Benchmarking factor selection and sensitivity: a case study with nursing courses
There is an increasing requirement in higher education (HE) worldwide to deliver excellence. Benchmarking is widely used for this purpose, but methodological approaches to the creation of benchmark metrics vary greatly. Approaches require selection of factors for inclusion and subsequent calculation of benchmarks for comparison. We describe an approach using machine learning to select input factors based on their value to predict completion rates of nursing courses. Data from over 36,000 students, from nine institutions over three years were included and weighted averages provided a dynamic baseline for year on year and within year comparisons between institutions. Anonymised outcomes highlight the variation in benchmarked performances between institutions and we demonstrate the value of accompanying sensitivity analyses. Our methods are appropriate worldwide, for many forms of data and at multiple scales of enquiry. We discuss our results in the context of HE management, highlighting the value of scrutinising benchmark calculations

Effects of the Fluoroquinolones Moxifloxacin and Levofloxacin on the QT Subintervals: Sex Differences in Ventricular Repolarization.
Women are associated with longer electrocardiographic QT intervals and increased proarrhythmic risks of QT-prolonging drugs. The purpose of this study was to characterize the differences in cardiac electrophysiology between moxifloxacin and levofloxacin in men and women and to assess the balance of inward and outward currents through the analysis of QT subintervals. Data from 2 TQT studies were used to investigate the impact of moxifloxacin (400 mg) and levofloxacin (1000 and 1500 mg) on QT subintervals using algorithms for measurement of J-Tpeak and Tpeak -Tend intervals. Concentration-effect analyses were performed to establish potential relationships between the ECG effects and the concentrations of the 2 fluoroquinolones. Moxifloxacin was shown to be a more potent prolonger of QT interval corrected by Fredericia (QTcF) and had a pronounced effect on J-Tpeak c. Levofloxacin had little effect on J-Tpeak c. For moxifloxacin, the concentration-effect modeling showed a greater effect for women on QTcF and J-Tpeak c, whereas for levofloxacin the inverse was true: women had smaller QTcF and J-Tpeak c effects. The different patterns in repolarization after administration of both drugs suggested a sex difference, which may be related to the combined IKs and IKr inhibitory properties of moxifloxacin versus IKr suppression only of levofloxacin. The equipotent inhibition of IKs and IKr appears to affect women more than men. Sex hormones are known to influence cardiac ion channel expression and differences in QT duration. Differences in IKr and IKs balances, influenced by sex hormones, may explain the results. These results support the impact of sex differences on the cardiac safety assessment of drugs
Students’ perceptions of their learning experiences: A repeat regional survey of healthcare students
Background Student experience is an international concern and recent research has focused on initiatives to improve students’ learning experiences and ultimately reduce attrition levels. Objective To determine similarities and differences between students’ perceptions of their learning experiences between 2011 and 2015 in relation to campus-based learning, placement-based learning and personal circumstances. Design A repeat online survey in 2011 and 2015; using a questionnaire developed from thematic analysis of narrative interviews with a subsample of the target population. Settings Nine universities in the North West of England. Participants A total of 1080 students completed the survey in 2011 and 1983 students in 2015 from a target population of all students studying on commissioned pre-registration healthcare education programmes. Methods An online survey was made available to all undergraduate students studying on Health Education funded programmes within the region and survey respondents were invited to give demographic information and rate their agreement to statements on four-point Likert-type responses. Results Responses to a repeat survey of healthcare studying in the North West of England in 2015 were strikingly similar overall to those of an original 2011 survey. Although the students were positive overall about their experiences, a number were dissatisfied with some aspects of their experiences - particularly in relation to initial support on campus and whilst studying on placement. Four years on from the original survey, despite a considerable investment in improving students’ experiences across the region, there appears to be little change in students’ perceptions of their learning experiences Conclusion In the short-term monitoring of student experience needs to be continued; and links to attrition (potential or actual) noted and acted upon. However, given that attrition from these courses has been a long-term problem and the complexity of its resolution a recurrent finding in the literature; new ways of framing and resolving the problem need to be considered
Rupture by damage accumulation in rocks
The deformation of rocks is associated with microcracks nucleation and
propagation, i.e. damage. The accumulation of damage and its spatial
localization lead to the creation of a macroscale discontinuity, so-called
"fault" in geological terms, and to the failure of the material, i.e. a
dramatic decrease of the mechanical properties as strength and modulus. The
damage process can be studied both statically by direct observation of thin
sections and dynamically by recording acoustic waves emitted by crack
propagation (acoustic emission). Here we first review such observations
concerning geological objects over scales ranging from the laboratory sample
scale (dm) to seismically active faults (km), including cliffs and rock masses
(Dm, hm). These observations reveal complex patterns in both space (fractal
properties of damage structures as roughness and gouge), time (clustering,
particular trends when the failure approaches) and energy domains (power-law
distributions of energy release bursts). We use a numerical model based on
progressive damage within an elastic interaction framework which allows us to
simulate these observations. This study shows that the failure in rocks can be
the result of damage accumulation
A portable and affordable extensional rheometer for field testing
Extensional shear testing is often needed to characterise the behaviour of complex fluids found in industry and nature. Traditional extensional rheometers are typically expensive, fragile and heavy and are only suited to making measurements in a laboratory environment. For some applications, it is necessary to make in situ rheological measurements where, for example, fluid properties change rapidly over time or where laboratory facilities are unavailable. This paper reports the development and validation of an inexpensive, lightweight and robust 'open source' extensional rheometer, Seymour II. Validation was carried out experimentally and computationally. Measurements on a Newtonian fluid (492 mPa s Brookfield silicone oil) yielded results of 510 ± 51 mPa s; these are comfortably within the range of ±10% which other authors have quoted for extensional techniques using laboratory rheometers. Comparison of the observed filament thinning dynamics to those obtained using computational fluid dynamics (CFD) gave good qualitative agreement. Use of Seymour II at the University of Cambridge Botanic Gardens revealed that the mucilage of the 'crane flower', , was a viscoelastic fluid whose extensional response could be described by a two-mode Giesekus equation. Albert Pierrepoint and the cultural persona of the twentieth-century hangman
Albert Pierrepoint was Britain’s most famous 20th-century hangman. This article utilises diverse sources in order to chart his public representation, or cultural persona, as hangman from his rise to prominence in the mid-1940s to his portrayal in the biopic Pierrepoint(2005). It argues that Pierrepoint exercised agency in shaping this persona through publishing his autobiography and engagement with the media, although there were also representations that he did not influence. In particular, it explores three iterations of his cultural persona – the Professional Hangman, the Reformed Hangman and the Haunted Hangman. Each of these built on and reworked historical antecedents and also communicated wider understandings and contested meanings in relation to capital punishment. As a hangman who remained in the public eye after the death penalty in Britain was abolished, Pierrepoint was an important, authentic link to the practice of execution and a symbolic figure in debates over reintroduction. In the 21st century, he was portrayed as a victim of the ‘secondary trauma’ of the death penalty, which resonated with worldwide campaigns

Signal transduction underlying the control of urinary bladder smooth muscle tone by muscarinic receptors and β-adrenoceptors
The normal physiological contraction of the urinary bladder, which is required for voiding, is predominantly mediated by muscarinic receptors, primarily the M3 subtype, with the M2 subtype providing a secondary backup role. Bladder relaxation, which is required for urine storage, is mediated by β-adrenoceptors, in most species involving a strong β3-component. An excessive stimulation of contraction or a reduced relaxation of the detrusor smooth muscle during the storage phase of the micturition cycle may contribute to bladder dysfunction known as the overactive bladder. Therefore, interference with the signal transduction of these receptors may be a viable approach to develop drugs for the treatment of overactive bladder. The prototypical signaling pathway of M3 receptors is activation of phospholipase C (PLC), and this pathway is also activated in the bladder. Nevertheless, PLC apparently contributes only in a very minor way to bladder contraction. Rather, muscarinic-receptor-mediated bladder contraction involves voltage-operated Ca2+ channels and Rho kinase. The prototypical signaling pathway of β-adrenoceptors is an activation of adenylyl cyclase with the subsequent formation of cAMP. Nevertheless, cAMP apparently contributes in a minor way only to β-adrenoceptor-mediated bladder relaxation. BKCa channels may play a greater role in β-adrenoceptor-mediated bladder relaxation. We conclude that apart from muscarinic receptor antagonists and β-adrenoceptor agonists, inhibitors of Rho kinase and activators of BKCa channels may have potential to treat an overactive bladder
