Fenomen dobra w perspektywie opieki hospicyjnej
An aim of this article is to depict a phenomenon of good, understood as a ethical attitude tied with specific psychological after-effects, in caring on dying people. As it is shown, the good itself lies both on basics of modern hospice movement, as well as in daily practice of first hospice that realizes its goals – that is St Christopher’s Hospice in London, which is an exemplar and source of inspiration for many others established in whole World
Palliative home care for children in Poland
Background. The aim of the study is to evaluate the current status of palliative
home care for children in Poland.
Material and methods. The survey based on the questionnaire, which analyzed
standard of care, annual number of patients and deaths. The annual number of treated
children per million of inhabitants was calculated. The study encompassed 31 hospices
(7 for children and 24 for adults).
Results. The majority of the hospices do not fulfil the minimal standard
established by the Institute of Mother and Child. The average standard was higher
in the children’s hospices than in the adults’ ones. In 2003 the number of patients
(< 18 years) was 329 (111 cancer patients), and the number of deaths 102. The
total home care area covers for 79% of the population of Poland. The number of
treated children per million inhabitants is diverse in different provinces, the
highest in the Lublin province and the lowest in the Świętokrzyski province.
Conclusions. The Polish system of palliative home care for children is
differentiated in regard to standard of care and the number of patients in respective
regions of Poland. There are still regions where palliative home care for children
is not accessible.Wstęp. Celem badania jest ocena aktualnego stanu domowej opieki paliatywnej
nad dziećmi w Polsce.
Materiał i metody. Za pomocą ankiety oceniono standard opieki oraz roczną
liczbę pacjentów i zgonów. Obliczono wskaźnik określający liczbę leczonych dzieci
na milion mieszkańców. Badaniem objęto 31 hospicjów (7 hospicjów dla dzieci i
24 hospicja dla dorosłych).
Wyniki. Większość hospicjów nie spełnia minimalnego standardu określonego
przez Instytut Matki i Dziecka. Średni standard w hospicjach dla dzieci jest wyższy
niż w hospicjach dla dorosłych. W 2003 roku liczba leczonych pacjentów (< 18 lat)
wyniosła 329 (w tym dzieci z chorobami nowotworowymi 111), a liczba zgonów 102.
Ludność zamieszkująca rejony objęte opieką przez badane hospicja to 79% populacji
kraju. Liczba leczonych pacjentów pediatrycznych na milion mieszkańców jest zróżnicowana
w poszczególnych województwach - najwyższa w województwie lubelskim, a najniższa
w świętokrzyskim.
Wnioski. W Polsce system domowej opieki paliatywnej nad dziećmi jest zróżnicowany
zarówno pod względem standardu świadczonej opieki, jak i liczby leczonych pacjentów
w poszczególnych regionach Polski. Nadal w naszym kraju istnieją obszary, gdzie
domowa opieka paliatywna nad dziećmi nie jest dostępna

This article characterizes the interactions and institutions in which parents can receive support after their child has been diagnosed with a lethal defect. First and foremost parents need information and emotional support when making decision regarding their child. The primary source of the aforementioned elements is the hospital personnel and the perinatal hospice- that is if parents decide to contact it. If the child is born alive and discharged from the hospital then the parents require support during its treatment (which more often than not is identical with palliativecare) and rehabilitation, even social rehabilitation. Children’s hospices can offer many services in this particular matter. On each stage of the child’s disease the contact with other parents of children with lethal defect is crucial. Visual communication of hospice orgnizations and its influence on shaping social awareness
W dyskursie medialnym bardzo silnie obecny jest temat śmierci, której obraz pozbawiony jest jej rzeczywistej wartości. Hospicja, które działają na rzecz poprawy jakości życia osób chorych terminalnie, poprzez komunikaty kierowane do swojego otoczenia przywracają tematowi śmierci jego właściwy wymiar, wpływając na postawy społeczeństwa i jego postrzeganie. Organizacje hospicyjne w codziennej działalności wykorzystują narzędzia z zakresu public relations. Wyspecjalizowany PR (związany ze służbą zdrowia i organizacjami pożytku publicznego) nastawiony jest na symetryczne komunikowanie dwukierunkowe, podczas którego wypracowuje się wzajemne zrozumienie na linii organizacja-otoczenie.In the media discourse the topic of death is very strongly represented and its image is deprived of the real value. Hospices, which work to improve the quality of life of terminally ill people, through messages directed to their environment, restore the right to death, affecting the attitudes and perceptions of society. Hospice organizations use public relations tools in their daily activities to reach as many members of their target groups as possible. Specialized PR (connected both with the health service and public relations of public benefit organizations) is focused on symmetrical bi-directional communication, during which mutual understanding on the line of the organization - the environment is developed
Economic, Social and Existential Dimension of the Functioning of Hospice Institutions and Hospice Voluntary Services
The functioning of hospice institutions may be considered based on three fundamental, functions
accomplished by it, i.e. existential, social and economic ones. Holistic evaluation of hospice care
requires taking into account actual and potential advantages, possible to achieve both through society
represented by all citizens, and through the economy.
The article has a survey character – the literature is based on critical analysis within the scope
ofthe subject raised. The work was made rich from two-year own experience of volunteer work.
For the purpose of realization of the subject raised, empirical examinations were also conducted based
on the method of shadow prices and logical matrix.
In this paper, the implementation of the function of an existential volunteer hospice is considered
on the basis of its ability to alleviate the suffering of patients’ mental and spiritual capacity and give
them practical help by volunteers. The usefulness of hospices, which is a part of their social function,
was studied by considering the point of existence and functioning of palliative and long-term care
centres. In order to define the economic function of a voluntary service, micro and macroeconomic
types of analysis were used. One should, however, emphasise that these examinations should have
an interdisciplinary character

Światowa Organizacja Zdrowia zdefiniowała opiekę paliatywną jako aktywną, całościową opiekę obejmującą
pacjentów z zaawansowaną postępującą chorobą. Jej realizacja wymaga zaangażowania interdyscyplinarnego
zespołu, złożonego z lekarzy, pielęgniarek, psychologów, pracowników socjalnych, duchownych i wolontariuszy.
Farmaceuta, jako członek tego zespołu, również powinien odgrywać istotną rolę w opiece paliatywno-
hospicyjnej. W ramach jego działań wyróżnia się obszary: kliniczny, organizacyjny, administracyjny
i edukacyjny. Opieka farmaceutyczna ma także wymiar ekonomiczny, co pozwala na oszczędne gospodarowanie
wydatkami dzięki racjonalizacji i optymalizacji kosztów terapii. Znajomość uwarunkowań prawnych
dotyczących świadczenia usług farmaceutycznych i aspektów organizacyjnych jest niezbędna dla zapewnienia
prawidłowej realizacji zadań przez farmaceutę i innych członków zespołu terapeutycznego
Opieka paliatywna w Polsce - od idei do praktyki (również lekarza rodzinnego)
Opieka paliatywna jest zespołową, wielodyscyplinarną i dynamiczną opieką, w centrum
której znajdują się pacjent i jego rodzina. Oczywistym celem szeroko pojętej opieki nad
przewlekle chorym, wobec niemożności jego wyleczenia, jest poprawa funkcjonowania
i przedłużenie życia przez zastosowanie nowoczesnych technik i sposobów terapii. We
współczesnym „społeczeństwie życia” często jednak zapomina się o drugim aspekcie
postępowania z człowiekiem chorym, czyli o uldze w cierpieniu, zapewnieniu dobrej
jakości życia oraz właściwej opieki w okresie umierania.
W niniejszym opracowaniu przedstawiono formy opieki paliatywnej dostępnej w Polsce
oraz praktyczne aspekty związane z jej funkcjonowaniem dotyczące bezpośrednio
lekarza rodzinnego i jego podopiecznych
