    Heard and Seen: Developing Inclusive Activities for Enabling the Voices of Children with Disabilities in Oral Health Research

    The Convention on the Rights of the Child argues for children’s involvement in research. Oral health research frequently excludes children with disabilities and their voices. This study takes a rights-based approach by devising methods to include disabled children in oral health research. This is an ethnographic study. Methods utilized interviews, guided tours, symbols, drawings, pictures, and games. The selection of method depended on the ability and preference of each child. Using pictures and games as prompts enabled child participation. The guided tour activity facilitated the development of relationships with the children. It also increased their ability to chat informally and appeared to reduce power imbalances compared to formal, structured interviewing. Focus group interviews, symbols, and drawings acted as barriers to children’s participation. Involving children with disabilities in oral health research requires using appropriate methodological designs and innovative, pluralistic methods drawn from differentdisciplines. This promotes a rights-based approach, which recognizes diversity and aims to reduce the discrimination and disempowerment of children with disabilities

    Implementation of health IT for cancer screening in US primary care: Scoping review

    BACKGROUND: A substantial percentage of the US population is not up to date on guideline-recommended cancer screenings. Identifying interventions that effectively improve screening rates would enhance the delivery of such screening. Interventions involving health IT (HIT) show promise, but much remains unknown about how HIT is optimized to support cancer screening in primary care. OBJECTIVE: This scoping review aims to identify (1) HIT-based interventions that effectively support guideline concordance in breast, cervical, and colorectal cancer screening provision and follow-up in the primary care setting and (2) barriers or facilitators to the implementation of effective HIT in this setting. METHODS: Following scoping review guidelines, we searched MEDLINE, CINAHL Plus, Web of Science, and IEEE Xplore databases for US-based studies from 2015 to 2021 that featured HIT targeting breast, colorectal, and cervical cancer screening in primary care. Studies were dual screened using a review criteria checklist. Data extraction was guided by the following implementation science frameworks: the Reach, Effectiveness, Adoption, Implementation, and Maintenance framework; the Expert Recommendations for Implementing Change taxonomy; and implementation strategy reporting domains. It was also guided by the Integrated Technology Implementation Model that incorporates theories of both implementation science and technology adoption. Reporting was guided by PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews). RESULTS: A total of 101 studies met the inclusion criteria. Most studies (85/101, 84.2%) involved electronic health record-based HIT interventions. The most common HIT function was clinical decision support, primarily used for panel management or at the point of care. Most studies related to HIT targeting colorectal cancer screening (83/101, 82.2%), followed by studies related to breast cancer screening (28/101, 27.7%), and cervical cancer screening (19/101, 18.8%). Improvements in cancer screening were associated with HIT-based interventions in most studies (36/54, 67% of colorectal cancer-relevant studies; 9/14, 64% of breast cancer-relevant studies; and 7/10, 70% of cervical cancer-relevant studies). Most studies (79/101, 78.2%) reported on the reach of certain interventions, while 17.8% (18/101) of the included studies reported on the adoption or maintenance. Reported barriers and facilitators to HIT adoption primarily related to inner context factors of primary care settings (eg, staffing and organizational policies that support or hinder HIT adoption). Implementation strategies for HIT adoption were reported in 23.8% (24/101) of the included studies. CONCLUSIONS: There are substantial evidence gaps regarding the effectiveness of HIT-based interventions, especially those targeting guideline-concordant breast and colorectal cancer screening in primary care. Even less is known about how to enhance the adoption of technologies that have been proven effective in supporting breast, colorectal, or cervical cancer screening. Research is needed to ensure that the potential benefits of effective HIT-based interventions equitably reach diverse primary care populations

    Arts education research over the years has highlighted the situation of non-specialist preservice primary arts teachers as having little confidence in their own artistic ability and their ability to teach the arts to children. Added to this, problems such a lack of resources, confidence, priority, time, knowledge and experience appear to inhibit the regular teaching of the arts by generalist classroom teachers while at the same time, face-to-face hours for preservice primary arts education have decreased significantly over the recent years. This paper describes how one subject within a Primary Teacher Education course responded to these challenges. This subject was based on Herrington, Oliver and Reeves’ (2003) framework for creating authentic learning environments then triangulates this authentic learning framework with what students wanted to learn in the subject and how they perceived they had developed their confidence and competence in creative arts educatio

    The present invention provides a system and method for creating on site, remote from a sterile environment, parenteral solutions in large volume parenteral containers for intravenous administration to a patient. In an embodiment, this system comprises an empty large volume container including at least one port for accessing an interior of the container. The port includes a sterilizing filter for sterilizing a fluid fed through the port into the container. A second container is provided including a solute and having means for coupling the second container to the large volume container and thereby providing fluid communication therebetween allowing the solute to be received within the interior of the container. A sterile water source is also provided including means for placing the sterile water source in fluid communication with the port and allowing water to flow from the sterile water source into the interior of the container. This allows the solute, and sterile water that has been fed through the filter, to create a parenteral solution in the large volume parenteral container

    Understandings of creative practice and pedagogy by teacher education communities in West Bank, Palestine, and North West England

    This is an Accepted Manuscript of an article published by Taylor & Francis in Discourse on 27/01/2020, available online: https://www.tandfonline.com/doi/full/10.1080/01596306.2019.1708706.This paper discusses a collaborative research project that aimed to explore approaches to creative practices and pedagogies by teacher education communities in the West Bank, Palestine, and North West England (Bethlehem and Chester). The project explored the values, attitudes and perceptions of teacher educators and student teachers in relation to creative pedagogies and the conditions under which they flourished in each community. We found that creativity was understood to take many forms, according to the cultural values and conditions present in each community, and that creative pedagogical forms emerged from the specificities of their cultural and political contexts. Creativity in education is a contentious issue in both cultures, but an area that both education communities wished to explore further. Despite the differences, there were surprising commonalities between the two communities about the value of creative practices and the relation of creativity to democratic and critical practices in the classroom

    Advancing In Situ Modeling of ICMEs: New Techniques for New Observations

    It is generally known that multi-spacecraft observations of interplanetary coronal mass ejections (ICMEs) more clearly reveal their three-dimensional structure than do observations made by a single spacecraft. The launch of the STEREO twin observatories in October 2006 has greatly increased the number of multipoint studies of ICMEs in the literature, but this field is still in its infancy. To date, most studies continue to use on flux rope models that rely on single track observations through a vast, multi-faceted structure, which oversimplifies the problem and often hinders interpretation of the large-scale geometry, especially for cases in which one spacecraft observes a flux rope, while another does not. In order to tackle these complex problems, new modeling techniques are required. We describe these new techniques and analyze two ICMEs observed at the twin STEREO spacecraft on 22-23 May 2007, when the spacecraft were separated by ~8 degrees. We find a combination of non-force-free flux rope multi-spacecraft modeling, together with a new non-flux rope ICME plasma flow deflection model, better constrains the large-scale structure of these ICMEs. We also introduce a new spatial mapping technique that allows us to put multispacecraft observations and the new ICME model results in context with the convecting solar wind. What is distinctly different about this analysis is that it reveals aspects of ICME geometry and dynamics in a far more visually intuitive way than previously accomplished. In the case of the 22-23 May ICMEs, the analysis facilitates a more physical understanding of ICME large-scale structure, the location and geometry of flux rope sub-structures within these ICMEs, and their dynamic interaction with the ambient solar wind

    Changes in expression and activity of the secretory pathway Ca2+ATPase 1 (SPCA1) in A7r5 vascular smooth muscle cells cultured at different glucose concentrations

    Diabetes mellitus-related vascular disease is often associated with both a dysregulation of Ca2+^{2+} homoeostasis and enhanced secretory activity in VSMCs (vascular smooth muscle cells). Here, we employ a commonly used rat cell line for VSMCs (A7r5 cells) to investigate the effects of glucose on the expression and activity of the SPCA1 (secretory pathway Ca2+^{2+}-ATPase 1; also known as ATP2C1), which is a P-type Ca2+^{2+} pump located in the Golgi apparatus that plays a key role in the secretory pathway. Our results show that mRNA expression levels of SPCA1 are significantly increased in A7r5 cells cultured in high glucose (25.0 mM)-supplemented medium compared with normal glucose (5.55 mM)-supplemented medium. SPCA1 protein expression levels and thapsigargin-insensitive Ca2+^{2+}-dependent ATPase activity were also consistent with a higher than normal expression level of SPCA1 in high-glucose-cultured A7r5 cells. Analysis of AVP (arginine-vasopressin)-induced cytosolic Ca2+^{2+} transients in A7r5 cells (after pre-treatment with thapsigargin) showed faster rise and decay phases in cells grown in high glucose medium compared with cells grown in normal glucose medium, supporting the observation of increased SPCA expression/activity. The significant levels of both Ca2+^{2+}-ATPase activity and AVP-induced Ca2+^{2+} transients, in the presence of thapsigargin, indicate that SPCA must play a significant role in Ca2+^{2+} uptake within VSMCs. We therefore propose that, if such increases in SPCA expression and activity also occur in primary VSMCs, this may play a substantial role in the aetiology of diabetes mellitus-associated vascular disease, due to alterations in Ca2+^{2+} homoeostasis within the Golgi apparatus
