    Vacuum therapy in complex treatment of patients with odontogenic inflammatory process of the maxillofacial area and neck

    ВАКУУМНАЯ ТЕРАПИЯРАНЫ, ХИРУРГИЧЕСКАЯ ОБРАБОТКАРАНЫ ЗАЖИВЛЕНИЕРЕГЕНЕРАЦИЯЧЕЛЮСТНО-ЛИЦЕВЫЕ ХИРУРГИЧЕСКИЕ ОПЕРАЦИИОСТЕОМИЕЛИТКОСТЕЙ БОЛЕЗНИ ИНФЕКЦИОННЫЕМИКРОЦИРКУЛЯЦИЯЦель. Оценить эффективность применения вакуумной системы в комплексном лечении пациентов с одонтогенными инфекционно-воспалительными заболеваниями челюстно-лицевой области и шеи. Материал и методы. Проведено обследование 178 пациентов с острым одонтогенным остеомиелитом челюсти, осложненным флегмоной прилежащих клетчаточных пространств. Для оказания помощи пациентам основной группы дополнительно использовалась система для вакуумной терапии ран. Пациентам группы сравнения лечение проводилось с использованием стандартных методов. Группу контроля составили 50 здоровых лиц. Деформируемость эритроцитов (ДЭ) оценивали по времени прохождения их суспензии стандартного расстояния по пористому фильтру. Адгезию лейкоцитарно-тромбоцитарной суспензии (ЛТС) исследовали, регистрируя изменения светопропускания суспензии лейкоцитов до и после инкубации вместе с волокнистым субстратом с помощью агрегометра АР 2110 "СОЛАР". Результаты. При включении в лечебный комплекс вакуумной терапии в более короткие сроки (5 (5; 6) суток) купировалась боль при пальпации очага воспаления, на 7-е (5; 8) сутки восстанавливалась конфигурация лица, гиперемия кожи купировалась на 5-е (5; 6) сутки, гнойная экссудация из раны прекращалась на 6-е (5; 7) сутки, создавались благоприятные условия для начала формирования грануляций на 7-е (6; 8) сутки. Выявлено снижение продолжительности лечения (9 (8; 10) суток) основной группы пациентов относительно пациентов группы сравнения – 10 (8;12) суток. Вакуумная терапия при завершении лечения способствует снижению повышенных в начале лечения показателей микроциркуляции до уровня здоровых лиц. Заключение. Применение вакуумной терапии в лечении пациентов с острым одонтогенным остеомиелитом челюсти, осложненным флегмоной прилежащих клетчаточных пространств, способствует сокращению сроков заживления раны и уменьшению продолжительности лечения с 10 (8; 12) до 9 (8; 10) суток. Выявлена нормализация скорости и степени агрегации ЛТС, ДЭ в плазме крови при завершении лечения пациентов с использованием отрицательного давления.Objective. To evaluate the effectiveness of the vacuum system in the complex treatment of patients with odontogenic inflammatory diseases of the maxillofacial area and neck. Methods. The examination of 178 patients with acute odontogenic osteomyelitis of the jaw, complicated by phlegmon of the adjacent tissue spaces, was conducted. To manage the patients of the main group vacuum system was additionally used. Patients in the comparison group were treated using standard methods. The control group consisted of 50 healthy individuals. The erythrocyte deformability (ED) was evaluated by the time of their suspension passing of a standard distance through a porous filter. The adhesion of leukocyte-platelet suspension (LTS) was investigated by recording the changes in the light transmission of the leukocyte’s suspension before and after incubation with a fibrous substrate using the AR 2110 "SOLARф" aggregometer. Results. When vacuum therapy was included in the medical complex, in shorter terms (5 (5-6) days) the pain during palpation of the inflammatory focus was stopped; the face configuration was restored on the 7 (5-8) day; the skin redness was stopped on the 5 (5-6) day; purulent exudation from the wound was stopped on the 6 (5-7) day; favorable conditions were created for the beginning of the formation of granulations on the 7 (6-8) day. A decrease in treatment terms (9 (8-10) days) of the main group of patients in relation to the duration of treatment in patients with standard complex treatment (10 (8-12) days) was revealed. Vacuum therapy at the end of treatment helps to reduce microcirculation indices elevated at the beginning of treatment to the level of healthy individuals. Conclusions. The use of vacuum therapy in the treatment of patients with acute odontogenic osteomyelitis of the jaw, complicated by phlegmon of the adjacent tissue spaces, reduces the wound healing terms and reduces the duration of treatment from 10 (8; 12) days to 9 (8; 10) days.     Purpose of the study: The aim of this study is to investigate the image of municipalities and also to determine the things that give the city a positive image of the territory. Methodology: The theoretical and methodological basis of the study is the ideas and paradigms that were developed in the works of leading domestic sociologists, cult urologists, economists, and marketers. The applied individual methods include comparative analysis, typology, classification method, questionnaire, method of analysis of social factors and events Results: The Program of socio-economic development must be amended in the form of a subprogram for the formation of a positive image of the municipality of Chekhov, since today there are no projects or programs aimed at creating a positive image of the municipality to one degree or another, its popularization and perception among the local population and among tourists. Moreover, information support of the main assets of the municipality of Chekhov (information about the advantages of the territory, photographs and advertising leaflets) in social networks, the media, on the Internet, that is, marketing of historical and cultural heritage with a priority on the work of A.P. Chekhov Applications of this study: The formation of a favorable image is of fundamental importance primarily for residents of the municipality. It affects the creation and support of social optimism among the population, trust in the authorities and confidence in the future. Novelty of the study: The image of the municipality is formed from the priority elements of its structure and a set of certain factors of development and promotion of the territory. So, the novelty of this study is to asses this image in modern life

    The impact of a local supply chain strategy on regional economic

    Abstract- The aim of this research is to explore socio-economic risks and challenges, in the modern globalized production landscape from an end-to-end supply chain perspective. Given the sensitivity of a Russian regional economy contribution to its supply chains, any improvement in supply chains strategy will be beneficial for mines and their stakeholders. The objectives of this work were to: (i) test the hypothesis that supply chain management strategy, by mines in a region, significantly affects regional economy; and (ii) develop a methodology to identify the critical backward commodities of a regional economic sector. The complexity of the strategic development of the region is conditioned by the fact that it is necessary to take into account the characteristics of all cities and settlements, to be able to integrate their plans into a single document, which allows to build a single development strategy for the entire territory. The development of a regional development strategy is the most important task of both regional science and the practical activities of regional authorities. The relevance of the study concerning the processes and tools of strategic planning for the development of Russian Federation constituent entities is conditioned by the fact that the quality of strategic documents that determines the success of the territory development in modern, dynamically developing socio-economic conditions. At present, the main agency responsible for the strategic approach to the development of the territory is the Ministry of Economic Development of the corresponding territory, which, in conjunction with the regional government, other ministries and departments, forms a document defining the main development vectors of the Russian Federation constituent entity, and the Ministry is responsible for the quality of this document. Orientation to the strengths or weaknesses of the region can act as the principles for the regional development strategy creation, and the content of the strategy determines which sectors will be priority in terms of their development support, which will be provided with additional sources of financing, and which will be developed independently, without state close attention

    Ideals and principles of common wellbeing under social and humanistic paradigm

    We attempt to specify ideals and principles of common wellbeing: saving economy, political attendance, full employment, education, equitable distribution. As a part of the study we have formulated the concept of so-called "other" society under the social-humanistic paradigm. As it was mentioned earlier, social systems can be described as open-end ones, which implies their constant interaction with environment expressed via matter, energy, and information exchange. Because of that, the functional and developmental regulations are characterized by the presence of constant alteration as well, which stipulates the high complexity of structuring valid prognoses. Subject vs. object feedback determines special peculiarities of social prognosis, namely, the ones that are capable of either self-fulfillment of self-destruction of prognostic model. In other words, the predicted future influences the actual future. In the conditions of the increasing instability and dynamics of social processes especially urgent is the problem of adequacy of innovative model. The specific feature of modern innovative processes is the fact that any information about the features of their occurrence is extremely fast becoming obsolete that, in turn, leads to the increase the gap between reality and model

    Constriction imposed by basement membrane regulates developmental cell migration

    The basement membrane (BM) is a specialized extracellular matrix (ECM), which underlies or encases developing tissues. Mechanical properties of encasing BMs have been shown to profoundly influence the shaping of associated tissues. Here, we use the migration of the border cells (BCs) of the Drosophila egg chamber to unravel a new role of encasing BMs in cell migration. BCs move between a group of cells, the nurse cells (NCs), that are enclosed by a monolayer of follicle cells (FCs), which is, in turn, surrounded by a BM, the follicle BM. We show that increasing or reducing the stiffness of the follicle BM, by altering laminins or type IV collagen levels, conversely affects BC migration speed and alters migration mode and dynamics. Follicle BM stiffness also controls pairwise NC and FC cortical tension. We propose that constraints imposed by the follicle BM influence NC and FC cortical tension, which, in turn, regulate BC migration. Encasing BMs emerge as key players in the regulation of collective cell migration during morphogenesis

    New sovereign states appeared after the collapse of the USSR, which have to develop democratic society for a long time. With the development of public administration decentralization, a significant part of the obligations and powers to manage the property of cities and rural areas, health, education, culture, and sports was transferred to the localities. The process of the legal field of municipality development began, which has not been completed to this day. The optimization of municipality management in modern conditions will allow to achieve the highest possible results, enhance the involvement of the local population in the development of the city, to address the issues of local importance more effectively. The great difficulty in the study of modern problems of local self-government is conditioned by the presence of different points of view on the issue of its essence, the absence of a unified theory of local self-government and the existence of different models and forms of its organization. The absence of the established interaction practice between population, government and judicial structures, the underdevelopment of political culture, apathy and lack of confidence in the authority points to the insufficiency of control forms and responsibility specified in the law of the Republic of Armenia on local self-government. The article substantiates the need to increase the importance of local self-government in society, allows to realize the diversity of population interests, satisfy their socio-economic needs, act as the most important means of Armenia region stabilization and development and the state as a whole, the formation of civil society. The municipality must solve local problems with its own forces and means, and be responsible for its decisions. Local governments should own and manage municipal property, including local finances independently - these are the necessary economic conditions for the exercise of their authority.Nuevos estados soberanos aparecieron después del colapso de la URSS, que deben desarrollar una sociedad democrática durante mucho tiempo. Con el desarrollo de la descentralización de la administración pública, una parte significativa de las obligaciones y poderes para administrar la propiedad de las ciudades y las zonas rurales, la salud, la educación, la cultura y el deporte se transfirieron a las localidades. Comenzó el proceso del campo legal del desarrollo del municipio, que no se ha completado hasta el día de hoy. La optimización de la gestión municipal en condiciones modernas permitirá lograr los mejores resultados posibles, mejorar la participación de la población local en el desarrollo de la ciudad, para abordar los problemas de importancia local de manera más efectiva. La gran dificultad en el estudio de los problemas modernos del autogobierno local está condicionada por la presencia de diferentes puntos de vista sobre el tema de su esencia, la ausencia de una teoría unificada del autogobierno local y la existencia de diferentes modelos y formas de su organización. La ausencia de la práctica establecida de interacción entre la población, el gobierno y las estructuras judiciales, el subdesarrollo de la cultura política, la apatía y la falta de confianza en la autoridad apuntan a la insuficiencia de formas de control y responsabilidad especificadas en la ley de la República de Armenia sobre el gobierno local. El artículo confirma la necesidad de aumentar la importancia del autogobierno local en la sociedad, permite darse cuenta de la diversidad de intereses de la población, satisfacer sus necesidades socioeconómicas, actuar como el medio más importante de estabilización y desarrollo de la región de Armenia y el estado como un en conjunto, la formación de la sociedad civil. El municipio debe resolver los problemas locales con sus propias fuerzas y medios, y ser responsable de sus decisiones. Los gobiernos locales deben poseer y administrar la propiedad municipal, incluidas las finanzas locales de forma independiente; estas son las condiciones económicas necesarias para el ejercicio de su autoridad. После распада СССР появились новые суверенные государства, которым предстоит долгий путь по построению демократического общества. С развитием децентрализации государственного управления значительная часть обязательств и полномочий по управлению имуществом городов и сельской местности, здравоохранением, образованием, культурой, спортом была передана на места. Начался процесс формирования правового поля муниципалитетов, который не завершён и по сей день. Оптимизация управления муниципалитетами в современных условиях позволит достичь максимально возможных результатов, активизации вовлечения местного населения в процесс развития города, более эффективному решению вопросов местного значения. Большая сложность в исследовании современных проблем местного самоуправления заключается в наличии разных точек зрения по вопросу о его сущности, отсутствии единой теории местного самоуправления и существовании разных моделей и форм его организации. Отсутствие сложившейся практики взаимодействия населения, властных и судебных структур, неразвитость политической культуры, апатия и отсутствие доверия к органам власти указывает на недостаточность обозначенных в законе Республики Армения о местном самоуправлении форм контроля и ответственности. В статье обосновывается необходимость повышения значимости местного самоуправления в жизни общества, позволяющая реализовать многообразие интересов населения, удовлетворить их социально-экономические потребности, выступать в качестве важнейшего средства стабилизации и развития регионов Армении и государства в целом, формирования гражданского общества. Муниципальное образование должно решать местные проблемы своими силами и средствами, и нести ответственность за свои решения. Органы местного самоуправления должны владеть и управлять муниципальной собственностью, в том числе самостоятельно распоряжаться местными финансами - это необходимые экономические условия для осуществления их властных полномочий
