Randomized Controlled Trial of the Effectiveness of Genetic Counseling and a Distance, Computer-Based, Lifestyle Intervention Program for Adult Offspring of Patients with Type 2 Diabetes: Background, Study Protocol, and Baseline Patient Characteristics
Relatives of type 2 diabetic patients are at a high risk of developing type 2 diabetes and should be regarded as target of intervention for diabetes prevention. However, it is usually hard to motivate them to implement preventive lifestyle changes, because of lack of opportunity to take advises from medical professionals, inadequate risk perception, and low priority for preventive behavior. Prevention strategy for them therefore should be highly acceptable and suited for them. The parallel, three-group trial is now being conducted to investigate the effects of genetic counseling and/or a computerized behavioral program on the prevention of type 2 diabetes in that population. The preventive strategies used in this study could provide a novel solution to the numbers of genetically high-risk individuals, if found to be effective. Sociodemographic predictors of depression among survivors of armed conflict with posttraumatic stress disorder in Dogonahawa, north central Nigeria
Background: Survivors of mass violence are at high risk of developing a wide range of psychological disorders. Nevertheless, little is known about the impact of trauma in post conflict low-income countries where armed conflict abounds.Objective: This study was carried out to assess the prevalence of comorbid depression among victims of armed- conflict in Dogonahawa, north-central Nigeria who developed posttraumatic stress disorder (PTSD) and those who did not. It also assessed the socio-demographic predictors for comorbid depression among those who developed PTSD.Method: A cross-sectional study that employed a multi stage sampling technique to select eligible subjects in Dogonahawa, north-central Nigeria. The PTSD module of Mini International Neuropsychiatric Interview (MINI) was used to assess for current symptoms of PTSD, while the Beck Depression Inventory-II (BDI-II) was used to assess for depression.Results: The results showed that 55.5% and 44.7% of the respondents had PTSD and PTSD with comorbid depression respectively. The difference in prevalence was statistically significant with adults diagnosed with PTSD in this community being significantly more likely to experience comorbid depression than those without PTSD (p< 0.001). Being a female gender and being the head of household were found to be the predictors for comorbid depression among respondents with PTSD.Conclusions: Mental health consequences of conflict continued to endure four years after the armed conflict. In the light of the above, mental health emergency interventions after exposure to traumatic event need to be developed to enhance healing and recovery.KeyWords: PTSD, co-morbid depression, sociodemographic predictors, depression, Armed, Conflict, Dogonahaw
Binge drinking and sexual assault among women in Jos Metropolis, Nigeria
Background: Alcohol related sexual assault is a growing epidemic world wide that affects mainly women. There is urgent need to empower women to identify behaviors and situations that may predispose them to sexual victimization.Objectives: The study was carried out to determine the relationship between binge drinking and sociodemographic factors. It also assessed the relationship between binge drinking and sexual assault.Methods: The cross-sectional descriptive study was carried out in Jos North Local Government Area from March to July, 2017. Multistage sampling technique was employed to select 272 participants aged 18 years and above who consented after obtaining ethical approval.Results: The total numbers of participants were 272 females with an age range of 18-60 years. The mean age was 28.8 ± 8.6 years. The socio-demographic variables significantly associated with binge drinking were marital status (X2 = 9.847, DF = 2, p = 0.007), educational status (X2 = 10.684, DF = 3, p = 0.014) and employment status (X2= 5.122, DF = 1, p = 0.024). Binge drinking was significantly associated with sexual assault (X2 = 10.732, DF = 1, p = 0.001). Previously married were significantly more likely to binge drink compared with never married and married. Those with no formal education were more likely to binge drink compared to those with tertiary education while the unemployed were less likely to binge drink compared with the employed. The sexually assaulted (P = 0.01, OR = 2.429, CI = 1.419-4.157) were 2 times more likely to binge drink.Conclusion: There was a significant relationship between binge drinking with marital status, employment, lower level of education and sexual assault. Women should be provided with information about the safe level of alcohol consumption and the many consequences of heavy drinking including sexual assault.Keywords: Binge drinking, women, sexual assault, socio-demographic, Jo
Posttraumatic stress disorder among internally displaced victims of Boko Haram terrorism in north-eastern Nigeria
Background: A large number of youth are often directly involved in armed-conflict, and therefore are at greater risk of developing a wide range of mental disorders including posttraumatic stress disorder (PTSD). However little is known about the prevalence of PTSD among youth who constitute a large work force.Objective: This study was carried out to assess the prevalence of posttraumatic stress disorder among Internally Displaced youth exposed to Boko Haram terrorism in North-Eastern Nigeria. It also assessed the socio-demographic factors associated with PTSD in this population.Method: A cross-sectional study that employed a consecutive sampling technique to select eligible subjects undergoing a Citizenship and leadership training at Citizenship and Leadership training institute Jos, Plateau State. The posttraumatic stress disorder module of Mini International Neuropsychiatric Interview was used to assess for current symptoms of PTSD.Results: The results show that more than two-third of the respondents were unemployed youth with secondary level of education and had low or no stable income. More than 63% of the respondents were diagnosed with PTSD. Educational status (p=0.002), marital status (p=0.001) and income before displacement (p=0.010) were the significant factors associated with PTSD.Conclusions: The results point to the importance of screening and treatment for traumatic armed conflict victims, with particular attention to youth who had low education and low income levels as well as being never married or widowed.Keywords: PTSD, sociodemographic factors, Bokoharam terrorism, North Eastern Nigeri
Intracellular Trafficking of the Amyloid β-Protein Precursor (APP) Regulated by Novel Function of X11-Like
Background: Amyloid beta (A beta), a causative peptide of Alzheimer's disease, is generated by intracellular metabolism of amyloid beta-protein precursor (APP). In general, mature APP (mAPP, N- and O-glycosylated form) is subject to successive cleavages by alpha- or beta-, and gamma-secretases in the late protein secretory pathway and/or at plasma membrane, while immature APP (imAPP, N-glycosylated form) locates in the early secretory pathway such as endoplasmic reticulum or cis-Golgi, in which imAPP is not subject to metabolic cleavages. X11-like (X11L) is a neural adaptor protein composed of a phosphotyrosine-binding (PTB) and two C-terminal PDZ domains. X11L suppresses amyloidogenic cleavage of mAPP by direct binding of X11L through its PTB domain, thereby generation of A beta lowers. X11L expresses another function in the regulation of intracellular APP trafficking. Methodology: In order to analyze novel function of X11L in intracellular trafficking of APP, we performed a functional dissection of X11L. Using cells expressing various domain-deleted X11L mutants, intracellular APP trafficking was examined along with analysis of APP metabolism including maturation (O-glycosylation), processing and localization of APP. Conclusions: X11L accumulates imAPP into the early secretory pathway by mediation of its C-terminal PDZ domains, without being bound to imAPP directly. With this novel function, X11L suppresses overall APP metabolism and results in further suppression of Ab generation. Interestingly some of the accumulated imAPP in the early secretory pathway are likely to appear on plasma membrane by unidentified mechanism. Trafficking of imAPP to plasma membrane is observed in other X11 family proteins, X11 and X11L2, but not in other APP-binding partners such as FE65 and JIP1. It is herein clear that respective functional domains of X11L regulate APP metabolism at multiple steps in intracellular protein secretory pathways
Recent secular trends in child and adolescent physical activity and sedentary behavior internationally : analyses of active healthy kids global alliance global matrices 1.0 to 4.0
We examined recent global secular trends in 5 indicators of child and adolescent physical activity and sedentary behavior (Overall Physical Activity, Organized Sport and Physical Activity, Active Play, Active Transportation, and Sedentary Behavior) and 4 influences on these (Family and Peers, School, Community and Environment, and Government). Active Healthy Kids Global Alliance letter grades (A+ to F) were assigned numbers from 15 to 2, with 0 assigned for missing/incomplete grades. Trends from Active Healthy Kids Global Alliance Global Matrices 1.0 (2014) to 4.0 (2022) were analyzed using linear mixed-effects models with level of economic development and gender inequity considered as potential moderators. Report card grades were generally relatively stable. Trends generally did not differ significantly by level of economic development (except for Active Transportation and Active Play), but gender inequality did significantly moderate trends for most of the indicators, with higher gender inequality associated with more adverse changes in grades. The number of "incomplete" grades decreased over time, but this did not reach statistical significance. While trends varied within and between countries, physical activity and sedentary behaviors, and the influences on these behaviors globally, were relatively stable over the past decade or so, albeit at undesirable levels
Fatal Outcome in Bacteremia is Characterized by High Plasma Cell Free DNA Concentration and Apoptotic DNA Fragmentation: A Prospective Cohort Study
INTRODUCTION: Recent studies have shown that apoptosis plays a critical role in the pathogenesis of sepsis. High plasma cell free DNA (cf-DNA) concentrations have been shown to be associated with sepsis outcome. The origin of cf-DNA is unclear. METHODS: Total plasma cf-DNA was quantified directly in plasma and the amplifiable cf-DNA assessed using quantitative PCR in 132 patients with bacteremia caused by Staphylococcus aureus, Streptococcus pneumoniae, ß-hemolytic streptococcae or Escherichia coli. The quality of cf-DNA was analyzed with a DNA Chip assay performed on 8 survivors and 8 nonsurvivors. Values were measured on days 1-4 after positive blood culture, on day 5-17 and on recovery. RESULTS: The maximum cf-DNA values on days 1-4 (n = 132) were markedly higher in nonsurvivors compared to survivors (2.03 vs 1.26 ug/ml, p<0.001) and the AUCROC in the prediction of case fatality was 0.81 (95% CI 0.69-0.94). cf-DNA at a cut-off level of 1.52 ug/ml showed 83% sensitivity and 79% specificity for fatal disease. High cf-DNA (>1.52 ug/ml) remained an independent risk factor for case fatality in a logistic regression model. Qualitative analysis of cf-DNA showed that cf-DNA displayed a predominating low-molecular-weight cf-DNA band (150-200 bp) in nonsurvivors, corresponding to the size of the apoptotic nucleosomal DNA. cf-DNA concentration showed a significant positive correlation with visually graded apoptotic band intensity (R = 0.822, p<0.001). CONCLUSIONS: Plasma cf-DNA concentration proved to be a specific independent prognostic biomarker in bacteremia. cf-DNA displayed a predominating low-molecular-weight cf-DNA band in nonsurvivors corresponding to the size of apoptotic nucleosomal DNA

Starved epithelial cells uptake extracellular matrix for survival
Extracellular matrix adhesion is required for normal epithelial cell survival, nutrient uptake and metabolism. This requirement can be overcome by oncogene activation. Interestingly, inhibition of PI3K/mTOR leads to apoptosis of matrix-detached, but not matrix-attached cancer cells, suggesting that matrix-attached cells use alternate mechanisms to maintain nutrient supplies. Here we demonstrate that under conditions of dietary restriction or growth factor starvation, where PI3K/mTOR signalling is decreased, matrix-attached human mammary epithelial cells upregulate and internalize β4-integrin along with its matrix substrate, laminin. Endocytosed laminin localizes to lysosomes, results in increased intracellular levels of essential amino acids and enhanced mTORC1 signalling, preventing cell death. Moreover, we show that starved human fibroblasts secrete matrix proteins that maintain the growth of starved mammary epithelial cells contingent upon epithelial cell β4-integrin expression. Our study identifies a crosstalk between stromal fibroblasts and epithelial cells under starvation that could be exploited therapeutically to target tumours resistant to PI3K/mTOR inhibition
Large- and small-size advantages in sneaking behaviour in the dusky frillgoby Bathygobius fuscus
Sneaking tactic, a male alternative reproductive tactic involving sperm competition, is generally adopted by small individuals because of its inconspicuousness. However, large size has an advantage when competition occurs between sneakers for fertilization of eggs. Here, we suggest that both large- and small-size advantages of sneaker males are present within the same species. Large sneaker males of the dusky frillgoby Bathygobius fuscus showed a high success rate in intruding into spawning nests because of their advantage in competition among sneaker males in keeping a suitable position to sneak, whereas small sneakers had few chances to sneak. However, small sneaker males were able to stay in the nests longer than large sneaker males when they succeeded in sneak intrusion. This suggests the possibility of an increase in their paternity. The findings of these sizespecific behavioural advantages may be important in considering the evolution of size-related reproductive traits
