Congenital Diarrhea and Cholestatic Liver Disease: Phenotypic Spectrum Associated with MYO5B Mutations
Myosin Vb (MYO5B) is a motor protein that facilitates protein trafficking and recycling in polarized cells by RAB11- and RAB8-dependent mechanisms. Biallelic MYO5B mutations are identified in the majority of patients with microvillus inclusion disease (MVID). MVID is an intractable diarrhea of infantile onset with characteristic histopathologic findings that requires life-long parenteral nutrition or intestinal transplantation. A large number of such patients eventually develop cholestatic liver disease. Bi-allelic MYO5B mutations are also identified in a subset of patients with predominant early-onset cholestatic liver disease. We present here the compilation of 114 patients with disease-causing MYO5B genotypes, including 44 novel patients as well as 35 novel MYO5B mutations, and an analysis of MYO5B mutations with regard to functional consequences. Our data support the concept that (1) a complete lack of MYO5B protein or early MYO5B truncation causes predominant intestinal disease (MYO5B-MVID), (2) the expression of full-length mutant MYO5B proteins with residual function causes predominant cholestatic liver disease (MYO5B-PFIC), and (3) the expression of mutant MYO5B proteins without residual function causes both intestinal and hepatic disease (MYO5B-MIXED). ÜÇ-ALTI YAŞ ARASI FARKLI ASTIM FENOTİPLERİNDEKİ ÇOCUKLARDA BRONŞİYAL HİPERREAKTİVİTE ÖZELLİKLERİ
Okul öncesi yaş grubunda; aynı klinikle karşımıza çıkan, fakat tedavileri
farklı olan vizing fenotipleri bulunmaktadır. Klinik pratikte, bu çocuklar arasında
ayırıcı tanı yapmak kolay olmamaktadır. Bu fenotiplerin altta yatan patofizyolojik
mekanizmalarının farklı olduğu düşünülmektedir. Astımda direkt ve indirekt yolla
bronş hiperreaktivitesini belirleyen, bronş provokasyon testleri patofizyoloji
hakkında ipuçları verebilmektedir. Bu nedenle; bu çalışma, astım bölümünde
düzenli izlenen üç-altı yaş grubundaki astım fenotipleri arasında, hava yolu
duyarlılığı farklarını araştırmak amacıyla yapılmıştır.
Metakolin ve adenozin 5’-monofosfat ile yapılan bronş provokasyon
testlerine bronkokonstriksiyon yanıtı veren çocukların sayısı; persistan vizing
grubunda, epizodik viral vizing grubuna göre daha fazla bulunmuştur. Transkutan
oksijen basıncında %15’lik azalmaya yol açan (PC15PtcO2) metakolin dozunun
persistan vizingli çocuklarda, epizodik viral vizingli çocuklara göre daha düşük
olması; hava yolu duyarlılığının persistan vizingli çocuklarda daha şiddetli
olduğunu ortaya çıkartmıştır. Ancak PC15PtcO2 adenozin 5’-monofosfat (AMP)
dozu açısından iki grup arasında fark saptanmamıştır. Her iki grup için de,
PC15PtcO2 metakolin ve PC15PtcO2 AMP arasında ilişki bulunmamıştır.
“Alıcı işlem karakteristiği eğrisi” (ROC eğrisi) kullanılarak, PC15PtcO2
metakolin için %87,2 duyarlılık, %55 seçicilik ile epizodik ve persistan vizing
ayrımı için en uygun sınır değer 0,24 mg/ml bulunmuştur. Pozitif olabilirlik oranı
(positive LR) 1,94; negatif olabilirlik oranı (negative LR) ise 0,23 bulunmuştur.
Adenozin 5’-monofosfat için hesaplanan eğri altında kalan alan anlamlı
bulunmamıştır.
Persistan vizingli çocukların periferik kan ortalama eozinofil sayıları,
epizodik viral vizingli çocuklara göre yüksek saptanmıştır. Ancak iki grup
arasında ortalama serum total IgE düzeyleri açısından fark saptanmamıştır. Ayrıca
iki grup için de; eozinofil sayısı ile AMP ve metakolin PC15PtcO2; hava yolu
cevabı arasında ilişki saptanmamıştır.
Çalışmamızın sonuçlarına göre; direkt ve indirekt bronş provokasyon
testleri, okul öncesi dönemde vizing fenotipleri arasından gerçek astımlı olanları
ayırmada yeterli değildir
Intestinal Failure and Aberrant Lipid Metabolism in Patients With DGAT1 Deficiency
BACKGROUND & AIMS: Congenital diarrheal disorders are rare inherited intestinal disorders characterized by intractable, sometimes life-threatening, diarrhea and nutrient malabsorption; some have been associated with mutations in diacylglycerol-acyltransferase 1 (DGAT1), which catalyzes formation of triacylglycerol from diacylglycerol and acyl-CoA. We investigated the mechanisms by which DGAT1 deficiency contributes to intestinal failure using patient-derived organoids. METHODS: We collected blood samples from 10 patients, from 6 unrelated pedigrees, who presented with early-onset severe diarrhea and/or vomiting, hypoalbuminemia, and/or (fatal) protein-losing enteropathy with intestinal failure; we performed next-generation sequencing analysis of DNA from 8 patients. Organoids were generated from duodenal biopsies from 3 patients and 3 healthy individuals (controls). Caco-2 cells and patient-derived dermal fibroblasts were transfected or transduced with vectors that express full-length or mutant forms of DGAT1 or full-length DGAT2. We performed CRISPR/Cas9-guided disruption of DGAT1 in control intestinal organoids. Cells and organoids were analyzed by immunoblot, immunofluorescence, flow cytometry, chromatography, quantitative real-time polymerase chain reaction, and for the activity of caspases 3 and 7. RESULTS: In the 10 patients, we identified 5 bi-allelic loss-of-function mutations in DGAT1. In patient-derived fibroblasts and organoids, the mutations reduced expression of DGAT1 protein and altered triacylglycerol metabolism, resulting in decreased lipid droplet formation after oleic acid addition. Expression of full-length DGAT2 in patient-derived fibroblasts restored formation of lipid droplets. Organoids derived from patients with DGAT1 mutations were more susceptible to lipid-induced cell death than control organoids. CONCLUSIONS: We identified a large cohort of patients with congenital diarrheal disorders with mutations in DGAT1 that reduced expression of its product; dermal fibroblasts and intestinal organoids derived from these patients had altered lipid metabolism and were susceptible to lipid-induced cell death. Expression of full-length wildtype DGAT1 or DGAT2 restored normal lipid metabolism in these cells. These findings indicate the importance of DGAT1 in fat metabolism and lipotoxicity in the intestinal epithelium. A fat-free diet might serve as the first line of therapy for patients with reduced DGAT1 expression. It is important to identify genetic variants associated with congenital diarrheal disorders for proper diagnosis and selection of treatment strategies
