    IMAGE-BASED RESPIRATORY MOTION EXTRACTION AND RESPIRATION-CORRELATED CONE BEAM CT (4D-CBCT) RECONSTRUCTION

    Accounting for respiration motion during imaging helps improve targeting precision in radiation therapy. Respiratory motion can be a major source of error in determining the position of thoracic and upper abdominal tumor targets during radiotherapy. Thus, extracting respiratory motion is a key task in radiation therapy planning. Respiration-correlated or four-dimensional CT (4DCT) imaging techniques have been recently integrated into imaging systems for verifying tumor position during treatment and managing respiration-induced tissue motion. The quality of the 4D reconstructed volumes is highly affected by the respiratory signal extracted and the phase sorting method used. This thesis is divided into two parts. In the first part, two image-based respiratory signal extraction methods are proposed and evaluated. Those methods are able to extract the respiratory signals from CBCT images without using external sources, implanted markers or even dependence on any structure in the images such as the diaphragm. The first method, called Local Intensity Feature Tracking (LIFT), extracts the respiratory signal depending on feature points extracted and tracked through the sequence of projections. The second method, called Intensity Flow Dimensionality Reduction (IFDR), detects the respiration signal by computing the optical flow motion of every pixel in each pair of adjacent projections. Then, the motion variance in the optical flow dataset is extracted using linear and non-linear dimensionality reduction techniques to represent a respiratory signal. Experiments conducted on clinical datasets showed that the respiratory signal was successfully extracted using both proposed methods and it correlates well with standard respiratory signals such as diaphragm position and the internal markers’ signal. In the second part of this thesis, 4D-CBCT reconstruction based on different phase sorting techniques is studied. The quality of the 4D reconstructed images is evaluated and compared for different phase sorting methods such as internal markers, external markers and image-based methods (LIFT and IFDR). Also, a method for generating additional projections to be used in 4D-CBCT reconstruction is proposed to reduce the artifacts that result when reconstructing from an insufficient number of projections. Experimental results showed that the feasibility of the proposed method in recovering the edges and reducing the streak artifacts

    Surrogate driven respiratory motion model derived from CBCT projection data

    Cone Beam Computed Tomography (CBCT) is the most common imaging method for Image Guided Radiation Therapy (IGRT). However due to the slow rotating gantry, the image quality of CBCT can be adversely affected by respiratory motion, as it blurs the tumour and nearby organs at risk (OARs), which makes visualization of organ boundaries difficult, in particular for organs in the thoracic region. Currently one approach to tackle the problem of respiratory motion is the use of respiratory motion model to compensate for the motion during CBCT image reconstruction. The overall goal of this work is to estimate the 3D motion, including the breath-to-breath variability, on the day of treatment directly from the CBCT projection data, without requiring any external devices. The work presented here consist of two main parts: firstly, we introduce a novel data driven method based on Principal Component Analysis PCA, with the goal to extract a surrogate signal related to the internal anatomy from the CBCT projections. Secondly, using the extracted signals, we use surrogate-driven respiratory motion models to estimate the patient’s 3D respiratory motion. We utilized a recently developed generalized framework that unifies image registration and correspondence model fitting into a single optimization. This enables the model to be fitted directly to unsorted/unreconstructed data (CBCT projection data), thereby allowing an estimate of the patient’s respiratory motion on the day of treatment. To evaluate our methods, we have used an anthropomorphic software phantom combined with CBCT projection simulations.     Shape-correlated statistical modeling and analysis for respiratory motion estimation

    Respiratory motion challenges image-guided radiation therapy (IGRT) with location uncertainties of important anatomical structures in the thorax. Effective and accurate respiration estimation is crucial to account for the motion effects on the radiation dose to tumors and organs at risk. Moreover, serious image artifacts present in treatment-guidance images such 4D cone-beam CT cause difficulties in identifying spatial variations. Commonly used non-linear dense image matching methods easily fail in regions where artifacts interfere. Learning-based linear motion modeling techniques have the advantage of incorporating prior knowledge for robust motion estimation. In this research shape-correlation deformation statistics (SCDS) capture strong correlations between the shape of the lung and the dense deformation field under breathing. Dimension reduction and linear regression techniques are used to extract the correlation statistics. Based on the assumption that the deformation correlations are consistent between planning and treatment time, patient-specific SCDS trained from a 4D planning image sequence is used to predict the respiratory motion in the patient's artifact-laden 4D treatment image sequence. Furthermore, a prediction-driven atlas formation method is developed to weaken the consistency assumption, by integrating intensity information from the target images and the SCDS predictions into a common optimization framework. The strategy of balancing between the prediction constraints and the intensity-matching forces makes the method less sensitive to variation in the correlation and utilizes intensity information besides the lung boundaries. This strategy thus provides improved motion estimation accuracy and robustness. The SCDS-based methods are shown to be effective in modeling and estimating respiratory motion in lung, with evaluations and comparisons carried out on both simulated images and patient images

    Real-time intrafraction motion monitoring in external beam radiotherapy

    © 2019 Institute of Physics and Engineering in Medicine. Radiotherapy (RT) aims to deliver a spatially conformal dose of radiation to tumours while maximizing the dose sparing to healthy tissues. However, the internal patient anatomy is constantly moving due to respiratory, cardiac, gastrointestinal and urinary activity. The long term goal of the RT community to 'see what we treat, as we treat' and to act on this information instantaneously has resulted in rapid technological innovation. Specialized treatment machines, such as robotic or gimbal-steered linear accelerators (linac) with in-room imaging suites, have been developed specifically for real-time treatment adaptation. Additional equipment, such as stereoscopic kilovoltage (kV) imaging, ultrasound transducers and electromagnetic transponders, has been developed for intrafraction motion monitoring on conventional linacs. Magnetic resonance imaging (MRI) has been integrated with cobalt treatment units and more recently with linacs. In addition to hardware innovation, software development has played a substantial role in the development of motion monitoring methods based on respiratory motion surrogates and planar kV or Megavoltage (MV) imaging that is available on standard equipped linacs. In this paper, we review and compare the different intrafraction motion monitoring methods proposed in the literature and demonstrated in real-time on clinical data as well as their possible future developments. We then discuss general considerations on validation and quality assurance for clinical implementation. Besides photon RT, particle therapy is increasingly used to treat moving targets. However, transferring motion monitoring technologies from linacs to particle beam lines presents substantial challenges. Lessons learned from the implementation of real-time intrafraction monitoring for photon RT will be used as a basis to discuss the implementation of these methods for particle RT

    Time dependent cone-beam CT reconstruction via a motion model optimized with forward iterative projection matching

    The purpose of this work is to present the development and validation of a novel method for reconstructing time-dependent, or 4D, cone-beam CT (4DCBCT) images. 4DCBCT can have a variety of applications in the radiotherapy of moving targets, such as lung tumors, including treatment planning, dose verification, and real time treatment adaptation. However, in its current incarnation it suffers from poor reconstruction quality and limited temporal resolution that may restrict its efficacy. Our algorithm remedies these issues by deforming a previously acquired high quality reference fan-beam CT (FBCT) to match the projection data in the 4DCBCT data-set, essentially creating a 3D animation of the moving patient anatomy. This approach combines the high image quality of the FBCT with the fine temporal resolution of the raw 4DCBCT projection data-set. Deformation of the reference CT is accomplished via a patient specific motion model. The motion model is constrained spatially using eigenvectors generated by a principal component analysis (PCA) of patient motion data, and is regularized in time using parametric functions of a patient breathing surrogate recorded simultaneously with 4DCBCT acquisition. The parametric motion model is constrained using forward iterative projection matching (FIPM), a scheme which iteratively alters model parameters until digitally reconstructed radiographs (DRRs) cast through the deforming CT optimally match the projections in the raw 4DCBCT data-set. We term our method FIPM-PCA 4DCBCT. In developing our algorithm we proceed through three stages of development. In the first, we establish the mathematical groundwork for the algorithm and perform proof of concept testing on simulated data. In the second, we tune the algorithm for real world use; specifically we improve our DRR algorithm to achieve maximal realism by incorporating physical principles of image formation combined with empirical measurements of system properties. In the third stage we test our algorithm on actual patient data and evaluate its performance against gold standard and ground truth data-sets. In this phase we use our method to track the motion of an implanted fiducial marker and observe agreement with our gold standard data that is typically within a millimeter

    A biomechanical approach for real-time tracking of lung tumors during External Beam Radiation Therapy (EBRT)

    Lung cancer is the most common cause of cancer related death in both men and women. Radiation therapy is widely used for lung cancer treatment. However, this method can be challenging due to respiratory motion. Motion modeling is a popular method for respiratory motion compensation, while biomechanics-based motion models are believed to be more robust and accurate as they are based on the physics of motion. In this study, we aim to develop a biomechanics-based lung tumor tracking algorithm which can be used during External Beam Radiation Therapy (EBRT). An accelerated lung biomechanical model can be used during EBRT only if its boundary conditions (BCs) are defined in a way that they can be updated in real-time. As such, we have developed a lung finite element (FE) model in conjunction with a Neural Networks (NNs) based method for predicting the BCs of the lung model from chest surface motion data. To develop the lung FE model for tumor motion prediction, thoracic 4D CT images of lung cancer patients were processed to capture the lung and diaphragm geometry, trans-pulmonary pressure, and diaphragm motion. Next, the chest surface motion was obtained through tracking the motion of the ribcage in 4D CT images. This was performed to simulate surface motion data that can be acquired using optical tracking systems. Finally, two feedforward NNs were developed, one for estimating the trans-pulmonary pressure and another for estimating the diaphragm motion from chest surface motion data. The algorithm development consists of four steps of: 1) Automatic segmentation of the lungs and diaphragm, 2) diaphragm motion modelling using Principal Component Analysis (PCA), 3) Developing the lung FE model, and 4) Using two NNs to estimate the trans-pulmonary pressure values and diaphragm motion from chest surface motion data. The results indicate that the Dice similarity coefficient between actual and simulated tumor volumes ranges from 0.76±0.04 to 0.91±0.01, which is favorable. As such, real-time lung tumor tracking during EBRT using the proposed algorithm is feasible. Hence, further clinical studies involving lung cancer patients to assess the algorithm performance are justified
