Enabling Prescription-based Health Apps
We describe an innovative framework for prescription of personalised health
apps by integrating Personal Health Records (PHR) with disease-specific mobile
applications for managing medical conditions and the communication with
clinical professionals. The prescribed apps record multiple variables including
medical history enriched with innovative features such as integration with
medical monitoring devices and wellbeing trackers to provide patients and
clinicians with a personalised support on disease management. Our framework is
based on an existing PHR ecosystem called TreC, uniquely positioned between
healthcare provider and the patients, which is being used by over 70.000
patients in Trentino region in Northern Italy. We also describe three important
aspects of health app prescription and how medical information is automatically
encoded through the TreC framework and is prescribed as a personalised app,
ready to be installed in the patients' smartphone
User interface design for mobile-based sexual health interventions for young people: Design recommendations from a qualitative study on an online Chlamydia clinical care pathway
Background: The increasing pervasiveness of mobile technologies has given potential to transform healthcare by facilitating clinical management using software applications. These technologies may provide valuable tools in sexual health care and potentially overcome existing practical and cultural barriers to routine testing for sexually transmitted infections. In order to inform the design of a mobile health application for STIs that supports self-testing and self-management by linking diagnosis with online care pathways, we aimed to identify the dimensions and range of preferences for user interface design features among young people. Methods: Nine focus group discussions were conducted (n=49) with two age-stratified samples (16 to 18 and 19 to 24 year olds) of young people from Further Education colleges and Higher Education establishments. Discussions explored young people's views with regard to: the software interface; the presentation of information; and the ordering of interaction steps. Discussions were audio recorded and transcribed verbatim. Interview transcripts were analysed using thematic analysis. Results: Four over-arching themes emerged: privacy and security; credibility; user journey support; and the task-technology-context fit. From these themes, 20 user interface design recommendations for mobile health applications are proposed. For participants, although privacy was a major concern, security was not perceived as a major potential barrier as participants were generally unaware of potential security threats and inherently trusted new technology. Customisation also emerged as a key design preference to increase attractiveness and acceptability. Conclusions: Considerable effort should be focused on designing healthcare applications from the patient's perspective to maximise acceptability. The design recommendations proposed in this paper provide a valuable point of reference for the health design community to inform development of mobile-based health interventions for the diagnosis and treatment of a number of other conditions for this target group, while stimulating conversation across multidisciplinary communities
E-Health: Value Proposition and Technologies Enabling Collaborative Healthcare
E-health, enabled by ubiquitous computing and communication technologies, is facilitating a fundamental shift in the age old praxis of healthcare. It is revolutionizing healthcare in the 21st century in much the same way as the arrival of modern medicine and vaccines did in the 19th century. E-health lies at the intersection of diverse disciplines including healthcare, computer science, information systems, economics, and political science. The broad impact of E-health on diverse domains, complexity of supporting technologies, and the dizzying interplay of theories bridging multiple disciplines creates a rich problem space for information system researchers and calls on to conduct cross-disciplinary research. In the current paper we present salient characteristics of e-health and discuss its value proposition. The value proposition of e-health presents the entire range of processes supporting the healthcare sector. We also present research opportunities as e-health takes center stage in the delivery of healthcare
Usability and feasibility of consumer-facing technology to reduce unsafe medication use by older adults
Background
Mobile health technology can improve medication safety for older adults, for instance, by educating patients about the risks associated with anticholinergic medication use.
Objective
This study's objective was to test the usability and feasibility of Brain Buddy, a consumer-facing mobile health technology designed to inform and empower older adults to consider the risks and benefits of anticholinergics.
Methods
Twenty-three primary care patients aged â„60 years and using anticholinergic medications participated in summative, task-based usability testing of Brain Buddy. Self-report usability was assessed by the System Usability Scale and performance-based usability data were collected for each task through observation. A subset of 17 participants contributed data on feasibility, assessed by self-reported attitudes (feeling informed) and behaviors (speaking to a physician), with confirmation following a physician visit.
Results
Overall usability was acceptable or better, with 100% of participants completing each Brain Buddy task and a mean System Usability Scale score of 78.8, corresponding to âGoodâ to âExcellentâ usability. Observed usability issues included higher rates of errors, hesitations, and need for assistance on three tasks, particularly those requiring data entry. Among participants contributing to feasibility data, 100% felt better informed after using Brain Buddy and 94% planned to speak to their physician about their anticholinergic related risk. On follow-up, 82% reported having spoken to their physician, a rate independently confirmed by physicians.
Conclusion
Consumer-facing technology can be a low-cost, scalable intervention to improve older adultsâ medication safety, by informing and empowering patients. User-centered design and evaluation with demographically heterogeneous clinical samples uncovers correctable usability issues and confirms the value of interventions targeting consumers as agents in shared decision making and behavior change
Why digital medicine depends on interoperability
Digital data are anticipated to transform medicine. However, most of today's medical data lack interoperability: hidden in isolated databases, incompatible systems and proprietary software, the data are difficult to exchange, analyze, and interpret. This slows down medical progress, as technologies that rely on these data - artificial intelligence, big data or mobile applications - cannot be used to their full potential. In this article, we argue that interoperability is a prerequisite for the digital innovations envisioned for future medicine. We focus on four areas where interoperable data and IT systems are particularly important: (1) artificial intelligence and big data; (2) medical communication; (3) research; and (4) international cooperation. We discuss how interoperability can facilitate digital transformation in these areas to improve the health and well-being of patients worldwide

Web Vulnerability Study of Online Pharmacy Sites
Consumers are increasingly using online pharmacies, but these sites may not provide an adequate level of security with the consumersâ personal data. There is a gap in this research addressing the problems of security vulnerabilities in this industry. The objective is to identify the level of web application security vulnerabilities in online pharmacies and the common types of flaws, thus expanding on prior studies. Technical, managerial and legal recommendations on how to mitigate security issues are presented. The proposed four-step method first consists of choosing an online testing tool. The next steps involve choosing a list of 60 online pharmacy sites to test, and then running the software analysis to compile a list of flaws. Finally, an in-depth analysis is performed on the types of web application vulnerabilities. The majority of sites had serious vulnerabilities, with the majority of flaws being cross-site scripting or old versions of software that have not been updated. A method is proposed for the securing of web pharmacy sites, using a multi-phased approach of technical and managerial techniques together with a thorough understanding of national legal requirements for securing systems
