    W styczniu 1994 roku w Krakowskim Centrum Onkologii rozpoczęto prospektywne badanie II fazy celem oceny skuteczności i toksyczności równoczesnego leczenia chemicznego wg schematu EP (etopozyt + cysplatyna) i napromienianiem chorych na DKRP w postaci ograniczonej. Do lutego 1998 roku zakończono leczenie 32 chorych. W całej grupie uzyskano 21% 3-letnich aktualizowanych przeżyć bezobjawowych. Przeprowadzono analizę wpływu na wyniki leczenia takich czynników jak: profilaktyczne napromienianie mózgu, opóźnienie radioterapii w stosunku do leczenia chemicznego, liczba serii chemioterapii, zaawansowanie kliniczne według TNM, obecność regresji potwierdzonej w badaniu histologicznym. Uzyskane wyniki wstępne wskazują na konieczność dalszych modyfikacji protokołu leczenia

    The Mechanical and material properties of 316LN austenitic stainless steel for the fusion application in cryogenic temperatures

    Due to the constant increase of claims for all materials used in superconducting magnets in "magnetic fusion reactors", the article deals with the possibilities of increasing the mechanical properties of austenitic stainless steel tested at cryogenic conditions that ensure the transport of Helium to magnets. The aim of the experimental plan was to increase the mechanical properties of the steel grade 316LN tested at 4.2K from the original value Steel A: YS = 1045 MPa, UTS = 1528 MPa, A = 33% to the value of YS = 1204 MPa,UTS = 1642 MPa, A = 34% and Steel B: YS = 1173 MPa, UTS = 1541 MPa, A = 28% to the value of YS = 1351 MPa, UTS = 1645 MPa, A = 17%. The increase in mechanical properties of the steel grade under examination has been made by means of heat processing in the conditions of annealing: Th1 = 625 ° C / th1 = 696 h. The mechanical properties of steel were evaluated using static tension tests at 4,2 K. The samples were placed in a cryostat filled with liquid helium. Except for the mechanical properties, there were also evaluated structural changes depending on the conditions of heat processing by light optical microscopy and EBSD (Electron Backscatter Diffraction).     Potential and current distribution in strongly anisotropic Bi(2)Sr(2) CaCu(2)O(8) single crystals at current breakdown

    Experiments on potential differences in the low-temperature vortex solid phase of monocrystalline platelets of superconducting Bi(2)Sr(2)CaCu(2)O(8) (BSCCO) subjected to currents driven either through an "ab" surface or from one such surface to another show evidence of a resistive/nonresistive front moving progressively out from the current contacts as the current increases. The depth of the resistive region has been measured by a novel in-depth voltage probe contact. The position of the front associated with an injection point appears to depend only on the current magnitude and not on its withdrawal point. It is argued that enhanced nonresistive superconducting anisotropy limits current penetration to less than the London length and results in a flat rectangular resistive region with simultaneous "ab" and "c" current breakdown which moves progressively out from the injection point with increasing current. Measurements in "ab" or "c" configurations are seen to give the same information, involving both ab-plane and c-axis conduction properties.Comment: 9 pages, 13 figures, typo error corrected, last section was refine

    Profiles of Diabetic Ketoacidosis in Multiethnic Diabetic Population of Malaysia

    Purpose: To outline first-time patient profiles of diabetic ketoacidosis (DKA) in the absence of reported incidence and mortality rates of DKA in Malaysian diabetic population.Methods: A retrospective cross-sectional study was designed and all medical records of patients with a discharge note of DKA were reviewed. Admissions from January 2009 to December 2011 were included. Data were analyzed in terms of socio-demographic variables in order to provide incidence and mortality rates. Medical history, as well as physical and biochemical characteristics were analyzed to report epidemiology of DKA patients.Results: Out of a total of 207 admissions for DKA, 132 were selected for the present study. Female (62.9 %), Malay ethnic (47.0 %) and the elderly (45.1 years and above) contributed most to DKA episodes. Type 2 diabetes mellitus (51.1 %) patients were prone to develop DKA. Most patients experienced mild to moderate episode of DKA by the time they sought medical attention. Although, there was no significant relationship between chronic co-morbidity and occurrence of DKA, hypertension (54.5 %), dyslipidemia (43.0 %) and cardiac disorders (35.6 %) were, however, the most frequently observed co-morbidities. Non-adherence (43.2 %), sepsis (31.9 %) and respiratory tract infection (12.2 %) were the most encountered precipitating factors for DKA episode. Mortality rate was as high as 17.6 %.Conclusion: With a higher incidence and mortality rate of DKA in Malaysia, the patterns observed in this study seem to be different from those of developed nations.     Headache and pregnancy. a systematic review

    This systematic review summarizes the existing data on headache and pregnancy with a scope on clinical headache phenotypes, treatment of headaches in pregnancy and effects of headache medications on the child during pregnancy and breastfeeding, headache related complications, and diagnostics of headache in pregnancy. Headache during pregnancy can be both primary and secondary, and in the last case can be a symptom of a life-threatening condition. The most common secondary headaches are stroke, cerebral venous thrombosis, subarachnoid hemorrhage, pituitary tumor, choriocarcinoma, eclampsia, preeclampsia, idiopathic intracranial hypertension, and reversible cerebral vasoconstriction syndrome. Migraine is a risk factor for pregnancy complications, particularly vascular events. Data regarding other primary headache conditions are still scarce. Early diagnostics of the disease manifested by headache is important for mother and fetus life. It is especially important to identify "red flag symptoms" suggesting that headache is a symptom of a serious disease. In order to exclude a secondary headache additional studies can be necessary: electroencephalography, ultrasound of the vessels of the head and neck, brain MRI and MR angiography with contrast ophthalmoscopy and lumbar puncture. During pregnancy and breastfeeding the preferred therapeutic strategy for the treatment of primary headaches should always be a non-pharmacological one. Treatment should not be postponed as an undermanaged headache can lead to stress, sleep deprivation, depression and poor nutritional intake that in turn can have negative consequences for both mother and baby. Therefore, if non-pharmacological interventions seem inadequate, a well-considered choice should be made concerning the use of medication, taking into account all the benefits and possible risks

    Experimental and numerical assessment of the effectiveness of FRP-based strengthening configurations for dapped-end RC beams

    This paper presents experimental and numerical assessments of the effectiveness of strengthening dapped-end reinforced concrete beams using externally bonded carbon fiber reinforced polymers (CFRPs). The research was prompted by a real application, in which the dapped-ends of several precast/prestressed concrete beams developed diagonal cracks due to errors during assembly. Hence, the dapped-ends were strengthened on-site using CFRP plates to limit further crack opening. In the empirical phase of the study, four similar specimens were tested: one unstrengthened reference specimen, two strengthened with high-strength CFRP plates, and one with high-modulus CFRP sheets. The specimens strengthened with plates had slightly higher load carrying capacity than the reference element, but failed by debonding, while the specimens strengthened with sheets showed no increase of capacity and failed by the fibers rupturing. Nonlinear finite element analysis of the specimens under the test conditions indicated that: (a) debonding is more likely to occur at the inner end of dapped-ends and (b) the capacity could have been increased by up to 20% if the plates had been mechanically anchored.The research work was partially supported by research Grant type A No. 27688/2005, awarded by the National University Research Council, Romania.
