Case Report of Obstructive Uropathy
Os autores descrevem o caso clÃnico de uma criança do sexo masculino, de 3 anos de idade, internada na Unidade de Nefrologia
do Hospital Dona Estefânia por infecção urinária. O inÃcio das queixas remonta a três meses antes do internamento
com o aparecimento de disúria e hematúria macroscópica na sequência do descolamento das aderências baiano prepuciais.
Posteriormente surgem alguns episódios febris de curta duração, sendo internado no decurso de um destes. Os exames complementares revelaram além de uma infecção urinária, um megauretero
obstrutivo à direita e a biópsia renal a existência de lesões graves e extensas caracterÃsticas de uma nefropatia obstrutiva. Os autores a partir deste caso questionam as limitações da ecografia no diagnóstico pré-natal das uropatias malformativas e a valorização de alguns sinais e sintomas no rastreio da infecção

Overabundance of alpha-elements in exoplanet host stars
We present the results for a chemical abundance analysis between
planet-hosting and stars without planets for 12 refractory elements for a total
of 1111 nearby FGK dwarf stars observed within the context of the HARPS GTO
programs. Of these stars, 109 are known to harbour high-mass planetary
companions and 26 stars are hosting exclusively Neptunians and super-Earths. We
found that the [X/Fe] ratios for Mg, Al, Si, Sc, and Ti both for giant and
low-mass planet hosts are systematically higher than those of comparison stars
at low metallicities ([Fe/H] < from -0.2 to 0.1 dex depending on the element).
The most evident discrepancy between planet-hosting and stars without planets
is observed for Mg. Our data suggest that the planet incidence is greater among
the thick disk population than among the thin disk for mettallicities bellow
-0.3 dex. After examining the [alpha/Fe] trends of the planet host and non-host
samples we conclude that a certain chemical composition, and not the Galactic
birth place of the stars, is the determinating factor for that. The inspection
of the Galactic orbital parameters and kinematics of the planet-hosting stars
shows that Neptunian hosts tend to belong to the "thicker" disk compared to
their high-mass planet-hosting counterparts.We also found that Neptunian hosts
follow the distribution of high-alpha stars in the UW vs V velocities space,
but they are more enhanced in Mg than high-alpha stars without planetary
companions. Our results indicate that some metals other than iron may also have
an important contribution to planet formation if the amount of iron is low.
These results may provide strong constraints for the models of planet

Chronic rhinosinusitis: role of biofilms
A rinossinusite crónica é uma doença muito prevalente e possui um impacto sócio-económico importante. É definida por um conjunto de sintomas, com duração superior a doze semanas, sendo os sinais da inflamação evidenciados pela rinoscopia anterior e endoscopia nasal. Também pode ser documentada imagiologicamente pela tomografia computadorizada. A patogénese desta doença permanece desconhecida. Entre os vários fatores causais que são apontados, destaca-se o papel dos biofilmes que têm despertado cada vez mais a atenção dos autores, nos últimos anos. As infeções associadas aos biofilmes têm-se revelado refractárias ao tratamento prolongado com antibióticos. Os biofilmes têm sido implicados em várias infecções da cabeça e pescoço nomeadamente na doença dentária e periodontal, otite média colesteatomatosa, otorreia associada a tubos de timpanostomia e amigdalite crónica. Recentemente, vários autores têm comprovado a sua implicação na rinossinusite crónica. Além disso, têm sido detetadas alterações estruturais na mucosa nasossinusal, por microscopia eletrónica, em doentes com rinossinusite crónica portadores de biofilmes. Têm-se sido detetados vários nÃveis de gravidade desde uma desorganização ciliar até a uma ausência completa de cÃlios e de células goblet . Metallicities for 13 nearby open clusters from high-resolution spectroscopy of dwarf and giant stars. Stellar metallicity, stellar mass, and giant planets
We present a study of accurate stellar parameters and iron abundances for 39
giants and 16 dwarfs in the 13 open clusters IC2714, IC4651, IC4756, NGC2360,
NGC2423, NGC2447 (M93), NGC2539, NGC2682 (M67), NGC3114, NGC3680, NGC4349,
NGC5822, NGC6633. The analysis was done using a set of high-resolution and
high-S/N spectra obtained with the UVES spectrograph (VLT). These clusters are
currently being searched for planets using precise radial velocities. For all
the clusters, the derived average metallicities are close to solar.
Interestingly, the values derived seem to depend on the line-list used. This
dependence and its implications for the study of chemical abundances in giants
stars are discussed. We show that a careful choice of the lines may be crucial
for the derivation of metallicities for giant stars on the same metallicity
scale as those derived for dwarfs. Finally, we discuss the implications of the
derived abundances for the metallicity- and mass-giant planet correlation. We
conclude that a good knowledge of the two parameters is necessary to correctly
disentangle their influence on the formation of giant planets.Comment: accepted for publication in A&
Chemical abundances of 1111 FGK stars from the HARPS GTO planet search program.Galactic stellar populations and planets
We performed a uniform and detailed abundance analysis of 12 refractory
elements (Na, Mg, Al, Si, Ca, Ti, Cr, Ni, Co, Sc, Mn and V) for a sample of
1111 FGK dwarf stars from the HARPS GTO planet search program. 109 of these
stars are known to harbour giant planetary companions and 26 stars are hosting
exclusively Neptunians and super-Earths. The main goals of this paper are i) to
investigate whether there are any differences between the elemental abundance
trends for stars of different stellar populations; ii) to characterise the
planet host and non-host samples in term of their [X/H]. The extensive study of
this sample, focused on the abundance differences between stars with and
without planets will be presented in a parallel paper. The equivalent widths of
spectral lines are automatically measured from HARPS spectra with the ARES
code. The abundances of the chemical elements are determined using a LTE
abundance analysis relative to the Sun, with the 2010 revised version of the
spectral synthesis code MOOG and a grid of Kurucz ATLAS9 atmospheres. To
separate the Galactic stellar populations we applied both a purely kinematical
approach and a chemical method. We found that the chemically separated (based
on the Mg, Si, and Ti abundances) thin and thick discs are also chemically
disjunct for Al, Sc, Co and Ca. Some bifurcation might also exist for Na, V,
Ni, and Mn, but there is no clear boundary of their [X/Fe] ratios. We confirm
that an overabundance in giant-planet host stars is clear for all the studied
elements.We also confirm that stars hosting only Neptunian-like planets may be
easier to detect around stars with similar metallicities as non-planet hosts,
although for some elements (particulary alpha-elements) the lower limit of

Novos dados petrográficos, geoquÃmicos e isotópicos sobre a área de concessão de Boa Fé (Montemor-o-Novo, Zona de Ossa-Morena)
A área do estudo situa-se na Zona de Cisalhamento de Montemor-o-Novo (Maciço de Évora, Zona de Ossa-Morena - ZOM). O trabalho incidiu sobre amostras de metassedimentos e metabasitos da Série Negra (Ediacarano), bem como de granitos tardi-variscos. Os metassedimentos vão de micaxistos a paragnaisses com a associação, de pico metamórfico, Qz+Pl+Bt+Ms+Opq ±Tur±Sil±Crd+Zrn. Os metabasitos são anfibolitos com paragénese Pl+Hbl+Qz+Mag+Ilm±Bt. Quer nos metassedimentos quer nos metabasitos, existem testemunhos de duas fases de deformação importantes, sendo uma xistosidade de crenulação a estrutura mais penetrativa. Aquelas duas fases de deformação dúctil deverão corresponder à s fases variscas D1 e D2 do autóctone da ZOM. Ocorreu ainda deformação posterior, testemunhada por kinks e processos de subgranulação e recristalização afetando alguns grãos em zonas confinadas à s proximidades imediatas de fraturas tardias, a qual deverá relacionar-se com a fase D3. Os granitos tardi-variscos são fortemente peraluminosos, tendo como minerais mais abundantes quartzo, microclina, plagioclase sódica e moscovite. Como acessórios ocorrem biotite, zircão, apatite, opacos, turmalina e silimanite. Em termos isotópicos, os valores de εSr320Ma variam entre +93 e +182, enquanto a gama de valores para εNd320Ma vai de -6.7 a -9.0, compatÃveis com um forte contributo de anatexia dos metassedimentos da Série Negra na génese dos magmas que deram origem aos granitos tardi-variscos.The study area is located within the Montemor-o-Novo Shear Zone (Évora Massif, Ossa-Morena Zone - OMZ). The analysed samples are of metasediments and metabasites of the Série Negra lithostratigraphic unit (Ediacaran) and of late-Variscan granites. The metasediments are micaschists to paragneisses displaying Qz+Pl+Bt+Ms+Opq±Tur±Sil±Crd+Zrn as peak metamorphic assemblage. The metabasites are amphibolites displaying the paragenesis Pl+Hbl+Qz+Mag+Ilm±Bt. Microstructures observed in both types of metamorphic rocks document two major ductile deformation phases. The most penetrative tectonic anisotropy is a crenulation schistosity. Those major deformation phases probably correspond to the Variscan D1 and D2, previously recognized in the autochthon of the OMZ. There is also evidence for a later deformation stage, likely related to the Variscan D3: some grains, in narrow zones adjacent to late fractures, show kinks or were affected by subgranulation and recrystallization. The studied late-Variscan granites are strongly peraluminous, and have quartz, microcline, Na-plagioclase and muscovite as the most abundant minerals. Biotite, zircon, apatite, opaques, tourmaline and sillimanite occur as accessory minerals. Considering isotope geochemistry, εSr320Ma varies between +93 and +182, whilst the εNd320Ma values go from -6.7 to -9.0. These data can be explained by a strong involvement of anatexis of the Série Negra metasediments in the genesis of the granitic magmas
Endocardite infecciosa: ainda mais desafios que certezas

Its increasing incidence, demographic shifts (affecting older patients), microbiology with higher rates of Staphylococcus infection, still frequent serious complications and substantial mortality make endocarditis a very complex disease. Despite this, innovations in the diagnosis, involving microbiology and imaging, and improvements in intensive care and cardiac surgical techniques, materials and timing can impact the prognosis of this disease. Ongoing challenges persist, including rethinking prophylaxis, improving the diagnosis criteria comprising blood culture-negative endocarditis and prosthetic valve endocarditis, timing of surgical intervention, and whether to perform surgery in the presence of ischemic stroke or in intravenous drug users. A combined strategy on infective endocarditis is crucial, involving advanced clinical decisions and protocols, a multidisciplinary approach, national healthcare organization and health policies to achieve better results for our patients.Após catorze décadas de evolução médica e tecnológica, a endocardite infeciosa continua a desafiar médicos no seu diagnóstico e manejo diário. O aumento da incidência, alterações demográficas (afetando pacientes mais idosos), microbiologia com taxas de infeção por Staphylococcus mais elevadas, com complicações graves ainda frequentes e uma mortalidade substancial tornam a endocardite uma doença muito complexa. Apesar de tudo, a inovação no seu diagnóstico, nomeadamente na área da microbiologia e imagem, e a melhoria nos cuidados intensivos e na cirurgia cardÃaca (quanto à s técnicas, materiais usados e momento de intervenção) podem ter um impacto no seu prognóstico. Os desafios persistem, incluindo repensar a profilaxia, melhorar os critérios de diagnóstico incluindo a endocardite com culturas negativas e endocardite de prótese valvar, o timing para a intervenção cirúrgica, e sua realização ou não na presença de acidente vascular cerebral isquêmico e em usuários de drogas intravenosas.
