Addressing illicit trafficking through a crime convergence lens: towards a model for cross-border cooperation in Africa
Scholars and practitioners alike have found illicit cross-border flows - from drugs to wildlife and weapons - to converge and reinforce each other while often being embedded in large transnational criminal networks. However, practitioners still lack the tools and guidance to effectively combat these links in an integrated fashion. Cross-border cooperation initiatives across Africa are in a unique position to mitigate illicit trafficking, and yet, there is no conceptual model that could guide initiatives in doing so. This Knowledge Note seeks to bridge this gap in the context of BICC's small arms control work in Africa. It provides an initial selection of six building blocks (I-VI) that could structure and guide our future work on cross-border cooperation to reduce the illicit trafficking of small arms and light weapons (SALW) and ammunition in East Africa and beyond. The proposed model brings together a review of practice and policy advice on different illicit flows, thus showing how they are connected. It embraces the opportunity to put on a broader lens and look beyond the realm of arms trafficking. Across the six building blocks, the willingness of all actors to engage in long-term, sustainable and evidence-based cooperation as well as to ensure political buy-in at all levels seem crucial for cross-border efforts to be effective. Follow-up engagements and testing in the field are to refine this desk study's initial model

Technical Note: Quantification of interferences of wet chemical HONO measurements under simulated polar conditions
International audienceIn the present pilot study, an optimized LOPAP instrument for the detection of nitrous acid (HONO) in the atmosphere (DL 0.2 pptV) was tested at the high alpine research station "Jungfraujoch" at 3580 m altitude in the Swiss Alps under conditions comparable to polar regions. HONO concentrations in the range <0.5?50 pptV with an average of 7.5 pptV were observed at the "Jungfraujoch". The diurnal profiles obtained exhibited clear maxima at noon and minima with very low concentration during the night supporting the proposed photochemical production of HONO. In good agreement with recent measurements at the South Pole, it was demonstrated, that interferences of chemical HONO instruments can significantly influence the measurements and lead to considerable overestimations, especially for low pollution level. Accordingly, the correction of interferences is of paramount importance for those instruments, which sample HONO on aqueous or humid surfaces

Neuroendokrinologische und psychophysiologische Auswirkungen von Musik als Mittel zur Stressreduzierung vor gynÀkologischen Operationen
Die Wartezeit vor operativen Eingriffen ist in der Regel fĂŒr die meisten Menschen
eine groĂe psychische Belastung. Messbar ist dieser enorme Stress neben
subjektiven Angaben zum persönlichen Befinden auch anhand objektiver
physiologischer VerÀnderungen wie z.B. von Blutdruck und Puls oder
endokrinologischen Stressparametern. Diese, durch die groĂen prĂ€operativen
Ăngste ausgelösten psychovegetativen Reaktionen, machen erhöhte Narkosedosen
erforderlich und verlangsamen die postoperative Wundheilung. Mit dieser Studie
sollte geprĂŒft werden, ob die Möglichkeit, prĂ€operativ Musik zu hören, neben
etablierten PrÀmedikationsmethoden messbare endokrinologische Stressparameter
beeinflusst und die subjektive SelbsteinschÀtzung der LebensqualitÀt verbessert.
133 Patientinnen (Durchschnittsalter 43,5 Jahre), einer gynÀkologischen Station, bei
denen ein operativer Eingriff bevorstand, wurden nach dem Zufallsprinzip in zwei
Gruppen aufgeteilt. Die eine Gruppe (n=63) erhielt die Möglichkeit, prĂ€operativ ĂŒber
einen Walkman Musik zu hören, die andere Gruppe (n=70) diente als Kontrolle und
bekam keine Musik. Zur Beurteilung des Einflusses der Musik auf den objektiven
Stress wurden bei dem gesamten Kollektiv zwei Blutentnahmen zur Bestimmung der
Hormone Prolaktin, Cortisol und DHEAS durchgefĂŒhrt, wovon die erste bei
stationÀrer Aufnahme und die zweite unmittelbar vor der Narkoseeinleitung erfolgte.
ZusÀtzlich waren alle Studienteilnehmerinnen angehalten, am ersten prÀ- und ersten
postoperativen Tag jeweils einen standardisierten Fragebogen (visuelle Analogskala
0-10) auszufĂŒllen, mit dem das subjektive Befinden erfasst wurde sowie die
psychische Hilfe der Musik beurteilt werden konnte.
Die durchgefĂŒhrte Untersuchung zeigte, dass unter dem Einfluss der Musik ein
signifikant geringerer Anstieg der den Stress widerspiegelnden Hormone Prolaktin
(p=0,009) und DHEAS (p<0,05) erfolgt, wohingegen Cortisol mit Musik zwar weniger
anstieg, der Unterschied aber keine statistische Signifikanz erreichte. Der subjektive
psychische Zustand der Frauen in Bezug auf Traurigkeit, Wohlbefinden und Ăngste
verbesserte sich perioperativ eindeutig mehr mit der Möglichkeit, vor der Operation
Musik zu hören, und die belastende Wartezeit auf den Eingriff wurde sogar als
signifikant leichter eingestuft (p<0,05). Am meisten hörten die Patientinnen
Popmusik, vor Meditation und Klassik. 71% empfanden beim Hören der Musik
Ablenkung, 51% Ruhe und 50% Entspannung, wÀhrend nur 3% Trauer und 2%
Angst fĂŒhlten. Die Möglichkeit, Musik hören zu können, empfanden die Frauen dabei
als sehr gut (8,5; 0-10) und hilfreich (7,5; 0-10) in der Operationsvorbereitung. Auf
den Zustand der Patientinnen im Aufwachraum sowie auf die Gesamtbewertung des
Klinikaufenthaltes hatte die prÀoperative Musik keinen Einfluss.
Insgesamt fĂŒhrte der Einsatz von Musik in der Wartezeit vor Operationen zu einer,
an endokrinologischen Parametern gemessenen, objektiv verminderten
Stressbelastung und wurde auch von den Patientinnen als sehr positiv und hilfreich
erlebt. Hieraus ergibt sich der durchaus sinnvolle Einsatz von Musik auch als
psychische StĂŒtze vor Eingriffen in der operativen GynĂ€kologie
Klinisch-funktionelle Nachuntersuchung von Patienten, die mittels B-Twin Expandable-Spinal-Spacer an der LendenwirbelsÀule operiert wurden
Die optimale Behandlung der degenerativen Spondylolisthesis wird vielfach diskutiert und ist nicht abschlieĂend geklĂ€rt. Auch das bestmögliche therapeutische Procedere bei sekundĂ€ren InstabilitĂ€ten nach knöcherner Dekompression und die optimale Versorgung von wiederholten Rezidiv-BandscheibenvorfĂ€llen mit konsekutiver Höhenminderung des Zwischenwirbelraums sind nicht eindeutig definiert.
Derzeit kommen verschiedene Therapien zum Einsatz: konservatives Vorgehen, interspinöse Implantate, minimalinvasive oder percutane Stabilisierungs-Operationen, ein- oder zweizeitig durchgefĂŒhrte 360° Verschraubung. Eine weitere Therapiemöglichkeit ist das PLIF-Verfahren (posterior lumbar interbody fusion), welches seit Jahren etabliert ist und gute Ergebnisse erzielt (70% - 90% Patientenzufriedenheit). Bisher konnte noch kein PLIF-Implantat als deutlich ĂŒberlegen identifiziert werden. Die guten Ergebnisse werden durch die zum Teil hohen Komplikationsraten beeintrĂ€chtigt: Verletzungen der Dura werden in 5,4% der FĂ€lle berichtet, bei 9-16% kommt es zu einer Verletzung der Nervenwurzel.
Ein neues Verfahren ist der B-Twin Expandable-Spinal-Spacer. Bei dem in der vorliegenden Arbeit untersuchten Patientenkollektiv wurde er als âstand alone PLIFâ eingesetzt. Im Gegensatz zu konventionellen PLIF-Cages wird der B-Twin erst im Bandscheibenfach auf seine volle GröĂe expandiert. Die Vorteile dieser Implantierungs-Methode bestehen vor allem in einer Schonung der umgebenden Strukturen, wie Muskulatur, Cauda Equina und Nervenwurzeln. Ziel der vorliegenden Arbeit war es, die Ergebnisse der B-Twin-Operation erstmals an einem europĂ€ischen Patienten-Kollektiv zu evaluieren.
Es erfolgte die klinische Untersuchung der mittels B-Twin operierten Patienten sowie die Auswertung von drei Messinstrumenten (Marburg Score, Oswestry Disability Index und Visuell analoge Schmerzskala) zu drei Messzeitpunkten: prĂ€operativ (T1), unmittelbar postopertiv (T2) und bei der Nachuntersuchung (T3). Es wurde ein Kollektiv von 97 Patienten untersucht. AnschlieĂend erfolgte die systematische Einbindung der hier vorliegenden Ergebnisse in die Geamtdatenbasis zum B-Twin-Verfahren. Hierzu wurden alle bis zum 1.Juni 2010 veröffentlichten Studien ĂŒber das B-Twin-Verfahren meta-analytisch zusammengefasst. Es wurden die Standardabweichungseinheiten (Cohens d) als MaĂ fĂŒr die EffektstĂ€rken berechnet. Insgesamt konnten so die postoperativen Ergebnisse von 261 Patienten ausgewertet werden. Von den operierten Patienten waren 56 Frauen (57,7%) und 41 MĂ€nner (42,3%), das Alter zum Zeitpunkt der Operation betrug im Durchschnitt 61,2 Jahre. Die Indikation zur Operation war bei 67 Patienten eine degenerative Spondylolisthesis (maximal Grad I nach Meyerding), bei 15 Patienten eine sekundĂ€re InstabilitĂ€t nach stattgehabter knöcherner Dekompression einer Spinalkanalstenose und bei 15 Patienten ein Rezidiv-Bandscheibenvorfall mit Höhenminderung des Bandscheibenfachs. Bei der Nachuntersuchung lag die Implantation des B-Twin im Mittel 26 Monate zurĂŒck. Der ODI-Wert war postoperativ (T2) und bei der Nachuntersuchung (T3) signifikant niedriger als prĂ€operativ (T1): ODI T1=36,60 SD=11,86; ODI T3=22,09 SD=9,06; p&lt;0.01. Auch die VAS-Werte waren postoperativ signifikant niedriger: VAS T1=8,7 SD=1,45; VAS T2=3,57 SD=2,80; VAS T3=3,74 SD=2,60; p&lt;0.05. Bei keinem der operierten Patienten wurde die Dura oder eine Nervenwurzel verletzt. Verletzungen von groĂen BlutgefĂ€Ăen traten ebenfalls in keinem Fall ein.
In der Zusammenschau lassen sich bei allen integrierten Studien sehr groĂe EffekstĂ€rken in Form von Cohens d beobachten: unmittelbar postoperativ lag der VAS-Wert im Mittel um d=4.02 Standardabweichungseinheiten unter dem prĂ€operativen Ausgangswert, zum Zeitpunkt der Nachuntersuchung (M=21,2 Monate) war d=5.20 SD . Der ODI lag zum Zeitpunkt der Nachuntersuchung im Mittel d=6,52 SD unter dem Ausgangswert.
Die vorliegende Arbeit zeigt die Wirksamkeit des B-Twin-Verfahrens in einem kleinen Patienten-Kollektiv, lĂ€sst weitere möglichst prospektiv und multizentrisch ausgelegte Studien jedoch keinesfalls unnötig werden. Die weiteren Arbeiten bestĂ€tigen ebenfalls die Wirksamkeit des Verfahrens. In keiner Studie sind gröĂere Komplikationen, wie Verletzungen der Dura oder eines groĂen BlutgefĂ€Ăes berichtet. Es ergeben sich Hinweise auf einen stĂ€rkeren Vorteil durch die Operation bei Patienten, deren RĂŒckenschmerzen noch nicht chronifiziert sind (Beschwerdedauer kĂŒrzer als zwei Jahre). Bei der vorliegenden Arbeit handelt es sich um eine retrospektive und monozentrische Beobachtungsstudie ohne Kontrollgruppe. Dieses Studiendesign entspricht nicht dem maximal erreichbaren Standard einer prospektiven und randomisierten Erhebung mit Kontrollgruppe
Characterisation of the photolytic HONO-source in the atmosphere simulation chamber SAPHIR
HONO formation has been proposed as an important OH radical source in simulation chambers for more than two decades. Besides the heterogeneous HONO formation by the dark reaction of NO<sub>2</sub> and adsorbed water, a photolytic source has been proposed to explain the elevated reactivity in simulation chamber experiments. However, the mechanism of the photolytic process is not well understood so far. As expected, production of HONO and NO<sub>x</sub> was also observed inside the new atmospheric simulation chamber SAPHIR under solar irradiation. This photolytic HONO and NO<sub>x</sub> formation was studied with a sensitive HONO instrument under reproducible controlled conditions at atmospheric concentrations of other trace gases. It is shown that the photolytic HONO source in the SAPHIR chamber is not caused by NO<sub>2</sub> reactions and that it is the only direct NO<sub>y</sub> source under illuminated conditions. In addition, the photolysis of nitrate which was recently postulated for the observed photolytic HONO formation on snow, ground, and glass surfaces, can be excluded in the chamber. A photolytic HONO source at the surface of the chamber is proposed which is strongly dependent on humidity, on light intensity, and on temperature. An empirical function describes these dependencies and reproduces the observed HONO formation rates to within 10%. It is shown that the photolysis of HONO represents the dominant radical source in the SAPHIR chamber for typical tropospheric O<sub>3</sub>/H<sub>2</sub>O concentrations. For these conditions, the HONO concentrations inside SAPHIR are similar to recent observations in ambient air
Cryo-electron Microscopy Structure of the 70S Ribosome from Enterococcus faecalis
Enterococcus faecalis is a gram-positive organism responsible for serious infections in humans, but as with many bacterial pathogens, resistance has rendered a number of commonly used antibiotics ineffective. Here, we report the cryo-EM structure of the E. faecalis 70S ribosome to a global resolution of 2.8 Ă
. Structural differences are clustered in peripheral and solvent exposed regions when compared with Escherichia coli, whereas functional centres, including antibiotic binding sites, are similar to other bacterial ribosomes. Comparison of intersubunit conformations among five classes obtained after three-dimensional classification identifies several rotated states. Large ribosomal subunit protein bL31, which forms intersubunit bridges to the small ribosomal subunit, assumes different conformations in the five classes, revealing how contacts to the small subunit are maintained throughout intersubunit rotation. A tRNA observed in one of the five classes is positioned in a chimeric pe/E position in a rotated ribosomal state. The 70S ribosome structure of E. faecalisnow extends our knowledge of bacterial ribosome structures and may serve as a basis for the development of novel antibiotic compounds effective against this pathogen
