    Feasibility of waveform capnography as a non-invasive monitoring tool during cardiopulmonary resuscitation

Cardiopulmonary resuscitation (CPR) is one of the key interventions for patient survival after SCA, a life-saving procedure that combines chest compressions and ventilations to maintain a minimal oxygenated blood flow.To deliver oxygen, an adequate blood flow must be generated, by effective CPR, during the majority of the cardiac arrest time. Although monitoring the quality of CPR performed by rescuers during cardiac arrest has been a huge step forward in resuscitation science, in 2013, a consensus statement from the American Heart Association prioritized a new type of CPR quality monitoring focused on the physiological response of the patient instead of how the rescuer is doing.To that end, current resuscitation guidelines emphasize the use of waveform capnography during CPR for patient monitoring. Among several advantages such as ensure correct tube placement, one of its most important roles is to monitor ventilation rate, helping to avoid potentially harmful over-ventilation. In addition, waveform capnography would enable monitoring CPR quality, early detection of ROSC and determining patient prognosis. However, several studies have reported the appearance of fast oscillations superimposed on the capnogram, hereinafter CC-artifact, which may hinder a feasible use of waveform capnography during CPR. In addition to the possible lack of reliability, several factors need to be taken into account when interpreting ETCO2 measurements. Chest compressions and ventilation have opposing effects on ETCO2 levels. Chest compressions increase CO2 concentration, delivering CO2 from the tissues to the lungs, whilst ventilations remove CO2 from the lungs, decreasing ETCO2. Thus, ventilation rate acts as a significant confounding factor.This thesis analyzes the feasibility of waveform capnography as non-invasive monitoring tool of the physiological response of the patient to resuscitation efforts. A set of four intermediate goals was defined.First, we analyzed the incidence and morphology of the CC-artifact and assessed its negative influence in the detection of ventilations and in ventilation rate and ETCO2 measurement. Third, we applied a novel strategy to model the impact of ventilations and ventilation rate on the exhaled CO2 measured in out-of-hospital cardiac arrest capnograms, which could allow to measure the change in ETCO2 attributable to chest compressions by removing the influence of concurrent ventilations. Finally, we studied if the assessment of the ETCO2 trends during chest compressions pauses could allow to detect return of spontaneous circulation, a metric that could be useful as an adjunct to other decision tool

    Sudden cardiac arrest (SCA) is the sudden cessation of the heart’s effective pumping function, confirmed by the absence of pulse and breathing. Without appropriate treatment, it leads to sudden cardiac death, considered responsible for half of the global cardiac disease deaths. Cardiopulmonary resuscitation (CPR) is a key intervention during SCA. Current resuscitation guidelines emphasize the use of waveform capnography during CPR in order to enhance CPR quality and improve patient outcomes. Capnography represents the concentration of the partial pressure of carbon dioxide (CO2) in respiratory gases and reflects ventilation and perfusion of the patient. Waveform capnography should be used for confirming the correct placement of the tracheal tube and monitoring ventilation. Other potential uses of capnography in resuscitation involve monitoring CPR quality, early identification of restoration of spontaneous circulation (ROSC), and determination of patient prognosis. An important role of waveform capnography is ventilation rate monitoring to prevent overventilation. However, some studies have reported the appearance of high-frequency oscillations synchronized with chest compressions superimposed on the capnogram. This chapter explores the incidence of chest compression artifact in out-of-hospital capnograms, assesses its negative influence in the automated detection of ventilations, and proposes several methods to enhance ventilation detection and capnography waveform

    Tesis inglés 218 p. -- Tesis euskera 220 p.Out-of-hospital cardiac arrest (OHCA ) is characterized by the sudden loss of the cardiac function, andcauses around 10% of the total mortality in developed countries. Survival from OHCA depends largelyon two factors: early defibrillation and early cardiopulmonary resuscitation (CPR). The electrical shock isdelivered using a shock advice algorithm (SAA) implemented in defibrillators. Unfortunately, CPR mustbe stopped for a reliable SAA analysis because chest compressions introduce artefacts in the ECG. Theseinterruptions in CPR have an adverse effect on OHCA survival. Since the early 1990s, many efforts havebeen made to reliably analyze the rhythm during CPR. Strategies have mainly focused on adaptive filtersto suppress the CPR artefact followed by SAAs of commercial defibrillators. However, these solutionsdid not meet the American Heart Association¿s (AHA) accuracy requirements for shock/no-shockdecisions. A recent approach, which replaces the commercial SAA by machine learning classifiers, hasdemonstrated that a reliable rhythm analysis during CPR is possible. However, defibrillation is not theonly treatment needed during OHCA, and depending on the clinical context a finer rhythm classificationis needed. Indeed, an optimal OHCA scenario would allow the classification of the five cardiac arrestrhythm types that may be present during resuscitation. Unfortunately, multiclass classifiers that allow areliable rhythm analysis during CPR have not yet been demonstrated. On all of these studies artefactsoriginate from manual compressions delivered by rescuers. Mechanical compression devices, such as theLUCAS or the AutoPulse, are increasingly used in resuscitation. Thus, a reliable rhythm analysis duringmechanical CPR is becoming critical. Unfortunately, no AHA compliant algorithms have yet beendemonstrated during mechanical CPR. The focus of this thesis work is to provide new or improvedsolutions for rhythm analysis during CPR, including shock/no-shock decision during manual andmechanical CPR and multiclass classification during manual CPR

    C-Trend parameters and possibilities of federated learning

    Abstract. In this observational study, federated learning, a cutting-edge approach to machine learning, was applied to one of the parameters provided by C-Trend Technology developed by Cerenion Oy. The aim was to compare the performance of federated learning to that of conventional machine learning. Additionally, the potential of federated learning for resolving the privacy concerns that prevent machine learning from realizing its full potential in the medical field was explored. Federated learning was applied to burst-suppression ratio’s machine learning and it was compared to the conventional machine learning of burst-suppression ratio calculated on the same dataset. A suitable aggregation method was developed and used in the updating of the global model. The performance metrics were compared and a descriptive analysis including box plots and histograms was conducted. As anticipated, towards the end of the training, federated learning’s performance was able to approach that of conventional machine learning. The strategy can be regarded to be valid because the performance metric values remained below the set test criterion levels. With this strategy, we will potentially be able to make use of data that would normally be kept confidential and, as we gain access to more data, eventually develop machine learning models that perform better. Federated learning has some great advantages and utilizing it in the context of qEEGs’ machine learning could potentially lead to models, which reach better performance by receiving data from multiple institutions without the difficulties of privacy restrictions. Some possible future directions include an implementation on heterogeneous data and on larger data volume.C-Trend-teknologian parametrit ja federoidun oppimisen mahdollisuudet. TiivistelmĂ€. TĂ€ssĂ€ havainnointitutkimuksessa federoitua oppimista, koneoppimisen huippuluokan lĂ€hestymistapaa, sovellettiin yhteen Cerenion Oy:n kehittĂ€mÀÀn C-Trend-teknologian tarjoamaan parametriin. Tavoitteena oli verrata federoidun oppimisen suorituskykyĂ€ perinteisen koneoppimisen suorituskykyyn. LisĂ€ksi tutkittiin federoidun oppimisen mahdollisuuksia ratkaista yksityisyyden suojaan liittyviĂ€ rajoitteita, jotka estĂ€vĂ€t koneoppimista hyödyntĂ€mĂ€stĂ€ tĂ€yttĂ€ potentiaaliaan lÀÀketieteen alalla. Federoitua oppimista sovellettiin purskevaimentumasuhteen koneoppimiseen ja sitĂ€ verrattiin purskevaimentumasuhteen laskemiseen, johon kĂ€ytettiin perinteistĂ€ koneoppimista. Kummankin laskentaan kĂ€ytettiin samaa dataa. Sopiva aggregointimenetelmĂ€ kehitettiin, jota kĂ€ytettiin globaalin mallin pĂ€ivittĂ€misessĂ€. Suorituskykymittareiden tuloksia verrattiin keskenÀÀn ja tehtiin kuvaileva analyysi, johon sisĂ€ltyi laatikkokuvioita ja histogrammeja. Odotetusti opetuksen loppupuolella federoidun oppimisen suorituskyky pystyi lĂ€hestymÀÀn perinteisen koneoppimisen suorituskykyĂ€. MenetelmÀÀ voidaan pitÀÀ pĂ€tevĂ€nĂ€, koska suorituskykymittarin arvot pysyivĂ€t alle asetettujen testikriteerien tasojen.     VLSI Circuits for Bidirectional Neural Interfaces

    Medical devices that deliver electrical stimulation to neural tissue are important clinical tools that can augment or replace pharmacological therapies. The success of such devices has led to an explosion of interest in the field, termed neuromodulation, with a diverse set of disorders being targeted for device-based treatment. Nevertheless, a large degree of uncertainty surrounds how and why these devices are effective. This uncertainty limits the ability to optimize therapy and gives rise to deleterious side effects. An emerging approach to improve neuromodulation efficacy and to better understand its mechanisms is to record bioelectric activity during stimulation. Understanding how stimulation affects electrophysiology can provide insights into disease, and also provides a feedback signal to autonomously tune stimulation parameters to improve efficacy or decrease side-effects. The aims of this work were taken up to advance the state-of-the-art in neuro-interface technology to enable closed-loop neuromodulation therapies. Long term monitoring of neuronal activity in awake and behaving subjects can provide critical insights into brain dynamics that can inform system-level design of closed-loop neuromodulation systems. Thus, first we designed a system that wirelessly telemetered electrocorticography signals from awake-behaving rats. We hypothesized that such a system could be useful for detecting sporadic but clinically relevant electrophysiological events. In an 18-hour, overnight recording, seizure activity was detected in a pre-clinical rodent model of global ischemic brain injury. We subsequently turned to the design of neurostimulation circuits. Three critical features of neurostimulation devices are safety, programmability, and specificity. We conceived and implemented a neurostimulator architecture that utilizes a compact on-chip circuit for charge balancing (safety), digital-to-analog converter calibration (programmability) and current steering (specificity). Charge balancing accuracy was measured at better than 0.3%, the digital-to-analog converters achieved 8-bit resolution, and physiological effects of current steering stimulation were demonstrated in an anesthetized rat. Lastly, to implement a bidirectional neural interface, both the recording and stimulation circuits were fabricated on a single chip. In doing so, we implemented a low noise, ultra-low power recording front end with a high dynamic range. The recording circuits achieved a signal-to-noise ratio of 58 dB and a spurious-free dynamic range of better than 70 dB, while consuming 5.5 ÎŒW per channel. We demonstrated bidirectional operation of the chip by recording cardiac modulation induced through vagus nerve stimulation, and demonstrated closed-loop control of cardiac rhythm
