Madonna of Loreto, Mola di Bari
The sanctuary of the Virgin of Loreto in the Marche region was one of the dominant pilgrimage centers of Europe during the early modern period, along with Saint Peter’s in Rome and Santiago de Compostela in Galicia. Cult following developed throughout the region during the fourteenth and fifteenth centuries, but once placed under papal tutelage, starting in the 1470s, it spread to other parts of Italy and many areas of Europe and America as well. The cult revolved around two objects: a sculpture of the Virgin and Christ Child believed to have been made by Saint Luke, and the Holy House of Nazareth, which according to legend was transported by angels from Holy Land to Italy in 1291-94. Captivated by the idea that a visit to Loreto meant actually inhabiting the house where the Annunciation took place and the Holy Family lived, many pilgrims returned to their hometowns to build a replica of the Holy House and/or establish a chapel. This painting is an example of how the cult spread to Puglia, where there are a number of other chapels and churches harboring Loretan images

Funerary monument (sepulchre) of Giacomo Bongiovanni
The tomb of Giacomo Bongiovanni is one of the few early sixteenth-century monuments preserved in the Basilica of Saint Nicholas in Bari. Most of the church walls are bare, owing to various conservation campaigns, most dramatically those between 1925 and 1934, when a desire to recover medieval “authenticity” stripped the church of centuries of history and art. Earlier sources reveal that in the sixteenth century, this church, like others in the city such as the cathedral, had side chapels, many with tombs of prominent local inhabitants. Sculptures and paintings ornamented them, a few of which are preserved in museums, for example the Virgin and Child with Saint Enrico of Upsala and Saint Anthony of Padua by Paris Bordone, originally in the cathedral chapel that belonged to the Tanzi family. Decontextualized and removed from their original architectural frame, such works are no longer entirely understandable, especially because they have lost the complementary artistic elements that collectively articulated funerary monuments. By contrast, the tomb of Bongiovanni offers a window onto the way in which the arts came together to commemorate the deceased at the height of the Renaissance in Puglia. La experiencia presenta el proceso de diseño de una propuesta de “cuñas motrices bilingües” generadas en un contexto extraescolar por miembros del grupo Actitudes para su posterior aplicación en el contexto escolar. Éstas relacionan las cuñas motrices que se llevan a la práctica en el contexto escolar y la secuenciación de competencias de la competencia lingüística en lengua extranjera del proyecto INCOBA. Se han utilizado ciclos de investigación-acción para generar experiencias que puedan transferirse al contexto escolar, por lo que el grupo de trabajo cuenta con profesorado en activo para el diseño de las mismas. Los resultados demuestran gran nivel de participación del alumnado y un nivel de satisfacción alto de los mismos y de sus familias

wedges" generated in an out-of-school context by members of the Actitudes group for
later application in the school context. These relate the driving wedges that are put into
practice in the school context and the sequencing of competences of the foreign
language proficiency of the INCOBA project. For its design, research-action cycles
have been used with the intention of generating experiences that can be transferred to
the school context, so that the working group has active teaching staff for their design.
The results demonstrate the high level of student participation and a high level of
satisfaction of the students and their families.[PT] A experiência mostra o processo de concepção de uma proposta de "cunhas motrizes
bilíngües" gerados em um contexto formal, por membros das atitudes do grupo para
posterior aplicação no contexto escolar. Estes se relacionam com as cunhas motrizes
bilíngües que estão sendo implementadas no contexto escolar e sequenciamento
habilidades de proficiência em língua estrangeira do projeto INCOBA. Têm sido usados
pesquisa-ação com a intenção de criar experiências que podem ser transferidos para o
contexto escolar, de modo que o grupo de trabalho tem professores ativos para projetarlos.
Os resultados demonstram o alto nível de participação dos alunos e um alto nível de
satisfação de si e suas famílias.S

Incidence, Clinical Characteristics and Management of Inflammatory Bowel Disease in Spain : Large-Scale Epidemiological Study
(1) Aims: To assess the incidence of inflammatory bowel disease (IBD) in Spain, to describe the main epidemiological and clinical characteristics at diagnosis and the evolution of the disease, and to explore the use of drug treatments. (2) Methods: Prospective, population-based nationwide registry. Adult patients diagnosed with IBD-Crohn's disease (CD), ulcerative colitis (UC) or IBD unclassified (IBD-U)-during 2017 in Spain were included and were followed-up for 1 year. (3) Results: We identified 3611 incident cases of IBD diagnosed during 2017 in 108 hospitals covering over 22 million inhabitants. The overall incidence (cases/100,000 person-years) was 16 for IBD, 7.5 for CD, 8 for UC, and 0.5 for IBD-U; 53% of patients were male and median age was 43 years (interquartile range = 31-56 years). During a median 12-month follow-up, 34% of patients were treated with systemic steroids, 25% with immunomodulators, 15% with biologics and 5.6% underwent surgery. The percentage of patients under these treatments was significantly higher in CD than UC and IBD-U. Use of systemic steroids and biologics was significantly higher in hospitals with high resources. In total, 28% of patients were hospitalized (35% CD and 22% UC patients, p < 0.01). (4) Conclusion: The incidence of IBD in Spain is rather high and similar to that reported in Northern Europe. IBD patients require substantial therapeutic resources, which are greater in CD and in hospitals with high resources, and much higher than previously reported. One third of patients are hospitalized in the first year after diagnosis and a relevant proportion undergo surgery
Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2
The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people. Efficacy of clozapine versus standard treatment in adult individuals with intellectual disability and treatment-resistant psychosis (CLOZAID): study protocol of a multicenter randomized clinical trial
BackgroundIntellectual disability (ID) affects approximately 1% of the worldwide population and individuals with ID have a higher comorbidity with mental illness, and specifically psychotic disorders. Unfortunately, among individuals with ID, limited research has been conducted since ID individuals are usually excluded from mental illness epidemiological studies and clinical trials. Here we perform a clinical trial to investigate the effectiveness of clozapine in the treatment of resistant psychosis in individuals with ID. The article highlights the complexity of diagnosing and treating psychopathological alterations associated with ID and advocates for more rigorous research in this field.MethodsA Phase IIB, open-label, randomized, multicenter clinical trial (NCT04529226) is currently ongoing to assess the efficacy of oral clozapine in individuals diagnosed with ID and suffering from treatment-resistant psychosis. We aim to recruit one-hundred and fourteen individuals (N=114) with ID and resistant psychosis, who will be randomized to TAU (treatment as usual) and treatment-with-clozapine conditions. As secondary outcomes, changes in other clinical scales (PANSS and SANS) and the improvement in functionality, assessed through changes in the Euro-QoL-5D-5L were assessed. The main outcome variables will be analyzed using generalized linear mixed models (GLMM), assessing the effects of status variable (TAU vs. Clozapine), time, and the interaction between them.DiscussionThe treatment of resistant psychosis among ID individuals must be directed by empirically supported research. CLOZAID clinical trial may provide relevant information about clinical guidelines to optimally treat adults with ID and treatment-resistant psychosis and the benefits and risks of an early use of clozapine in this underrepresented population in clinical trials.Trial registrationClinicaltrials.gov: NCT04529226.
