    Relationship between cardiovascular risk and lipid testing in one health care system: a retrospective cohort study.

    BackgroundThe US Preventive Services Taskforce (USPSTF) recommends routine lipid screening beginning age 35 for men [1]. For women age 20 and older, as well as men age 20-34, screening is recommended if cardiovascular risk factors are present. Prior research has focused on underutilization but not overuse of lipid testing. The objective is to document over- and under-use of lipid testing in an insured population of persons at low, moderate and high cardiovascular disease (CVD) risk for persons not already on statins.MethodsThe study is a retrospective cohort study that included all adults without prior CVD who were continuously enrolled in a large integrated healthcare system from 2005 to 2010. Measures included lipid test frequency extracted from administrative data and Framingham cardiovascular risk equations applied using electronic medical record data. Five year lipid testing patterns were examined by age, sex and CVD risk. Generalized linear models were used to estimate the relative risk for over testing associated with patient characteristics.ResultsAmong males and females for whom testing is not recommended, 35.8 % and 61.5 % received at least one lipid test in the prior 5 years and 8.4 % and 24.4 % had two or more. Over-testing was associated with age, race, comorbidity, primary care use and neighborhood income. Among individuals at moderate and high-risk (not already treated with statins) and for whom screening is recommended, between 21.4 % and 25.1 % of individuals received no screening in the prior 5 years.ConclusionsBased on USPSTF lipid screening recommendations, this study documents substantial over-testing among individuals with low CVD risk and under-testing among individuals with moderate to high-risk not already on statins. Opportunity exists to better focus lipid screening efforts appropriate to CVD risk

    Ancient road systems have often been used by archaeologists to reconstruct interaction and political ties among prehistoric settlements. Roads built by the ancient Maya offer many insights into the political geography of the area, particularly in the northern lowlands where hieroglyphic texts are rare. This study examines ethnohistoric, historic, and archaeological data that suggest that a regional road, some 300 km in length, once spanned the northern lowlands from the modern location of MĂ©rida to the east coast facing the island of Cozumel. The political implications of such a road, if it once existed, are discussed

    Development of a Transition Between an Energy-Absorbing Concrete Barrier and a Rigid Concrete Butress

    From 2010 to 2015, MwRSF researchers developed the RESTORE barrier, which is a restorable MASH TL-4 median barrier with a steel and concrete rail supported by elastomer posts and steel skids. The research effort reported herein describes the initial development of a transition from the RESTORE barrier to a rigid TL-4 concrete buttress. The previously-developed RESTORE barrier LS-DYNA model was validated against three full-scale vehicle crash tests. Several design concepts were generated through a series of brainstorming efforts. The primary transition concept consisted of a pin and loop connection between the RESTORE barrier and rigid concrete buttress, which was designed and evaluated with LS-DYNA computer simulation. Vehicle and system behavior were investigated using MASH test designation nos. 4-20, 4-21, and 4-22. Six horizontal gusset plates and drop-down pin allowed for limited deflection and rotation at the transition joint, but provided shear continuity between the two systems. A rounded-edge cover plate mitigated vehicle snag on the transition joint hardware. Eleven impact points were evaluated with each vehicle model to determine critical impact points for use in a future full-scale crash testing program. All occupant risk measures and vehicle stability were within MASH limits. Further design modifications are recommended to limit stresses in the transition joint hardware and to reduce excessive occupant compartment deformation that occurred when the small car impacted the concrete buttress end

    Differential respiratory health effects from the 2008 northern California wildfires: A spatiotemporal approach

    AbstractWe investigated health effects associated with fine particulate matter during a long-lived, large wildfire complex in northern California in the summer of 2008. We estimated exposure to PM2.5 for each day using an exposure prediction model created through data-adaptive machine learning methods from a large set of spatiotemporal data sets. We then used Poisson generalized estimating equations to calculate the effect of exposure to 24-hour average PM2.5 on cardiovascular and respiratory hospitalizations and ED visits. We further assessed effect modification by sex, age, and area-level socioeconomic status (SES). We observed a linear increase in risk for asthma hospitalizations (RR=1.07, 95% CI=(1.05, 1.10) per 5µg/m3 increase) and asthma ED visits (RR=1.06, 95% CI=(1.05, 1.07) per 5µg/m3 increase) with increasing PM2.5 during the wildfires. ED visits for chronic obstructive pulmonary disease (COPD) were associated with PM2.5 during the fires (RR=1.02 (95% CI=(1.01, 1.04) per 5µg/m3 increase) and this effect was significantly different from that found before the fires but not after. We did not find consistent effects of wildfire smoke on other health outcomes. The effect of PM2.5 during the wildfire period was more pronounced in women compared to men and in adults, ages 20–64, compared to children and adults 65 or older. We also found some effect modification by area-level median income for respiratory ED visits during the wildfires, with the highest effects observed in the ZIP codes with the lowest median income. Using a novel spatiotemporal exposure model, we found some evidence of differential susceptibility to exposure to wildfire smoke
