    The article discusses the current characteristics of trade unionism in Italy. First, however, attention is paid to the initial imprinting of the model, which stemmed from the circumstances in which the trade unions were reconstituted at the end of WWII, and whose far-reaching consequences are still apparent today. In fact, because of original divisions along ideological lines, and within a context of enduring voluntarism and low institutionalisation, the Italian trade unions, which acquired large followings and strong organizational capacity and influence over time, still tend to oscillate between behaving as either organizations or social movement according to convenience and to pressures applied by the rank and file

    Italian politics have undergone momentous change in the 2007–2017 decade under the impact of the eurozone crisis, whose peak in 2011–2013 could be equated to the earlier watershed years of 1992–1994. The lasting impact of the upheaval in Italian politics in the early 1990s could still be felt in the decade of economic recession, but there were also new challenges prompted by a crisis that had its roots in international financial contagion and which unravelled under the shadow of both recession and austerity. The changes were of an economic, social, cultural, institutional, policy-oriented and political nature. If one central quintessentially political theme stands out by the end of this decade it is the apparent exhaustion of the quest for bipolarisation that was initiated in the early 1990s

    Collective bargaining in Italy primarily takes place at two negotiation levels: at the industry or sectorial level, which addresses issues such as the periodic determination of the pay and conditions for the industry or sector, and at the company level or plant level, which addresses specific workplace problems. Until the early 1990s, the balance between the two levels changed according to external circumstances. It was with the Tripartite Agreement of 23 July 1993 that the competencies, procedures and issues pertaining the two levels were clarified. Ever since, other agreements between trade unions and employers' organizations addressed the issues related the two levels. The agreement signed in April 2009 makes clear that greater decentralization of bargaining is seen as a mechanism to "re-launch" a growth in productivity and therefore of real incomes. This poses enormous challenges to employee's representatives as they assume a key role for the social innovation and competitiveness of the company.Drawing from interviews and a survey, it seems that Italian HR managers are aware of this key role of employee representatives, so much that they would like them to be more competent, open to change and less conflictual. They believe that a more cooperative environment leads to better agreements for both parties at the negotiation table

    There is limited clinical evidence on the utility of the monitoring of Epstein-Barr virus (EBV) DNAemia in the pre-emptive management of post-transplant lymphoproliferative disease (PTLD) in solid organ transplant (SOT) recipients. We investigated current preventive measures against EBV-related PTLD through a web-based questionnaire sent to 669 SOT programmes in 35 European countries. This study was performed on behalf of the ESGICH study group from the European Society of Clinical Microbiology and Infectious Diseases. A total of 71 SOT programmes from 15 European countries participated in the study. EBV serostatus of the recipient is routinely obtained in 69/71 centres (97%) and 64 (90%) have access to EBV DNAemia assays. EBV monitoring is routinely used in 85.9% of the programmes and 77.4% reported performing pre-emptive treatment for patients with significant EBV DNAemia levels. Pre-emptive treatment for EBV DNAemia included reduction of immunosuppression in 50.9%, switch to mammalian target of rapamycin inhibitors in 30.9%, and use of rituximab in 14.5% of programmes. Imaging by whole-body 18-fluoro-deoxyglucose positron emission tomography (FDG-PET) is used in 60.9% of centres to rule out PTLD and complemented computer tomography is used in 50%. In 10.9% of centres, FDG-PET is included in the first-line diagnostic workup in patients with high-risk EBV DNAemia. Despite the lack of definitive evidence, EBV load measurements are frequently used in Europe to guide diagnostic workup and pre-emptive reduction of immunosuppression. We need prospective and controlled studies to define the impact of EBV monitoring in reducing the risk of PTLD in SOT recipients

    What is the role of the placebo effect for pain relief in neurorehabilitation? Clinical implications from the Italian Consensus Conference on Pain in Neurorehabilitation

    Background: It is increasingly acknowledged that the outcomes of medical treatments are influenced by the context of the clinical encounter through the mechanisms of the placebo effect. The phenomenon of placebo analgesia might be exploited to maximize the efficacy of neurorehabilitation treatments. Since its intensity varies across neurological disorders, the Italian Consensus Conference on Pain in Neurorehabilitation (ICCP) summarized the studies on this field to provide guidance on its use. Methods: A review of the existing reviews and meta-analyses was performed to assess the magnitude of the placebo effect in disorders that may undergo neurorehabilitation treatment. The search was performed on Pubmed using placebo, pain, and the names of neurological disorders as keywords. Methodological quality was assessed using a pre-existing checklist. Data about the magnitude of the placebo effect were extracted from the included reviews and were commented in a narrative form. Results: 11 articles were included in this review. Placebo treatments showed weak effects in central neuropathic pain (pain reduction from 0.44 to 0.66 on a 0-10 scale) and moderate effects in postherpetic neuralgia (1.16), in diabetic peripheral neuropathy (1.45), and in pain associated to HIV (1.82). Moderate effects were also found on pain due to fibromyalgia and migraine; only weak short-term effects were found in complex regional pain syndrome. Confounding variables might have influenced these results. Clinical implications: These estimates should be interpreted with caution, but underscore that the placebo effect can be exploited in neurorehabilitation programs. It is not necessary to conceal its use from the patient. Knowledge of placebo mechanisms can be used to shape the doctor-patient relationship, to reduce the use of analgesic drugs and to train the patient to become an active agent of the therapy

After a general introduction, a particular emphasis is put on the Territorial Employment Pacts (TEPs) of EU origin. In the conclusions, the highly controversial issue of the best way of regulating such pacts is discussed
