    Randomised controlled trial of specialist nurse intervention in heart failure

    <p>Objectives. To determine whether specialist nurse intervention improves outcome in patients with chronic heart failure.</p> <p>Design. Randomised controlled trial.</p> <p>Setting. Acute medical admissions unit in a teaching hospital.</p> <p>Participants. 165 patients admitted with heart failure due to left ventricular systolic dysfunction. The intervention started before discharge and continued thereafter with home visits for up to 1 year.</p> <p>Main outcome measures. Time to first event analysis of death from all causes or readmission to hospital with worsening heart failure.</p> <p>Results. 31 patients (37%) in the intervention group died or were readmitted with heart failure compared with 45 (53%) in the usual care group (hazard ratio=0.61, 95% confidence interval 0.33 to 0.96).Compared with usual care, patients in the intervention group had fewer readmissions for any reason (86 v 114, P=0.018), fewer admissions for heart failure (19 v 45, P<0.001) and spent fewer days in hospital for heart failure (mean 3.43 v 7.46 days, P=0.0051).</p> <p>Conclusions. Specially trained nurses can improve the outcome of patients admitted to hospital with heart failure.</p&gt

    Avalanche Breakdown Timing Statistics for Silicon Single Photon Avalanche Diodes

    CCBY Silicon-based Single Photon Avalanche Diodes (SPADs) are widely used as single photon detectors of visible and near infrared photons. There has however been a lack of models accurately interpreting the physics of impact ionization (the mechanism behind avalanche breakdown) for these devices. In this work, we present a statistical simulation model for silicon SPADs that is capable of predicting breakdown probability, mean time to breakdown and timing jitter. Our model inherently incorporates carriers & #x0027; dead space due to phonon scattering and allows for non-uniform electric fields. Model validation included avalanche gain, excess noise factor, breakdown voltage, breakdown probability, and timing statistics. Simulating an n on-p and a p-on-n SPAD design using our model, we found that the n-on-p design offers significantly improved mean time to breakdown and timing jitter characteristics. For a breakdown probability of 0.5, mean time to breakdown and timing jitter from the n-on-p design were 3 and 4 times smaller compared to those from the p on n design.     Chern-Simons Reduction and non-Abelian Fluid Mechanics

    We propose a non-Abelian generalization of the Clebsch parameterization for a vector in three dimensions. The construction is based on a group-theoretical reduction of the Chern-Simons form on a symmetric space.     We study the space of geometric and open string moduli of type IIB compactifications from the perspective of complex structure deformations of F-theory. In order to find a correspondence, we work in the weak coupling limit and for simplicity focus on compactifications to 6 dimensions. Starting from the topology of D7-branes and O7-planes, we construct the 3-cycles of the F-theory threefold. We achieve complete agreement between the degrees of freedom of the Weierstrass model and the complex structure deformations of the elliptic Calabi-Yau.     Herd-level risk factors of bovine tuberculosis in England and Wales after the 2001 foot-and-mouth disease epidemic

    We present the results of a 2005 case–control study of bovine tuberculosis (bTB) breakdowns in English and Welsh herds. The herd management, farming practices, and environmental factors of 401matched pairs of case and control herds were investigated to provide a picture of herd-level risk factors in areas of varying bTB incidence. A global conditional logistic regression model, with region-specific variants, was used to compare case herds that had experienced a confirmed bTB breakdown to contemporaneous control herds matched on region, herd type, herd size, and parish testing interval. Contacts with cattle from contiguous herds and sourcing cattle from herds with a recent history of bTB were associated with an increased risk in both the global and regional analyses. Operating a farm over several premises, providing cattle feed inside the housing, and the presence of badgers were also identified as significantly associated with an increased bTB risk. Steps taken to minimize cattle contacts with neighboring herds and altering trading practices could have the potential to reduce the size of the bTB epidemic. In principle, limiting the interactions between cattle and wildlife may also be useful; however this study did not highlight any specific measures to implement

    The stability analysis of a generalized Dicke model, in the semi-classical limit, describing the interaction of a two-species Bose-Einstein condensate driven by a quantized field in the presence of Kerr and spontaneous parametric processes is presented. The transitions from Rabi to Josephson dynamics are identified depending on the relative value of the involved parameters. Symmetry-breaking dynamics are shown for both types of coherent oscillations due to the quantized field and nonlinear optical processes.Comment: 12 pages, 5 figures.     Fixing D7 Brane Positions by F-Theory Fluxes

    To do realistic model building in type IIB supergravity, it is important to understand how to fix D7-brane positions by the choice of fluxes. More generally, F-theory model building requires the understanding of how fluxes determine the singularity structure (and hence gauge group and matter content) of the compactification. We analyse this problem in the simple setting of M-theory on K3xK3. Given a certain flux which is consistent with the F-theory limit, we can explicitly derive the positions at which D7 branes or stacks of D7 branes are stabilised. The analysis is based on a parameterization of the moduli space of type IIB string theory on T^2/Z_2 (including D7-brane positions) in terms of the periods of integral cycles of M-theory on K3. This allows us, in particular, to select a specific desired gauge group by the choice of flux numbers.Comment: 41 pages, 2 figures; latex font correcte
