Management of Diabetic Patients with Administration of Insulin for Oral Surgery : Two cases of less common types of diabetes mellitus

Types of diabetes mellitus less common than the main three (insulin-dependent, non-insulin-dependent, and malnutrition-related) show different diabetic conditions depending on the nature of the original diseases, thus complicating the control of these types for surgery. We report the successful management of two cases of less common types of diabetes, in which the administration of insulin was required for diabetic control prior to and during dental surgery. In two cases of diabetes mellitus, one developing after hyperthyoidism and the other due to total resection of the pancreas in pancreatic carcinoma, careful determination of the insulin dosage was necessary due to high sensitivity to the drug. In the case of total pacreatectomy in particular, there is a risk of developing hypoglycemia due to the wide daily fluctuation in glucose level. For such surgical patients care is necessary to avoid hypoglycemic seizures, and to prevent sepesis and local infection by administering sufficient antibiotics, in order to minimize surgical trauma. Preparation of agricultural low-cost super water absorbent

概述了合成类吸水树脂在农用上的进展,提出采用低成本且工艺成熟的水溶液法,以丙烯酸(AA)、丙烯酰胺(AM)为基体、接枝可溶性淀粉进行三元单体共聚,经钠基膨润土改性,复合引发剂、复合交联剂制备低成本、易降解的合成类吸水性树脂。经实验改进和工艺条件优化后,树脂吸去离子水倍率可达1 940.9 g/g,吸自来水899.8g/g。采用红外光谱仪、热重分析仪、扫描电镜、X射线衍射仪等分析手段对合成树脂进行了分析表征,证实了产物为淀粉、丙烯酸及丙烯酰胺的共聚物。The utilization status of the synthetic absorbent resin in agricultural applications was summarized.An improved process of three monomer copolymer system was proposed by using an aqueous solution polymerization.These three monomers are soluble starch,acrylic acid and acrylamide,which are modified by sodium bentonite,using a composite initiator and cross-linker.By improving the experiments and optimizing the process conditions,the absorption rate of the synthetic absorbent resins is up to 1 940.9 g/g in deionized water,and 899.8 g/g in tap water.In addition,the structure and performance of the synthetic absorbent resin were characterized by using infrared spectroscopy,thermal gravimetric analysis,scanning electron microscopy and X-ray diffraction,which shows that it is an copolymer of starch,acrylic acid and acrylamide

Serum prostate specific antigen for the early detection of prostate cancer in outpatients of internal medicine

1994年6月からの1年間, 高血圧等の慢性疾患で通院中の102例(60歳代36例, 70歳代49例, 80歳以上17例)について, chemiluminescence法により前立腺特異抗原(PSA)を測定した.PSA 3.0ng/ml以上(正常値は2.22ng/ml)の例は36例(35.3%)で, うち20例は泌尿器科を受診して直腸内指診(DRE)と経直腸的超音波検査(TRUS)を行った.16例ではTRUS誘導前立腺生検を行ったが, PSA 10ng/ml以上の15例中7例で前立腺癌を発見した.即ち, 前立腺癌発見率は6.9%(7/102)で, 日本人についての従来の健診による発見率の約4倍と高率であった.PSAによるスクリーニングは前立腺癌早期発見に有用と思われるScreening was performed on 102 patients aged 60 years or older who visited the Outpatient Department of Internal Medicine in Tane General Hospital between June 1994 and May 1995. Of those screened, 36 patients (35.3%) had elevated prostate specific antigen (PSA) values, and 20 of them visited the Urological Department, and underwent digital rectal examination (DRE) and transrectal ultrasonography (TRUS) for further screening. Of the 16 patients who underwent ultrasound guided biopsies of the prostate, 7 patients were found to have prostate cancer. Therefore, the cancer detection rate was 6.9%, which was about 4 times higher than that of mass-screening examinations previously reported in the Japanese literature. These findings suggested that the screening using PSA in outpatients of Internal Medicine may be useful for the early detection of prostate cancer
