    Understanding unequal ageing: towards a synthesis of intersectionality and life course analyses

    Intersectionality has received an increasing amount of attention in health inequalities research in recent years. It suggests that treating social characteristics separately—mainly age, gender, ethnicity, and socio-economic position—does not match the reality that people simultaneously embody multiple characteristics and are therefore potentially subject to multiple forms of discrimination. Yet the intersectionality literature has paid very little attention to the nature of ageing or the life course, and gerontology has rarely incorporated insights from intersectionality. In this paper, we aim to illustrate how intersectionality might be synthesised with a life course perspective to deliver novel insights into unequal ageing, especially with respect to health. First we provide an overview of how intersectionality can be used in research on inequality, focusing on intersectional subgroups, discrimination, categorisation, and individual heterogeneity. We cover two key approaches—the use of interaction terms in conventional models and multilevel models which are particularly focussed on granular subgroup differences. In advancing a conceptual dialogue with the life course perspective, we discuss the concepts of roles, life stages, transitions, age/cohort, cumulative disadvantage/advantage, and trajectories. We conclude that the synergies between intersectionality and the life course hold exciting opportunities to bring new insights to unequal ageing and its attendant health inequalities

    Fine particulate matter air pollution and cognitive function among U.S. older adults

    Objectives. There is growing interest in understanding how exposures in the residential environment relate to cognitive function in older adults. The goal of this study is to determine if neighborhood-level exposure to fine particulate matter air pollution (PM 2.5 ) is associated with cognitive function in a diverse, national sample of older U.S. adults. Methods. We use cross-sectional data on non-Hispanic black and white men and women aged 55 and older from the 2001/2002 Americans' Changing Lives Study (N = 780). EPA air monitoring data were linked to respondents using census tract identifiers. Cognitive function was assessed with tests of working memory and orientation. Negative binomial regression models were used to examine the association between PM 2.5 and the number of errors on the cognitive assessment. Results. Older adults living in areas with high concentrations of PM 2.5 had an error rate 1.5 times greater than those exposed to lower concentrations, net of individual and neighborhood-level demographic and socioeconomic characteristics. Discussion. This study adds to a growing body of research demonstrating the importance of air pollution to cognitive function in older adults. Improvements to air quality may be an important mechanism for reducing age-related cognitive decline

    Increasing attention is being paid to the importance of built environment characteristics for participation, especially among people with various levels of impairment or activity limitations. The purpose of this research was to examine the role of specific characteristics in the urban environment as they interact with underlying impairments and activity limitations to either promote or hinder participation in life situations. Using data from the Chicago Community Adult Health Study (2001-2003) in the United States, we used logistic regression to examine the effect of built environment characteristics on three indicators of participation (interpersonal interaction, obtaining preventive health care, and voting) among adults age 45+ (N = 1225). We examined effects across two levels of spatial scale: the census tract and block group. One in five adults reported difficulty walking 2-3 blocks unaided, but their odds of engaging in regular interpersonal interaction was 45% higher when they lived in areas with higher residential security. For the thirty-six percent of adults who reported visual impairment, and the odds of obtaining preventive health care were over 20% lower when living in an area with heavy traffic. Residing in an area with a high proportion of streets in poor condition was associated with 60% lower odds of voting among those with underlying difficulty with mobility activities. Results varied across levels of spatial scale.     Mobility Disability and the Urban Built Environment

    Research on the effects of the built environment in the pathway from impairment to disability has been largely absent. Using data from the Chicago Community Adult Health Study (2001–2003), the authors examined the effect of built environment characteristics on mobility disability among adults aged 45 or more years (n = 1,195) according to their level of lower extremity physical impairment. Built environment characteristics were assessed by using systematic social observation to independently rate street and sidewalk quality in the block surrounding each respondent's residence in the city of Chicago (Illinois). Using multinomial logistic regression, the authors found that street conditions had no effect on outdoor mobility among adults with only mild or no physical impairment. However, among adults with more severe impairment in neuromuscular and movement-related functions, the difference in the odd ratios for reporting severe mobility disability was over four times greater when at least one street was in fair or poor condition (characterized by cracks, potholes, or broken curbs). When all streets were in good condition, the odds of reporting mobility disability were attenuated in those with lower extremity impairment. If street quality could be improved, even somewhat, for those adults at greatest risk for disability in outdoor mobility, the disablement process could be slowed or even reversed

    Measurement of the Local Food Environment: A Comparison of Existing Data Sources

    Studying the relation between the residential environment and health requires valid, reliable, and cost-effective methods to collect data on residential environments. This 2002 study compared the level of agreement between measures of the presence of neighborhood businesses drawn from 2 common sources of data used for research on the built environment and health: listings of businesses from commercial databases and direct observations of city blocks by raters. Kappa statistics were calculated for 6 types of businesses—drugstores, liquor stores, bars, convenience stores, restaurants, and grocers—located on 1,663 city blocks in Chicago, Illinois. Logistic regressions estimated whether disagreement between measurement methods was systematically correlated with the socioeconomic and demographic characteristics of neighborhoods. Levels of agreement between the 2 sources were relatively high, with significant (P < 0.001) kappa statistics for each business type ranging from 0.32 to 0.70. Most business types were more likely to be reported by direct observations than in the commercial database listings. Disagreement between the 2 sources was not significantly correlated with the socioeconomic and demographic characteristics of neighborhoods. Results suggest that researchers should have reasonable confidence using whichever method (or combination of methods) is most cost-effective and theoretically appropriate for their research design
