    Tilt Test in Elderly Patients with Syncope of Unknown Etiology: Experience with Pharmacological Stimulation with Nitroglycerin

    Está demonstrado o papel do teste de inclinação na avaliação de doentes com síncope de causa não esclarecida. Nos idosos, a prevalência de episódios sincopais aumenta, associa-se a pior prognóstico e, frequentemente, implica o recurso a múltiplos exames de diagnóstico. Objectivos: Avaliar a utilidade do teste de inclinação com recurso a nitroglicerina sub-lingual como agente provocativo em idosos com síncope de etiologia desconhecida. Métodos: Foram estudados 46 doentes consecutivos com > 65 anos (56,5% do sexo feminino; 74 ± 6 anos) por síncope de etiologia não esclarecida. Em 25 doentes(54%) não havia evidência de patologia cardíaca, tendo os restantes cardiopatia hipertensiva (14 doentes) ou isquémica (7 doentes). O protocolo incluiu massagem do seio carotídeo em decúbito a após ortostatismo passivo. O teste de inclinação foi efectuado sob monitorização contínua do electrocardiograma e tensão arterial, com inclinação a 70º durante 20 minutos. Na ausência de síncope, administrou-se 500 mcg de nitroglicerina sub-lingual com monitorização por mais 20 minutos. O teste de inclinação foi considerado positivo quando houve reprodução da sintomatologia acompanhada de bradicardia e/ou hipotensão arterial (respostas cardio-inibitória, vasodepressora ou mista). Considerou-se haver hipotensão ortostática nos casos com queda da tensão arterial sistólica > 20 mmHg ou da tensão arterial diastólica > 10 mmHg nos 3 minutos após ortostatismo; nos doentes com sintomas associados à descida gradual, paralela, da tensão arterial sistólica e diastólica durante o teste de inclinação passivo, admitiu-se um perfil do tipo disautonómico; nos doentes com descida gradual da tensão arterial após nitroglicerina, considerou-se haver resposta exagerada aos nitratos. Resultados: Ocorreu reprodução de sintomas em 34 doentes (73,9%): 19,6% no teste de inclinação passivo (resposta neurocardiogénica vasodepressora – 3 doentes, hipersensibilidade do seio carotídeo – 1 doente, hipotensão ortostática – 1 doente, perfil disautonómico – 4 doentes) e 54,3% após nitroglicerina (resposta neurocardiogénica vasodepressora – 12 doentes, mista – 5 doentes , cardio-inibitória – 2 doentes e resposta exagerada aos nitratos – 6 doentes). A síncope foi neurocardiogénica em 47,8% (teste passivo – 13,6%, pós-nitroglicerina – 86,4%). Em 16,2% dos doentes submetidos a nitroglicerina ocorreu resposta exagerada aos nitratos. Não se registaram complicações durante o exame. Conclusões: O teste de inclinação em idosos com síncope de etiologia não esclarecida: 1. contribui para o diagnóstico diferencial no estudo etiológico da síncope, 2. quando potenciado pela nitroglicerina associa-se a um aumento significativo do número de respostas positivas, e permite identificar um número considerável de doentes com resposta exagerada aos nitratos

    A cooperative multi-robot team for the surveillance of shipwreck survivors at sea

    The sea as a very extensive area, renders difficult a pre-emptive and long-lasting search for shipwreck survivors. The operational cost for deploying manned teams with such proactive strategy is high and, thus, these teams are only reactively deployed when a disaster like a shipwreck has been communicated. To reduce the involved financial costs, unmanned robotic systems could be used instead as background surveillance teams patrolling the seas. In this sense, a robotic team for search and rescue (SAR) operations at sea is presented in this work. Composed of an Unmanned Surface Vehicle (USV) piggybacking a watertight Unmanned Aerial Vehicle (UAV) with vertical take-off and landing capabilities, the proposed cooperative system is capable of search, track and provide basic life support while reporting the position of human survivors to better prepared manned rescue teams. The USV provides long-range transportation of the UAV and basic survival kits for victims.     Issues and Opportunities in Exotic Hadrons

    The last few years have been witness to a proliferation of new results concerning heavy exotic hadrons. Experimentally, many new signals have been discovered that could be pointing towards the existence of tetraquarks, pentaquarks, and other exotic configurations of quarks and gluons. Theoretically, advances in lattice field theory techniques place us at the cusp of understanding complex coupled-channel phenomena, modelling grows more sophisticated, and effective field theories are being applied to an ever greater range of situations. It is thus an opportune time to evaluate the status of the field. We have developed an electroenzymatic ATP regeneration system consisting in a gold electrode modified with a floating phospholipid bilayer that allows coupling the catalytic activity of two membrane-bound enzymes: NiFeSe hydrogenase from Desulfovibrio vulgaris and F1Fo-ATP synthase from Escherichia coli. Thus, H2 is used as a fuel for producing ATP.     Integration of industrial IoT architectures for dynamic scheduling

    Technology advancement in smart sensors, cloud computing, and decision support systems is pressuring the industry to adopt more flexible data-based solutions. One of the areas that can benefit from this progress is dynamic scheduling, which can improve production efficiency by tracking resource availability, job changes, and user commands. Challenges include setting triggers through the simultaneous integration of information from cloud-based enterprise databases and operational sources in the Industrial Internet of Things (IIoT). This paper proposes an integrated Information Technology - Operational Technology solution to support dynamic scheduling and rescheduling operations in a personalized production environment. It presents an implementation to a real-world application: an analytical quality control laboratory in the pharmaceutical industry. The resulting integration of intelligent sensors and business events in a fog computing architecture allows the generation of rescheduling triggers to specific online events. This paper focus on the definition of the online events affecting the operations and the cloud-fog-edge IIoT architecture used to support the implementation, and on the rescheduling triggers. The optimized reschedule of the use case shows that moving the computation closer to the cloud improves the CPU run time for larger instances. However, the combined CPU run time with the data exchange and querying introduces a non-negligible communication delay for smaller instances. In situations where fast scheduling solutions are required, fog computing near the edge is the best approach. On the other hand, for larger-size instances, moving the computation closer to the cloud is the recommended approach

    Two charged strangeonium-like structures observable in the Y(2175)→ϕ(1020)π+π−Y(2175) \to \phi(1020)\pi^{+} \pi^{-} process

    Via the Initial Single Pion Emission (ISPE) mechanism, we study the ϕ(1020)π+\phi(1020)\pi^{+} invariant mass spectrum distribution of Y(2175)→ϕ(1020)π+π−Y(2175) \to \phi(1020)\pi^{+} \pi^{-}. Our calculation indicates there exist a sharp peak structure (Zs1+Z_{s1}^+) close to the KKˉ∗K\bar{K}^\ast threshold and a broad structure (Zs2+Z_{s2}^+) near the K∗Kˉ∗K^\ast\bar{K}^\ast threshold. In addition, we also investigate the ϕ(1680)→ϕ(1020)π+π−\phi(1680) \to \phi(1020)\pi^{+} \pi^{-} process due to the ISPE mechanism, where a sharp peak around the KKˉ∗K\bar{K}^\ast threshold appears in the ϕ(1020)π+\phi(1020)\pi^{+} invariant mass spectrum distribution. We suggest to carry out the search for these charged strangeonium-like structures in future experiment, especially Belle II, Super-B and BESIII.Comment: 7 pages, 5 figures. Accepted by Eur. Phys. J.

    Weighting for the beat: Using a dance cue to facilitate turning in people with Parkinson's Disease and freezing of gait

Inability to initiate a step from FOG is likely underpinned, at least in part, by a deficient preparatory weight-shift. Conscious attempts to weight-shift in preparation to step can improve success of initiating forward steps following FOG. However, FOG often occurs during turning, where weight-shifting is more complex and risk of falling is higher. We explored the effectiveness of a dance-based (‘cha-cha’) weight-shifting strategy to re-initiate stepping following FOG during turning. Results suggest that this simple movement strategy can enhance turning steps following FOG, without compromising safety.Parkinson's UK project grant (K-1604)

    The sulfur pathway and diagnosis of sulfate depletion in grapevine

    Sulfur is an essential nutrient to all plant species. Plants assimilate sulfur in a well-described pathway, which has been taken up by roots. Regulatory mech- anism has been the subject of many research papers. However, recent studies highlighted differences between crop plants and the model plant Arabidopsis thaliana. Our work focuses on the identification of genes involved in the sulfur metabolism in the Vitis vinifera genome, and their response to sulfur deficiency and other abiotic stress endured by grapevine in the field, namely water stress. Here, we describe the identification and brief characterization of the first assimilation enzymes involved in the sulfur pathway, the enzyme responsible for sulfur activa- tion, ATP sulfurylase (ATPS), and the two enzymes that reduce sulfate to sulfide, Adenosine 50-phosphosulate reductase (APR) and Sulfite reductase (SiR). A reduc- tion was observed in the number of ATPS and APR isoforms identified in V. vinifera genome when compared to A. thaliana or Glycine max genomes. Two ATPS isoforms were present in the Vitis genome, of which only ATPS1 transcript was detected in the tested tissues, and one APR isoform, suggesting an absence of redundancy in the role of both enzymes. ATPS1, APR and SiR transcript level was up-regulated in response to 2 days exposure to sulfur deficiency in V. vinifera cell cultures, which was completely reversed by the addition of GSH to the culture medium. Apparently, oxidative stress triggered GSH has a pivotal role in the regulation of ATPS1, APR and SiR transcription level, since their up-regulation was observed in mRNA from field grapevine berries under water stress, which is known to induce oxidative stress.info:eu-repo/semantics/publishedVersio

    Heat and water stress induce unique transcriptional signatures of heat-shock proteins and transcription factors in grapevine

    Grapevine is an extremely important crop worldwide. In southern Europe, post-flowering phases of the growth cycle can occur under high temperatures, excessive light, and drought conditions at soil and/or atmospheric level. In this study, we subjected greenhouse grown grapevine, variety Aragonez, to two individual abiotic stresses, water deficit stress (WDS), and heat stress (HS). The adaptation of plants to stress is a complex response triggered by cascades of molecular networks involved in stress perception, signal transduction, and the expression of specific stress-related genes and metabolites. Approaches such as array-based transcript profiling allow assessing the expression of thousands of genes in control and stress tissues. Using microarrays, we analyzed the leaf transcriptomic profile of the grapevine plants. Photosynthesis measurements verified that the plants were significantly affected by the stresses applied. Leaf gene expression was obtained using a high-throughput transcriptomic grapevine array, the 23K custom-made Affymetrix Vitis GeneChip. We identified 1,594 genes as differentially expressed between control and treatments and grouped them into ten major functional categories using MapMan software. The transcriptome of Aragonez was more significantly affected by HS when compared with WDS. The number of genes coding for heat-shock proteins and transcription factors expressed solely in response to HS suggesting their expression as unique signatures of HS. However, a cross-talk between the response pathways to both stresses was observed at the level of AP2/ERF transcription factors

    Reference gene validation for quantitative RT-PCR during biotic and abiotic stresses in Vitis vinifera

    Grapevine is one of the most cultivated fruit crop worldwide with Vitis vinifera being the species with the highest economical importance. Being highly susceptible to fungal pathogens and increasingly affected by environmental factors, it has become an important agricultural research area, where gene expression analysis plays a fundamental role. Quantitative reverse transcription polymerase chain reaction (qRT-PCR) is currently amongst the most powerful techniques to perform gene expression studies. Nevertheless, accurate gene expression quantification strongly relies on appropriate reference gene selection for sample normalization. Concerning V. vinifera, limited information still exists as for which genes are the most suitable to be used as reference under particular experimental conditions. In this work, seven candidate genes were investigated for their stability in grapevine samples referring to four distinct stresses (Erysiphe necator, wounding and UV-C irradiation in leaves and Phaeomoniella chlamydospora colonization in wood). The expression stability was evaluated using geNorm, NormFinder and BestKeeper. In all cases, full agreement was not observed for the three methods. To provide comprehensive rankings integrating the three different programs, for each treatment, a consensus ranking was created using a non-weighted unsupervised rank aggregation method. According to the last, the three most suitable reference genes to be used in grapevine leaves, regardless of the stress, are UBC, VAG and PEP. For the P. chlamydospora treatment, EF1, CYP and UBC were the best scoring genes.
