    Identification of the regional context to increase the use of species mixtures

    Construction of solutions for maintaining the economic and environmental sustainability of the arable sector relies on demonstration and understanding of local practical knowledge. This report presents the results of a survey of growers to identify local practical knowledge of current species mixtures, the reasons for these current mixtures as well as barriers and enablers towards more intercropping approaches.The key characteristics of those conducting food intercropping are contrasted against those who grow for forage and those who do not undertake these particular types of intercropping. A novel network approach is used to link the main types of intercropping and this indicates the diversity of intercropping strategies occurring across Europe. Moreover, farmers currently conducting food intercropping emerge as curiosity driven, generally younger and more focused on promotion of individual values around the environment. This seems to agree with the literature on both organic and agro-ecological farming practices. However, farmers have a core of belief around the economic sustainability of the business.     Estudio bibliométrico sobre disertaciones y tesis en enfermería con aproximación fenomenológica: tendencia y perspectivas

    This study aimed to analyze the scientific production of nursing dissertations and theses with a phenomenological approach published from 1981 to 2002. A descriptive, retrospective and bibliometric research was carried out. The material analysis, which is part of the database of the catalogue Information on Nursing Research and Researchers, resulted in 217 studies, 71,9% from the master's course and 28,1% from the doctoral course. Among the obtained results, it stands out that the theoretical-philosophical reference chosen to support the data discussion found in investigations has not been used emphatically. There is a need to discuss the unveiled data, through the subjects' discourse, in the light of the philosophical reference framework mentioned in the research trajectory.Este trabajo tuvo como objetivo analizar la producción científica sobre disertaciones y tesis de enfermería con aproximación fenomenológica publicadas de 1981 a 2002. Fue una investigación del tipo descriptivo, retrospectivo y bibliométrico. El análisis del material perteneciente a la base de datos del catálogo Informaciones sobre Investigaciones y Investigadores en Enfermería resultó en 217 estudios: el 71,9% de maestría y el 28,1% del doctorado. Entre los resultados alcanzados, se destaca que el referencial teórico-filosófico elegido para soportar el análisis de datos encontrados en las investigaciones no ha sido utilizados con énfasis. Se destaca la necesidad de discutir los datos encontrados a través del discurso de los sujetos, a la luz del referencial teórico-filosófico mencionado en la trayectoria de investigación.Este estudo teve por objetivo analisar a produção científica sobre dissertações e teses de enfermagem com abordagem fenomenológica, publicadas de 1981 a 2002. Trata-se de pesquisa do tipo descritiva, retrospectiva e bibliométrica.     Amniotic Fluid Embolism. Is a New Pregnancy Possible? Case Report.

    Amniotic fluid embolism (AFE) is a rare but potentially catastrophic clinical condition, characterized by a combination of signs and symptoms that reflect respiratory distress, cardiovascular collapse and disseminated intravascular coagulation (DIC). Its pathogenesis is still unclear. More recently, the traditional view of obstruction of pulmonary capillary vessels by amniotic fluid emboli as the main explanation for the etiology has been ruled out, and immunologic factors and the activation of the inflammatory cascade took on an important role. Amniotic fluid embolism has an unpredictable character, its diagnosis is exclusively clinical, and the treatment consists mainly of cardiovascular support and administration of blood products to correct the DIC. No diagnostic test is recommended until now, though multiple blood markers are currently being studied. The authors present a case report of a woman who had survived AFE in her previous pregnancy and had a subsequent pregnancy without recurrence, providing one more clinical testimony of the low risks for the pregnancy after AFE.info:eu-repo/semantics/publishedVersio

    An early adoption of ecological management practices by farmers is a potential route to achieve sustainable and greener goals for agriculture. This briefing note outlines an economic assessment of four different ecological practices on Scottish livestock farms. The ecological practices used are; ecological area, reducing farm inputs, organic adoption and agro-forestry. The results suggest that ecological area and reduced farm inputs have potential financial benefits on farms and can be easily adopted by farmers. The organic and agro-forestry systems, however, require capital investment to establish and hence provide a challenging prospect of adoption without a provision of financial support<br/

    The Spatial Distribution of Absolute Skeletal Muscle Deoxygenation During Ramp-Incremental Exercise Is Not Influenced by Hypoxia.

    Time-resolved near-infrared spectroscopy (TRS-NIRS) allows absolute quantitation of deoxygenated haemoglobin and myoglobin concentration ([HHb]) in skeletal muscle. We recently showed that the spatial distribution of peak [HHb] within the quadriceps during moderate-intensity cycling is reduced with progressive hypoxia and this is associated with impaired aerobic energy provision. We therefore aimed to determine whether reduced spatial distribution of skeletal muscle [HHb] was associated with impaired aerobic energy transfer during exhaustive ramp-incremental exercise in hypoxia. Seven healthy men performed ramp-incremental cycle exercise (20 W/min) to exhaustion at 3 fractional inspired O2 concentrations (FIO2): 0.21, 0.16, 0.12. Pulmonary O2 uptake (VO₂) was measured using a flow meter and gas analyser system. Lactate threshold (LT) was estimated non-invasively. Absolute muscle deoxygenation was quantified by multichannel TRS-NIRS from the rectus femoris and vastus lateralis (proximal and distal regions). VO₂peak and LT were progressively reduced (p < 0.05) with hypoxia. There was a significant effect (p < 0.05) of FIO2 on [HHb] at baseline, LT, and peak. However the spatial variance of [HHb] was not different between FIO2 conditions. Peak total Hb ([Hbtot]) was significantly reduced between FIO2 conditions (p < 0.001). There was no association between reductions in the spatial distribution of skeletal muscle [HHb] and indices of aerobic energy transfer during ramp-incremental exercise in hypoxia. While regional [HHb] quantified by TRS-NIRS at exhaustion was greater in hypoxia, the spatial distribution of [HHb] was unaffected. Interestingly, peak [Hbtot] was reduced at the tolerable limit in hypoxia implying a vasodilatory reserve may exist in conditions with reduced FIO2

    Think twice before starting a new trial; what is the impact of recommendations to stop doing new trials?

    ObjectivesIn evidence-based medicine, we base our conclusions on the effectiveness of interventions on the results of high-quality meta-analysis. If a new randomized controlled trial (RCT) is unlikely to change the pooled effect estimate, conducting the new trial is a waste of resources. We evaluated whether recommendations not to conduct further RCTs reduced the number of trials registered for two scenarios.MethodsAnalysis of registered trials on the World Health Organisation (WHO) International Clinical Trials Registry Platform (ICTRP). We regarded trial protocols relevant if they evaluated the effectiveness of (1) exercise for chronic low back pain (LBP) and (2) cognitive behavioural therapy (CBT) for chronic pain. We calculated absolute and relative numbers and change of registered trials in a pre-set time window before and after publication of the recommendations, both published in 2012.ResultsWe found 1,574 trials registered in the WHO trial registry for exercise in LBP (459 before 2012; 1,115 after) and 5,037 trials on chronic pain (1,564 before 2012; 3,473 after). Before 2012, 13 trials on exercise for LBP (out of 459) fit the selection criteria, compared to 42 trials (out of 1,115) after, which represents a relative increase of 33%. Twelve trials (out of 1,564) regarding CBT for chronic pain, fit the selection criteria before 2012 and 18 trials (out of 3,473) after, representing a relative decrease of 32%. We found that visibility, media exposure and strength of the recommendation were related to a decrease in registered trials.ConclusionsRecommendations not to conduct further RCTs might reduce the number of trials registered if these recommendations are strongly worded and combined with social media attention
