    Development of a prototype sensor-integrated urine bag for real-time measuring.

    The urine output is a rapid bedside test for kidney function, and reduced output is the common biomarker for an acute kidney injury (AKI). The consensus definition of the symptom is used urine output <0.5 ml/kg/hour for ≥6 hours to define AKI. If a patient is suspected to have this problem, the urine output monitoring needs to be done hourly, and this task consumes a lot of time, and easily affected by human errors. Moreover, available evidences in literatures indicate that more frequent patient monitoring could impact clinical decision making and patient’s outcome. However, it is not possible for nurses to dedicate their precious time manually up to minute manually measurements. To date, there is no reliable device has been used in the clinical routine. From the literatures, only a few automated devices were found with the ability to automatically monitor urine outputs, and could reduce nurse workload and at the same time enhance work performance, but these still have some limitations to measure human urine. In this thesis presents the development and testing for such a device. The research was aimed at building a prototype that could be measured a small amount of urine output, and transit information via wireless to a Cloud database with inexpensive and less complex components. The concept is to provide a real-time measurement and generates data records in Cloud database without requiring any intervention by the nurse. The initial experiment was done measure small amount of liquid using a dropvolume calculation technique. An optical sensor was placed in a medical dropper to record number of counted-drops, the Mean Absolute Percent Error from the test is reported ±3.96% for measuring 35 ml of liquid compared with the ISO standard. The second prototype was developed with multi-sensors, including photo interrupter sensor, infrared proximity sensor, and ultrasonic sensor, to detect the dripping and urine flow. However, the optical sensor still provided the most accuracy of all. The final prototype is based on the combination of optical sensor for detecting drops to calculated urine flow rate and its volume, and weight scales to measurement the weight of collected urine in a commercial urine meter. The prototype also provides an alert in two scenarios; when the urine production is not met the goals, and when the urine container is almost full, the system will automatically generate alarms that warn the nurse. Series of experimentation tests have been conducted under consultant of medical professional to verify the proper operation and accuracy in the measurement. The results are improved from the previous prototype. The mean error found of this version is 1.975% or ≈ ±1.215 ml. when measure 35ml of urine under the average density value of urine (1.020). These tests confirm the potential application of the device by assisting nurse to monitor urine output with the accuracy in the measurement. The use of the Cloud based technology has not been previously reported in the literature as far as can be ascertained. These results illustrated the capability, suitability and limitation of the chosen technology

    Alternative site for the placement of totally implantable vascular access device (TIVAD). A case report of two successful TIVAD implantations in the thigh after femoral vein catheterization

    Background: Totally implantable venous access devices (TIVADs) have improved the quality of life for seriously ill and cancer patients. These devices represent a convenient option when long-term venous access is indicated. The Subclavian and Internal Jugular Veins are the vessels of choice for catheterization [1]. However, if it is not possible to catheterize them, an alternative vein should be sought for [2]. Femoral vein can be used in such cases [3].Clinical problem: In 2 cases, it was not possible to catheterise any vein ending in the Superior Vena Cava and implant a TIVAD in the chest wall, although this was very necessary for them. Femoral vein was chosen despite higher risk of complications.Case 1: A 47 years old female with a metastatic breast cancer and infected ulcerations of the anterior chest wall. Veins in both arms were occluded. Her implanted TIVAD could not be used. Case 2: A 44 years old female who had a newly diagnosed lung cancer and Superior Vena Cava Syndrome. She was treated by a high-dose anti-coagulants.Surgical intervention: The catheter was inserted in the left femoral vein using ultrasound-guided percutaneous technique. After making a small incision, PORT-A-CATH® II POWER P.A.C. single-lumen standard port was implanted subcutaneously in the anterior surface of the left thigh. Verification of the catheter’s tip intra-operatively was difficult in Case 1 due to fluoroscopy problems. Prior consideration of the required instruments prevented the occurrence of a similar problem in Case 2. We performed these operations in the University Hospital of Norrland in Sweden in 2013.Follow-up: Apart from later adjustment of the catheter positioning in Case 1, we did not get any complications or problems with the use of the TIVAD. Frequent flushing of the device was recommended. Patients’ and staff’s satisfaction were good. Conclusion: Placement of TIVAD in the thigh is to be considered when the veins of the neck and upper arm are not accessible or the area on the chest wall is not appropriate for implanting the device.     Advancing Torpor Inducing Transfer Habitats for Human Stasis to Mars

    SpaceWorks Enterprises, Inc. (SEI) has conducted an evaluation of an advanced habitat system designed to transportcrews between the Earth and Mars. This new and innovative habitat concept is capable of placing crew members ininactive, torpor states during transit phases of a deep space mission. This substantially reduces the mass and size ofthe habitat, which ultimately leads to significant reductions in the overall architecture size.Our approach for achieving this is based on extending the current and evolving medical practice of TherapeuticHypothermia (TH) - a proven and effective treatment for various traumatic injuries. TH is a medical treatment thatlowers a patient's body temperature by just 5 to 10 degrees Fahrenheit causing human metabolic rate to decreasesignificantly and the body to enter an unconscious state. This method avoids the intractable challenges often associatedwith cell metabolic cessation through cryogenic freezing and other highly speculative approaches.The initial results obtained from the research and analysis conducted in the Phase I effort warranted further study ofthis concept and technology. The specific objectives of the continued work include:1. Addressing critical medical aspects and risks for inducing torpor via Therapeutic Hypothermia and theapproach for providing nutrition and hydration for the crew during torpor. 2. Focusing on mitigation aspects and technology potential for solving key human spaceflight challenges. 3. Addressing critical engineering aspects of the design that may impact the initial performance and cost resultsobtained in Phase I. 4. Examining the broader extensibility and enabling capabilities of this concept through applicability toadditional exploration missions beyond Mars. 5. Establishing a technology development roadmap, addressing both medical and engineering aspects, thatindicate a logical and scientifically achievable path forward for maturation of this technology

    The authors will present a comprehensive account of the neurological aspects of SARS-CoV-2 infection. The aim is to provide a practical clinical book which will serve as a guide for clinicians from all specialties involved in the management of COVID-19 patients. The authors share the extensive clinical experience gained in major hospitals in Lombardy, the first European region to face the COVID-19 emergency in 2020. All are recognized international experts in their respective fields and have been involved in the management of COVID-19 cases from the very beginning of the Italian SARS-CoV-2 outbreak. The text begins with a description of pathobiological and pathophysiological aspects related to the involvement of the nervous system, moving on to the discussion of the neurological complications observed in COVID-19 patients; these range from central to peripheral symptoms, and can occur in the acute or post-acute phases of the disease. Further topics are: neuropathology, seizures and EEG, neuroimaging, delirium, encephalomyelitis, stroke, psychopathology and psychiatry, neuropsychology and cognitive impairment, neuromuscu-lar disorders, and the impact of COVID-19 on other pre-existing neurological disorders. In addi-tion, the book will discuss the new developments in teleneurology approaches, which have been a direct response to the ongoing pandemic.
