    Hepatic Lipidosis in Banded Armadillos (Euphractus sexcinctus) Bred in Captivity

    Background: Wild animals bred in captivity may develop steatosis due to unbalanced dietary intake and obesity caused by sedentary behaviour. In some rural areas, six-banded armadillos are raised in captivity inside some metal barrels, with restricted space, and are often overfeed. After getting fattening, armadillos they are slaughtered and used as food. Theunfavorable conditions may somehow contribute to likely underlie the development of many diseases, among them we can including hepatic steatosis. The present report aims to describe the pathological fndings of hepatic lipidosis in banded armadillos (Euphractus sexcinctus), bred in captivity.Case: The animals were three female adult six-banded armadillos (Euphractus sexcinctus), caught in the native forest of the city of Natal, Rio Grande do Norte, Brazil and raised in domestic environment in a small enclosure and fed with human food leftovers. After eight months kept in these conditions armadillos had severe obesity and then were donated to the city zoo, where they were quarantined and presented clinical symptoms of anorexia for two weeks, weight loss and exhaustion. After being found dead during daily inspection carried out by the veterinarian zoo, the six-banded armadillos were sent to the Animal Pathology Laboratory of Federal Rural University of the Semi-Arid (UFERSA) for pathological examination.The animals presented with a history of anorexia were extremely obese and the autopsy detected had extensive deposits of fat in the carcass and liver. Histopathologically, the liver cells hepatocytes showed marked intracytoplasmic vacuolization and with peripheral displacement of the nucleus to the periphery, with and positive staining for lipids using scarlet Sudan III. To our knowledge, this is the frst report of hepatic lipidosis in banded.Discussion: Hepatic lipidosis can be categorized into normal liver and mild, moderate, or severe fatty liver. In severe cases, it is estimated that 80% of the hepatocytes are affected. Therefore, we classifed the hepatic lipidosis of the three studied female six-banded armadillos as severe, since the lipids were deposited diffusely. Based on the severity of the lesions and absence of other fndings, we concluded here that hepatic lipidosis contributed to the cause of the six-banded armadil los death. The weight of the animals in this report may be associated with innapropriate enclosure dimensions and a diet based on human food leftovers. Such diet was reportedly composed of rice, beens, spaghetti, and bread, which are food with high levels of carbohydrate. The armadillos of the Euphractus sexcinctus species usually weigh between 3 and 6 kg and hence, the animals of this report showed twice the normal weight. The occurrence of hepatic lipidosis associated with anorexia triggered by a stressful event such as a change of environment and diet armadillos observed in the present study is in agreement with literature data pointing anorexia in obese animals as a major predisposing factors for the occurrence of hepatic lipidosis by promoting energy restriction, peripheral lipolysis and large intake of fatty acids to the liver.Keywords: Euphractus sexcinctus, armadillos, anatomopathology, liver, lipidosis

    Avaliação das salas de imunização de uma capital do nordeste do Brasil

    The evaluation by the National Program of the different quality rooms of the determined protocols (PNI) makes an assessment of the various factors that can interfere with the local asymbiological vaccine. With this, it is possible to obtain satisfactory results and to reorder the execution and offer of quality services. The present study aimed to evaluate the immunization rooms in the city of São Luís, Maranhão. This is an evaluative study carried out in 65 Municipal Vaccine rooms, carried out in São Paulo, in a Supervision Room (PAISSV). Most rooms were considered good (58.6%), but none were ideal. The most impacting were found in the room temperature that was not between 18ºC to 20ºC (67.69%), data and opening time of the bottle were not recorded (75.38%), control cards were not organized by return data (70.77%), the search for counterfeiters was not carried out (60.00%), there was no distribution box so as not to turn off the circuit breaker in the electrical search room (78.46%) and there was no active a preventive maintenance and/or correction program for the refrigerator (64.62%). Public vaccination rooms in São Luís were performed as good. Thus, it is necessary to supervise, monitor and evaluate professionals, with permanent education strategies.A avaliação das salas de vacina se faz necessária pelos diversos fatores que podem interferir na qualidade dos imunobiológicos presentes no local e assim comprometer sua eficácia, por meio da identificação de problemas que comprometem a concretização dos protocolos determinados pelo Programa Nacional de Imunizações (PNI). Com isso, é possível obter resultados satisfatórios e reordenar a execução e a oferta de serviços de qualidade. O presente estudo objetivou avaliar as salas de imunização da cidade de São Luís, Maranhão. Trata-se de estudo avaliativo realizado no município de São Luís – MA, em 65 salas que administram vacinas através do Instrumento de Supervisão em Sala de Vacinação (PAISSV). Nenhuma sala foi considerada como ideal, mas 58,46% obtiveram o escore bom. Contudo, 67,69% das salas apresentaram temperatura diferente de 18ºC a 20ºC, 75,38% não realizava o registro de data e hora de abertura do frasco, 70,77% não organizava os cartões de controle por data de retorno, em 60,00% não era realizada a busca ativa de faltosos, o informativo não desligar o disjuntor na caixa de distribuição elétrica não estava presente em 78,46% e o programa de manutenção preventiva e/ou corretiva para o refrigerador era inexistente em 64,62%. As salas de vacinas dos serviços de saúde pública de São Luís foram consideradas como boas, porém não exime a necessidade de fiscalização e avaliação, juntamente com estratégias de qualificação dos profissionais

    The weaning period of piglets is characterized by physiological alterations, such as decreased weight gain, increased reactive oxygen species (ROS) and increased serum cortisol levels with possible effects on the immune response. The effect of parenteral administration of vitamins A, D and E on production performance, oxidative metabolism, and the function of polymorphonuclear leukocytes (PMNLs) was assessed in piglets during the weaning period. The sample was comprised of 20 male piglets that were given an injectable ADE vitamin combination (135,000 IU vitamin A, 40,000 IU vitamin D and 40mg vitamin E/animal) at 20 and 40 days of age. Weight gain, concentration of reduced glutathione (GSH), malondialdehyde (MDA), superoxide dismutase (SOD) and the microbicidal and phagocytic activity of PMNLs were assessed. No difference was observed in the average piglet weight during the study; however, a greater percentage of weight gain was observed after weaning in the treated group. The concentrations of GSH and SOD did not differ between groups, although lipid peroxidation was greater in the control group at 60 days of age. The investigated variables of oxidative metabolism were correlated as follows: -0.41 for GSH and MDA, -0.54 for GSH and SOD and 0.34 for MDA and SOD. The intensity of intracellular ROS production, the percentage of ROS-producing PMNLs and the intensity of phagocytosis by PMNLs did not differ between treatment groups. Administration of the injectable ADE combination improved the percentage of weight gain between 20 and 40 days of age, decreased oxidative stress at 60 days of age and did not influence the function of PMNLs in piglets.O período de desmame nos leitões é caracterizado por alterações fisiológicas como menor ganho de peso, aumento na produção de espécies reativas de oxigênio (EROs) e aumento na concentração plasmática de cortisol com possíveis implicações para a resposta imune. Foi avaliado o efeito da administração parenteral das vitaminas A, D e E sobre o desempenho produtivo, o metabolismo oxidativo e a função de leucócitos polimorfonucleares (PMNLs) em suínos durante esta fase de crescimento. Foram utilizados 20 leitões, machos, com 20 dias de idade que receberam ADE injetável (135.000 UI vitamina A, 40.000 UI vitamina D e 40mg vitamina E/animal), aos 20 e 40 dias de idade. Foi determinado o ganho de peso e as concentrações de glutationa reduzida (GSH), malondialdeído (MDA) e superóxido dismutase (SOD) e a capacidade microbicida e fagocítica dos PMNLs. Não houve diferença entre o peso vivo médio durante o experimento, porém maior ganho de peso percentual foi observado 20 dias após o desmame para o grupo tratado. As concentrações de GSH e SOD não diferiram entre os grupos, porém a lipoperoxidação foi maior no grupo controle aos 60 dias de idade. As correlações entre as variáveis do metabolismo oxidativo foram -0,41 para GSH e o MDA, -0,54 para GSH e SOD e 0,34 para MDA e SOD. A intensidade da produção intracelular de EROs, a porcentagem de PMNLs que produziram EROs e a intensidade de fagocitose dos PMNLs não diferiram entre os tratamentos. A administração de ADE injetável melhorou o ganho de peso percentual no período de 20 a 40 dias de idade, diminuiu o estresse oxidativo aos 60 dias de idade e não influenciou função dos PMNLs dos leitões

    Effectiveness and safety of first-generation protease inhibitors in real-world patients with hepatitis C virus genotype 1 infection in Brazil: a multicenter study

    OBJECTIVE: To evaluate the effectiveness and safety of first-generation protease inhibitors for the treatment of genotype 1 hepatitis C virus-infected patients at Brazilian reference centers. METHODS: This multicenter cross-sectional study included hepatitis C virus genotype 1 monoinfected patients treated with Peg-interferon, ribavirin, and either boceprevir (n=158) or telaprevir (n=557) between July 2013 and April 2014 at 15 reference centers in Brazil. Demographic, clinical, virological, and adverse events data were collected during treatment and follow-up. RESULTS: Of the 715 patients, 59% had cirrhosis and 67.1% were treatment-experienced. Based on intention-to-treat analysis, the overall sustained viral response was 56.6%, with similar effectiveness in both groups (51.9% for boceprevir and 58% for telaprevir, p=0.190). Serious adverse events occurred in 44.2% of patients, and six deaths (0.8%) were recorded. Cirrhotic patients had lower sustained viral response rates than non-cirrhotic patients (46.9% vs. 70.6%, p 465 years, diagnosis of cirrhosis, and abnormal hemoglobin levels/platelet counts prior to treatment were associated with serious adverse events.     Discrepancies in breast cancer’s oncological outcomes between public and private institutions in the southeast region of Brazil: a retrospective cohort study

    BackgroundBrazil is a middle-income country with inequalities in its healthcare system. The disparities between public and private services affect the diagnosis and treatment of patients with breast cancer. The aim of this study is to assess whether disease-free survival (DFS) and overall survival (OS) are different in public and private specialized centers.Patient and methodsA retrospective cohort study with 1,545 breast cancer patients diagnosed from 2003 to 2011 at Barretos Cancer Hospital—BCH (public group, N = 1,408) and InORP Oncoclinicas (private group, N = 137) was conducted. A 1:1 propensity score matching (PSM) analysis was used to adjust the differences between the groups’ characteristics (n = 137 in each group).ResultsThe median age at diagnosis was 54.4 years. Estimated DFS rates at 1, 5, and 10 years were 96.0%, 71.8%, and 59.6%, respectively, at BCH and 97.8%, 86.9%, and 78%, respectively, at InORP (HR: 2.09; 95% confidence interval [CI], 1.41–3.10; p < 0.0001). Estimated OS rates at 1, 5, and 10 years were 98.1%, 78.5%, and 65.4%, respectively, at BCH and 99.3%, 94.5%, and 91.9%, respectively, at InORP (HR: 3.84; 95% CI, 2.16–6.82; p < 0.0001). After adjustment by PSM, DFS and OS results in 1, 3, and 5 years remained worse in the public service compared to the private service.ConclusionPatients treated in a public center have worse DFS and OS after a follow-up period of more than 5 years.
