    Stateless HOL

    We present a version of the HOL Light system that supports undoing definitions in such a way that this does not compromise the soundness of the logic. In our system the code that keeps track of the constants that have been defined thus far has been moved out of the kernel. This means that the kernel now is purely functional. The changes to the system are small. All existing HOL Light developments can be run by the stateless system with only minor changes. The basic principle behind the system is not to name constants by strings, but by pairs consisting of a string and a definition. This means that the data structures for the terms are all merged into one big graph. OCaml - the implementation language of the system - can use pointer equality to establish equality of data structures fast. This allows the system to run at acceptable speeds.     Compilation of extended recursion in call-by-value functional languages

    This paper formalizes and proves correct a compilation scheme for mutually-recursive definitions in call-by-value functional languages. This scheme supports a wider range of recursive definitions than previous methods.     Initiality for Typed Syntax and Semantics

    We give an algebraic characterization of the syntax and semantics of a class of simply-typed languages, such as the language PCF: we characterize simply-typed binding syntax equipped with reduction rules via a universal property, namely as the initial object of some category. For this purpose, we employ techniques developed in two previous works: in [2], we model syntactic translations between languages over different sets of types as initial morphisms in a category of models. In [1], we characterize untyped syntax with reduction rules as initial object in a category of models. In the present work, we show that those techniques are modular enough to be combined: we thus characterize simply-typed syntax with reduction rules as initial object in a category. The universal property yields an operator which allows to specify translations - that are semantically faithful by construction - between languages over possibly different sets of types. We specify a language by a 2-signature, that is, a signature on two levels: the syntactic level specifies the types and terms of the language, and associates a type to each term. The semantic level specifies, through inequations, reduction rules on the terms of the language. To any given 2-signature we associate a category of models. We prove that this category has an initial object, which integrates the types and terms freely generated by the 2-signature, and the reduction relation on those terms generated by the given inequations.     Pseudoconvex domains spread over complex homogeneous manifolds

    Using the concept of inner integral curves defined by Hirschowitz we generalize a recent result by Kim, Levenberg and Yamaguchi concerning the obstruction of a pseudoconvex domain spread over a complex homogeneous manifold to be Stein. This is then applied to study the holomorphic reduction of pseudoconvex complex homogeneous manifolds X=G/H. Under the assumption that G is solvable or reductive we prove that X is the total space of a G-equivariant holomorphic fiber bundle over a Stein manifold such that all holomorphic functions on the fiber are constant.Comment: 21 page

    Mature autologous dendritic cell vaccines in advanced non-small cell lung cancer: a phase I pilot study

    Conselho Nacional de Desenvolvimento Científico e Tecnológico (CNPq)Background: Overall therapeutic outcomes of advanced non-small-cell lung cancer (NSCLC) are poor. The dendritic cell (DC) immunotherapy has been developed as a new strategy for the treatment of lung cancer. The purpose of this study was to evaluate the feasibility, safety and immunologic responses in use in mature, antigen-pulsed autologous DC vaccine in NSCLC patients. Methods: Five HLA-A2 patients with inoperable stage III or IV NSCLC were selected to receive two doses of 5 x 107 DC cells administered subcutaneous and intravenously two times at two week intervals. The immunologic response, safety and tolerability to the vaccine were evaluated by the lymphoproliferation assay and clinical and laboratorial evolution, respectively. Results: The dose of the vaccine has shown to be safe and well tolerated. The lymphoproliferation assay showed an improvement in the specific immune response after the immunization, with a significant response after the second dose (p = 0.005). This response was not long lasting and a tendency to reduction two weeks after the second dose of the vaccine was observed. Two patients had a survival almost twice greater than the expected average and were the only ones that expressed HER-2 and CEA together.     South African Menopause Society revised consensus position statement on menopausal hormone therapy

    The South African Menopause Society (SAMS) consensus position statement on menopausal hormone therapy (HT) 2014 is a revision of the SAMS Council consensus statement on menopausal HT published in the SAMJ in May 2007. Information presented in the previous statement has been re-evaluated and new evidence has been incorporated. While the recommendations pertaining to HT remain similar to those in the previous statement, the 2014 revision includes a wider range of clinical benefits for HT, the inclusion of non-hormonal alternatives such as selective serotonin reuptake inhibitors and serotonin noradrenaline reuptake inhibitors for the management of vasomotor symptoms, and an appraisal of bioidentical hormones and complementary medicines used for treatment of menopausal symptoms. New preparations that are likely to be more commonly used in the future are also mentioned. The revised statement emphasises that commencing HT during the 'therapeutic window of opportunity' maximises the benefit-to-risk profile of therapy in symptomatic menopausal women

    HPV testing in primary screening of older women

    Certain types of the human papilloma virus (HPV) are well established as the primary cause of cervical cancer. Several studies have shown that HPV testing can improve the detection rate of high-grade cervical intraepithelial neoplasia (CIN), but these have been carried out primarily in younger women. In this study we evaluated the role of HPV testing as an adjunct to cytology in women aged 35 or over. An additional aim was to evaluate commercially available kits for HPV testing. A total of 2988 eligible women aged 34 or more attending for a routine smear in 40 general practitioner practices received HPV testing in addition to routine cytology, after having given written informed consent. Samples were assayed by polymerase chain reaction (PCR) and two versions of the Hybrid Capture test for HPV, and women were invited for colposcopy if there was any cytological abnormality (including borderline smears) or the PCR test was positive. Any apparent abnormality was biopsied and loop-excision was performed as necessary. CIN was judged by histology; 42 women had high-grade CIN, of which six were cytology negative (86% sensitivity for borderline or worse) and three had a borderline smear (79% sensitivity for mild dyskaryosis or worse). The positive predictive value of a borderline smear was only 3.1%. Eleven high-grade lesions were negative by the PCR HPV test (sensitivity 74%). The first generation Hybrid Capture II test had a similar sensitivity but an unacceptably high false positive rate (18.3%), while the newer Hybrid Capture II microtitre kit had a 95% sensitivity and a 2.3% positivity rate in normal women when used at a 2 pg ml−1 cut-off (positive predictive value 27%). Cytology performed very well in this older cohort of women. The newer Hybrid Capture II microtitre test may be a useful adjunct, especially if the results reported here are reproducible in other studies. A combined screening test offers the possibility of greater protection and/or longer screening intervals, which could reduce the overall cost of the screening programme. © 1999 Cancer Research Campaig

    A formalized general theory of syntax with bindings

    We present the formalization of a theory of syntax with bindings that has been developed and refined over the last decade to support several large formalization efforts. Terms are defined for an arbitrary number of constructors of varying numbers of inputs, quotiented to alpha-equivalence and sorted according to a binding signature. The theory includes a rich collection of properties of the standard operators on terms, such as substitution and freshness. It also includes induction and recursion principles and support for semantic interpretation, all tailored for smooth interaction with the bindings and the standard operators
