    Retinal Dysplasia Mimicking Retinoblastoma.

    Retinal dysplasia represents a congenital disorder characterized by abnormal proliferation of retinal tissue causing leukocoria. We present a case of an infant with bilateral leukocoria, clinically diagnosed as retinoblastoma, followed by enucleation of the left eye. Microscopy however, demonstrated retinal dysplasia consisting of a disorderly proliferation of retinal tissue with formation of rosettes, mimicking retinoblastoma. Microscopic features that aid in differentiating this lesion from retinoblastoma are discussed

    A cross sectional study of water quality from dental unit water lines in dental practices in the West of Scotland

    OBJECTIVE: To determine the microbiological quality of water from dental units in a general practice setting and current practice for disinfection of units. DESIGN: A cross-sectional study of the water quality from 40 dental units in 39 general practices and a questionnaire of the disinfection protocols used in those practices. SETTING: NHS practices in primarydental care. SUBJECTS: Thirty-nine general practices from the West of Scotland. METHODS: Water samples were collected on two separate occasions from dental units and analysed for microbiological quality by the total viable count (TVC) method. Water specimens were collected from the triple syringe, high speed outlet, cup filler and surgery tap. Each participating practitioner was asked to complete a questionnaire. Results Microbial contamination was highest from the high speed outlet followed by the triple syringe and cup filler. On average, the TVC counts from the high speed water lines at 37 degrees C and for the high speed lines, triple syringe and cup filler at 22 degrees C were significantly higher than that from the control tap water specimens. The study included units from 11 different manufacturers with ages ranging from under one year to over eight years. The age of the dental unit analysed did not appear to influence the level of microbial contamination. Five of the practices surveyed used disinfectants to clean the dental units but these had no significant effect on the microbiological quality of the water. The majority of dental units (25 out of 40) were never flushed with water between patients. A number of different non-sterile irrigants were used for surgical procedures. CONCLUSION: The microbiological quality of water from dental units in general dental practice is poor compared with that from drinking water sources. Suitable sterile irrigants should be used for surgical procedures in dental practice. Further work is required for pragmatic decontamination regimens of dental unit water lines in a general dental practice setting

    Background: hypertension is a State where the systolic blood pressure is greater than or equal to 140 mmHg, diastolic blood pressure and greater than or equal to 90 mmHg. The world health organization (WHO) report there is 12.8% of deaths caused by hypertension worldwide, and as much as 40% of adults had suffered from hypertension. Purpose: to know how the walk against the blood pressure in the elderly with hypertension in the House social services Tresna Werdha (BPSTW) unit Budhi Sublime, Kasongan, Bantul, Yogyakarta. Method: the type of this research is quantitative approach to time series design (circuit design time). The population in this research as much as 88 respondents with samples as many as 12 respondents taken using purposive sampling technique. Results: Wilcoxon test results on samples obtained results for the z value of systole count =-2.271 and p value = 0.023 α < 0.05. In addition the results to calculate the z value of diastole =-2.530 and p value = 0.011 α < 0.05 meaning there influence between walk against blood pressure in elderly hypertension. Conclusion: walking affects blood pressure changes (down) on the elderly with hypertension

    Pattern of irritable bowel syndrome and its impact on quality of life in primary health care center attendees, Suez governorate, Egypt

    Introduction: Irritable bowel syndrome (IBS) is one of the most common disorders diagnosed by gastroenterologists and a common cause of general practice visits. Although this disease is not life threatening, patients with IBS seem to be seriously affected in their everyday life. The study was designed to explore the pattern of IBS in clinical practice and the impact on the quality of life. Methods: This is a case control descriptive study. 117 individuals were included in this study. Rome II criteria were used for the diagnosis of IBS. Impact of IBS on patient's quality of life was determined by irritable bowel syndrome quality of life (IBS-QOL) questionnaire. Results: Prevalence of IBS among the study sample was 34.2%. 10% were IBS-Diarrhea, 37.5% were IBS-Constipation and 52.5% were alternators. There is statistical insignificant relationship between IBS (+) and age while it was a significant relation regarding gender (more common among women 80%). There is statistical significance relationship between IBS (+) on one hand and marital status and occupational status on the other hand. Patients with IBS had statistically significant lower scores for all IBS- QOL domains compared with the control group. Conclusion: IBS is a prevalent disorder that affects females more than males and it has significant impacts on work, lifestyle and social well-being.Key words: Irritable bowel syndrome, quality of life, Roma I

    Solid Biomass to Medium CV Gas Conversion With Rich Combustion

    A modified cone calorimeter for controlled atmosphere combustion was used to investigate the gases released from fixed bed rich combustion of solid biomass. The cone calorimeter was used with 50 kW/m2 of radiant heat that simulated a larger gasification system. The test specimen in the cone calorimeter is 100mm square and this sits on a load cell so that the mass burn rate can be determined. Pine wood was burned with fixed air ventilation that created rich combustion at 1.5–4 equivalence ratio, Ø. The raw exhaust gas was sampled using a multi-hole gas sample probe in a discharge chimney above the cone heater, connected via heated sample lines, filters and pumps to the heated Gasmet FTIR. The FTIR was calibrated for 60 species, including 40+ hydrocarbons. The hydrogen in the gas was computed from the measured CO concentration using the water-gas shift reaction. The exhaust gas temperature was also measured so that the sensible heat from the gasification zone was included in the energy balance. The GCV of the pine was 18.8 MJ/kgpine and at the optimum Ø the energy in the rich combustion zone gases was 14.5 MJ/kgpine, which is a 77% energy conversion from solid biomass to a gaseous fuel feed for potential gas turbine applications. This conversion efficiency is comparable with the best conventional gasification of biomass and higher than most published conversion efficiencies for coal gasifiers. Of the energy in the gas from the rich combustion 35% was from the CO, 20% from hydrogen, 35% from hydrocarbons and 10% sensible heat. Ash remained in the rich burning gasification zone. As the biomass is a carbon neutral fuel there is no need to convert the gasified gases to hydrogen, with the associated energy losses

    Dating the Sea of Marmara sediments by a uniform mixing model

    The sedimentation rates and Pb-210 fluxes on sediment surfaces were measured in the north, northwestern and southwestern parts of the Sea of Marmara. Each core had varying thickness of constant Pb-210 activity regions followed by a decreasing part with sediment depth. The sedimentation rates of the samples collected from the Bosporus and the Dardanelles could not be analysed due to the homogenization of activity in the strong currents of these straits. A uniform mixing model is proposed for the simultaneous analysis of sedimentation rates, Pb-210 fluxes and mixing depths from the experimental data. In this model, the parameters were obtained by minimizing the multi-dimensional parameter space using a grid search algorithm. The Pb-210 fluxes were found to be about 0.048 Bq cm(-2) year(-1) for all sampling sites. The mass sedimentation rates were 0.19 and 0.073 g cm(-2) year(-1) at the shelves of the Bosporus and the Dardanelles and 0.055 and 0.064 g cm(-2) year(-1) in the northwestern and middle northern basins, respectively, of the Sea of Marmara.
