    Brain macrophages in human cortical contusions as indicator of survival period

    The aim of this study was to establish a morphologic time scheme with which cases of cerebral contusion with unknown survival periods can be dated. Our study of 275 cases was limited to qualitative and quantitative changes in macrophages. The appearance of macrophages and their distribution as well as their content of neutral fat, esterified cholesterol, erythrocytes, siderin, hematoidin, and ceroid were correlated with the survival period. For each cytologic criterium, the observation period, distribution-free limits of tolerance, and relative frequency of identification in different survival periods were determined, and the limits of confidence calculated. The findings permit the dating of trauma in cases with unknown survival periods. Moreover, the probability of this dating was calculated

    Sensitivity of Bunker Cave to climatic forcings highlighted through multi-annual monitoring of rain-, soil-, and dripwaters

    The last two decades have seen a considerable increase in studies using speleothems as archives of past climate variability. Caves under study are now monitored for a wide range of environmental parameters and results placed in context with speleothem data. The present study investigates trends from a seven year long monitoring of Bunker Cave, northwestern Germany, in order to assess the hydraulic response and transfer time of meteoric water from the surface to the cave. Rain-, soil-, and dripwater were collected from August 2006 to August 2013 at a monthly to bimonthly resolution and their oxygen and hydrogen isotope composition was measured. Furthermore, drip rates were quantified. Due to different drip characteristics, annual mean values were calculated for the drip rates of each drip site. Correlations of the annual mean drip rate of each site with precipitation and infiltration demonstrate that the annual infiltration, and thus the annual precipitation control the inter-annual drip-rate variability for all except one site. The hydraulic response is not delayed on an annual basis. All drip sites display identical long-term trends, which suggests a draining of a common karst reservoir over these seven years of monitoring. Correlations of soil- and dripwater monthly δ18O and δD values with atmospheric temperature data reveal water transfer times of 3 months to reach a depth of 40 cm (soilwater at site BW 2) and 4 months for 70 cm depth (soilwater at site BW 1). Finally, the water reaches the cave chambers (15 to 30 m below land surface) after ca. 2.5 years. Consequently, a temporal offset of 29 to 31 months (ca. 2.5 years) between the hydraulic response time (no time lag on annual basis) and the water transfer time (time lag of 29 to 31 months) was found, which is negligible with regard to Bunker Cave speleothems because of their slow growth rates. Here, proxies recording precipitation/infiltration and temperature are registered on a decadal scale. Variations in drip rate and thus precipitation and infiltration are recorded by δ13C and Mg/Ca ratios in speleothem calcite. Speleothem δ18O values reflect both temperature and precipitation signals due to drip rate-related fractionation processes. We document that long-term patterns in temperature and precipitation are recorded in dripwater patterns of Bunker Cave and that these are linked to the North Atlantic Oscillation (NAO)

    Permeation control in hydrogel-layered patterned PET membranes with defined switchable pore geometry – Experiments and numerical simulation

    AbstractPermeation through polymeric membranes can be controlled by surface coating of a polyethylene terephthalate (PET) membrane with poly(N-isopropylacrylamide) (PNIPAAm) and inserting pores of defined geometry. When the temperature of the system rises above the volume phase transition temperature, the pores open, which allows permeation of formerly blocked particles. The exact control of the temperature allows defined change of the pore size and therefore enables separation abilities. Free swelling experiments are conducted to obtain the swelling behaviour of PNIPAAm. Then, a temperature expansion model is derived in order to simulate this behaviour with the finite element tool ABAQUS. The gained results are in excellent agreement with the observed shape change. Membranes with permeation control of particles can be used for biomedical application in microfluidics to analyse the size distribution of cells or in chemical information processing as a transistor-like component for an information-bearing chemical species. The possibility to simulate the behaviour of such permeation systems allows computer aided design and prediction of permeation abilities in these areas

    Bunker Cave stalagmites: an archive for central European Holocene climate variability

    Holocene climate was characterised by variability on multi-centennial to multi-decadal time scales. In central Europe, these fluctuations were most pronounced during winter. Here we present a record of past winter climate variability for the last 10.8 ka based on four speleothems from Bunker Cave, western Germany. Due to its central European location, the cave site is particularly well suited to record changes in precipitation and temperature in response to changes in the North Atlantic realm. We present high-resolution records of δ18O, δ13C values and Mg/Ca ratios. Changes in the Mg/Ca ratio are attributed to past meteoric precipitation variability. The stable C isotope composition of the speleothems most likely reflects changes in vegetation and precipitation, and variations in the δ18O signal are interpreted as variations in meteoric precipitation and temperature. We found cold and dry periods between 8 and 7 ka, 6.5 and 5.5 ka, 4 and 3 ka as well as between 0.7 and 0.2 ka. The proxy signals in the Bunker Cave stalagmites compare well with other isotope records and, thus, seem representative for central European Holocene climate variability. The prominent 8.2 ka event and the Little Ice Age cold events are both recorded in the Bunker Cave record. However, these events show a contrasting relationship between climate and δ18O, which is explained by different causes underlying the two climate anomalies. Whereas the Little Ice Age is attributed to a pronounced negative phase of the North Atlantic Oscillation, the 8.2 ka event was triggered by cooler conditions in the North Atlantic due to a slowdown of the thermohaline circulation

    Characteristics of clinical-pharmacological recommendations in psychiatry.

    OBJECTIVE Psychiatric patients in general, and elderly psychiatric patients in particular, are at risk of adverse drug reactions due to comorbidities and inappropriate polypharmacy. Interdisciplinary and clinical-pharmacologist-led medication reviews may contribute to medication safety in the field of psychiatry. In this study, we reported the frequency and characteristics of clinical-pharmacological recommendations in psychiatry, with a particular focus on geriatric psychiatry. METHOD A clinical pharmacologist, in collaboration with the attending psychiatrists and a consulting neurologist, conducted interdisciplinary medication reviews in a general psychiatric ward with a geropsychiatric focus at a university hospital over a 25-week period. All clinical and pharmacological recommendations were recorded and evaluated. RESULTS A total of 316 recommendations were made during 374 medication reviews. Indications/contraindications of drugs were the most frequently discussed topics (59/316; 18.7 %), followed by dose reductions (37/316; 11.7 %), and temporary or permanent discontinuation of medications (36/316; 11.4 %). The most frequent recommendations for dose reduction involvedbenzodiazepines (9/37; 24.3 %). An unclear or absent indication was the most common reason for recommending temporary or permanent discontinuation of the medication (6/36; 16.7 %). CONCLUSION Interdisciplinary clinical pharmacologist-led medication reviews represented a valuable contribution to medication management in psychiatric patients, particularly the elderly ones

    Retrospective Analysis of Efficacy and Toxicity of Stereotactic Body Radiotherapy and Surgical Resection of Adrenal Metastases from Solid Tumors.

    BACKGROUND This single-center retrospective study aimed to evaluate the efficacy and toxicity profiles of stereotactic body radiotherapy (SBRT) and surgical resection in patients with adrenal metastases originating from solid tumors. METHODS/MATERIALS Patients with advanced tumor conditions or comorbidities typically received SBRT, whereas those considered physically fit underwent standard surgical treatment. Endpoints included local control (LC), progression free survival (PFS), overall survival (OS), and complication rates (CR). RESULTS 41 patients with 48 adrenal metastases were included, with 27 (65.9%) patients receiving SBRT and 14 (34.1%) patients undergoing adrenalectomy. One- and two-year LC values were 100% for both periods after adrenalectomy, and 70.0% and 52.5% after SBRT (p = 0.001). PFS showed values of 40.2% and 32.1% at one and two years after adrenalectomy and of 10.6% for both periods after SBRT (p = 0.223). OS was 83.3% both one and two years after surgery and 67.0% and 40.2% after SBRT (p = 0.031). There was no statistically significant difference between the two groups regarding acute complications (p = 0.123). CONCLUSION Despite potential confounders, adrenalectomy exhibited statistically significant superior LC and OS compared to SBRT in managing adrenal metastases, while both treatment methods displayed acceptable toxicity profiles. However, patient selection bias must be taken into account when directly comparing the two therapy modalities. Nevertheless, the study provides new and important results for the scientific and medical communities regarding oncological outcomes after SBRT or surgical resection of adrenal metastases

    Gammaretrovirus-mediated correction of SCID-X1 is associated with skewed vector integration site distribution in vivo

    We treated 10 children with X-linked SCID (SCID-X1) using gammaretrovirus-mediated gene transfer. Those with sufficient follow-up were found to have recovered substantial immunity in the absence of any serious adverse events up to 5 years after treatment. To determine the influence of vector integration on lymphoid reconstitution, we compared retroviral integration sites (RISs) from peripheral blood CD3(+) T lymphocytes of 5 patients taken between 9 and 30 months after transplantation with transduced CD34(+) progenitor cells derived from 1 further patient and I healthy donor. Integration occurred preferentially in gene regions on either side of transcription start sites, was clustered, and correlated with the expression level in CD34(+) progenitors during transduction. In contrast to those in CD34(+) cells, RISs recovered from engrafted CD3(+)T cells were significantly overrepresented within or near genes encoding proteins with kinase or transferase activity or involved in phosphorus metabolism. Although gross patterns of gene expression were unchanged in transduced cells, the divergence of RIS target frequency between transduced progenitor cells and post-thymic T lymphocytes indicates that vector integration influences cell survival, engraftment, or proliferation

    Functional Magnetic Resonance Imaging of Motor Cortex Activation in Schizophrenia

    Previous fMRI studies of sensorimotor activation in schizophrenia have found in some cases hypoactivity, no difference, or hyperactivity when comparing patients with controls; similar disagreement exists in studies of motor laterality. In this multi-site fMRI study of a sensorimotor task in individuals with chronic schizophrenia and matched healthy controls, subjects responded with a right-handed finger press to an irregularly flashing visual checker board. The analysis includes eighty-five subjects with schizophrenia diagnosed according to the DSM-IV criteria and eighty-six healthy volunteer subjects. Voxel-wise statistical parametric maps were generated for each subject and analyzed for group differences; the percent Blood Oxygenation Level Dependent (BOLD) signal changes were also calculated over predefined anatomical regions of the primary sensory, motor, and visual cortex. Both healthy controls and subjects with schizophrenia showed strongly lateralized activation in the precentral gyrus, inferior frontal gyrus, and inferior parietal lobule, and strong activations in the visual cortex. There were no significant differences between subjects with schizophrenia and controls in this multi-site fMRI study. Furthermore, there was no significant difference in laterality found between healthy controls and schizophrenic subjects. This study can serve as a baseline measurement of schizophrenic dysfunction in other cognitive processes
