    Detection of ectopic gastric mucosa using scintigraphy in pediatric patients

    Introdução: A cintigrafia é frequentemente usada na deteção de mucosa gástrica ectópica no Divertículo de Meckel (DM). Este estudo teve como objetivo avaliar a sua acuidade em crianças com suspeita de DM sintomático. Material e Métodos: Foram revistos os registos médicos dos pacientes submetidos a cintigrafia com pertecnetato de [99mTc] entre 2004 e 2018 num centro hospitalar terciário. Os critérios de exclusão foram: idade > 18 anos, registos médicos incompletos, indicação para cintigrafia pouco clara e óbito. Foram recolhidos os dados demográficos, as manifestações clínicas e achados histopatológicos. A cintigrafia foi realizada de acordo com as guidelines SNMMI / EANM. Os doentes foram divididos nos grupos A (hemorragia digestiva aguda) e B (outras manifestações). Resultados: Foram incluídos no estudo 131 exames (74% crianças do sexo masculino; idade 5 [2-10] anos). O grupo A incluiu 116 pacientes e o grupo B 15 (dor abdominal, n = 6; invaginação atípica, n = 9). O resultado foi positivo em 29 crianças. Em 5 doentes, apesar do resultado negativo, foi detetada mucosa gástrica ectópica. Os valores de sensibilidade, especificidade, VPP, VPN e acuidade foram de 84%, 97%, 90%, 95% e 94%, respetivamente. Não houve diferenças estatisticamente significativas entre os grupos relativamente aos resultados e à acuidade da cintigrafia. Discussão e conclusão: A cintigrafia com pertecnetato de [99mTc] apresenta uma acuidade elevada na deteção de mucosa gástrica ectópica em crianças com hemorragia digestiva aguda, bem como face a outras formas sintomáticas da DM.Background: [99mTc] Pertechnetate Scintigraphy is frequently used to detect ectopic gastric mucosa in Meckel's Diverticulum (MD). The present study aimed to assess its accuracy in paediatric patients suspected of symptomatic MD. Material and Methods: All the medical records of patients that underwent [99mTc] pertechnetate scintigraphy from 2004 to 2018 at a tertiary centre were reviewed. The exclusion criteria were: age above 18 years, incomplete medical records, unclear indication for scintigraphy and death. Demographic data, clinical manifestations, outcome, and histopathology findings were collected. The scans were performed according to SNMMI/EANM guidelines. Patients were divided into group A presenting acute digestive bleeding, and group B with other presentations. Results: 131 scans performed in children (74% males; age 5 [2-10] years) were included. Group A encompassed 116 patients, and group B 15 (abdominal pain, n=6; atypical intussusception, n=9). Overall, there were 29 positives scans. Ectopic gastric mucosa was found in an additional 5 patients with negative scans. The sensitivity, specificity, PPV, NPV and accuracy values were 84%, 97%, 90%, 95% and 94% respectively. There were no statistically significant differences between the groups concerning scan findings and accuracy. Discussion and conclusion: [99mTc] pertechnetate scintigraphy presents a high accuracy in detecting ectopic gastric mucosa in children with acute digestive bleeding as well as in front of suspicion of other symptomatic forms of MD

    The urban structure is a modeller of wind fields and an active modifier of radiation conditions, trapping, reflecting and shadowing much more than the rural areas. In a smaller scale the different albedos and the heat capacity of the materials are also able to modify the thermal patterns. The main purposes of this work are to create a land use/urban structure map for climatic purposes and to estimate heat fluxes with numerical modelling and Landsat images are.     A Typed Model for Dynamic Authorizations

    Security requirements in distributed software systems are inherently dynamic. In the case of authorization policies, resources are meant to be accessed only by authorized parties, but the authorization to access a resource may be dynamically granted/yielded. We describe ongoing work on a model for specifying communication and dynamic authorization handling. We build upon the pi-calculus so as to enrich communication-based systems with authorization specification and delegation; here authorizations regard channel usage and delegation refers to the act of yielding an authorization to another party. Our model includes: (i) a novel scoping construct for authorization, which allows to specify authorization boundaries, and (ii) communication primitives for authorizations, which allow to pass around authorizations to act on a given channel. An authorization error may consist in, e.g., performing an action along a name which is not under an appropriate authorization scope. We introduce a typing discipline that ensures that processes never reduce to authorization errors, even when authorizations are dynamically delegated.Comment: In Proceedings PLACES 2015, arXiv:1602.0325

    An observational model for spatial logics

    Spatiality is an important aspect of distributed systems because their computations depend both on the dynamic behaviour and on the structure of their components. Spatial logics have been proposed as the formal device for expressing spatial properties of systems. We define CCS∥, a CCS-like calculus whose semantics allows one to observe spatial aspects of systems on the top of which we define models of the spatial logic. Our alternative definition of models is proved equivalent to the standard one. Furthermore, logical equivalence is characterized in terms of the bisimilarity of CCS∥

    Dynamic Role Authorization in Multiparty Conversations

    Protocol specifications often identify the roles involved in communications. In multiparty protocols that involve task delegation it is often useful to consider settings in which different sites may act on behalf of a single role. It is then crucial to control the roles that the different parties are authorized to represent, including the case in which role authorizations are determined only at runtime. Building on previous work on conversation types with flexible role assignment, here we report initial results on a typed framework for the analysis of multiparty communications with dynamic role authorization and delegation. In the underlying process model, communication prefixes are annotated with role authorizations and authorizations can be passed around. We extend the conversation type system so as to statically distinguish processes that never incur in authorization errors. The proposed static discipline guarantees that processes are always authorized to communicate on behalf of an intended role, also covering the case in which authorizations are dynamically passed around in messages.Comment: In Proceedings BEAT 2014, arXiv:1408.556

    Keep on Running - Understanding and influencing sustained participation in recreational running

    This dissertation sought to provide a comprehensive understanding of recreational running in adults. Specifically, it comprises a set of three studies that were designed to i) characterize recreational running in Portuguese adults; ii) study correlates of recreational runners (e.g. motivation, goals, consequences); and iii) explore how recreational running might be effectively promoted in the long term. Study 1 aimed to estimate the prevalence of recreational running, and explore demographic, behavioral and psychological characteristics of the “Portuguese Recreational Runner”. It characterizes runners and describes the relative importance of running for total physical activity (PA). Study 2 reflects the state of the art regarding psychological and behavioral correlates of recreational running. It suggested that intrinsic motives are key antecedents of recreational running and that mood and wellbeing are the most frequently associated outcomes. Finally, study 3 described the development of a brief, theory-based intervention, delivered in a digital format, aimed at increasing the maintenance of running behaviors over time. Additionally, results from the pilot study informs about adjustments to future interventions to improve its acceptability. The findings in this thesis highlight the importance of recreational running as a PA promotion strategy, and the need for long-term individualized approaches for its implementation. Lessons learned can inform future research initiatives, focused on testing the KoR intervention, and also recreational running promotion policies.Esta dissertação teve como objetivo ampliar a compreensão da corrida recreativa em adultos. Especificamente, integra um conjunto de três estudos que foram concebidos para i) caracterizar a corrida recreativa em adultos Portugueses; ii) estudar correlatos da corrida recreativa (p.ex., motivação, objetivos, consequências); e iii) explorar como a corrida recreativa pode ser eficazmente promovida a longo prazo. O estudo 1 teve como objetivo estimar a prevalência da corrida recreativa e explorar as características demográficas, comportamentais e psicológicas do “Corredor Recreativo Português”. Além da caracterização, revelou a importância relativa da corrida para a atividade física (AF) total. O estudo 2 reflete o estado da arte relativamente aos correlatos psicológicos e comportamentais da corrida recreativa. Sugeriu que os motivos intrínsecos são antecedentes fundamentais da corrida recreativa, e que o humor e bem-estar são os efeitos mais frequentemente associados. Finalmente, o estudo 3 descreveu o desenvolvimento de uma intervenção breve, baseada em teoria, implementada em formato digital, com o objetivo de aumentar a manutenção dos comportamentos de corrida no longo prazo. Além disso, os resultados do estudo piloto indicam alguns ajustes na intervenção, a fim de melhorar sua aceitabilidade. Os resultados desta tese destacam a importância da corrida recreativa como estratégia de promoção da AF e a necessidade de abordagens individualizadas de longo prazo para a sua implementação. As lições aprendidas podem informar futuras iniciativas de investigação, focadas em testar a intervenção KoR e, também, políticas de promoção de corrida recreativa
