    Internet-based psychoeducation for bipolar disorder: a qualitative analysis of feasibility, acceptability and impact

    <p>Background: In a recent exploratory randomised trial we found that a novel, internet-based psychoeducation programme for bipolar disorder (Beating Bipolar) was relatively easy to deliver and had a modest effect on psychological quality of life. We sought to explore the experiences of participants with respect to feasibility, acceptability and impact of Beating Bipolar.</p> <p>Methods: Participants were invited to take part in a semi-structured interview. Thematic analysis techniques were employed; to explore and describe participants’ experiences, the data were analysed for emerging themes which were identified and coded.</p> <p>Results: The programme was feasible to deliver and acceptable to participants where they felt comfortable using a computer. It was found to impact upon insight into illness, health behaviour, personal routines and positive attitudes towards medication. Many participants regarded the programme as likely to be most beneficial for those recently diagnosed.</p> <p>Conclusions: An online psychoeducation package for bipolar disorder, such as Beating Bipolar, is feasible and acceptable to patients, has a positive impact on self-management behaviours and may be particularly suited to early intervention. Alternative (non-internet) formats should also be made available to patients.</p&gt

    Nicotine-Mediated ADP to Spike Transition: Double Spiking in Septal Neurons

    The majority of neurons in lateral septum (LS) are electrically silent at resting membrane potential. Nicotine transiently excites a subset of neurons and occasionally leads to long lasting bursting activity upon longer applications. We have observed simultaneous changes in frequencies and amplitudes of spontaneous action potentials (AP) in the presence of nicotine. During the prolonged exposure, nicotine increased numbers of spikes within a burst. One of the hallmarks of nicotine effects was the occurrences of double spikes (known also as bursting). Alignment of 51 spontaneous spikes, triggered upon continuous application of nicotine, revealed that the slope of after-depolarizing potential gradually increased (1.4 vs. 3 mV/ms) and neuron fired the second AP, termed as double spiking. A transition from a single AP to double spikes increased the amplitude of after-hyperpolarizing potential. The amplitude of the second (premature) AP was smaller compared to the first one, and this correlation persisted in regard to their duration (half-width). A similar bursting activity in the presence of nicotine, to our knowledge, has not been reported previously in the septal structure in general and in LS in particular

    Modulation of HCN channels in lateral septum by nicotine

    The effects of addictive drugs most commonly occur via interactions with target receptors. The same is true of nicotine and its multiple receptors in a variety of cell types. However, there are also side effects for given substances that can dramatically change cellular, tissue, organ, and organism functions. In this study, we present evidence that nicotine possesses such properties, and modulates neuronal excitability. We recorded whole-cell voltages and currents in neurons situated in the dorsal portion of the lateral septum in acute coronal brain slices of adult rats. Our experiments in the lateral septum revealed that nicotine directly affects HCN – hyperpolarization-activated cyclic nucleotide gated non-selective cation channels. We demonstrate that nicotine effects persist despite the concurrent application of nicotinic acetylcholine receptors’ antagonists – mecamylamine, methyllycaconitine, and dihydro-βerythroidine. These results are novel in regard to HCN channels in the septum, in general, and in their sensitivity to nicotine, in particular

    REDEPEND-REACT: an architecture analysis tool

    The i* approach includes the construction of strategic dependency models (SD) developed to model information systems that can be applied in a wide variety of contexts. We are interested in the formulation of metrics over i* models to be used in components system architectures analysis. These metrics are defined to analyse the models with respect to some properties that are interesting for the system being modelled, such as security, efficiency or accuracy. Metrics are defined in terms of the actors and dependencies of the models and the results of their evaluation are used to inform multiple component selection. As the definition and evaluation of the property metrics are cumbersome activities, the need of tool support arises. Thus, we have enlarged REDEPEND, a Microsoft Visio plug-in that graphically models system goals using the i* formalism, to support the definition and evaluation of architectural properties obtaining the REDEPEND-REACT prototype.Peer ReviewedPostprint (published version

    Mixed states: Still a modern psychopathological syndrome?

    The aim of this review is to evaluate whether the DSM-5 concept of mixed features \u201cspecifier\u201d provides a definition that reflects the richness and multiplicity of this psychopathological picture pointing out the historical development, clinical concepualisation and proposed therapeutic approach to mixed states. We review and discuss the recent evidence on the presence of mixed features during mania and depression and summarise findings on the conceptualisation of mixed states. Electronic searches of all English-language papers were performed in the MEDLINE and PUBMED database using and cross-listing key words: mixed state, mixed features, bipolar disorder, major depressive disorder, mania, hypomania, depression. The mixed categorical-dimensional concept used in the DSM- 5 broadens the concept of mixed episodes, introducing substantial changes to the diagnosis of mixed states. This definition appears more appropriate for less severe forms of mixed states presenting clear and detectable mood symptoms with evident improvement compared to the DSM-IV, as the possibility of classifying depression \u201cwith mixed features\u201d. The transition from the classical definition of mixed states to the one reported in the DSM-5 has determined a complex modification of the concept of mixed state. The DSM-IV-TR description, based on the co-presence of symptoms of opposite polarity, was extremely reductive and did not capture the sub-syndromal symptoms of the opposite pole experienced in bipolar and major depressive disorders. The DSM-5 definition of mixed features \u201cspecifier\u201d represents a valid tool to improve the recognition and proper treatment of bipolar mixed patients, reducing misdiagnosis and mistreatment associated with chronic and repetitive exposure to antidepressants and sedatives, although the mixed categorical-dimensional concept does not adequately reflect some overlapping mood criteria, such as mood lability, irritability and psychomotor agitation

    A goal-based round-trip method for system development

    In most cases information system development can be seen as an exercise of business process reengineering, either because it automates some human-based processes or because a legacy system is going to be replaced. From this point of view, we can say that the specification of the system-to-be goes from the observation and analysis of the current system to the specification of the system-to-be, going through the construction and evaluation of alternatives. Goal-oriented models are a valuable formalism to support the strategic analysis of the current process. In this paper, we propose a method supporting that round-trip engineering process, focusing in the prescriptive construction of strategic i* models and the systematic generation of alternatives. Several requirements engineering techniques are used in order to model the existing process, which allow a reliable generation and evaluation of alternatives as well as the reuse of strategic knowledge for information system development.Peer ReviewedPostprint (author's final draft

    Modelling the influence of RKIP on the ERK signalling pathway using the stochastic process algebra PEPA

    This paper examines the influence of the Raf Kinase Inhibitor Protein (RKIP) on the Extracellular signal Regulated Kinase (ERK) signalling pathway [5] through modelling in a Markovian process algebra, PEPA [11]. Two models of the system are presented, a reagent-centric view and a pathway-centric view. The models capture functionality at the level of subpathway, rather than at a molecular level. Each model affords a different perspective of the pathway and analysis. We demonstrate the two models to be formally equivalent using the timing-aware bisimulation defined over PEPA models and discuss the biological significance

    Complex organic molecules in comets C/2012 F6 (Lemmon) and C/2013 R1 (Lovejoy): detection of ethylene glycol and formamide

    A spectral survey in the 1 mm wavelength range was undertaken in the long-period comets C/2012 F6 (Lemmon) and C/2013 R1 (Lovejoy) using the 30 m telescope of the Institut de radioastronomie millim\'etrique (IRAM) in April and November-December 2013. We report the detection of ethylene glycol (CH2_2OH)2_2 (aGg' conformer) and formamide (NH2_2CHO) in the two comets. The abundances relative to water of ethylene glycol and formamide are 0.2-0.3% and 0.02% in the two comets, similar to the values measured in comet C/1995 O1 (Hale-Bopp). We also report the detection of HCOOH and CH3_3CHO in comet C/2013 R1 (Lovejoy), and a search for other complex species (methyl formate, glycolaldehyde).Comment: Accepted for publication as a Letter in Astronomy and Astrophysic
