    Use of MRI to identify enlarged inferior gluteal and ischioanal lymph nodes and associated findings related to the primary disease

    PURPOSE:We aimed to draw attention to the lymph nodes at the inferior gluteal and ischioanal regions and evaluate the lesions accompanying them using 3.0 T magnetic resonance imaging (MRI).METHODS:In total, 22 patients (15 men, 7 women; mean age, 50±11.2 years; age range, 32–71 years) were included in this study. The patients’ medical records were reviewed. MRI data were reviewed on a picture archiving and communication system workstation by two radiologists in consensus. Lymph node location, laterality, number, and size were documented.RESULTS:The primary disorders causing the enlargement of inferior gluteal lymph nodes (n=16) were perianal fistula of cryptoglandular origin (n=5), perianal fistula associated with Crohn’s disease (n=2), decubitus ulcers (n=2), presacral abscess (n=1), non-Hodgkin lymphoma (n=2), prostate cancer invading urethra and anorectal junction (n=1), endometrium cancer invading the urethra and vagina (n=1), and anal cancer (n=2). The pathologies causing the enlargement of ischioanal lymph nodes (n=6) were perianal fistula of cryptoglandular origin (n=4), subcutaneous inflammation of gluteal region related to Crohn’s disease (n=1), and prostate cancer (n=1).CONCLUSION:The infectious and neoplastic lesions involving the anal canal, distal rectum, gluteal region, prostate, and urethra are the possible causes of inferior gluteal and ischioanal lymph node enlargement. Lymphoproliferative diseases can also affect these node groups. MRI is an important method to identify enlarged inferior gluteal and ischioanal lymph nodes and define associated findings

    Diagnostic accuracy and safety of CT-guided fine needle aspiration biopsy of pulmonary lesions with non-coaxial technique: a single center experience with 442 biopsies

    PURPOSE:We aimed to evaluate the diagnostic accuracy and safety of computed tomography (CT)-guided biopsy of pulmonary lesions with fine needle aspiration (FNA) using non-coaxial technique.METHODS:We analyzed 442 patients who underwent CT-guided lung biopsy with FNA and non-coaxial technique to determine the diagnostic outcomes, complication rates, and independent risk factors for diagnostic failure and pneumothorax.RESULTS:Diagnostic accuracy, sensitivity, and specificity were 97.6%, 97.3%, and 100%, respectively. Age and >35 mm lesion size were significant risk factors for diagnostic failure. The rates of pneumothorax and chest tube placement were 19% and 2.9%, respectively. Middle and lower lobe location, lesion to pleura distance >7.5 mm, and >45° needle trajectory angle were significant risk factors for pneumothorax.CONCLUSION:CT-guided FNA of pulmonary lesions with non-coaxial technique is a safe and reliable method with a relatively low pneumothorax rate and an acceptably high diagnostic accuracy

    In this work, the concept of sensor management is introduced and stochastic dynamic programming based resource allocation approach is proposed to track multiple targets. The core of this approach is to use Lagrange relaxation for decreasing the state space dimension. By this approximation, the overall problem is separated into components instead of using joint Markov model to optimize large scale stochastic control problem. The aim of this study is to adaptively allocate radar resources in an optimal way in order to maintain track qualities for multi-target case. The radar is electronically steered antenna radar. Resource allocation is done only for tracking excluding the search beams. Adaptive target tracking is performed by Kalman filter. Problem is modeled as a set of controlled Markov chains each dedicated to one track. Time scale is divided into two levels that are called as micro management and macro management. During the thesis, we deal with macro management part that aims to construct a policy which is optimal for a given objective function under the resource constraints. Stochastic dynamic programming with constraints in the sense of [32] is the method used. In this thesis, five different scenarios are constructed and corresponding algorithms are confirmed by simulation results. The performances of the algorithms are also compared.     1959 Amsterdam doğumlu Almanya kökenli kadın sanatçı Rineke Dijkstra 1992 yılından bu yana uluslararası üne sahip bir sanatçıdır. Diane Arbus, Thomas Struth vb. fotoğraf sanatçılarının yanında Vermeer ve Verspronck'un çalışmalarından da etkilenmesi onu fotoğraflarının resimsel gücünü açıklamamıza yardımcı olur. Dijkstra ergenlerin, yeni doğum yapmış kadınların, matadorların, sokakta yürüyen insanların, plajdaki gençlerin; yapmacıksız, samimi hikayelerini not eder nitelikte çalışmalarıyla kişisel belgesel fotoğrafçılığın sıradışı temsilcileri arasındadır. Onun niyetini, anlatıların insanlarla arasındaki mesafeyi azaltmak olarak okuyabiliriz. Dijkstra'nın fotoğrafları bizi her zaman çerçevenin ötesini sorgulamaya yöneltecek şekilde düzenlenmiştir. The fact that she was influenced by art photographers like Diane Arbus, Thomas Struth etc. as well as Vermeer and Verspronckhelps to explain the painterly supremacy of her photographs. In a manner of noting down the candid storiesof adolescents, puerperant women, toreadors, people walking on the street, youngsters on the beach, Dijkstra is amongst the extraordinary representatives of personal documentary photography. We can suggest that her intention is to reduce the distance between narratives and people. Dijkstra's photographs are arranged in a way to lead us question what is beyond the frame and astonishes masses with her candid photographs that reveal personal mythologies, the loneliness of people and their psychological moods that triggers humane emotions in the spectator. Without a desire to find a place in museums, her pictures growing yellow in time Dijkstra prioritizes a relational attitude with her viewers. This paper will help to analyze the underlying intentions and thoughts behind her series and lead us to reconsider the place and importance of photography within the context of contemporary art

    The effect of cooking methods on some quality characteristics of gluteus medius

    Abstract In this research, the effects of cooking by convection oven and cooking by sous-vide method on the physicochemical, textural, sensorial and microbiological of gluteus medius cuts were investigated. For the sous-vide method, the meat was vacuumed sealed in special packaging material (Mylar®Cook) before being stored at + 1 °C for 0, 15, and 30 days. The samples cooked with the sous-vide method had higher moisture content (P < 0.001) and lower cooking loss than oven roasted samples. TBARS content was affected by cooking method (P < 0.01) and storage time ( P < 0.01). The 2-thiobarbituric acid reactive substances (TBARS) values of the convection oven samples were higher than those cooked with sous-vide method. The WBSF values were affected by the cooking methods. The samples cooking with oven significantly lower shear (P < 0.0001) force values than those for the sous-vide method. Samples cooked by the convection oven had higher L* (lightness), a* (redness), b* (yellowness) than the cooked with sous-vide method (P < 0.0001). The sliced meat samples cooked by oven were given higher sensory ratings than those cooked by sous-vide except texture scores

    Effect of Different Production Methods on the Mechanical and Microstructural Properties of Hypereutectic Al-Si Alloys

    In this study, the effects of different production methods like melt spinning, high-energy ball milling, and combined melt spinning and high-energy ball milling on the mechanical and microstructural properties of hypereutectic Al-20Si-5Fe alloys were investigated. While microstructural and spectroscopic analyses were performed using scanning electron microscopy and X-ray diffractometry, mechanical properties were measured using a depth-sensing indentation instrument with a Berkovich tip. Microstructural and spectroscopic analyses demonstrate that high-energy ball milling process applied on the melt-spun Al-20-Si-5Fe alloy for 10 minutes brings about a reduction in the size of silicon particles and intermetallic compounds. However, further increase in milling time does not yield any significant reduction in size. High-energy ball milling for 10 minutes on the starting powders is not enough to form any intermetallic phase. According to the depth-sensing indentation experiments, high-energy milling of melt-spun Al-20Si-5Fe alloys shows an incremental behavior in terms of hardness values.     Explaining Young Consumers' Online Purchase Behavior under Risky Conditions: Perspectives from Mental Accounting

    As the internet becomes a new shopping channel, which threats traditional shopping, it also causes dramatic changes in consumer market. In the light of theories claiming that consumers act with limited rationality and make their preferences with the motive of avoiding loss, the main aim of this study is to measure the impact of loss variables on the consumers' decisions about online shopping in Turkey. The originality of this study is to categorise and analyse loss variables as 'the ones that happen in the process of shopping' and 'the ones that happen out of the process of shopping'. Following the literature stating that consumers tend to act irrationally, the study also concludes that consumers tend to be away from the economical rationality at a great extent under the loss conditions. The findings show that any type of loss either physically or mentally related to consumers' shopping has an effect on consumers' irrational behaviors. This study indicates that this effect is higher in the conditions of loss in the process of shopping than out of the process of shopping

    Lung ultrasound and computed tomographic findings in pregnant woman with COVID-19

    Imaging modalities play a crucial role in the management of suspected COVID-19 patients. Before reverse transcription polymerase chain reaction (RT-PCR) test results are positive, 60-93% of patients have positive chest computed tomographic (CT) findings consistent with COVID-19. We report a case of positive lung ultrasound findings consistent with COVID-19 in a woman with an initially negative RT-PCR result. The lung ultrasound-imaging findings were present between the negative and subsequent positive RT-PCR tests and correlated with CT findings. The point-of-care lung-ultrasound examination was easy to perform and, as such, could play an important role in the triage of women with suspected COVID-19. The neonatal swabs, cord blood and placental swab RT-PCR tests were negative for SARS-CoV-2, a finding consistent with the published literature suggesting no vertical transmission of this virus in pregnant women.
