    Upper eyelid angiolipoma associated friction in the glide zone

    A 74-year-old man was examined for a mass in the left upper eyelid. It was inconspicuous in inspection. There was no visual restriction, however a feeling of friction on the superior orbit during lid movement. Through eyelid crease lid incision, a solitary lesion appeared in the preaponeurotic space that orginated from the upper tarsal plate. Excisional biopsy revealed adipose proliferation with patch form vessels. This case represents the second reported angiolipoma of the eyelid in English literature

    Estimating the properties of ground-waste-brick mortars using DNN and ANN

    Demir, Ilhami/0000-0002-8230-4053WOS: 000456418500009In this study, deep-neural-network (DNN)- and artificial-neural-network (ANN)- based models along with regression models have been developed to estimate the pressure, bending and elongation values of ground-brick (GB)-added mortar samples. This study is aimed at utilizing GB as a mineral additive in concrete in the ratios 0.0%, 2.5%, 5.0%, 7.5%, 10.0%, 12.5% and 15.0%. In this study, 756 mortar samples were produced for 84 different series and were cured in tap water (W), 5% sodium sulphate solution (SS5) and 5% ammonium nitrate solution (AN5) for 7 days, 28 days, 90 days and 180 days. The developed DNN models have three inputs and two hidden layers with 20 neurons and one output, whereas the ANN models have three inputs, one output and one hidden layer with 15 neurons. Twenty-five previously obtained experimental sample datasets were used to train these developed models and to generate the regression equation. Fifty-nine non-training-attributed datasets were used to test the models. When these test values were attributed to the trained DNN, ANN and regression models, the brick-dust pressure as well as the bending and elongation values have been observed to be very close to the experimental values. Although only a small fraction (30%) of the experimental data were used for training, both the models performed the estimation process at a level that was in accordance with the opinions of experts. The fact that this success has been achieved using very little training data shows that the models have been appropriately designed. In addition, the DNN models exhibited better performance as compared with that exhibited by the ANN models. The regression model is a model whose performance is worst and unacceptable; further, the prediction error is observed to be considerably high. In conclusion, ANN- and DNN-based models are practical and effective to estimate these values

    Pulmonary artery banding and venous bidirectional cava-pulmonary shunt for two-stage arterial switch procedure in late referral of patients with transposition of the great arteries and intact ventricular septum: Midterm results

    PubMed ID: 31230611Objective: Two-stage arterial switch operation and left ventricle retraining are necessary for the patients with left ventricle dysfunction and transposition of great vessels with intact ventricular septum (TGA-IVS) who are referred late.Material and methods: Forty-seven patients with the diagnosis of TGA-IVS and left ventricle dysfunction who underwent arterial switch operation in our centre between July 2013 and August 2017 were analysed retrospectively. The inclusion criteria for left ventricle retraining were patients older than 2 months of age at presentation, having an echocardiographic left ventricle mass index of less than 35 g/m, and having an echocardiographic banana-shaped left ventricle geometric appearance. The patients were divided into two groups: pulmonary artery banding and Blalock Taussig shunt were performed as the initial surgical procedure for later arterial switch operation in Group I (n = 19) and pulmonary artery banding and bidirectional cava-pulmonary shunt in Group 2 (n = 28).Results: The average age was found to be 122.3 ± 45.6 days in Group I and 145.9 ± 37.2 days in Group II. There was no statistically significant difference (p = 0.232 versus p = 0.373) between the average left ventricle mass index of the two groups neither before the first stage nor the second stage (26.6 ± 4.8 g/m versus 25.0 ± 4.9 g/m and 70.5 ± 12 g/m versus 673.8 ± 12.0 g/m, respectively). The average time interval for the left ventricle to retrain was 97.7 ± 42.9 days for Group I and 117.3 ± 40.3 days for Group II, significantly lower in Group I (p = 0.027). The time spent in ICU, length of the period during which inotropic support was required, and the duration of hospital stay were significantly higher in Group I (p<0.001, p < 0.001, and p < 0.00, respectively).Conclusion: Pulmonary artery banding and bidirectional cava-pulmonary shunt can be performed as a safe and effective alternative to pulmonary artery banding and arterial Blalock Taussig shunt for patients with TGA-IVS in whom arterial switch operation is needed beyond the neonatal period. This approach involves a shorter hospital stay and fewer post-operative complications. © Cambridge University Press 2019.

    Supracoronary myotomy for myocardial bridges in the setting of hypertrophic cardiomyopathy: off-pump experience

    Myocardial bridges (MB) are rarely observed but well known pathology of the major epicardial coronary arteries which are embedded in the overlying myocardial tissue. it is most frequently found in young patients with hypertrophic cardiomyopathy (HCM). Myocardial bridges are associated with myocardial ischemia and infarction, cardiac arrythmias and sudden death. The present case series report the outcomes of three symptomatic patients with hypertrophic cardiomyopathy who under-went myocardial muscle debridges. They were operated using beating heart technique without cardiopulmonary bypass. The authors conclude that off-pump supracoronary muscle myotomy is a feasible treatment modality in the young age group with non-obstuctive hypertrophic cardiomyopathy

    Multi-hop driver-parcel matching problem with time windows

    Crowdsourced shipping can result in significant economic and social benefits. For a shipping company, it has a potential cost advantage and creates opportunities for faster deliveries. For the society, it can provide desirable results by reducing congestion and air pollution. Despite the great potential, crowdsourced shipping is not well studied. With the aim of using the spare capacities along the existing transportation flows of the crowd to deliver small-to-medium freight volumes, this paper defines the multi-driver multi-parcel matching problem and proposes a general ILP formulation, which incorporates drivers’ maximum detour, capacity limits, and the option of transferring parcels between drivers. Due to the high computational complexity, we develop two heuristics to solve the problem. The numerical study shows that crowdsourced shipping can be an economic viable and sustainable option, depending on the spatial characteristics of the network and drivers’ schedules.
