    Performance of the resurfaced hip. Part 1: the influence of the prosthesis size and positioning on the remodelling and fracture of the femoral neck

    Hip resurfacing is an established treatment for osteoarthritis in young active patients. Failure modes include femoral neck fracture and prosthesis loosening, which may be associated with medium-term bone adaptation, including femoral neck narrowing and densification around the prosthesis stem.Finite element modelling was used to indicate the effects of prosthesis sizing and positioning on the bone remodelling and fracture strength under a range of normal and traumatic loads, with the aim of understanding these failure modes better.The simulations predicted increased superior femoral neck stress shielding in young patients with small prostheses, which required shortening of the femoral neck to give an acceptable implant–bone interface. However, with a larger prosthesis, natural femoral head centre recreation in the implanted state was possible; therefore stress shielding was restricted to the prosthesis interior, and its extent was less sensitive to prosthesis orientation. With valgus orientation, the implanted neck strength was, at worst, within 3 per cent of its intact strength.The study suggests that femoral neck narrowing may be linked to a reduction in the horizontal femoral offset, occurring if the prosthesis is excessively undersized. As such, hip resurfacing should aim to reproduce the natural femoral head centre, and, for valgus prosthesis orientation, to avoid femoral neck fracture

Possibly, though, insubstantive aspects of the question, such as the incidence of incorrect answers that are readily dismissed, also affect the difficulty level into which a question is classified.     Does a PEEK femoral TKA implant preserve intact femoral surface strains compared with CoCr? A preliminary laboratory study

    BACKGROUND: Both the material and geometry of a total knee arthroplasty (TKA) component influence the induced periprosthetic bone strain field. Strain, a measure of the local relative deformation in a structure, corresponds to the mechanical stimulus that governs bone remodeling and is therefore a useful in vitro biomechanical measure for assessing the response of bone to new implant designs and materials. A polyetheretherketone (PEEK) femoral implant has the potential to promote bone strains closer to that of natural bone as a result of its low elastic modulus compared with cobalt-chromium (CoCr).QUESTIONS/PURPOSES: In the present study, we used a Digital Image Correlation (DIC) technique to answer the following question: Does a PEEK TKA femoral component induce a more physiologically normal bone strain distribution than a CoCr component? To achieve this, a DIC test protocol was developed for periprosthetic bone strain assessment using an analog model; the protocol aimed to minimize errors in strain assessment through the selection of appropriate analysis parameters.METHODS: Three synthetic bone femurs were used in this experiment. One was implanted with a CoCr femoral component and one with a PEEK femoral component. The third (unimplanted) femur was intact and used as the physiological reference (control) model. All models were subjected to standing loads on the corresponding polyethylene (ultrahigh-molecular-weight polyethylene) tibial component, and speckle image data were acquired for surface strain analysis using DIC in six repeat tests. The strain in 16 regions of interest on the lateral surface of each of the implanted bone models was plotted for comparison with the corresponding strains in the intact case. A Wilcoxon signed-rank test was used to test for difference at the 5% significance level.RESULTS: Surface analog bone strain after CoCr implantation indicated strain shielding (R2 = 0.6178 with slope, ? = 0.4314) and was lower than the intact case (p = 0.014). The strain after implantation with the PEEK implant deviated less from the intact case (R2 = 0.7972 with slope ? = 0.939) with no difference (p = 0.231).CONCLUSIONS: The strain shielding observed with the contemporary CoCr implant, consistent with clinical bone mineral density change data reported by others, may be reduced by using a PEEK implant.CLINICAL RELEVANCE: This bone analog in vitro study suggests that a PEEK femoral component could transfer more physiologically normal bone strains with a potentially reduced stress shielding effect, which may improve long-term bone preservation. Additional studies including paired cadaver tests are necessary to test the hypothesis further

    Spatially Resolved Stellar Populations of Eight GOODS-South AGN at z~1

    We present a pilot study of the stellar populations of 8 AGN hosts at z~1 and compare to (1) lower redshift samples and (2) a sample of nonactive galaxies of similar redshift. We utilize K' images in the GOODS South field obtained with the laser guide star adaptive optics (LGSAO) system at Keck Observatory. We combine this K' data with B, V, i, and z imaging from the ACS on HST to give multi-color photometry at a matched spatial resolution better than 100 mas in all bands. The hosts harbor AGN as inferred from their high X-ray luminosities (L_X > 10^42 ergs/s) or mid-IR colors. We find a correlation between the presence of younger stellar populations and the strength of the AGN, as measured with [OIII] line luminosity or X-ray (2-10 keV) luminosity. This finding is consistent with similar studies at lower redshift. Of the three Type II galaxies, two are disk galaxies and one is of irregular type, while in the Type I sample there only one disk-like source and four sources with smooth, elliptical/spheroidal morphologies. In addition, the mid-IR SEDs of the strong Type II AGN indicate that they are excited to LIRG (Luminous InfraRed Galaxy) status via galactic starbursting, while the strong Type I AGN are excited to LIRG status via hot dust surrounding the central AGN. This supports the notion that the obscured nature of Type II AGN at z~1 is connected with global starbursting and that they may be extincted by kpc-scale dusty features that are byproducts of this starbursting.Comment: 56 pages, 39 figures, accepted to A

    Spectral Templates from Multicolor Redshift Surveys

    Understanding how the physical properties of galaxies (e.g. their spectral type or age) evolve as a function of redshift relies on having an accurate representation of galaxy spectral energy distributions. While it has been known for some time that galaxy spectra can be reconstructed from a handful of orthogonal basis templates, the underlying basis is poorly constrained. The limiting factor has been the lack of large samples of galaxies (covering a wide range in spectral type) with high signal-to-noise spectrophotometric observations. To alleviate this problem we introduce here a new technique for reconstructing galaxy spectral energy distributions directly from samples of galaxies with broadband photometric data and spectroscopic redshifts. Exploiting the statistical approach of the Karhunen-Loeve expansion, our iterative training procedure increasingly improves the eigenbasis, so that it provides better agreement with the photometry. We demonstrate the utility of this approach by applying these improved spectral energy distributions to the estimation of photometric redshifts for the HDF sample of galaxies. We find that in a small number of iterations the dispersion in the photometric redshifts estimator (a comparison between predicted and measured redshifts) can decrease by up to a factor of 2.Comment: 25 pages, 9 figures, LaTeX AASTeX, accepted for publication in A

    The Hubble Deep Field North SCUBA Super-map III - Optical and near-infrared properties of submillimetre galaxies

    We present a new sub-mm Super-map in the HDF-North region (GOODS-North field), containing 40 statistically robust sources at 850 microns. This map contains additional data, and several new sources, including one of the brightest blank-sky extragalactic sub-mm sources ever detected. We have used the ACS HST images and ground-based near-IR observations from GOODS to develop a systematic approach for counterpart identification. 72 per cent of our sources with optical coverage have a unique optical counterpart using our new techniques for counterpart identification, and an additional 18 per cent have more than one possibility that meet our criteria in the ACS images. We have found a much higher ERO rate than other sub-mm surveys, due to the increased depth in the optical images. The median photometric redshift (and quartile range), from optical and near-infrared data, is 1.7 (1.3-2.5) for the radio-detected sub-mm sources, and rises to 2.3 (1.3-2.7) for the radio-undetected sub-sample. We find interesting correlations between the 850 micron flux and both the i magnitude and the photometric redshift, from which there appears to be an absence of high redshift faint counterparts to the lower flux density SCUBA sources. While the quantitative morphologies span a range of values, in general the sub-mm galaxies show larger sizes and a higher degree of asymmetry than other galaxy populations at the same redshifts.Comment: 20 pages, 11 eps figures.
