    Design of a game-based training environment to enhance mental health care professionals' skills in using e-mental health:Multiple methods user requirements analysis

    Background: A major factor hampering the adoption of technology in mental health care is a lack of knowledge and skills. Serious gaming offers a potentially effective strategy to enhance the skills needed through experiencing and learning-by-doing in a playful way. However, serious gaming solutions are not widely available for mental health care. Therefore, the development of a game-based training environment in mental health care was pursued in a design project. The first step in such a design project is to identify user requirements that should be met. Objective: This study aims to deliver user requirements that inform the design of a game-based training environment for mental health care professionals. This environment aims to support mental health care professionals’ knowledge and skill enhancement regarding the use of e–mental health (eMH); for example, video calling, mobile apps, web-based treatment modules, and techniques such as virtual or augmented reality. Methods: We used an exploratory multiple methods design consisting of a web-based questionnaire, co-design sessions, and interviews. To ensure a good representation of the target user group, professionals from various disciplines within mental health care were included in the research. The multiple methods design facilitates a broad view of user needs and in-depth knowledge of specific design requirements. We describe the protocol for this research project in a protocol paper published in the JMIR Research Protocols in February 2021. Results: The user requirements analysis revealed three types of users for the envisioned game-based training environment: mental health care professionals who want to learn about the basic possibilities of eMH, mental health care professionals who want to develop their eMH skills to the next level, and mental health care professionals who want to experiment with new technologies. This reflects the diversity of needs that were identified, as well as the need to develop a diversity of suitable scenarios in the environment. User requirements analysis shows that the focus of a training environment should be on increasing knowledge about the possibilities of eMH, focusing on experiencing the benefits in particular situations, and building confidence in using eMH in a therapeutic setting. This requires careful consideration of the suitable game characteristics. Conclusions: Improvement of mental health care professionals’ skills in eMH requires an environment that is user driven and flexible, and simultaneously incorporates contextual factors that are relevant for its implementation in practice. This user requirements analysis contributes to the understanding of the issues that should be considered in the development of a game-based training environment. This shows that there are multiple and diverse learning needs among mental health care professionals. Various client populations, services, and situations demand various options for training.     Investigating resilience patterns based on within-subject changes in sleep and resting heart rate variability

    Occupational stress can cause all kinds of health problems. Resilience interventions that help employees deal with and adapt to adverse events can prevent these negative consequences. Due to advances in sensor technology and smartphone applications, relatively unobtrusive self-monitoring of resilience-related outcomes is possible. With models that can recognize intra-individual changes in these outcomes and relate them to causal factors within the employee’s own context, an automated resilience intervention that gives personalized, just-in-time feedback can be developed. The Wearables and app-based resilience Modelling in employees (WearMe) project aims to develop such models. A cyclical conceptual framework based on existing theories of stress and resilience is presented, as the basis for the WearMe project. The included concepts are operationalized and measured using sleep tracking (Fitbit Charge 2), heart rate variability measurements (Elite HRV + Polar H7) and Ecological Momentary Assessment (mobile app), administered in the morning (7 questions) and evening (12 questions). The first (ongoing) study within the WearMe project investigates the feasibility of the developed measurement cycle and explores the development of such models in social studies students that are on their first major internship. Analyses will target the development of both within-subject (n=1) models, as well as between-subjects models. The first results will be shared at the Health By Tech 2019 conference in Groningen. If successful, future work will focus on further developing these models and eventually exploring the effectiveness of the envisioned personalized resilience system

    Guidelines for the effective use of telemedicine in public healthcare in resource constrained settings

    On September 8 2000, world leaders gathered at the United Nations summit and resolved to help citizens in the world's poorest countries to achieve a better life by the year 2015. This resolve was outlined in the Millennium Development Goals that were subsequently published with goals 4, 5 and 6 specific to healthcare. The integration of ICT‘s in the remote delivery of services has opened new avenues from which centralised, scarce resources can be accessed remotely for the benefit of the general population. Telemedicine has made great strides in the developed world with remote populations benefiting from the improved access to healthcare. In the Eastern Cape Province of South Africa, the sub-field of teleradiology has shown promise for enabling the wider delivery of specialist services. However, in resource constrained settings such as developing countries, telemedicine has had limited success and as a result, the equitable access to healthcare for remote populations remains inconceivable. This is exacerbated by the migration of healthcare professionals both domestically and internationally. The public sector has suffered the most with acute staff shortages in the public healthcare institutions, more so in rural and remote areas. This study identifies the prevailing challenges posing as barriers to the effective use of telemedicine services in the Public health sector in resource constrained settings and provides recommendations and guidelines aimed at facilitating the adoption and effective use of telemedicine. Challenges are identified from literature and from the first person accounts of specific role-players who are directly involved with telemedicine in their respective institutions. Participants are drawn from institutions offering telemedicine services within the Eastern Cape Province of South Africa. Challenges identified from literature are collated with those identified from the participant interviews to provide a concise list of factors that is used as input to the recommendation and guideline development process. The results thus far point to an enthusiastic environment coupled with a semi-capable infrastructure but however hampered by staff shortages and a general lack of support structures and propulsion mechanisms to adequately encourage the wider use of telemedicine.     Diffusion of Technology in Small to Medium Medical Providers in Saudi Arabia

    The Saudi ministry of health reported that government health care spending doubled from 2008 to 2011. To address increased demand, the government encouraged small to medium enterprise (SME) growth. However, SME leaders could not leverage technology as a growth enabler because they lacked strategies to address operating inefficiencies associated with technology. Only 50% of hospitals fully implemented information technology. The purpose of this phenomenological study was to explore lived experiences of SME leaders on strategies needed to accelerate technology implementation. This exploration drew on a conceptual framework developed from Wainwright and Waring\u27s framework addressing issues of technology adoption. Data were collected from semistructured interviews of 20 SME leaders in Saudi Arabia. A modified van Kaam method was used to analyze participants\u27 interview transcripts in search of common themes. The main themes were strategies to address human resources, clinical teams, funding, and organizational and leadership alignment to accelerate the diffusion of technology. Findings indicated that insurance companies influence SME operations, growth, and survival. Analysis of findings revealed the need for change in management, training, implementation follow up, and staff retention to accelerate technology implementation. Application of findings has the potential to promote positive social change in guiding SME leaders to be change agents and enabling them to create a reliable, sustainable health care delivery system
