Investigating and learning lessons from early experiences of implementing ePrescribing systems into NHS hospitals:a questionnaire study
Background: ePrescribing systems have significant potential to improve the safety and efficiency of healthcare, but they need to be carefully selected and implemented to maximise benefits. Implementations in English hospitals are in the early stages and there is a lack of standards guiding the procurement, functional specifications, and expected benefits. We sought to provide an updated overview of the current picture in relation to implementation of ePrescribing systems, explore existing strategies, and identify early lessons learned.Methods: a descriptive questionnaire-based study, which included closed and free text questions and involved both quantitative and qualitative analysis of the data generated.Results: we obtained responses from 85 of 108 NHS staff (78.7% response rate). At least 6% (n = 10) of the 168 English NHS Trusts have already implemented ePrescribing systems, 2% (n = 4) have no plans of implementing, and 34% (n = 55) are planning to implement with intended rapid implementation timelines driven by high expectations surrounding improved safety and efficiency of care. The majority are unclear as to which system to choose, but integration with existing systems and sophisticated decision support functionality are important decisive factors. Participants highlighted the need for increased guidance in relation to implementation strategy, system choice and standards, as well as the need for top-level management support to adequately resource the project. Although some early benefits were reported by hospitals that had already implemented, the hoped for benefits relating to improved efficiency and cost-savings remain elusive due to a lack of system maturity.Conclusions: whilst few have begun implementation, there is considerable interest in ePrescribing systems with ambitious timelines amongst those hospitals that are planning implementations. In order to ensure maximum chances of realising benefits, there is a need for increased guidance in relation to implementation strategy, system choice and standards, as well as increased financial resources to fund local activitie

Computerization of workflows, guidelines and care pathways: a review of implementation challenges for process-oriented health information systems
There is a need to integrate the various theoretical frameworks and formalisms for modeling clinical guidelines, workflows, and pathways, in order to move beyond providing support for individual clinical decisions and toward the provision of process-oriented, patient-centered, health information systems (HIS). In this review, we analyze the challenges in developing process-oriented HIS that formally model guidelines, workflows, and care pathways. A qualitative meta-synthesis was performed on studies published in English between 1995 and 2010 that addressed the modeling process and reported the exposition of a new methodology, model, system implementation, or system architecture. Thematic analysis, principal component analysis (PCA) and data visualisation techniques were used to identify and cluster the underlying implementation ‘challenge’ themes. One hundred and eight relevant studies were selected for review. Twenty-five underlying ‘challenge’ themes were identified. These were clustered into 10 distinct groups, from which a conceptual model of the implementation process was developed. We found that the development of systems supporting individual clinical decisions is evolving toward the implementation of adaptable care pathways on the semantic web, incorporating formal, clinical, and organizational ontologies, and the use of workflow management systems. These architectures now need to be implemented and evaluated on a wider scale within clinical settings
Factors that promote or inhibit the implementation of e-health systems: an explanatory systematic review
OBJECTIVE: To systematically review the literature on the implementation of e-health to identify: (i) barriers and facilitators to e-health implementation, and (ii) outstanding gaps in research on the subject.METHODS: MEDLINE, EMBASE, CINAHL, PSYCINFO and the Cochrane Library were searched for reviews published between 1 January 1995 and 17 March 2009. Studies had to be systematic reviews, narrative reviews, qualitative metasyntheses or meta-ethnographies of e-health implementation. Abstracts and papers were double screened and data were extracted on country of origin; e-health domain; publication date; aims and methods; databases searched; inclusion and exclusion criteria and number of papers included. Data were analysed qualitatively using normalization process theory as an explanatory coding framework.FINDINGS: Inclusion criteria were met by 37 papers; 20 had been published between 1995 and 2007 and 17 between 2008 and 2009. Methodological quality was poor: 19 papers did not specify the inclusion and exclusion criteria and 13 did not indicate the precise number of articles screened. The use of normalization process theory as a conceptual framework revealed that relatively little attention was paid to: (i) work directed at making sense of e-health systems, specifying their purposes and benefits, establishing their value to users and planning their implementation; (ii) factors promoting or inhibiting engagement and participation; (iii) effects on roles and responsibilities; (iv) risk management, and (v) ways in which implementation processes might be reconfigured by user-produced knowledge.CONCLUSION: The published literature focused on organizational issues, neglecting the wider social framework that must be considered when introducing new technologies.<br/
The GUIDES checklist: development of a tool to improve the successful use of guideline-based computerised clinical decision support
Background: Computerised decision support (CDS) based on trustworthy clinical guidelines is a key component
of a learning healthcare system. Research shows that the effectiveness of CDS is mixed.
Multifaceted context, system, recommendation and implementation factors may potentially affect the success of
CDS interventions. This paper describes the development of a checklist that is intended to support professionals to
implement CDS successfully.
Methods: We developed the checklist through an iterative process that involved a systematic review of evidence
and frameworks, a synthesis of the success factors identified in the review, feedback from an international expert
panel that evaluated the checklist in relation to a list of desirable framework attributes, consultations with patients
and healthcare consumers and pilot testing of the checklist.
Results: We screened 5347 papers and selected 71 papers with relevant information on success factors for
guideline-based CDS. From the selected papers, we developed a 16-factor checklist that is divided in four domains,
i.e. the CDS context, content, system and implementation domains. The panel of experts evaluated the checklist
positively as an instrument that could support people implementing guideline-based CDS across a wide range of
settings globally. Patients and healthcare consumers identified guideline-based CDS as an important quality
improvement intervention and perceived the GUIDES checklist as a suitable and useful strategy.
Conclusions: The GUIDES checklist can support professionals in considering the factors that affect the success of
CDS interventions. It may facilitate a deeper and more accurate understanding of the factors shaping CDS
effectiveness. Relying on a structured approach may prevent that important factors are missed
Implementing Real-Time Computerized Clinical Decision Support During Trauma Resuscitations: Lessons Learned From Simulation.
Through the combination of clinical expertise, research, and patient values evidence-based practice (EBP) is becoming the new standard of patient care. Despite the main benefits of EBP medical providers are hesitant to implement based on a multitude of barriers. In order to increase adoption of EBP we need to develop deliver systems that bring best evidence in an easy to use format to the bedside.
We tested the implementation of a computerized clinical decision support documentation platform during trauma team resuscitations. The simulation sessions were geared to help identify opportunities for improvement in our implementation methodology.
During the simulation sessions an improvement in care was not identified in comparison to previous in situ simulation sessions. We did identify several opportunities to improve patient care through the use of disruptive alerts, additional training, and a time period for familiarization with the product

approach will help them to succeed in the future.
The socio-technical proposition is simply that any work system comprises
both a social system (including the staff, their working practices, job roles,
culture and goals) and a technical system (the tools and technologies that
support and enable work processes). These elements together form a
single system comprising interacting parts. The technical and the social
elements need to be jointly designed (or redesigned) so that they are
congruent and support one another in delivering a better service.
Focusing on one aspect alone is likely to be sub-optimal and wastes
money (Clegg, 2008). Thus projects that just focus on the IT will almost
always fail to deliver the full benefits
User-centered visual analysis using a hybrid reasoning architecture for intensive care units
One problem pertaining to Intensive Care Unit information systems is that, in some cases, a very dense display of data can result. To ensure the overview and readability of the increasing volumes of data, some special features are required (e.g., data prioritization, clustering, and selection mechanisms) with the application of analytical methods (e.g., temporal data abstraction, principal component analysis, and detection of events). This paper addresses the problem of improving the integration of the visual and analytical methods applied to medical monitoring systems. We present a knowledge- and machine learning-based approach to support the knowledge discovery process with appropriate analytical and visual methods. Its potential benefit to the development of user interfaces for intelligent monitors that can assist with the detection and explanation of new, potentially threatening medical events. The proposed hybrid reasoning architecture provides an interactive graphical user interface to adjust the parameters of the analytical methods based on the users' task at hand. The action sequences performed on the graphical user interface by the user are consolidated in a dynamic knowledge base with specific hybrid reasoning that integrates symbolic and connectionist approaches. These sequences of expert knowledge acquisition can be very efficient for making easier knowledge emergence during a similar experience and positively impact the monitoring of critical situations. The provided graphical user interface incorporating a user-centered visual analysis is exploited to facilitate the natural and effective representation of clinical information for patient care
