    Altered Central Nutrient Sensing in Male Mice Lacking Insulin Receptors in Glut4-Expressing Neurons

    Insulin signaling in the central nervous system influences satiety, counterregulation, and peripheral insulin sensitivity. Neurons expressing the Glut4 glucose transporter influence peripheral insulin sensitivity. Here, we analyzed the effects of insulin receptor (IR) signaling in hypothalamic Glut4 neurons on glucose sensing as well as leptin and amino acid signaling. By measuring electrophysiological responses to low glucose conditions, we found that the majority of Glut4 neurons in the ventromedial hypothalamus (VMH) were glucose excitatory neurons. GLUT4-Cre-driven insulin receptor knockout mice with a combined ablation of IR in Glut4-expressing tissues showed increased counterregulatory response to either 2-deoxyglucose-induced neuroglycopenia or systemic insulin-induced hypoglycemia. The latter response was recapitulated in mice with decreased VMH IR expression, suggesting that the effects on the counterregulatory response are likely mediated through the deletion of IRs on Glut4 neurons in the VMH. Using immunohistochemistry in fluorescently labeled hypothalamic Glut4 neurons, we showed that IR signaling promoted hypothalamic cellular signaling responses to the rise of insulin, leptin, and amino acids associated with feeding. We concluded that hypothalamic Glut4 neurons modulated the glucagon counterregulatory response and that IR signaling in Glut4 neurons was required to integrate hormonal and nutritional cues for the regulation of glucose metabolism

    Mortality due to external causes in youth in state of Bahia / Mortalidade por causas externas em jovens no estado da Bahia

    Objetivo: descrever a mortalidade por causas externas no Estado da Bahia, no período de 2000 a 2011, segundo dados sociodemográficos das vítimas, tipos de causas e taxas de mortalidade. Métodos: trata-se de um estudo descritivo de série temporal (2000 a 2011), com indivíduos nas faixas etárias de 15 a 19 anos e 20 a 29 anos e residentes no estado da Bahia. Os dados foram obtidos a partir do Sistema de Informações sobre Mortalidade (SIM). Resultados: foram registrados 44.536 óbitos por causas externas, especificamente devido a agressões (53,6%) sendo em sua maioria indivíduos do sexo masculino (90,7%), onde 2010 foi o ano com maior registro (CM 134,7). Conclusão: destaca-se a importância de medidas preventivas e de promoção da saúde, por meio de políticas públicas e de educação em saúde, para redução da morbimortalidade por estes agravos.Descritores: Causas Externas; Mortalidade; Jovens

    A plumieridine-rich fraction from Allamanda polyantha inhibits chitinolytic activity and exhibits antifungal properties against Cryptococcus neoformans

    Cryptococcosis is a fungal infection caused mainly by the pathogenic yeasts Cryptococcus neoformans and Cryptococcus gattii. The infection initiates with the inhalation of propagules that are then deposited in the lungs. If not properly treated, cryptococci cells can disseminate and reach the central nervous system. The current recommended treatment for cryptococcosis employs a three-stage regimen, with the administration of amphotericin B, flucytosine and fluconazole. Although effective, these drugs are often unavailable worldwide, can lead to resistance development, and may display toxic effects on the patients. Thus, new drugs for cryptococcosis treatment are needed. Recently, an iridoid named plumieridine was found in Allamanda polyantha seed extract; it exhibited antifungal activity against C. neoformans with a MIC of 250 µg/mL. To address the mode of action of plumieridine, several in silico and in vitro experiments were performed. Through a ligand-based a virtual screening approach, chitinases were identified as potential targets. Confirmatory in vitro assays showed that C. neoformans cell-free supernatant incubated with plumieridine displayed reduced chitinase activity, while chitinolytic activity was not inhibited in the insoluble cell fraction. Additionally, confocal microscopy revealed changes in the distribution of chitooligomers in the cryptococcal cell wall, from a polarized to a diffuse cell pattern state. Remarkably, further assays have shown that plumieridine can also inhibit the chitinolytic activity from the supernatant and cell-free extracts of bacteria, insect and mouse-derived macrophage cells (J774.A1). Together, our results suggest that plumieridine can be a broad-spectrum chitinase inhibitor

    Brazilian recommendations on the safety and effectiveness of the yellow fever vaccination in patients with chronic immune-mediated inflammatory diseases

    Background: In Brazil, we are facing an alarming epidemic scenario of Yellow fever (YF), which is reaching the most populous areas of the country in unvaccinated people. Vaccination is the only effective tool to prevent YF. In special situations, such as patients with chronic immune-mediated inflammatory diseases (CIMID), undergoing immunosuppressive therapy, as a higher risk of severe adverse events may occur, assessment of the risk-benefit ratio of the yellow fever vaccine (YFV) should be performed on an individual level. Main body of the abstract: Faced with the scarcity of specific orientation on YFV for this special group of patients, the Brazilian Rheumatology Society (BRS) endorsed a project aiming the development of individualized YFV recommendations for patients with CIMID, guided by questions addressed by both medical professionals and patients, followed an internationally validated methodology (GIN-McMaster Guideline Development). Firstly, a systematic review was carried out and an expert panel formed to take part of the decision process, comprising BRS clinical practitioners, as well as individuals from the Brazilian Dermatology Society (BDS), Brazilian Inflammatory Bowel Diseases Study Group (GEDIIB), and specialists on infectious diseases and vaccination (from Tropical Medicine, Infectious Diseases and Immunizations National Societies); in addition, two representatives of patient groups were included as members of the panel. When the quality of the evidence was low or there was a lack of evidence to determine the recommendations, the decisions were based on the expert opinion panel and a Delphi approach was performed. A recommendation was accepted upon achieving ≥80% agreement among the panel, including the patient representatives. As a result, eight recommendations were developed regarding the safety of YFV in patients with CIMID, considering the immunosuppression degree conferred by the treatment used. It was not possible to establish recommendations on the effectiveness of YFV in these patients as there is no consistent evidence to support these recommendations. Conclusion: This paper approaches a real need, assessed by clinicians and patient care groups, to address specific questions on the management of YFV in patients with CIMID living or traveling to YF endemic areas, involving specialists from many areas together with patients, and might have global applicability, contributing to and supporting vaccination practices.     O império dos mil anos e a arte do "tempo barroco": a águia bicéfala como emblema da Cristandade

    The article reveals that between the mid-17th century and the mid-18th century, in the ecclesiastical world of the religious orders (Jesuits, franciscans, Carmelites, Cistercians, Augustinian, etc.) and the episcopate, there was the progressive adoption of the imperial symbol, the double-headed eagle, attribute of the Christian Empire, the germanic Holy Roman Empire emblem. However, in the religious field, this imperial eagle of the baroque time appears without the political insignia (sword, scepter and the imperial orb), adorning altars, monstrances, trumphal archs, facades of temples, doors, walls, domes, pulpits, sacred washbasins, sculptures and paintings of the Virgin and Child, liturgical robes, etc.; therefore related to the cult and the dogmas of the Catholic faith - in artistic works, the association between the double eagle and the flesh-spirit or human-divine unity, axial principle of the Catholic faith, represented by Virgin-Mother and Christ, is often indicated directly. The research has located and identified numerous remnants of the double eagle emblem in religious field in Portugal and Spain and in their conquests and dominions in America, Asia and Africa, and also in Italy, and the historiography on painful birth of the modern era in the West has never realized this phenomenon. Symbolizing absolute power, universal power, these works with sacred significance, as the ecclesiastical discourse of the time, demonstrate movement occurred in religious plane. With the aim to assert not only spiritual but also temporal power of Christ and his mystical body, the Church was impelled by the idea of restoring the "Republica Christiana" or Christendon - disrupted by conflicts of power and faith - and of introducing a Universal Apostolic Monarchy extended to all mankind: the Empire of the Last Days, the Empire of Christ in the world, the fifth Empire
