temperature drops by roughly 60 per cent. Within a radius r<450 kpc,
the entropy profile has an approximate power-law form with index 1.1, as
expected for gravitationally collapsed gas in hydrostatic equilibrium. Beyond
r~450 kpc, however, the temperature and metallicity drop abruptly, and the
entropy profile becomes flatter, staying consistently below the expected value
by a factor of 2-2.5. The most likely explanation for the unusually shallow
density profile and the flattening of entropy at large radius is clumping in
the ICM. Our data provide direct observational evidence that the ICM is
enriched by metals all the way to r_200 to at least Z=0.1 Solar.Comment: Accepted for publication in MNRA
Parallel-plate Flow Chamber and Continuous Flow Circuit to Evaluate Endothelial Progenitor Cells under Laminar Flow Shear Stress
The overall goal of this method is to describe a technique to subject adherent cells to laminar flow conditions and evaluate their response to well quantifiable fluid shear stresses1.
Our flow chamber design and flow circuit (Fig. 1) contains a transparent viewing region that enables testing of cell adhesion and imaging of cell morphology immediately before flow (Fig. 11A, B), at various time points during flow (Fig. 11C), and after flow (Fig. 11D). These experiments are illustrated with human umbilical cord blood-derived endothelial progenitor cells (EPCs) and porcine EPCs2,3.
This method is also applicable to other adherent cell types, e.g. smooth muscle cells (SMCs) or fibroblasts.
The chamber and all parts of the circuit are easily sterilized with steam autoclaving In contrast to other chambers, e.g. microfluidic chambers, large numbers of cells (> 1 million depending on cell size) can be recovered after the flow experiment under sterile conditions for cell culture or other experiments, e.g. DNA or RNA extraction, or immunohistochemistry (Fig. 11E), or scanning electron microscopy5. The shear stress can be adjusted by varying the flow rate of the perfusate, the fluid viscosity, or the channel height and width. The latter can reduce fluid volume or cell needs while ensuring that one-dimensional flow is maintained. It is not necessary to measure chamber height between experiments, since the chamber height does not depend on the use of gaskets, which greatly increases the ease of multiple experiments. Furthermore, the circuit design easily enables the collection of perfusate samples for analysis and/or quantification of metabolites secreted by cells under fluid shear stress exposure, e.g. nitric oxide (Fig. 12)6
