    Endovascular Therapy of Pseudoaneurysm and Arteriovenous Fistula in a Renal Graft

    A 31 year old male Caucasian received a renal cadaveric allograft. Reconstruction of an inferior polar artery was corrected pre-implantation. Delayed graft function occurred leading to dialysis support for one month. Graft biopsies(days 7, 15) showed acute tubular necrosis(ATN) and no rejection. Serial ultrasound (US), performed on average weekly, were compatible with ATN. On day 31, Doppler US and a CAT scan suggested for the first time a pseudoaneurysm adjacent to the implantation of the graft artery on the external iliac artery. For clinical and technical reasons, arteriography was only performed on day 67, when serum creatinine was 3.3 mg/dl. It showed a large pseudoaneurysm with an arteriovenous fistula to the right common iliac vein. Compression of the right external iliac artery was clear. In an attempt to close the arteriovenous fistula, the communication with the pseudoaneurysm was embolised with gelfoam and metallic coils with partial success. One week later, by right femoral approach a covered wallstent was placed immediately below the origin of the graft artery.Subsequent Doppler US and arteriography con-firmed closure of the communication with thepseudoaneurysm and of the arteriovenous fistula. The calibre of the right external iliac artery was then normal. By month 18, serum creatinine is stable at 2.1 mg/dl. We can only speculate on the origin of thepseudoaneurysm and of the AV fistula, whichwere not evident until one month post-transplantation. Backtable surgery was performed on thepolar not the main graft artery. Invasive angiography was irreplaceable in this unusual clinical situation

    Optimal planning and campaign scheduling of biopharmaceutical processes using a continuous-time formulation

    This work addresses the optimal planning and campaign scheduling of biopharmaceutical manufacturing processes, considering multiple operational characteristics, such as the campaign schedule of batch and/or continuous process steps, multiple intermediate deliveries, sequence dependent changeovers operations, product storage restricted to shelf-life limitations, and the track-control of the production/campaign lots due to regulatory policies. A new mixed integer linear programing (MILP) model, based on a Resource Task Network (RTN) continuous time single-grid formulation, is developed to comprise the integration of all these features. The performance of the model features is discussed with the resolution of a set of industrial problems with different data sets and process layouts, demonstrating the wide application of the proposed formulation. It is also performed a comparison with a related literature model, showing the advantages of the continuous-time approach and the generality of our model for the optimal production management of biopharmaceutical processes

    Pedagogy subjects in the formation and in the construction of representations about teachers' work: views of preservice Chemistry and Physics teachers

    Este estudo tem como objetivo analisar as concepções de licenciandos em Química e em Física sobre a prática profissional e as marcas deixadas pelas disciplinas pedagógicas na sua formação, assim como identificar sinais de mudanças em suas concepções durante a realização do curso de Licenciatura. Para tanto, textos foram produzidos por 54 alunos matriculados em cursos de Licenciatura de duas universidades públicas do estado de São Paulo, nos quais os licenciandos expressaram suas experiências de formação e apontaram elementos que consideram que terão alguma influência em sua atuação profissional. Os textos revelaram que as ideias que os licenciandos carregam são consistentes com muitas das visões e objetivos apresentados em cursos de formação de professores. Nas produções dos estudantes faz-se presente o desejo de compartilhar e promover em seus futuros alunos o interesse pela Química e a Física. Os seus comentários refletiram visões de professores que tiveram no passado, os quais gostariam de imitar, e de outros que não gostariam de ser. Os comentários também tornaram evidente a vontade dos licenciandos em se estabelecerem como professores que se preocupam com seus alunos. Os textos foram analisados segundo a Análise do Discurso de linha francesa, como tem sido divulgada por Eni Orlandi, especialmente a noção de formação discursiva.The objective of this study is to analyze the views of preservice chemistry and physics teachers about the professional practice and the influence of pedagogic disciplines on their formation. Furthermore, it aims at identifying changes in preservice teachers' opinions and beliefs throughout their education. Hence, texts were produced by 54 undergraduate students who were undertaking preservice courses in two public universities in the state of São Paulo, in which they expressed their experiences throughout the course and mentioned relevant aspects about their professional development. The texts revealed that their ideas were consistent concerning many of the views and goals of teacher preparation courses. Their statements reflected desires to share and stimulate interest for chemistry and physics in their future students. Their comments evidenced past experiences they would like to emulate and others they would rather not. They also expressed their worries about becoming teachers who do care for their students. The texts were analyzed by Discourse Analysis, in its French approach, according to Eni Orlandi, especially the concept of discursive formation

    Apresenta-se um caso clínico referente a doente de 35 anos, do sexo masculino sem antecedentes pessoais relevantes, admitido no serviço de urgência por quadro de toracalgia e tosse produtiva com alterações electrocardiográficas sugestivas de pericardite. Inicialmente admitido pelo Serviço de Cardiologia, com melhoria do quadro clínico após terapêutica anti- -inflamatória; contudo, no internamento houve como intercorrência pneumonia de provável etiologia bacteriana, complicada por derrame pleural. Após a alta,foi referenciado à consulta de pneumologia, onde se manteve o estudo etiológico do derrame persistente, tendo vindo a complicar-se o seu quadro com alterações das cavidades cardíacas e múltiplos nódulos pulmonares, sugestivos de endocardite subaguda com embolização séptica pulmonar. Internado no serviço de Pneumologia e submetido a videotoracoscopia, foi-lhe diagnosticado angiossarcoma cardíaco com metastização pulmonar. Assistiu-se a uma rápida evolução do quadro clínico, quase fulminante, com falência cardíaca e óbito do doente sem ter iniciado radioterapia ou quimioterapia adjuvante

    Echocardiographic evaluation including tissue Doppler imaging in New Zealand white rabbits sedated with ketamine and midazolam

    Limited data are available on the use of more recent echocardiographic parameters in the rabbit. Echocardiographic examination, including conventional echocardiography and tissue Doppler imaging (TDI), was performed on 26 male New Zealand white rabbits under ketamine-midazolam sedation. Particular emphasis was placed on the more recent systolic and diastolic parameters, such as myocardial performance index (Tei index) and mitral annular motion (from septal and lateral sides of the left ventricle) obtained using pulsed TDI. Parameters that assessed systolic and diastolic function (fractional shortening, Tei index, and maximal mitral E- and A-wave velocities) were comparable to those reported in the literature for rabbits in the awake state. The less cardiodepressive anaesthetic protocol could offer a good alternative in performing echocardiographic evaluation whenever such caution is necessary. TDI is feasible in healthy rabbits and potentially suitable for the investigation of left ventricle systolic and diastolic function

    Childhood obesity and reduction of hours of sleep: results from cross-sectional study

    Childhood obesity is a prevalent condition in modern societies which can negatively impact people's health during life. Although there are several factors contributing to obesity, sleep duration has been pointed out as an important risk factor. This study, held in the district of Porto, Portugal, aimed to analyze the association between sleep duration and the nutritional status of school-aged children studying in Portugal. 1396 students from 35 and 3 public and private schools, respectively, were contacted, of which 829 participated in this study (9.2 +/- 0.38 years old; 50.3% male; 49.7% female). Age, sex, sleep duration, height, weight and body composition were registered trough a questionnaire. According to the obtained z-score for Body Mass Index (BMI), 22.9% of the children were overweight and 15.2% were obese. The mean of sleep duration was 9.7 +/- 1.08 hours and z-score for BMI was 0.78 +/- 1.21, demonstrating a statistically significant (p<0.01) weak negative correlation (r=-0.15) between sleep duration and z-score of BMI. Results were also stratified and analyzed by sex, and the same finding was observed. Therefore, the results suggest a negative association between sleep duration and overweight/obesity in these children. Although more studies are necessary, this finding highlights the need of a global approach which includes sleep assessment and counselling when promoting an appropriate nutritional status

    Emerging biosensing technologies for neuroinflammatory and neurodegenerative disease diagnostics

    Neuroinflammation plays a critical role in the onset and progression of many neurological disorders, including Multiple Sclerosis, Alzheimer’s and Parkinson’s diseases. In these clinical conditions the underlying neuroinflammatory processes are significantly heterogeneous. Nevertheless, a common link is the chronic activation of innate immune responses and imbalanced secretion of pro and anti-inflammatory mediators. In light of this, the discovery of robust biomarkers is crucial for screening, early diagnosis, and monitoring of neurological diseases. However, the difficulty to investigate biochemical processes directly in the central nervous system (CNS) is challenging. In recent years, biomarkers of CNS inflammatory responses have been identified in different body fluids, such as blood, cerebrospinal fluid, and tears. In addition, progress in micro and nanotechnology has enabled the development of biosensing platforms capable of detecting in real-time, multiple biomarkers in clinically relevant samples. Biosensing technologies are approaching maturity where they will become deployed in community settings, at which point screening programs and personalized medicine will become a reality.     Feohifomicosis Producida por Alternaria Infectoria con Presentación Clínica de Múltiples Lesiones Vegetantes en un Paciente Sometido a un Trasplante Renal

    The genus Alternaria is one of the most common black moulds and appears to be increasing as a causative agent of subcutaneous phaeohyphomycosis, particularly among immunosuppressed patients. A 53-year-old patient who had received a kidney transplant presented with multiple verrucous lesions on the distal extremities. Positive histopathology and cultures, in addition to rDNA ITS region sequencing, identified the fungal isolate as Alternaria infectoria. Oral itraconazole was administered for 10 months. A follow-up at 15 months demonstrated no signs of infection. Clinical manifestations of cutaneous alternariosis vary significantly and only a few cases have been described in the literature. Although optimal treatment options remain controversial, this case of phaeohyphomycosis was successfully treated with itraconazole monotherapy

    Current trends on seaweeds: Looking at chemical composition, phytopharmacology, and cosmetic applications

    Seaweeds have received huge interest in recent years given their promising potentialities. Their antioxidant, anti-inflammatory, antitumor, hypolipemic, and anticoagulant effects are among the most renowned and studied bioactivities so far, and these effects have been increasingly associated with their content and richness in both primary and secondary metabolites. Although primary metabolites have a pivotal importance such as their content in polysaccharides (fucoidans, agars, carragenans, ulvans, alginates, and laminarin), recent data have shown that the content in some secondary metabolites largely determines the effective bioactive potential of seaweeds. Among these secondary metabolites, phenolic compounds feature prominently.
