    Effects of forward head and rounded shoulder posture on scapular kinematics, muscle activity, and shoulder coordination

    Forward head and rounded shoulder posture (FHRSP) has been identified as a potential risk factor for the development of shoulder pain. The mechanism through which forward head and rounded shoulder can facilitate shoulder injury is not well understood. Altered scapular kinematics, muscle activity, and shoulder joint coordination due to FHRSP may lead to the development of shoulder pain. However, there is little evidence to support the influence of FHRSP on scapular kinematics, muscle activity, and shoulder joint coordination. Therefore, the purpose of this study was to compare scapular kinematics, muscle activity, and shoulder joint coordination in individuals with and without FHRSP. Eighty volunteers without shoulder pain were classified as having FHRSP or ideal posture. An electromagnetic tracking system together with hard-wired surface electromyography was used to collect three-dimensional scapular kinematics concurrently with muscle activity of the upper and lower trapezius as well as the serratus anterior during iv loaded shoulder flexion and an overhead reaching task. Separate mixed model analyses of variance were used to compare three dimensional scapular kinematics, muscle activity, and shoulder joint coordination during the ascending and descending phases of the loaded flexion and overhead reaching tasks. Individuals with FHRSP displayed significant increases in scapular upward rotation, internal rotation, and anterior tipping during the loaded flexion and reaching tasks. Significant decreases in serratus anterior muscle activation during the ascending phase of the flexion and reaching tasks were also noted. These scapular kinematic and muscle activation patterns are similar to those reported in individuals with shoulder pain. Additionally, uncoupled scapulohumeral coordination strategies were also observed for scapular upward rotation and anterior tipping. The observed uncoupling suggests an out-of-phase relationship between the humerus and these scapular rotations in individuals with FHRSP. Considered together, these results suggest FHRSP influences scapular kinematics, muscle activity, and shoulder joint coordination. FHRSP and its effects on shoulder kinematics, muscle activity, and shoulder coordination should be examined as a potential risk factor in the development of shoulder pain. Assessment and treatment of FHRSP should be considered in the prevention and interventions of shoulder pain

    Probabilistic Risk Analysis (PRA) of a Mobile Offshore Drilling Unit (MODU) Dynamic Positioning System (DPS)

    Probabilistic Risk Assessment (PRA) has been utilized by NASA in a variety of space oriented projects. It has served as one of the primary risk identification and ranking tools. Recent developments in the oil and gas industry have presented opportunities for NASA to lend their PRA expertise to both ongoing and developmental projects within the industry. As a result, NASA has entered into an agreement with Anadarko Petroleum Company (APC) to collaboratively develop PRAs for different aspects of the subsea drilling and completion process of well development. This paper documents how PRA was applied to estimate the probability that a Mobile Offshore Drilling Unit (MODU) equipped with a generically configured Dynamic Positioning System (DPS) loses location and needs to initiate an emergency disconnect. Since this project was in essence a pilot project, the PRA described in this paper is intended to be generic such that the vessel meets the general requirements of an International Maritime Organization (IMO) Maritime Safety Committee (MSC)/Circ. 645 Class 3 dynamically positioned vessel. The results of this analysis are not intended to be applied to any specific drilling vessel, although provisions were made to allow the analysis to be configured to a specific vessel if required

    2009 Space Shuttle Probabilistic Risk Assessment Overview

    Loss of a Space Shuttle during flight has severe consequences, including loss of a significant national asset; loss of national confidence and pride; and, most importantly, loss of human life. The Shuttle Probabilistic Risk Assessment (SPRA) is used to identify risk contributors and their significance; thus, assisting management in determining how to reduce risk. In 2006, an overview of the SPRA Iteration 2.1 was presented at PSAM 8 [1]. Like all successful PRAs, the SPRA is a living PRA and has undergone revisions since PSAM 8. The latest revision to the SPRA is Iteration 3. 1, and it will not be the last as the Shuttle program progresses and more is learned. This paper discusses the SPRA scope, overall methodology, and results, as well as provides risk insights. The scope, assumptions, uncertainties, and limitations of this assessment provide risk-informed perspective to aid management s decision-making process. In addition, this paper compares the Iteration 3.1 analysis and results to the Iteration 2.1 analysis and results presented at PSAM 8

    Long-Term Health Effects of Neutering Dogs: Comparison of Labrador Retrievers with Golden Retrievers

    Our recent study on the effects of neutering (including spaying) in Golden Retrievers in markedly increasing the incidence of two joint disorders and three cancers prompted this study and a comparison of Golden and Labrador Retrievers. Veterinary hospital records were examined over a 13-year period for the effects of neutering during specified age ranges: before 6 mo., and during 6–11 mo., year 1 or years 2 through 8. The joint disorders examined were hip dysplasia, cranial cruciate ligament tear and elbow dysplasia. The cancers examined were lymphosarcoma, hemangiosarcoma, mast cell tumor, and mammary cancer. The results for the Golden Retriever were similar to the previous study, but there were notable differences between breeds. In Labrador Retrievers, where about 5 percent of gonadally intact males and females had one or more joint disorders, neutering at ,6 mo. doubled the incidence of one or more joint disorders in both sexes. In male and female Golden Retrievers, with the same 5 percent rate of joint disorders in intact dogs, neutering at ,6 mo. increased the incidence of a joint disorder to 4–5 times that of intact dogs. The incidence of one or more cancers in female Labrador Retrievers increased slightly above the 3 percent level of intact females with neutering. In contrast, in female Golden Retrievers, with the same 3 percent rate of one or more cancers in intact females, neutering at all periods through 8 years of age increased the rate of at least one of the cancers by 3–4 times. In male Golden and Labrador Retrievers neutering had relatively minor effects in increasing the occurrence of cancers. Comparisons of cancers in the two breeds suggest that the occurrence of cancers in female Golden Retrievers is a reflection of particular vulnerability to gonadal hormone removal

    Stability of Extemporaneously Prepared Lansoprazole Suspension at Two Temperatures

    OBJECTIVE The purpose of this study was to examine the stability of a generic lansoprazole product in a 3 mg/mL sodium bicarbonate suspension under room temperature and refrigerated conditions. METHODS Lansoprazole suspensions (3 mg/mL) were prepared in triplicate using an 8.4% sodium bicarbonate vehicle for each storage condition (room temperature and refrigerated). During 1 month, samples from each replicate were periodically removed and analyzed for lansoprazole concentration by liquid chromatography–tandem mass spectrometry (LC-MS/MS). Each sample was spiked with 10 mg/L omeprazole to serve as the internal standard. A positive electrospray LC-MS/MS method was validated over the calibration range of 5 to 25 mg/L using Food and Drug Administration Guidance. The identities of the analyte and internal standard in the samples were verified by monitoring the MS/MS transitions of m/z 370 to m/z 252 and m/z 346 to m/z 198 for lansoprazole and omeprazole, respectively. Additionally, the pH of the suspensions was monitored throughout the study. RESULTS The stability of lansoprazole in the oral sodium bicarbonate suspension under refrigeration is compromised prior to what has been previously reported in the literature. Samples kept at room temperature lost \u3e10% of the lansoprazole after 48 hours compared with the refrigerated samples, which maintained integrity up to 7 days. No statistically significant difference was found between the pH of the room temperature and refrigerated suspension samples, indicating that this factor is not the cause for the differences in stability at these two conditions. CONCLUSIONS This study suggests that the extemporaneously compounded lansoprazole oral suspension prepared in 8.4% sodium bicarbonate should not be stored in plastic oral syringes longer than 48 hours at room temperature and no longer than 7 days when refrigerated. These data indicate an expiration time earlier than that previously reported for the refrigerated product (14 days)
