    The Effectiveness of Maintenance Pharmacotherapies for Non-Small Cell Lung Cancer

    Although current recommendations for the treatment of advanced non-small cell lung cancer (NSCLC) include a maximum of six cycles of platinum-based combination therapy as a first-line approach, most patients experience progression within 3–4 months. Therefore, a new treatment strategy, maintenance therapy, has been proposed, and several large randomized prospective controlled trials have shown benefits with maintenance therapy. Maintenance therapy can be classified as either continuation maintenance, which is defined as a prolongation of a part of the first-line chemotherapy or molecularly targeted agent until progression, or switch-maintenance, which is defined as the administration of a different cytotoxic chemotherapy or molecularly targeted agent immediately after induction therapy. In this article, recent results from large randomized phase III trials regarding maintenance therapy are reviewed in order to evaluate the role of maintenance therapy in NSCLC

    Spirituality, Religiosity, Depression, Anxiety, and Drug-Use Consequences During Methadone Maintenance Therapy

    Substance addiction is damaging to the health of persons, families, and society. Often the person with addiction has decreased spirituality and religiosity and suffers from anxiety, depression, or both, increasing the risk for continued substance use and its concomitant negative consequences. The study purpose was to describe spirituality and religiosity, among persons enrolled in methadone maintenance therapy and to examine associations between spirituality, religiosity, anxiety, depression, and drug-use consequences. Using a descriptive and cross-sectional correlational design, 108 participants completed questionnaires assessing the study variables. Spiritual well-being was similar to other addiction samples and lower than healthy person samples. Most participants described themselves as spiritual or religious though religious participation was lower than in their past. The analysis indicated that spirituality, religiosity, depression, anxiety, and negative drug-use consequences are interrelated in the person with addiction. Higher anxiety was predictive of negative drug-use consequences

    Continued-Maintenance Therapy for PCNSL

    Background. PCNSL is mainly treated with HD-MTX-based chemotherapy with or without WBRT. However, As WBRT is associated with delayed neurotoxicity leading to dementia in the elderly, many institutes reported benefits of intensive chemotherapy or high-dose chemotherapy with ASCT. We investigated whether treatment with HD-MTX and rituximab, followed by continued-maintenance HD-MTX monotherapy (3.5g / m2), improves overall survival (OS). Methods. In this retrospective, single-center trial 52 immunocompetent patients with newly diagnosed PCNSL were included. All were treated between January 2005 and December 2017. The controls were 18 patients who, between 2005 and 2011, had received 3 cycles of HD-MTX and then adjuvant treatment with WBRT. In 2011 we started HD-MTX continued-maintenance therapy to treat 34 PCNSL patients. In the induction phase, these patients received HD-MTX every 14 days until a complete response (CR) was observed. When CR was obtained, maintenance therapy with HD-MTX (3.5g / m2) was delivered every three months. Results. In 3-year overall survival (OS) there was a statistically significant difference between the two groups [controls : 33.1% (95%, CI 12.4 - 55.7%) ; maintenance group : 74.9% (95%, CI 55.6 - 86.7%) (p < 0.02)]. Conclusion : The induction of HD-MTX based chemotherapy followed by continued-maintenance HD-MTX monotherapy improved OS compared with chemoradiotherapy consisting of HD-MTX followed by WBRT

    Psycho active substance abuse is a common problem throughout the world, and it is associated with the increase of mortality and morbidity. One of substitution therapy program is methadone maintenance therapy program. The main problem is most of patients stop following the maintenance program before they feel the therapeutic effects of it. Compliance in taking daily medication is the focus in achieving the health status of patients. The research objective is to determine the relationship between the knowledge level with methadone maintenance therapy program in the public health center of Parakan, District of Temanggung. The research method was cross sectional approach. The population in this study were all patients of methadone maintenance therapy program in the public health centers of Parakan, District of Temanggung. Samples on this research were 31 patients and they are all active on methadone maintenance therapy program in the public health center of Parakan, District of Temanggung. Data analysis was performed using Chi Square (x2) formula correlation test. The results show there is a relationship between knowledge level with methadone maintenance therapy program in the public health center of Parakan, District of Temanggung with p value 0.001, with 13 respondents (42%) are in a good level of knowledge category and methadone treatment in obidiance catagory is 11 respondents (35.5%). It is concluded that there is a relationship between methadone maintenance therapy program in the public health center of Parakan, District of Temanggung
