    Scots Abroad, Nationalism at Home: Kailyard and Kilt as Gatekeepers? 1885-1979

    The emigration of the Scots from the 18th to the 20th century has produced a diaspora. The thesis outlines how many diasporas are involved in the nationalist projects of their homeland. However, over the chronology of this study and beyond, whilst there were active movements to amend or end the Union of 1707, it has been found that the Scots were not. The thesis then proposes some explanations for this. Chapters one and two introduce methods, research material and context; they describe the Union, the emigrations and diasporas. The study uses for comparison purposes the Irish and Norwegian diasporas. Lines of enquiry such as nationalism, the use of soft power and gatekeeping behaviour are presented, with a discussion of Scottish nationalism. The study examines the approach to involving the diaspora of five groups; both SHRAs, the International Scots Home Rule League, the National Convention and the NPS/SNP. The response of Scottish MPs in the diaspora in England to the many attempts to legislate for home rule is also examined. The approach to the diaspora was found to be badly executed and targeted. Few visits were made, and only to the US and Canada. Communication was unfocussed and spasmodic. The Scottish associational clubs were frequently used as a conduit. A small part of the whole diaspora, these acted as gatekeepers, selectively mobilising for themselves as an elite which had no need of nationalism as they could succeed without it. Comparing the Irish, whose diaspora successfully supported its nationalist causes at home, is instructive. The study concludes that the spasmodic and amateurish nature of contact, the nature of the Associations and that of the diaspora itself were the main culprits in this case of a diaspora indifferent to the fate of nationalism in its home land

    Use of MRI in the diagnosis of fetal brain abnormalities in utero (MERIDIAN): a multicentre, prospective cohort study.

    BACKGROUND: In-utero MRI (iuMRI) has shown promise as an adjunct to ultrasound but the comparative diagnostic performance has been poorly defined. We aimed to assess whether the diagnostic accuracy and confidence of the prenatal diagnosis of fetal brain abnormalities is improved with iuMRI and assess the clinical impact and patient acceptability of iuMRI. METHODS: We did a multicentre, prospective, cohort study in the UK, at 16 fetal medicine centres, of pregnant women aged 16 years or older whose fetus had a brain abnormality detected by ultrasound at a gestational age of 18 weeks or more, had no contraindications to iuMRI, and consented to enter the study. Women carrying a fetus suspected of having a brain anomaly on ultrasound had iuMRI done within 14 days of ultrasound. The findings were reviewed by two independent panels and used to estimate diagnostic accuracy and confidence by comparison with outcome diagnoses. Changes in diagnosis, prognosis, and clinical management brought about by iuMRI and patient acceptability were assessed. FINDINGS: Participants were recruited between July 29, 2011, and Aug 31, 2014. The cohort was subdivided by gestation into the 18 weeks to less than 24 weeks fetus cohort (n=369) and into the 24 weeks or older fetus cohort (n=201). Diagnostic accuracy was improved by 23% (95% CI 18-27) in the 18 weeks to less than 24 weeks group and 29% (23-36) in the 24 weeks and older group (p<0·0001 for both groups). The overall diagnostic accuracy was 68% for ultrasound and 93% for iuMRI (difference 25%, 95% CI 21-29). Dominant diagnoses were reported with high confidence on ultrasound in 465 (82%) of 570 cases compared with 544 (95%) of 570 cases on iuMRI. IuMRI provided additional diagnostic information in 387 (49%) of 783 cases, changed prognostic information in at least 157 (20%), and led to changes in clinical management in more than one in three cases. IuMRI also had high patient acceptability with at least 95% of women saying they would have an iuMRI study if a future pregnancy were complicated by a fetal brain abnormality. INTERPRETATION: iuMRI improves diagnostic accuracy and confidence for fetal brain anomalies and leads to management changes in a high proportion of cases. This finding, along with the high patient acceptability, leads us to propose that any fetus with a suspected brain abnormality on ultrasound should have iuMRI to better inform counselling and management decisions.     RNA-Seq reveals changes in human placental metabolism, transport and endocrinology across the first-second trimester transition.

    The human placenta is exposed to major environmental changes towards the end of the first trimester associated with full onset of the maternal arterial placental circulation. Changes include a switch from histotrophic to hemotrophic nutrition, and a threefold rise in the intraplacental oxygen concentration. We evaluated their impact on trophoblast development and function using RNA-sequencing (RNA-Seq) and DNA-methylation analyses performed on the same chorionic villous samples at 7-8 (n=8) and 13-14 (n=6) weeks of gestation. Reads were adjusted for fetal sex. Most DEGs were associated with protein processing in the endoplasmic reticulum (ER), hormone secretion, transport, extracellular matrix, vasculogenesis, and reactive oxygen species metabolism. Transcripts higher in the first trimester were associated with synthesis and ER processing of peptide hormones, and glycolytic pathways. Transcripts encoding proteins mediating transport of oxygen, lipids, protein, glucose, and ions were significantly increased in the second trimester. The motifs of CBX3 and BCL6 were significantly overrepresented, indicating the involvement of these transcription factor networks in the regulation of trophoblast migration, proliferation and fusion. These findings are consistent with a high level of cell proliferation and hormone secretion by the early placenta to secure implantation in a physiological low-oxygen environment

    Comparative cervical profiles of adult and under-18 front-row rugby players: implications for playing policy

    Objective To compare the cervical isometric strength, fatigue endurance and range of motion of adult and under-18 age-grade front-row rugby players to inform the development of a safe age group policy with particular reference to scrummaging.Design Cross-sectional cohort study.Setting ‘Field testing’ at Murrayfield stadium.Participants 30 high-performance under-18 players and 22 adult front-row rugby players.Outcome measures Isometric neck strength, height, weight and grip strength.Results Youth players demonstrated the same height and grip strength as the adult players; however, the adults were significantly heavier and demonstrated substantially greater isometric strength (p<0.001). Only two of the ‘elite’ younger players could match the adult mean cervical isometric strength value. In contrast to school age players in general, grip strength was poorly associated with neck strength (r=0.2) in front-row players; instead, player weight (r=0.4) and the number of years’ experience of playing in the front row (r=0.5) were the only relevant factors in multivariate modelling of cervical strength (R2=0.3).Conclusions Extreme forces are generated between opposing front rows in the scrum and avoidance of mismatch is important if the risk of injury is to be minimised. Although elite youth front-row rugby players demonstrate the same peripheral strength as their adult counterparts on grip testing, the adults demonstrate significantly greater cervical strength. If older youths and adults are to play together, such findings have to be noted in the development of age group policies with particular reference to the scrum

    Over two billion people are using the Internet at present, assisted by the mediating activities of software agents which deal with the diversity and complexity of information. There are, however, ethical issues due to the monitoring-and-surveillance, data mining and autonomous nature of software agents. Considering the context, this study aims to comprehend stakeholders' perspectives on the social network service environment in order to identify the main considerations for the design of software agents in social network services in the near future. Twenty-one stakeholders, belonging to three key stakeholder groups, were recruited using a purposive sampling strategy for unstandardised semi-structured e-mail interviews. The interview data were analysed using a qualitative content analysis method. It was possible to identify three main considerations for the design of software agents in social network services, which were classified into the following categories: comprehensive understanding of users' perception of privacy, user type recognition algorithms for software agent development and existing software agents enhancement

    An ideal journey: Making bus travel desirable

This paper explores the ways in which people use their travel-time on local buses, and explains how this knowledge can assist with efforts in many ‘auto-centric’ societies to make bus travel more attractive and encourage a shift away from excessive private car use. Framing the discussion around the concept of an ‘ideal bus journey’, this paper examines whether travel-time activities on-board the bus give subjective value to the journey experience. Particular attention is given to emergent mobile Information and Communications Technologies, which are rapidly reconfiguring the ways in which we can inhabit and use mobile spaces such as the bus. This paper reports a novel mixed-methodology, creating a synthesised analysis of online discussions, focus groups, and a large-scale questionnaire survey of 840 bus users in Bristol, UK. The findings demonstrate that the bus is a very active space, with high levels of travel-time activity. The most popular activities on the bus are those related to relaxation and personal benefit, such as reading, listening to music, and browsing the internet. It is the passengers themselves that are largely in control of their in-vehicle experience, being able to craft a range of different positive journey experiences through travel-time activity. However, negative experiences are very common, and there is a need to challenge unfavourable public perception and media representations of bus travel to create a more positive cultural construction of the bus which would allow for the concept of an ‘ideal journey’ to be more easily realised. Passengers are the main creators of their travel-time experience, however there is much that can be done by bus operators to facilitate different types of activity and encourage a desirable public space. The overarching message is that there is a distinct opportunity to unlock travel-time activity as a ‘Unique Selling Point’ of the bus. Creating a perception of the bus journey as a desirable piece of time will allow local bus services to compete with the car on their own terms, and assist with international efforts to encourage people out of their cars and onto public transport for some trips

    Making decisions on academic library opening hours is complex with many pressures on managers. This research surveys senior academic library managers from the UK, using a questionnaire to reveal views on library opening hours, the decision making process, and the pressures which influenced their decisions. A variety of factors were found, in particular satisfying undergraduate demands. The research also revealed the sources of information important in making decisions on opening hours and the influence of ‘political’ issues in the decision making process. Some institutions remove complexity by utilising 24/7 opening, though this is not an option for many

    Background: Ultrasonography has been the mainstay of antenatal screening programmes in the UK for many years. Technical factors and physical limitations may result in suboptimal images that can lead to incorrect diagnoses and inaccurate counselling and prognostic information being given to parents. Previous studies suggest that the addition of in utero magnetic resonance imaging (iuMRI) may improve diagnostic accuracy for fetal brain abnormalities. These studies have limitations, including a lack of an outcome reference diagnosis (ORD), which means that improvements could not be assessed accurately. Objectives: To assess the diagnostic impact, acceptability and cost consequence of iuMRI among fetuses with a suspected fetal brain abnormality. Design: A pragmatic, prospective, multicentre, cohort study with a health economics analysis and a sociological substudy. Setting: Sixteen UK fetal medicine centres. Participants: Pregnant women aged ≥ 16 years carrying a fetus (at least 18 weeks’ gestation) with a suspected brain abnormality detected on ultrasonography. Interventions: Participants underwent iuMRI and the findings were reported to their referring fetal medicine clinician. Main outcome measures: Pregnancy outcome was followed up and an ORD from postnatal imaging or postmortem autopsy/imaging collected when available. Developmental data from the Bayley Scales of Infant Development and questionnaires were collected from the surviving infants aged 2–3 years. Data on the management of the pregnancy before and after the iuMRI were collected to inform the economic evaluation. Two surveys collected data on patient acceptability of iuMRI and qualitative interviews with participants and health professionals were undertaken. Results: The primary analysis consisted of 570 fetuses. The absolute diagnostic accuracies of ultrasonography and iuMRI were 68% and 93%, respectively [a difference of 25%, 95% confidence interval (CI) 21% to 29%]. The difference between ultrasonography and iuMRI increased with gestational age. In the 18–23 weeks group, the figures were 70% for ultrasonography and 92% for iuMRI (difference of 23%, 95% CI 18% to 27%); in the ≥ 24 weeks group, the figures were 65% for ultrasonography and 94% for iuMRI (difference of 29%, 95% CI 23% to 36%). Patient acceptability was high, with at least 95% of respondents stating that they would have iuMRI again in a similar situation. Health professional interviews suggested that iuMRI was acceptable to clinicians and that iuMRI was useful as an adjunct to ultrasonography, but not as a replacement. Across a range of scenarios, iuMRI resulted in additional costs compared with ultrasonography alone. The additional cost was consistently < £600 per patient and the cost per management decision appropriately changed was always < £3000. There is potential for reporting bias from the referring clinicians on the diagnostic and prognostic outcomes. Lower than anticipated follow-up rates at 3 years of age were observed. Conclusions: iuMRI as an adjunct to ultrasonography significantly improves the diagnostic accuracy and confidence for the detection of fetal brain abnormalities. An evaluation of the use of iuMRI for cases of isolated microcephaly and the diagnosis of fetal spine abnormalities is recommended. Longer-term follow-up studies of children diagnosed with fetal brain abnormalities are required to fully assess the functional significance of the diagnoses.
