    Relentless Engagement with State Educational Policy Reform: Collaborating to Change the Writing Placement Conversation

    This article describes the educational reform efforts surrounding writing placement in one state context. We propose that placement offers a particularly useful engagement point because it is often controlled by state-level policies and it directly impacts the lived experience of first-year college students. To document how we worked across institutions in our state, we describe a series of events that occurred over several years and that fostered collaborative exchanges. Then, we explore the challenges and opportunities afforded by our long-term engagement with policymakers.     The effects of mechanochemical synthesis and postsynthetic grinding on the spin crossover material [Fe(atrz)3](BF4)2 was examined in detail using a combination of X-ray diffraction, magnetometry, EXAFS and TEM. Mechanochemical synthesis yielded a different polymorph (ÎČ-phase) to the solution synthesised sample (α-phase), with a lower temperature spin crossover. Milling duration did not significantly affect this temperature but did result in the production of smaller nanoparticles with a narrower size distribution. It is also possible to convert from α- to the ÎČ-phase via postsynthetic grinding

    Enhanced spectral histology in the colon using high-magnification benchtop FTIR imaging

    ArticleLabel-free imaging of cells and tissues is a promising tool to study the molecular alterations for improved cancer diagnosis. In this regard, the vibrational spectroscopic method of FTIR imaging has been employed to study the histo-pathological features and the alterations were correlated to a disease state. The aim of this study was to see if higher magnification images resulting in pixel sizes smaller than the diffraction limit of mid-IR wavelengths with two NA objectives could provide cellular and subcellular resolution of key diagnostic features. To this effect, FTIR spectroscopic imaging has been carried out directly on paraffinized colon tissue sections using a benchtop imaging system consisting of two different IR objectives with a NA of 0.81 and 0.62. The high NA objective (0.81) at a high magnification (63×) provided a pixel size of 0.63 × 0.63 ÎŒm2 in comparison to the standard magnification (12×) where the pixel size is 3.3 × 3.3 ÎŒm2. The second objective (0.62 NA) at a high magnification (36×) provided a pixel size of 1.1 × 1.1 ÎŒm2 in comparison to the standard magnification (7×) pixel size of 5.5 × 5.5 ÎŒm2. The spectral images were corrected for paraffin and other spectral interferences using a modified EMSC algorithm and subjected to cluster analysis in order to appreciate the histopathological details. Comparison of the IR cluster analysis results to adjacent haematoxylin and eosin stained tissue sections, which were used as the morphological controls, revealed various levels of histological and cellular organization. Together with large scale features such as the glandular and connective tissue regions, small scale features like goblet cells, interfaces between tissue types especially the peri-cryptal fibroblastic sheath were observed. Noticeable differences between the intracellular mucin and secreted mucin were also observed.     Infrared Spectroscopic Analysis in the Differentiation of Epithelial Misplacement From Adenocarcinoma in Sigmoid Colonic Adenomatous Polyps

The main aim of the work is to test, as a proof-of concept study, the ability of the infrared spectroscopic imaging approach to differentiate epithelial misplacement from adenocarcinoma in sigmoid colonic adenomatous polyps. Methods: Ten samples from each of the four diagnostic groups, normal colonic mucosa, adenomatous polyps with low grade dysplasia, epithelial misplacement in adenomatous polyps and adenocarcinoma, were analysed using IR spectroscopic imaging and data processing methods. IR spectral images were subjected to data pre-processing and cluster analysis based segmentation to identify epithelial, connective tissue and stromal regions. Statistical analysis was carried out using principal component analysis and linear discriminant analysis based cross validation, to classify spectral features according to the pathology, and the diagnostic attributes were compared. Results: The combined 4-group classification model on an average showed a sensitivity of 64%, a specificity of 88% and an accuracy of 76% for prediction based on a ‘single spectrum’, whilst a ‘majority-vote’ prediction on an average showed a sensitivity of 73%, a specificity of 90% and an accuracy of 82%. The 2-group model, for the differential diagnosis of epithelial misplacement versus adenocarcinoma, showed an average sensitivity and specificity of 82.5% for prediction based on a ‘single spectrum’ whilst a ‘majority-vote’ classification showed an average sensitivity and specificity of 90%. A 92% area under the curve (AUC) value was obtained when evaluating the classifier using the Receiver Operating Characteristics (ROC) curves. Conclusions: IR spectroscopy shows promise in its ability to differentiate epithelial misplacement from adenocarcinoma in tissue sections, considered as one of the most challenging endeavours in population-wide diagnostic histopathology. Further studies with larger series, including cases with challenging diagnostic features are required to ascertain the capability of this modern digital pathology approach. In the long-term, IR spectroscopy based pathology which is relatively low-cost and rapid, could be a promising endeavour to consider for integration into the existing histopathology pathway, in particular for population based screening programmes where large number of samples are scrutinised.National Institute for Health Research (NIHR

    A major radio outburst in III Zw 2 with an extremely inverted, millimeter-peaked spectrum

    III Zw 2 is a spiral galaxy with an optical spectrum and faint extended radio structure typical of a Seyfert galaxy, but also with an extremely variable, blazar-like radio core. We have now discovered a new radio flare where the source has brightened more than twenty-fold within less than two years. A broad-band radio spectrum between 1.4 and 666 GHz shows a textbook-like synchrotron spectrum peaking at 43 GHz, with a self-absorbed synchrotron spectral index +2.5 at frequencies below 43 GHz and an optically thin spectral index -0.75 at frequencies above 43 GHz. The outburst spectrum can be well fitted by two homogenous, spherical components with equipartition sizes of 0.1 and 0.2 pc at 43 and 15 GHz, and with magnetic fields of 0.4 and 1 Gauss. VLBA observations at 43 GHz confirm this double structure and these sizes. Time scale arguments suggest that the emitting regions are shocks which are continuously accelerating particles. This could be explained by a frustrated jet scenario with very compact hotspots.     Digital consultations for weight management in the NHS: A qualitative evaluation

The objective of the present study was to investigate patients’ experiences of digital weight management services (DWMS) provided by the National Health Service (NHS). Particular emphasis was placed on examining the perceived benefits and limitations of DWMS so as to identify potential means of improving provision. Sixteen patients (eight male; eight female) accessing digital consultations at one of two West Midlands (UK) NHS trusts, participated in semi-structured interviews. Interviews were transcribed verbatim and analysed via thematic analysis. We identified three overarching themes and associated sub-themes that reflect the perceived benefits and limitations of service provision as identified by patients. These were technology acceptability (sub-themes ‘challenges’, ‘requirements/facilitators’, and ‘beneficial features’); treatment acceptability (sub-themes ‘treatment features’, ‘patient attributes’, and ‘practitioner skills’); and treatment efficacy (sub-themes ‘treatment features’, ‘patient attributes’, and ‘practitioner skills’). Themes identified in this study have informed recommendations intended to enhance acceptability of DWMS technology and treatment, potentially encouraging engagement and increasing treatment efficacy.     Blood pressure variability and cardiovascular risk in the PROspective study of pravastatin in the elderly at risk (PROSPER)

    Variability in blood pressure predicts cardiovascular disease in young- and middle-aged subjects, but relevant data for older individuals are sparse. We analysed data from the PROspective Study of Pravastatin in the Elderly at Risk (PROSPER) study of 5804 participants aged 70–82 years with a history of, or risk factors for cardiovascular disease. Visit-to-visit variability in blood pressure (standard deviation) was determined using a minimum of five measurements over 1 year; an inception cohort of 4819 subjects had subsequent in-trial 3 years follow-up; longer-term follow-up (mean 7.1 years) was available for 1808 subjects. Higher systolic blood pressure variability independently predicted long-term follow-up vascular and total mortality (hazard ratio per 5 mmHg increase in standard deviation of systolic blood pressure = 1.2, 95% confidence interval 1.1–1.4; hazard ratio 1.1, 95% confidence interval 1.1–1.2, respectively). Variability in diastolic blood pressure associated with increased risk for coronary events (hazard ratio 1.5, 95% confidence interval 1.2–1.8 for each 5 mmHg increase), heart failure hospitalisation (hazard ratio 1.4, 95% confidence interval 1.1–1.8) and vascular (hazard ratio 1.4, 95% confidence interval 1.1–1.7) and total mortality (hazard ratio 1.3, 95% confidence interval 1.1–1.5), all in long-term follow-up. Pulse pressure variability was associated with increased stroke risk (hazard ratio 1.2, 95% confidence interval 1.0–1.4 for each 5 mmHg increase), vascular mortality (hazard ratio 1.2, 95% confidence interval 1.0–1.3) and total mortality (hazard ratio 1.1, 95% confidence interval 1.0–1.2), all in long-term follow-up. All associations were independent of respective mean blood pressure levels, age, gender, in-trial treatment group (pravastatin or placebo) and prior vascular disease and cardiovascular disease risk factors. Our observations suggest variability in diastolic blood pressure is more strongly associated with vascular or total mortality than is systolic pressure variability in older high-risk subjects
