    Background: Malnutrition among hospitalized patients increases length of stay (LOS) and carries extra hospitalization costs. Objective: To review the impact of malnutrition on hospital LOS and costs in Europe. Methods: PubMed and Google Scholar search. All articles from January 2004 until November 2014 were identified. Reference lists of relevant articles were also manually searched. Results: Ten studies on LOS and nine studies on costs were reviewed. The methods used to assess malnutrition and to calculate costs differed considerably between studies. Malnutrition led to an increased LOS ranging from 2.4 to 7.2 days. Among hospitalized patients, malnutrition led to an additional individual cost ranging between 1640 V and 5829 V. At the national level, the costs of malnutrition ranged between 32.8 million V and 1.2 billion V. Expressed as percentage of national health expenditures, the values ranged between 2.1% and 10%. Conclusions: In Europe, malnutrition leads to an increase in LOS and in hospital costs, both at the individual and the national level. Standardization of methods and results reported is needed to adequately compare results between countries. © 2015 European Society for Clinical Nutrition and Metabolism. Published by Elsevier Ltd.     Revisited: Therapeutic and toxic blood concentrations of more than 1100 drugs and other xenobiotics.

    In order to assess the significance of drug/substance levels measured in intensive care medicine and clinical and forensic toxicology as well as for therapeutic drug monitoring, it is essential that a comprehensive collection of data is readily available. We revisited and expanded our 2012 compilation of therapeutic and toxic plasma concentration ranges as well as half-lives of now more than 1100 drugs and other xenobiotics.Data have been abstracted from original papers, text books, and previous compilations and have been completed with data collected in our own forensic and clinical toxicology laboratories. We compiled the data presented in the table and the corresponding annotations over the past 30+ years. A previous compilation was completely double-checked, revised, and updated, if necessary. In addition, more than 200 substances, especially drugs who have been introduced since 2012 to the market as well as illegal drugs and other xenobiotics which became known to cause intoxications were added. We carefully referenced all data. Moreover, the annotations providing details were updated and revised, when necessary.For more than 1100 drugs and other xenobiotics, therapeutic ("normal") and, if data was available, toxic, and comatose-fatal plasma/blood concentrations as well as elimination half-lives were compiled in a table.In case of intoxications, the blood concentration of the substance and/or metabolite better predicts the clinical severity of the case when compared to the assumed amount and time of ingestion. Comparing and contrasting the clinical case against the data provided, including the half-life, may support the decision for or against further intensive care. In addition, the data provided are useful for the therapeutic monitoring of pharmacotherapies, to facilitate the diagnostic assessment and monitoring of acute and chronic intoxications as well as to support forensic and clinical expert opinions

    Wooden pallets exceed furniture and other solid wood products as the largest single use of sawn hardwood logs in the USA. Most wooden pallets are constructed from two types of pallet parts (Figure 1): (1) stringers—the structural center members that support the pallet load and (2) deckboards—the top and bottom facing members that provide dimensional stability and product placement. There are many variants of this basic design, but most pallets contain solid wood components that are produced from lumber or from the center cant material of logs. Cant material has a high percentage of defect area and is generally not highly valuable for other solid wood products. Therefore, the pallet manufacturing industry must make use of low-quality raw materials and yet produce a product that remains in service for many trips

    Classifying Defects in Pallet Stringers by Ultrasonic Scanning

    Detecting and classifying defects are required to grade and sort pallet parts. Use of quality parts can extend the life cycle of pallets and can reduce long-term cost. An investigation has been carried out to detect and classify defects in yellow-poplar (Liriodendron tulipifera, L.) and red oak (Quercus rubra, L.) stringers using ultrasonic scanning. Data were collected for sound and unsound knots, bark pockets, decay, holes, and wane using rolling transducers in a pitch-catch arrangement. Data from eight ultrasonic variables—energy, pulse length, time of flight (TOF)-amplitude, TOF-energy, TOF-centroid, energy value, energy pulse value, and peak frequency—were used to classify defects. Three different types of classifiers were used to categorize defects—a multi-layer perceptron network (MLP), a probabilistic neural network (PNN), and a k-nearest neighbor (KNN) classifier. Mean values for the energy variables demonstrated statistically significant differences between clear wood and defects and among defect types. Mean values for the TOF variables did not differ significantly between clear wood and knots. All three types of classifiers were able to distinguish defected from clear wood in oak with accuracies above 95%; accuracies for yellow-poplar were somewhat lower for the MLP and PNN classifiers. Among the defect classes, decay exhibited the highest recognition rate for both yellow-poplar and oak. Wane and holes in oak were readily confused owing to their common loss of transducer contact. Overall accuracy at the data-point level varied from 69-78%. Simple post-processing operations are expected to improve that substantially. Based on accuracy performance alone, the MLP and KNN appear equally preferable for this task

    Does the Isotropy of the CMB Imply a Homogeneous Universe? Some Generalised EGS Theorems

    We demonstrate that the high isotropy of the Cosmic Microwave Background (CMB), combined with the Copernican principle, is not sufficient to prove homogeneity of the universe -- in contrast to previous results on this subject. The crucial additional factor not included in earlier work is the acceleration of the fundamental observers. We find the complete class of irrotational perfect fluid spacetimes admitting an exactly isotropic radiation field for every fundamental observer and show that are FLRW if and only if the acceleration is zero. While inhomogeneous in general, these spacetimes all possess three-dimensional symmetry groups, from which it follows that they also admit a thermodynamic interpretation. In addition to perfect fluids models we also consider multi-component fluids containing non-interacting radiation, dust and a quintessential scalar field or cosmological constant in which the radiation is isotropic for the geodesic (dust) observers. It is shown that the non-acceleration of the fundamental observers forces these spacetimes to be FLRW. While it is plausible that fundamental observers (galaxies) in the real universe follow geodesics, it is strictly necessary to determine this from local observations for the cosmological principle to be more than an assumption.     Bayesian analysis of spatially distorted cosmic signals from Poissonian data

    Reconstructing the matter density field from galaxy counts is a problem frequently addressed in current literature. Two main sources of error are shot noise from galaxy counts and insufficient knowledge of the correct galaxy position caused by peculiar velocities and redshift measurement uncertainty. Here we address the reconstruction problem of a Poissonian sampled log-normal density field with velocity distortions in a Bayesian way via a maximum a posteriory method. We test our algorithm on a 1D toy case and find significant improvement compared to simple data inversion. In particular, we address the following problems: photometric redshifts, mapping of extended sources in coded mask systems, real space reconstruction from redshift space galaxy distribution and combined analysis of data with different point spread functions.Comment: 19 pages, 10 figures, accepte
