    Do paranoid delusions exist on a continuum with subclinical paranoia? A multi-method taxometric study

    Background There is widespread interest in whether psychosis exists on a continuum with healthy functioning. Previous research has implied that paranoia, a common symptom of psychosis, exists on a continuum but this has not been investigated using samples including both patients and non-patients and up-to-date taxometric methods. Aim To assess the latent structure of paranoia in a diverse sample using taxometric methods. Method We obtained data from 2836 participants, including the general population as well as at-risk mental state and psychotic patients using the P-scale of the Paranoia and Deservedness Scale. Data were analysed using three taxometric procedures, MAMBAC, MAXEIG and L-MODE (Ruscio, 2016), and two sets of paranoia indicators (subscales and selected items from the P scale), including and excluding the patient groups. Results Eleven of the twelve analyses supported a dimensional model. Using the full sample and subscales as indicators, the MAMBAC analysis was ambiguous. Overall, the findings converged on a dimensional latent structure. Conclusions A dimensional latent structure of paranoia implies that the processes involved in sub-clinical paranoia may be similar to those in clinical paranoia

    Epidemiology of bloodstream infections in the burn icu

    The study included data on 80 adult patients hospitalized in the ICU of the Yekaterinburg Burn Center in 2018 - 2019, who underwent at least one bacteriological blood test. In a small observational study, bloodstream infection did not lead to an increase in mortality, but is an important factor in increasing the incidence (length of hospitalization and ICU). In 70,5% of cases, the source of bloodstream infections are P. aeruginosa and S. aureus.     Primary objectives: It has been proposed that dissociation plays a role in the aetiology of hallucinatory experiences. The present study examined the relationship between reports of auditory hallucinations and dissociative experiences in the daily lives of patients with psychosis. The influence of everyday stressors on dissociation and on the hypothesised relationship between dissociation and hallucinatory experiences was also investigated. Finally, this study examined the association between hallucinations and other non-dissociative dysfunctional strategies used to suppress unpleasant mental events (i.e. experiential avoidance). Method: Forty-two patients with a diagnosis of schizophrenia spectrum disorder and 23 healthy controls were studied for six days using the Experience Sampling Method (ESM), a structured self-assessment diary technique. Results: Patients who hallucinated during the assessment period (n = 21) reported elevated levels of dissociation compared to non-hallucinating patients and healthy controls. Within the hallucinating patient group, auditory hallucinations were significantly predicted by both dissociation and experiential avoidance, although only the effect of dissociation remained significant after controlling for comorbid paranoia. Dissociation predicted the occurrence of auditory hallucinations especially under high stress. Hallucinating patients also reported a greater increase in dissociation in response to minor daily life stress compared to clinical and non-clinical controls. Conclusions: These results further support the link between auditory hallucinations and experiences of dissociative detachment, and might inform future investigations into the mechanisms underlying this association. Interventions designed to reduce dissociation should be studied as potential treatments for auditory hallucinations

    Objective: The experience sampling method (ESM) represents a valuable way of assessing clinical phenomena in real world settings and across time. Despite its theoretical advantages, using this methodology in psychiatric populations is challenging. This paper acts as a guide to researchers wishing to employ this approach when investigating mental illness. Method: The contents represent the opinions of researchers around the United Kingdom and the Netherlands who are experienced at using the ESM. Results: In ESM studies, participants are required to fill in questions about their current thoughts, feelings and experiences when prompted by an electronic device (e.g. a wristwatch, PDA). Entries are typically made at fixed or random intervals over 6 days. This article outlines how to design and validate an ESM diary. We then discuss which sampling procedure to use and how to increase compliance through effective briefing and telephone sessions. Debriefing, data management and analytical issues are considered, before suggestions for future clinical uses of the ESM are made. Conclusion: The last decade has seen an increase in the number of studies employing the ESM in clinical research. Further research is needed to examine the optimal equipment and procedure for different clinical groups
