Communication Strategy Concept for Private Health Care Facility of Optometry
Předmětem diplomové práce je analýza obsahu stávající komunikační strategie v nestátním zdravotnickém zařízení optometrie, vyhodnocení jejího vnímání zákazníkem a návrh opatření, vedoucích k podpoře výkonnosti podniku. První část se věnuje popisu teorií, jejichž znalost je k sestavení komunikačního plánu nezbytná. Ve druhé části je proveden rozbor současné externí komunikace v konkrétní společnosti a vyhodnocení její efektivity na základě dotazníkového šetření. V závěrečné části jsou navrženy změny a vhodná komunikační opatření, která zvýší výkonnost podniku.The subject of the thesis is an analysis of the effectiveness of marketing communication strategy in private health care facility of optometry, evaluation of customer perception and design measures to encourage business performance. The first part describes the theories, which knowledge of is necessary to build a communication plan. The second part is an analysis of the current external communication within the organization and evaluation of its effectiveness based on a questionnaire survey. The final part includes suggestions for changes and appropriate communication measures to increase business performance.

Rezumat. Oftalmopatia tiroidiana sau boala oculară tiroidiană este o patologie inflamatorie a orbitei asociată cu boala tiroidiană autoimună
care determină hipertrofia și fibroza țesutului adipos al orbitei și a mușchilor striați. Patologia prezintă morbiditate
crescută cu defect estetic și funcțional. Boala Graves este cea mai frecventă patologie autoimună, iar circa 50% dintre pacienți
pot dezvolta orbitopatia tiroidiană.Actuality. Thyroid ophthalmopathy or thyroid
eye disease (TED) is an orbital inflammatory condition
associated with autoimmune thyroid disorders,
which determine hypertrophy and fibrosis of orbital
fat and striated muscle, presenting increased
morbidity: aesthetically and functionally. Graves’
disease is the most common autoimmune disorder.
Approximately 50% of patients with Graves’ disease
may develop thyroid orbitopathy. The orbital target
of the immune response is probably the pluripotent
orbital fibrocyte. The diagnosis of TED is established
based on three aspects of the disease: clinical findings,
thyroid function and antibody tests, imaging
characteristics. The most important clinical features
of orbitopathy are lid retraction, proptosis, divergent
visual axis and dystyroid optic neuropathy. Inflammatory
phase is managed by conservative medical
treatment and chronic fibrotic phase by surgical.
Objectives. To present the epidemiology of
TED and possible risk factors; the pathophysiological
mechanism and clinical manifestations of thyroid
ophthalmopathy; to elucidate the stages and
the criteria of diagnosis; selection of treatments;
clinical case presentation. Materials and Methods. This study is a case
report, based on a retrospective analysis of a patient,
using different diagnostic methods presented
in the article.
Discussion and conclusion. Thyroid ophthalmopathy
is an orbital inflammatory condition,
which is associated with autoimmune thyroid disease
that causes hypertrophy and cicatrizing of
orbital fat and striated muscles. Although it is selflimiting,
it can significantly disrupt the aesthetic aspect,
vision and quality of life of patient. TED has a
biphasic course, with a progressive („active”) phase
that lasts 6-18 months, followed by a stable („inactive”)
phase. These phases are classified as „clinical
activity”. Immunomodulatory medication and radiotherapy
used in the temporary active phase may
limit the destructive consequences of the immune
cascade. TED management is ideally performed using
a multidisciplinary team (endocrinologist, ophthalmologist,
rheumatologist, oncologist), each
having an area for expertise
The footprint of the SARS-CoV-2 pandemic in the activity of the ophthalmology and optometry clinic SUMPh „Nicolae Testemițanu”
SUMPh «Nicolae Testemitanu», Department of Ophthalmology and OptometryScopul lucrării: stabilirea unor particularități
în activitatea clinicii de oftalmologie, cauzate de
pandemia COVID-19 (anul 2020), comparativ cu
activitatea în perioada prepandemică (anul 2019).
Materiale și metode: a fost efectuată o analiză
a activității didactice și curative a clinicii de oftalmologie
și optometrie pe parcursul anilor 2019 și 2020,
fiind totodată elucidate particularitățiile specifice
cauzate de pandemia COVID-19.
Rezultate: activitatea clinicii de oftalmologie și
optometrie a fost axată pe realizarea următoarelor
sarcini principale: 1) asigurarea realizării programului
didactic; 2) asigurarea asistenței oftalmologice
pentru cazurile de urgență medicală; 3) asigurarea
asistenței medicale specializate programate. Îndeplinirea
acestor deziderate este în permanentă corelare
cu respectarea strictă a recomandărilor ce țin de
siguranța instituției. În acest context, pe parcursul
anului 2020 au fost implementate reguli mai stricte
care țin de asigurarea securității antiepidemice, și
anume: purtarea permanentă de către tot personalul
clinicii a echipamentului de protecție (costume,
măști, viziere, ochelari, mănuși), instalarea în toate
încăperile a dozatoarelor cu dezinfectant, cuarțarea
și aerisirea periodică a tuturor spațiilor, limitarea
maximală a accesului persoanelor neautorizate în
incinta clinicii. Procesul didactic pentru studenți
a fost transferat în mare măsură în regim on-line.
De menționat este implicarea activă a medicilorrezidenți
atât în activitatea clinicii de oftalmologie
și optometrie, cât și în prima linie în lupta cu infecția
SARS-CoV-2, examinând suprafața oculară la patul bolnavului. Asistența specializată urgentă a fost acordată
în cadrul Departamentului Medicină Urgentă a
spitalului municipal „Sfânta Treime” cu respectarea
strictă a regulilor antiepidemice. A fost înregistrat un
număr similar de adresări (cu circa 4% mai puține).
Numărul de spitalizări a pacienților de urgență, de
asemenea, s-a micșorat cu aproximativ 4,5%. Datele
statistice înregistrează modificări în structura spitalizărilor
pacienților cu următoarele patologii: cataractă
- o micșorare cu 27%, glaucom – o micșorare
cu 36%, patologia pleoapelor - o micșorare cu 52%,
procese inflamatorii ale corneei – o majorare cu circa
12 %. În această perioadă s-a înregistrat o creștere
substanțială a numărului de transplanturi de membrană
amniotică la pacienții cu ulcere corneene (de
circa 2 ori). În legătura cu punerea în aplicare a stării
de carantină, pe o perioadă de 2 luni, activitatea
chirurgicală în mare măsură a fost sistată, fapt care
a cauzat diminuarea numărului de intervenții efectuate
cu circa 19%. În condițiile create s-a recurs la o
eficientizare maximală a procesului curativ, ceea ce
a redus durata spitalizării pacienților cu 0,4 zile (de
la 3,6 la 3,2 zile) din contul perioadei preoperatorii.
Concluzii. În pofida tuturor împedimentelor
provocate de pandemia COVID-19, activitatea clinicii
de oftalmologie și optometrie nu a suferit modificări
substanțiale, cu excepția transferării procesului
didactic în spațiul on-line, acordarea unei atenții
maximale securității antiepidemiologice și sporirea
intensității activității curative.Summary. The aim of the current study is to establish some particularities in the activity of the ophthalmology clinic caused by the
COVID-19 pandemic compared to the activity in the pre-pandemic period. An analysis of the didactic and curative activity
of the ophthalmology and optometry clinic was followed up during the years 2019 - 2020, while also elucidating the specific
features caused by the COVID-19 pandemic. Despite all the impediments caused by the COVID-19 pandemic, the activity of
the ophthalmology clinic has not undergone substantial changes, except for the transfer of the teaching process in the online
space, paying maximum attention to anti-epidemiological security and increasing the intensity of curative activity

“Myopia computer”. The “Myopia Calculator” application is a program that allows users to find out about the evolution of
the patient’s myopia depending on the selected treatment, the degree of myopia, age, the annual gradient of progression of the
initial myopia and family factor. The “Myopia Calculator” application can be installed in any gadget that supports the operation
of the screen and allows the visualization of the numerical value and the graphical visualization of the result for reading
and interpretation.Introducere. Creșterea exponențială a
pacienților miopi și integrarea tehnologiilor
informaționale în sistemul medical creează
necesitatea implementării programelor de diagnostic,
tratament și recomandări pentru aceste
persoane. Scopul studiului constă în prezentarea aplicației
„Myopia calculator”. Rezultat. Aplicația „Myopia Calculator” este
un program care permite utilizatorului să se informeze
cu privire la evoluția miopiei pacientului în
dependență de tratamentul selectat, gradul miopiei, vârsta, gradientul anual de progresare a miopiei
inițial și prezența sau absența factorului familial.
Aplicația „Myopia Calculator” poate fi instalată în
orice dispozitiv care susține funcționarea acesteia și permite vizualizarea pe ecran a valorilor numerice,
precum și vizualizarea grafică a cestora pentru citire

Instituirea specialităţii de optometrie în Republica Moldova: premise şi direcţii de dezvoltare
Background. Optometry is a healthcare profession that is autonomous and regulated. Optometrists are
the primary healthcare practitioners of the eye who provide comprehensive eye care. This paper attests
the necessity and importance of establishing the specialty of Optometry in the Republic of
Moldova. Objective of the study. Reflecting the premises for establishing the optometry specialty in
the Republic of Moldova and determining the strategic directions for the development of this
profession. Material and Methods. A descriptive study was performed. Historical, statistical and
comparative methods were used. Results. The Optometry specialty has been approved for the Training
Domain Professional Nomenclature and higher education specialties (GD, Nr. 482/2017). Enrolled 53
students in the period 2017-2019, in the Optometry specialty. 34 students from the specialty of
Optometry and 4 professors from SUMPh "Nicolae Testemitanu" attended practical internships and
exchange of experience at the University of South-Eastern Norway. 7 students from the University of
South-Eastern Norway followed the practical undergraduate internship (12 weeks) at SUMPh "Nicolae
Testemitanu" and other 2 students have consolidated their research skills within the Master's program
at SUMPh "Nicolae Testemitanu". Conclusion. Optometrists trained at USMF "Nicolae Testemitanu"
will ensure the prevention and reduction of cases of preventable blindness. One of the priorities is the
accreditation of the diploma at European level according to ECOO and the elaboration of the master's
and doctoral program in optometry. Introducere. Optometria este o profesie autonomă din domeniul medical care se ocupă cu măsurarea și
studiul acuității vizuale și a deficienţelor de vedere, cât și cu identificarea soluțiilor necesare corectării
lor. Lucrarea dată atestă necesitatea şi importanţa instituirii specialităţii de Optometrie în
Moldova. Scopul lucrării. Reflectarea premiselor de instituire a specialităţii de optometrie în Republica
Moldova şi determinarea direcţiilor strategice de dezvoltare a acestei profesii. Material și Metode. A
fost efectuat un studiu descriptiv. Au fost utilizate metodele istorică, statistică și
comparativă. Rezultate. A fost instituită specialitatea de optometrie în cadrul USMF „Nicolae
Testemițanu" aprobată prin Hotărârea de Guvern, (HG Nr. 482/2017). Au fost înmatriculaţi 53 de
studenţi în perioada 2017-2019, la specialitatea Optometrie. 34 de studenţi de la specialitatea de
Optometrie și 4 profesori din cadrul USMF „Nicolae Testemițanu" au urmat stagii practice și schimb de
experiență la Universitatea de Sud-Est din Norvegia. 7 studenți din cadrul Universității de Sud-Est din
Norvegia au urmat stagiul practic de licență (12 săptămâni) şi 2 studenți şi-au consolidat abilităţile de
cercetare în cadrul programului Masterat la USMF „Nicolae Testemițanu". Concluzii. Optometriştii
formaţi în cadrul USMF „Nicolae Testemițanu" vor asigura prevenirea şi reducerea cazurilor de cecitate
evitabilă. Una din priorităţi este acreditarea diplomei la nivel european şi elaborarea programului de
masterat și doctorat în optometrie
Agreement in dry eye management between optometrists and general practitioners in primary health care in the Netherlands

Methods: A web-based survey was used to investigate the agreement in symptoms associated with dry eye, causes of developing dry eye, and investigative techniques used in practice, between GPs and optometrists. Additional questions surveyed knowledge of the latest research, and co-management of dry eye disease in primary healthcare. The anonymised questionnaire contained 16 forced-choice questions with Likert scales, and was sent to 1471 general medical practitioners and 870 registered optometrists. The response data was stored on an online database, and was converted directly to text format for analysis using SPSS 21 statistical analysis software.
Results: 138 optometrists and 93 GPs responded to the survey (Cronbach α = 0.885, optometrists, and 0.833, GPs). Almost no agreement was found for all the questions: a statistically significant difference (Chi-square p 0.0001), and dry eye symptoms, except for ‘burning sensation of the eye’ and ‘irritation of the eye’ as agreed symptoms, and agreement that dry eye is an age-related disease. Conclusions: As the optometrist and the GP are the gatekeepers for secondary healthcare, the fundamental differences in the methods of investigation and interpretation of dry eye-related symptoms, the possible cause of developing dry eye disease, and the therapy given by GPs and optometrists in the Netherlands, may have a significant impact on consistency of patient care.The authors extend their appreciation to the University of Applied Sciences Utrecht for funding this researc

Manufacturer changes lead to clinically important differences between two editions of the TNO stereotest
Purpose: Stereoacuity tests used in clinical practice should be repeatable and reproducible. However, it has been observed in a clinical setting that new editions of the TNO stereotest appear to give different values from those obtained using previous versions. The purpose of the present research was to investigate this observation.
Methods: One hundred and twenty-one Dutch subjects, 88 (73%) females and 33 (27%) males, with an average age of 34.0 years (range 18–55) had their stereoacuity measured using two different versions of the TNO stereoacuity test (TNO 13 and TNO 15). The TNO was tested in a counterbalanced order so that consecutive subjects started with alternate editions to avoid bias.
Results: There was a significant difference (p < 0.001) between the median value for stereoacuity measured with TNO 13 (30 s of arc) and TNO 15 (60 s of arc). The bias between the two test versions was -0.23 Log arcseconds (95% limits of the differences: 0.15 to -0.60 Log arcseconds).
Conclusion: This study reveals that results obtained with two different editions of a commonplace stereoacuity test are not comparable. New versions come on the market at regular intervals and the assumption that they will give the same results as previous versions may not be valid. Besides the statistically significant difference between the TNO 13 and TNO 15, the Bland-Altman plot also showed a considerable bias and the 95% limits of the differences between the TNO 13 and TNO 15 are more than two steps on the Log arcsecond scale. This difference between two editions of the TNO stereotests is not clinically acceptable and therefore it is inappropriate to use the two versions of the test interchangeably. It is important in both research and clinical records to specify the edition of the TNO test used

Explicitation du raisonnement clinique chez des optométristes de niveaux compétent et expert : développement d'un modèle de raisonnement clinique en optométrie
L'optométrie a récemment connu une expansion rapide de son champ de pratique, ce qui rehausse l'importance pour les optométristes actuels et futurs de développer leur raisonnement clinique. Ce dernier n'ayant jamais été investigué en optométrie, il est quasi-absent des programmes de formation en optométrie. Cette compétence reste donc implicite et plus difficile à apprendre. Cette étude de cas avait pour objectifs de déterminer les processus de raisonnement clinique d'optométristes de niveaux compétent et expert, puis à les comparer entre eux pour faire ressortir ce qui caractérise l'expertise. L'étude visait ensuite à valider un modèle de raisonnement clinique. Le raisonnement clinique a été explicite chez quatre optométristes de chacun des niveaux compétent et expert par une rétroaction vidéo aidée de l'entretien d'explicitation. Un modèle de raisonnement clinique a ensuite été validé auprès de cinq optométristes de niveau expert réunis en groupe de discussion. Les résultats démontrent que les optométristes des deux niveaux utilisent les mêmes processus de raisonnement clinique, mais qu'ils les mettent en oeuvre différemment. L'expertise se caractérise par une meilleure prise en compte du patient, une meilleure planification, une capacité à réfléchir pendant des tâches cognitivement exigeantes, une représentation du cas clinique précoce, une meilleure anticipation des résultats et une élaboration des stratégies d'intervention répartie sur toute la consultation. Le modèle de raisonnement clinique issu de cette étude est centré sur la construction progressive de la représentation du cas clinique et des diagnostics, en parallèle avec l'élaboration des stratégies d'intervention. Le patient et les connaissances de l'optométriste y occupent une place capitale. L'explicitation du raisonnement clinique et la validation d'un modèle de raisonnement clinique spécifique à l'optométrie sont susceptibles d'entrainer des retombées en pratique, en formation et en recherche. Cette étude a notamment fourni des pistes pour l'enseignement du raisonnement clinique dans une perspective de développement de l'expertise professionnelle
Measuring of Corneal Thickness of Contact Lens Wearers with Keratoconus and Keratoplasty by Means of Optical Coherence Tomography (OCT)
To measure the corneal thickness and the depth of the precorneal tear film of contact lens wearers with keratoconus or keratoplasty and to reconfirm the identification and classification of the keratoconus with optical coherence tomography (OCT). The cornea and precorneal tear film of 123 eyes with keratoconus, of 39 eyes after keratoplasty and 8 eyes after LASIK were examined with an OCT (Zeiss VisanteTM) and a keratograph (Oculus). Visual acuity was determined. The mean age of all patients was 42.7 years (s=9). There were 35% female patients and 65% were male patients. The central corneal thickness of 123 eyes with keratoconus was 467 ± 73 mm. The nasal and especially the inferior corneal periphery exhibit a 9% lesser thickness (426 ± 83 mm). The cornea with keratoconus is thinner in the 90° meridian, than in the 180° meridian [p<0.01). This could be a clinically relevant result for the reduction of astigmatism after keratoplastic surgery. The central corneal thickness of 39 eyes with keratoplasty was 555 ± 65 mm. These eyes showed peripheral parts with even less thickness. The thickness of the precorneal tear film of 114 contact lens wearers with keratoconus was 89 ± 42 mm in the horizontal meridian, 113 ± 56 mm in the vertical meridian. All the comparative results in case of keratoconus, keratoplasty and the depth of the precorneal tear film had high statistical significance (p<0.001). Optical coherence tomography is particularly suitable for the examination of eyes with keratoconus and keratoplasty.
